5.Diagnostic value of T-cell enzyme-linked immunosorbent spot assay in pleural effusion and peripheral blood from patients with tuberculous pleurisy

Objective To evaluate the diagnostic value of T-cell enzyme-linked immunosorbent spot assay (T-SPOT.TB) using both pleural effusion and peripheral blood in tuberculous pleurisy.Methods One hundred and two cases of in patients with pleural effusion treated in Anhui Chest Hospital from January 2014 to October 2015 were enrolled in this study.T-SPOT.TB was performed using both serous effusion mononuclear cells (SEMC) and peripheral blood mononuclear cells (PBMC),and the diagnostic sensitivity and specificity were calculated.Chi square test was used for categorical variables and MannWhitney U test was used for continuous variables in non-normal distribution.Results Of the 102 participants,71 (69.61 %) were microbiologically or clinically diagnosed with tuberculous pleurisy and 31 (30.39 %) were diagnosed with other diseases.In tuberculous pleurisy group,the median spot forming cells (SFC) of T-SPOT.TB for early secretory antigenic target 6 (ESAT-6) and culture filtrate protein 10 (CFP-10) using SEMC were 159/2.5 × 105 (120/2.5 × 105-200/2.5× 105) and 160/2.5 × 105 (110/2.5 × 105-210/2.5 × 105),respectively;and the median SFC of T-SPOT.TB using PBMC were 30/2.5×105 (15/2.5×105-55/2.5×105) and 40/2.5×105(8/2.5×105-87/2.5 ×105),respectively.The SFC counts of SEMC were obviously higher than those of PBMC,and differences were statistically significant (for ESAT-6,Z=-7.818;for CFP-10,Z=-7.120,both P<0.05).The ROC curve analysis showed that using a cut-off value of 38 SFC per 2.5 × 105 SEMC,the area under the curve is 0.969.The sensitivity and specificity of T-SPOT.TB using PBMC were 90.14% and 77.42%,respectively;and those of T-SPOT.TB using SEMC were 95.77% and 93.55%,respectively.When combined the T-SPOT.TB assay using both PBMC (≥6 spots) and SEMC (≥38 spots),the sensitivity and specificity were 90.14% and 96.77%,respectively.Conclusion The joint detection of T-SPOT.TB using both PBMC and SEMC can be an effective diagnostic method for tuberculous pleurisy.
6.The significance of standardized fetal ultrasonographic screening in the first trimester

Objective To evaluate the significance of standardized ultrasound examination of fetal structures in the ifrst trimester (11-13+6 weeks). Methods The ultrasound data of 29 858 fetuses who underwent the ifrst trimester screening were analyzed retrospectively in Beijing Obstetrics and Gynecology Hospital of Capital Medical University from January 2010 to December 2012 and followed up the cases with structural abnormalities and increased nuchal translucency (NT). Results Among 29 858 fetuses, 328 structural defects were detected in 284 fetuses (0.95%) by ultrasound in the first trimester, including fetal hydrops (124 cases), choroid plexus cysts (36 cases), exencephaly (32 cases), gastroschisis (24 cases), exomphalos (22 cases), cardiac defects (19 cases), megacystis (14 cases), spine abnormalities (10 cases), meningoceles/encephaloceles (9 cases), alobar holoprosencephaly (8 cases), hydrocephaly (7 cases), abnormalities of extremities (7 cases), acrania (6 cases), amniotic band syndrome (4 cases), abdominal cyst (3 cases) and conjoined twins (3 cases). Pregnancy was terminated in all cases (115) with exencephaly, gastroschisis, exomphalos, meningoceles/encephaloceles, alobar holoprosencephaly, hydrocephaly,abnormalities of extremities, acrania, amniotic band syndrome and conjoined twins, and the defects were verified by the gross appearance of fetuses. Two cases of megacystis were turned out to be normal during follow-up while 1 case was proved to be aneuploid and 1 other case progressed to multiple abnormalities during the second trimester. Most choroid plexus cysts, except 1 case of aneuploid, were normal during the second and the third trimester follow-ups by ultrasound. Thirteen cases of fetal hydrops were found to be aneuploids and 4 hydrops cases were proved to be complicated with other structural defects during the second and the third trimester. NT was increased in 422 cases (1.41%), among which 122 cases (28.91%) were complicated with structural defects and/or fetal hydrops. Ninty-nine cases with increased NT underwent chromosomal examination and 38 cases were found to be aneuploides. During the follow-up of 298 cases with increased NT who continued pregnancy, 21 structural abnormalities were found, including cardiac defects (14 cases), neural abnormalities (4 cases) and diaphragmatic hernia (3 cases). Conclusion The standardized fetal ultrasound in the first trimester is a effective tool for the screening of aneupolides and major structural abnormalities.
7.The effect of HLA matching on patient/graft survival rate of renal transplant recipients positive for PRA

Objective To elucidate the effect of human leukocyte antigen(HLA)cross-reactive groups(CREGs)matching on the patient/graft survival rate of renal transplant recipients positive for penal-reactive antibody(PRA).Methods The PRA level and the HLA antibody specificity of renal transplant candidates were assayed using the LAT1240,LM720R,and SSP2LB kits.The appropriate donors were selected according to the CREGs matching standard.Results 112 recipients included 43 positive cases of HLA classⅠantibody,39 positive cases of HLA classⅡantibody,and 30 positive cases of bothⅠandⅡ.The HLA mismatching cases of 0,1,2,3,4 and 5 allele(s)were 6,39,38, 21,7 and 1,respectively.Hyper-acute rejection(2 cases),acute rejection(18 cases),chronic rejec- tion(5 cases),delayed graft function(4 cases),graft excision(one case)due to rejection and deaths (13 cases,5 with a functioning graft)occurred after transplant.Ninety-nine recipients and 96 grafts survived.The 1-,3-,and 5-year graft survival rate was 91.96%,86.96%,and 86.21%,respec- tively.Conclusion The HLA matching rate of patient/graft was elevated significantly and the effect of high PRA level was decreased according to the principle of CREGs,which increased the patient/graft survival rate remarkably in sensitized recipients.
8.Behcet's disease with neurological involvement: a case report

Behcet's Disease is a chronic systemic disorder characterized by recurrent oral and genital ulcerations, ocular inflammation, having manifestations related to skin and joints. Neurologic signs can also be observed seldomly during the course of the disease. In this report a case of Behcet's disease with neurological involvement was presented.
9.Effects of amniotic membrane on proliferation and differentiation of human retinal pigment epithelial cell

Background Human retinal pigment epithelial (RPE) cell transplantation treating retinal degenerative diseases is a researching topic,and the source of human RPE cells is a key problem.Many biological carriers can be used for the preparation of RPE cell layer.However,some advantages,such as cytotoxicity,lack of stability and immunologic reaction etc.are still existed.To study an ideal biological carrier is very important.Objective This experimental was to determine the effects of amniotic membrane on the proliferation and differentiation of human RPE cells and the possibility as a scaffold for RPE cell transplantation.Methods ARPE19 cell line cells were cultured and passaged in DMEM/F12 medium with 10% fetal bovine serum,and 8-12generation of cells were used.The cells were divided into two groups.One group of cells were incubated on the denuded amniotic membrane,and the other group of cells were cultured in the medium (control group).MTT was performed to detect the A492 value of RPE cells for the evaluation of cell proliferation ability 24,48,72,96 hours after culture.Cell morphology was compared by histopathological examination 3 weeks after culture.The mRNA expression of pigment epithelium-derived factor (PEDF),N-cadherin,β-catenin and cell connection related proteins in the cells of both groups were assayed using reverse transcription polymerase chain reaction (RT-PCR).Ultrastructure of the cells was observed under the transmission and scan electronic microscope 3 weeks after culture.Results The number of ARPE-19 cells cultured on denuded amniotic membrane was decreased significantly in comparison with the normal culture plate(F=41.760,P =0.000).Histopatholy also showed that the cell density on amniotic membrane was lower than of normal cells on plate surface.Moreover,the expression level of claudin 1 mRNA,N-cadherin mRNA and PEDF mRNA were significantly up-regulated in denuded amniotic membrane group in comparison with control group (t=15.828,P=0.000 ;t=6.839,P=0.002 ;t=14.667,P=0.000),but the expression of Connexin 43 mRNA was down-regulated in denuded amniotic membrane group compared with control group(t=3.358,P=0.024).Ultrastructural examination revealed that ARPE-19 cells cultured on amniotic membrane exhibited a polygonal epithelial phenotype with cilium on the apical side,however,the cells cultured on normal culture plate displayed fusiform shape and uneven thickness.Conclusions Amniotic membrane plays a promoting effect on the differentiation of ARPE-19 cells and a inhibitory effect on the proliferation of ARPE-19 cells,suggesting that amniotic membrane might be an useful scaffold for the preparation of functionally mature RPE cells for clinical transplantation.
10.Effects of Kinesio Taping on Muscle Strength of Knees in Female Students

Objective To investigate the effects of Kinesio taping on function of knees in girls. Methods 20 female students were tested the peak torque of centripedal and eccentric contraction of knee flexion and extension with Biodex isokinetic testing at 60°/s angular velocity, with Kinesio taping, placebo taping and without taping. Their amplitude root of mean square (RMS) of surface electromyography of vastus medialis (VM), rectus femoris (RM) and vastus lateralis (VL) were also recorded. Results For centripedal isokinetic contraction, the relative extensors peak torque was the most as Kinesio taping (P<0.01). There was no significant difference in relative flexor peak torque (P> 0.05). Standardized RMS of VM and VL were the least as Kinesio taping (P<0.01), but not significantly different of RF among all the conditions (P>0.05). For eccentric isokinetic contraction, the relative extensor peak torque of both extensors and flexors were the most as Kinesio taping (P<0.01), while the Standardized RMS of VM and VL were the least (P<0.01), but no significant difference was observed in RF (P> 0.05). Conclusion Kinesio taping may enhance the strength of centripedal, eccentric contractions of quadriceps and eccentric contractions of hamstring, and increase the muscle fibers recruitment of VM and VL.
