Chinese Journal of Geriatrics 2012;31(8):662-665
Objective To understand the clinical feature of the elderly patients with diabetes during coronary angiography, and analyze the risk factors of contrast-induced nephropathy (CIN).Methods The clinical data of 269 elderly patients who had undergone coronary angiography and percutaneous coronary intervention(PCI) from January 2007 to December 2009 in our hospital were analyzed retrospectively. The patients were divided into two groups:CIN group and non-CIN group. The possible risk factors for CIN,such as glycemic control, diabetic complication, renal function, volume of contrast medium, inflammatory state, therapy of perioperative period, past medical history were analyzed and compared between the two groups. Results In 269 elderly patients with diabetes, the incidence of CIN was 9.3 % (25/269).According to estimated glomerular filtration rate (e-GFR),the patients were divided into four subgroup:≥90 ml/min,89-60 ml/min,59-30 ml/min,<29 ml/min.The incidences of CIN for the subgroups were 2.2%(1/45),4.4%(6/135),17.3%(14/81) and 50 % (4/8),respectively.Multivariate logistic gradual regressive analysis showed that loop diuretic use (OR> 6.07),preoperative e-GFR(<60 ml/min) (OR>3.27),volume of contrast medium (≥200ml) (OR>3.26),chronic kidney disease(CKD) (OR>2.80) (P=0.001,0.024,0.015,0.048) were indepen-dent risk factors for CIN (P<0.05). Conclusions Loop diuretic use, preoperative GFR (<60 ml/min),volume of contrast medium (≥200 ml) and CKD are independent risk factors of CIN.

UNLABELLEDOBJECTIVE To investigate the effect of Nepsilon-(carboxymethyl) lysine albumin (CMLs), a primary advanced glycation end products (AGEPs) isoform in diabetic body, on the function and angiogenesis of adipose tissue-derived stem cells (ADSCs) and the protective effect of Danhong Injection (DH). METHODS Human ADSCs were cultured and separated from human subcutaneous fatty tissue using enzymatic digestion and centrifugation. The morphology was observed using optical microscope and differentiation capacities assessed. Cells were exposed to 5 different interventions respectively for 24 h, i.e., PBS, 60 1 microg/mL BSA, 60 microg/mL CML-BSA, 100 microL/mL DH, and 60 micro./mL CML-BSA +100 microL/mL DH. Their effect on the proliferation, migration, apoptosis, and secretion were observed using WST-1 assay, Transwell assay, Annexin V-FITC/PI flow meter test reagent kit, human VEGF reagent kit, ELISA reagent kit, respectively. The effect on ADSCs angiogenesis was observed by in vitro angiogenesis test.
RESULTSCompared with the BSA group, the capacities of proliferation and migration could be significantly inhibited by CML-BSA, the apoptosis promoted, the secretion of VEGF reduced, and the angiogenesis of ADSCs weakened (P < 0.05). Compared with the blank control group, 100 microL/mL DH could significantly promote the proliferation and migration capacities of ADSCs, inhibit apoptosis of ADSCs, increase the secretion of VEGF, and improve the angiogenesis of ADSCs (P < 0.05). Compared with the CML-BSA group, the inhibition of CML-BSA on the proliferation and migration capacities of ADSCs could be significantly reversed, the promotion of CML-BSA on the apoptosis of ADSCs improved, the secretion of VEGF increased, and the angiogenesis of ADSCs elevated (P < 0.05).
CONCLUSIONclusion CMLs could significantly inhibit the proliferation and migration capacities of ADSCs, promote their apoptosis, and inhibit their angiogeneses, which could be improved by DH.

Acta Physiologica Sinica 2009;61(6):559-566
The goal of the present study was to explore high-frequency rotation sensation function damage of the crista of the horizontal semicircular canal induced by gentamicin and its morphological basis. The guinea pigs were randomly divided into four groups (group 1, 2, 3 and control group, n=20). The animals of treated groups received gentamicin subcutaneously (50 mg/kg per day) for 1 week (group 1), 2 weeks (group 2) and 3 weeks (group 3), respectively. The animals of control group were administered with equal volume of saline subcutaneously. Videonystagmography (VNG) evoked by ice water or high-frequency rotations, and vestibular evoked potential (VsEP) evoked by low- and high-frequency rotations were recorded after the administration. After VNG and VsEP were examined, the cristae of the horizontal semicircular canals were prepared for scanning and transmission electron microscopy. The results are as follows: (1) In VNG examination, after ice water stimulation, no obvious nystagmus was observed in the animals of group 3, and there was no significant difference in nystagmus frequency and duration among group 1, 2 and control group (P>0.05). After high- frequency rotations, no obvious nystagmus was yet observed in the animals of group 3. There were significant differences in the nystagmus frequency and duration of the VNG waves between the experimental group 2 and control group (P<0.01), and no difference was found between group 1 and control group (P>0.05). (2) In VsEP examination, there was no significant difference in the parameters of VsEP among group 1, 2 and control group (P>0.05), and VsEP was not observed in group 3 after low-frequency rotations. After high- frequency rotations, compared with control group, the latencies and amplitudes of P1 and P2 in group 2 significantly decreased (P<0.01), while no changes were found in group 1 (P>0.05). Group 3 had no response of VsEP to high-frequency rotations. (3) Electron microscopy was used to observe the crista hair cells of the four groups. In control group, the hair cells were normal. In group 1, almost normal appearance with slight edema of the hair cells was shown. In group 2, some hair cells in the central top of the crista necrosed, but other hair cells in the peripheral area showed normal appearance. In group 3, vestibular hair cells were entirely impaired or necrosis. These results show gentamicin selectively induces high-frequency rotation sensation function damage of the crista of the horizontal semicircular canal. The damage is concentrated on the central top area and then subsequently extended to the peripheral area of vestibular crista. It can be inferred that the hair cells in the central top area of the crista are involved in high-frequency rotation vestibular perception.
