1.Analysis of 654 ADR Cases Reported in Our Hospital

OBJECTIVE:To survey the status quo of ADR occurred in our hospital.METHODS:654 ADR reports were collected from our hospital during 2003~2009 and analyzed statistically.RESULTS:Of total 654 ADR cases,the ratio of male to female was 1∶1.67.20.34%of ADR occurred to people aged from 41 to 50.452 ADR cases were induced by intravenous administration accounting for 69.11%and 177 ADR cases were induced by oral administration,accounting for 27.06%.342 ADR cases were caused by antibiotics(52.29%),among which 127 cases by cephalosporins(37.13%),91 cases by quinolones(26.61%)and 54 cases by macrocyclic lactone(15.79%);92 cases were caused by cardiovascular system drugs(14.07%).Main clinical manifestations were lesion of skin and its appendants(25.54%)and digestive system injury(22.48%).CONCLUSION:Great importance should be attached to ADR monitoring,rational use of antibiotics and close monitoring of TCM injection.
2.How to improve the level of endoscopic diagnosis and treatment of early gastrointestinal cancer: private opinions.

At present, the level of diagnosis and treatment of early gastrointestinal cancer has a larger gap between China and developed countries. In recent years, the advent of new endoscopic imaging techniques has improved the detection rate of early gastrointestinal cancer. Along with this, endoscopic submucosal dissection(ESD) enables radical resection for early cancer. In this article, we discuss how to improve the level of endoscopic diagnosis and treatment of early gastrointestinal cancer.

5.Adverse Drug Reactions in Our Hospital:Analysis of 258 Cases

OBJECTIVE:To investigate the characteristics and general pattern of the adverse drug reactions (ADR) occurred in our hospital. METHODS:A total of 258 ADR cases collected from June 2006 to June 2008 were analyzed statistically in respect of patients' age and sex,drug variety,clinical manifestations,routes of administraiton and dosage forms etc. RESULTS:Of the total 258 ADR cases,the majority(64.34%) were induced by antimicrobial drugs; 45.74% presented with lesions of skin and its appendants,and 77.91% were induced by intravenous way. CONCLUSION:To develop and strengthen ADR monitoring and reporting should be regarded as the key to enhance medication safety.
6.Expression of bFGF in chronic interstitial nephritis

Objective To study the expression of basic fibroblast growth factor(bFGF)in the kidney tissues of chronic interstitial nephritis(CIN).Methods Expression of bFGF was assayed in 30 patients with CIN and 5 normal persons by SP method of immunohistochemical technique.The average gray value and average optical density of the bFGF positive parts and its ratio with the whole visual field had been measured with video analysis system.Results The expression of bFGF in the kidney tissues of CIN was significantly stronger than that in normal tissues.The average gray value,average optical density and the positive areas were significantly high compared with that of normal cases.Conclusion The bFGF can promote the proliferation of fibroblasts and monocytes in renal interstitial patients and lead to renal interstitial fibrosis in the end.
7.Expression of NF-kB during the replacement of primary and permanent teeth in dog

Objective:To observe the expression of nuclear factor kappa B(NF-kB) during the replacement of primary and permanent teeth in dog. Methods:By immunohistochemistry, expression of NF-kB was examined in the tissues around deciduous tooth root at resorption stage in 2 dogs aged 3-4 months.Results:NF-kB positive signals were observed in the osteoclasts up to the permanent tooth germ and in odontoclasts in the medial wall of the pulp cavity of deciduous teeth.Conlusion:NF-kB may involve in the eruption of permanent teeth during the stage of the replacement of primary and permanent teeth.
8.A comparative study on different postoperative drainage ways in total knee arthroplasty

Objective To compare the clinical outcomes of different drains in total knee arthroplasty.Methods Fifty-five patients,who had accepted primary unilateral total knee arthroplasty from October 2010 to November 2012,were included in the comparative study.Based on different drainage ways,25 patients were arranged into autotransfusion drain group,18 into routine drain group,and 12 into delayed drain group.Same operative procedure and postoperative care were provided to all patients.The total amount of postoperative drainage (including intraoperative blood loss),information of allogenic blood transfusion,body temperatures of patients on postoperative 1st,3rd,7th day,pre-and post-operative hemoglobin level,and postoperative swelling,wound healing quality and range of motion were recorded and compared.Results The amount of drainage was significantly more in autotransfusion drain group (799.2±196.7ml)than those in other two groups (381.7±129.2 ml in delayed drain group and 666.1±155.0ml in routine drain group).The postoperative hemoglobin level in delayed drain group (91.50±7.92 g/L)was similar to that in autotransfusion group (91.96±9.57 g/L),while significantly higher than that in routine drain group (81.33±9.86 g/L).The highest body temperature of patients in autotransfusion drain group was higher than those in other two groups.Allogenic blood transfusion was performed in 8 patients (44.4%) in routine drain group,4 patients (16.0%) in autotransfusion drain group and 2 patients (16.7%) in delayed drain group.There were no differences between groups in swelling,wound healing quality,and range of motion.Conclusion Delayed release of drain in total knee arthroplasty could reduce blood loss,chances of allogenic blood transfusion and postoperative fever,and extra cost,which can bring more benefits to patients.
9.Expression of Serum Interleukin-4,-6,-8 and Tumor Necrosis Factor-? in Children with Henoch-Schonlein Purpura

Objective To investigate changes of serum levels of interleukin(IL)-4,-6,-8 and tumor necrosis factor-?(TNF-?),and probe their clinical significance in children with Henoch-Schonlein purpura(HSP)/ HenochSchonlein purpura nephritis(HSPN).Methods Serum levels of IL-4,-6,-8 and TNF-? of 45 children with HSP/HSPN and 43 healthy children were examined by enzyme linked immunosorbent assay(ELISA),changes and correlation between the aforementioned cytokines in children with HSP /HSPN were analyzed.Results 1.Serum levels of IL-4,-6,-8 and TNF-? of children with HSP were higher than those of control group(P0.05);The serum level of TNF-? was positively correlated to the serum levels of IL-6,-8 of patients with HSP(r=0.670 P
10.Evaluation on dosage-based efficacy-toxicity correlation of Tripterygium wilfordii against immune inflammation in mice.

OBJECTIVETo study the anti-immune inflammation efficacy and toxicity of Tripterygium wilfordii decoction, in order to provide experimental basis for studies on its "efficacy-toxicity" correlation.
METHODThe delayed hypersensitivity model was established by dinitrofluorobenzene in mice. Different doses of T. wilfordii decoction was administered for 5 consecutive days. The ear swelling inhibition ratio and the toxic action were observed. After the final administration, the biochemical indexes of PGE2, TNF-α, IL-2, ALT, AST, PA, TBA, TBIL in serum were detected, and the visceral indexes of heart, liver, spleen and kidney were measured.
RESULTThe DNFB-induced ear swelling could be notably inhibited by multiple oral administration of T. wilfordii decoction, with the ED50 and its 95% confidence limit of 0.34 (0.21-0.42) g x kg(-1). The contents of PGE2, TNF-α, IL-2 in serum decreased in a dose-dependent manner. The activities of serum AST, ALT, TBA, TBIL and the PA content reduced.
CONCLUSIONT. wilfordii decoction shows a significant anti-immune inflammation efficacy within the dosage range between 0.59 and 2.34 g x kg(-1) in a dose-dependent manner. With a certain hepatotoxicity, high dose (2.34-4.68 g x kg(-1)) of T. wilfordii decoction can cause substantial liver injury, with a dose dependence in liver function index. Therefore, the efficacy and toxicity of T. wilfordii is dose dependent, which provides reference for preventing adverse drug reactions in clinic and developing early-warning schemes and ensure the clinical medication safety of T. wilfordii.
