1.Analysis of depression by TCM

Based on analysis of the relationship between depression and seven emotions,etiopathogenisis and pathogenesis of depression,the relationship between depression and stagnation of liver qi,it showed that depression had highly correlation with sorrow,grief.The etiopathogenisis and pathogenesis of depression included body factor,exogenous pathogen,associated diseases,social psychic factor,etc.The essence of primary depression,endogenous depression and depressive emotion with dysthymia was deficient,and yang-deficinecy was the key pathologic basis.Exogenous evil was an indirect cause of depression.Because depressive emotion had connection with stagnation of liver qi at some extent,it should distinguish depression from stagnation of liver qi.
2.The status of studies on the treatment and prognosis estimation in idiopathic macular hole

Idiopathic macular hole (IMH) refers to full thickness defects of retina in macular area with no clear reasons.The management of IMH includes vitrectomy combined with internal limiting membrane (ILM) peeling and pharmacological vitreolysis.But ILM peeling may damage the inner retina;novel techniques,such as inverted ILM flap technique and foveola non-peeling ILM surgery,autologous ILM transplantation had made the method of ILM peeling more diversified with less damage.Pharmacological vitreolysis targeting fibronectin and laminin is considered to work in a two-step mechanism,involving both vitreoretinal separation and vitreous liquefaction.Furthermore,IMH judgment and prognosis indicators like ellipsoid zone,macular hole index,hole formation factor,diameter hole index and tractional hole index based on spectral domain optical coherence tomography enriched the assessment of macular hole diameter,depth and shape.How to make full use of new interventions to reduce the incidence of macular hole and obtain a better visual acuity with closed holes is an important direction for future research.
3.Application of Lornoxicam during Craniotomy

ObjectiveTo evaluate the effect of lornoxicam used for craniotomy. Methods60 neurosugical patients, ASA physical I~II, were randomly allocated into three groups to receive normal saline in controlled group (GroupⅠ), lornoxicam 8 mg (Group Ⅱ) and lornoxicam 24 mg (Group Ⅲ) intravenously 10~15 min before anesthesia. The end-tidal concentration of isoflurane was measured. The volumes of bleeding, transfusion, fluid infusion and urine were recorded. The time of consciousness, psychomotor and cognitive recoveries from general anesthesia were observed. The VAS scores of pain were evaluated 48 h after operation.ResultsThe concentrations of end-tidal isoflurane in the controlled group were significantly higher than other groups (P<0.01). There was no difference among the three groups in the volume of bleeding, transfusion, infusion and urine. The recovery time of conscious, psychomotor and cognitive from general anesthesia were shorter in group Ⅱ and Ⅲ (P<0.01). The total dose of tramadol and VAS score after the operation were no difference among the three groups.ConclusionThe preoperative application of lornoxicam can reduce the concentrations of end-tidal isoflurane significantly, smooth the recovery from anesthesia.
5.In vitro culture of high-purity osteoclasts induced by macrophage colony stimulating factor/receptor activator of nuclear factor kappa B ligand: Optimal dosage investigation

BACKGROUND: Macrophage colony-stimulating factor (M-CSF)/receptor activator of nudeer kappa B ligand (RANKL), two types of cytokines co-induce myeloid stem cells to form osteoclasts, is a kind of new method to harvest ostaoclasts with high purity and quantity, but there is lack of uniform cultivation standard. OBJECTIVE: To construct an effective M-CSF/RANKL induced mice myeloid stem cells inducing osteoclast differentiation cultivation system. METHODS: Myeloid stem cells ware obtained from ICR mice and then cultured for 24 hours in a-minimum essential medium containing M-CSF, at cell density of 10~7/L, 10~8/L, 10~9/L. Then 10 μg/L M-CSF and 20, 50, 100 μg/L RANKL were added into culture medium. Tartaric-resistant acid phosphatase stained was performed to observe the transition process from stem cell to osteoclast, as well as cell morphology and stain situation after culture, and positive stained osteoclasts were counted. We compared the influence of different induction conditions to the quantity of osteoclast. RESULTS AND CONCLUSION: A small quantity of osteoclasts contained many red positive beads in the intracytoplasm were observed at 3 days. There were positive beads with hypochromatic dikeryon in cells. A large amount of positively stained osteoclests were seen after 6-day cultudng, which maintained dikaryon. After 9-day culturing, positively stained colossal multinudear cells occurred, became larger and maintained three nuclei. At certain cell density, 100 μg/L RANKL could induce to form more osteoclasts compared with other 2 concentrations (P < 0.05); at certain RANKL concentration, the osteoclasts formation at cells density of 10~8/L was dramatically greater than other 2 cell densities (P < 0.05); the number of osteoclasts was the most when the concentration of RANKL was 100 μg/L and cell density of 10~8/L (P < 0.05). When osteoclasts are induced by M-CSF/RANKL from mudne myeloid stem cells, the best concentration of RANKL is 100 μg/L and cells density is 10~8/L.
6.Characteristics of rehabilitation therapy for patients with total knee replacement

OBJECTIVE: To analyze the influencing factor, as well as rehabilitation method after total knee replacement.METHODS: Chinese Journal Full-text Database and PubMed database were retrieved with key words of artificial knee joint, athroplasty, and rehabilitation, from January 1999 to June 2009. The literature search was restricted to Chinese and English languages. The evaluation indexes were the knee range of movement, function, muscle strength, and joint stability. Accordingly, clinical research concerning rehabilitation treatment of total knee replacement was included, and the animal experiment was excluded.RESULTS: A total of 492 literatures were obtained by initial search with computer. On the basis of inclusion criteria, the related papers were analyzed. Rehabilitation training after total knee replacement includes pain control, joint activity training, strength training, proprioceptive training and walking training. Knee movement range was one of the important indicators in evaluating total knee replacement, which was also the most important component in the knee-rating system. Pain reduced and normal walking function was important to observe the therapeutic effects after total knee replacement. Strength training was an important part of rehabilitation training, early rehabilitation not only can regain the muscle strength, but also can promote blood circulation and prevent deep vein thrombosis of lower limbs.CONCLUSION: Rehabilitation training can restore muscle strength, joint activity and walking ability as quickly as possible, and control pain greatly, which influence therapeutic effects of replacement.
