1.Clinical observation of a modified anterior fixation technique for thoracolumbar fractures

Objective To modify the anterior fixation technique for thoracolumbar fractures so as to reduce blood loss and improve the success rate of the first vertebral screw implantation. Methods Only surface artery of the injured vertebrae was ligated and the discs adjacent to the burst vertebra excised. Then,the mass autograft ilium was harvested with most cartilage terminal lamina left. The bursted vertebra was resected so that the vertebral canal could be decompressed thoroughly. Structural ilium graft was performed,followed by Z-plate being placed and locked. Results The modified operative technique was used to treat 87 cases,of which 76 cases of fresh thoracolumbar fracture had hemorrhage capacity about 400-800 ml (average 560 ml) as well as operation time for 2.5-3 hours,and 11 cases with old fractures had hemorrhage capacity about 800-1 200 ml (average 1 080 ml) as well as operative time for 3.5-4 hours. Eighty-one cases with kyphosis deformity was completely corrected,four cases had kyphosis deformity of 5?-8? and two lordosis. A total of 39 cases with post-operative paralysis basically recovered,28 partially recovered,nine recovered fractionally and 11 was beyond recovery. No aggravation was found in post-operative nerve syndrome. Conclusions The modified anterior fixation technique is excellent in reducing blood loss,shortening the operation time and increasing the success rate of the primary vertebral screw implantation. [

Three-Layer thought is an important proposition in Chinese traditional philosophy. This thought embodies the Chinese people's cosmology and methodology and exerts a far-reaching influence on various aspects of Chinese culture. The embodiment of Three-Layer thought in the theory and practice of acupuncture and moxibustion from naming of acupoints, principles of treatment, needling instruments, prescription of acupoints as well as needling techniques is elaborated and briefly analyzed. Thus it illustrates the comprehensive application of Three-Layer thought in acupuncture and moxibustion through the history and the significance of Chinese traditional philosophy in the science of acupuncture and moxibustion.

OBJECTIVE:To study the effects of 1,25-droxyvitamin D3 on nitric oxide and nitric oxide synthase in peripheral blood mononuclear cells(PBMC)in patients with spinal tuberculosis and to explore its immunoregulation function.METHODS:Patients' PBMC were assigned to test group in which DHVD3 was added or control group in which culture medium was added,then cultured after BCG vaccine inoculation.Cultured supernatant fluid from isolated PBMC was collected at 0,3,6,9,12 days respectively for measuring of relative optical density(OD)of NO by nitric oxide kit.Total RNA extracted from the PBMC was reverse transcribed to cDNA and amplificated by fluorecent real-time quantitative PCR on the sixth day;mRNA of target gene iNOS and reference gene 3-GAPD were analyzed and compared by relative quantitative method 2-??Ct.RESULTS:OD value of NO and mRNA of iNOS in test group were all higher than in control group(P

6.Meta analysis of low molecular weight heParins on severe acute Pancreatitis

Objective To elevate the efficacy and safety of low molecular weiGht heparins on severe acute pancreatitis. Methods Random or controlled clinical trials literatures( CRT,CTT ) which met the inclusion criteria were collected by computer retrieval combined with manual screeninG,and the data were analyzed by Meta-analysis. Results There were 6 randomized controlled study,includinG 626 cases severe acute pancreatitis which divided into low molecular weiGht heparin Group(n=3l2)and control Group(n=3l4). There were statistical siGnificant differences between Groups in terms of the mortality rate cure rate,complication rate, operation rate,averaGe hospitalization time( P<0. 05). while the parameters of coaGulation showed no siGnificant difference between two Groups. Conclusion Conventional treatment of SAP with LMWH is superior to conventional therapy in improvinG patients'outcome and decreasinG the mortality. Initial subcutaneous therapy with the LMWH appeared to be effective and safe,which cannot lead to bleedinG tendency.
7.Microbial Population Distribution in Intensive Care Unit: Analysis of Its Alteration

OBJECTIVE To study the alteration of microbial population distribution in intensive care unit (ICU) for the reference to clinical drug administration. METHODS The distributed features of 413 strains of infection germs detected among the patients of ICU were statistically analyzed during the years of 2002 to 2005. RESULTS The main germs of 413 strains were Pseudomonas aeruginosa(87 strains), Acinetobacter baumannii(48 strains), Stenotrophomonas maltophilia(34 strains), Staphylococcus epidermidis(30 strains), S. aureus (20 strains) and Escherichia coli (19 strains). The first 3 species were P. aeruginosa (21.07%), A. baumannii(11.62%) and Sten. maltophilia (8.23%). CONCLUSIONS Most pathogens in ICU are Gram-negative bacteria and the proportion of Sten. maltophilia increases gradually. It is a very important clinical value to inspect the bacterial distribution and antibiotics should be used rationally.

Objective To analyze the possible effective intervention for heat stroke(HS)associated with multiple organ dysfunction syndrome(MODS)as a result of cross-country training.Methods All the data of causes of HS,clinical manifestations,laboratory findings,clinical interventions and outcome were collected and analyzed in five cases of HS associated with MODS.Results Four patients survived and one died of severe DIC and MODS.The levels of creatin kinase and myoglobulin in survived patients lowered gradually 4-5 days after effective treatments.No sequela was found in survived patients.Conclusions The therapeutic principle for the patients of HS associated with MODS is early commencement of hypothermia,fluid expansion,anticoagulation,blood purification and deep-sedation.
10.Action potential of cardiac pacemaker cells differentiated from mouse mesenchymal stem cells after HCN4 gene modification

Objective To investigate the potassium currents of the cardiac pacemaking cells induced and differentiated from rat mesenchymal stem cells (MSCs) modified by HCN4 gene. Methods Identified cardiac pacemaking cells were adopted as the experiment group, and the sinoatrial node cells of original infant rat cultured in the same period were regarded as the control group. Whole cell patch was used to measure the action potential of the pacemaking cells and sinoatrial node cells. Results Action potential of automatic depolarization at dilatation was recorded in both the differentiated cardiac pacemaking cells and sinoatrial node cells. There was no significant difference on amplitudes of resting potential, amplitudes and cycle of action potential [(-50?2.8) vs (-55?5.5),(-60?2.5) vs (-65?2.5),(240?57) ms vs (250?60) ms], but the field potential was much lower in cardiac pacemaking cells than the control group[(-30?2.5) vs (-55?5.5),P
