3.Changing strategy of severe acute pancreatitis management

In 1989, Fitz defined and classified severe acute pancreatitis (SAP) and he thought that surgical treatment of SAP is feasible. Since then, the strategy of treating SAP was a controversial issue over past decades. Currently, relevant literatures reported that medical or minimally invasive treatments are superior to surgical treatment, while it is too early to make the final conclusion because of the complexity of SAP. From 1989 to 2008, 1852 patients with acute pancreatitis were treated at the General Hospital of PLA, and the clinical data of 18 patients who died of SAP were retrospectively analyzed. Based on the analysis, we concluded that medical conservative therapy and surgical operative therapy should not be opposed to each other. Selecting ideal timing and appropriate operation on the basis of pathological changes of retroperitoneum and pancreatitis should be considered seriously, and the new concept of "miniaturization of trauma" should also be introduced in treating SAP.

8. Relationship between macrophage autophagy and atherosclerosis:research advances

Macrophage plays important roles in the formation of atherosclerotic plaque,and the role of macrophage autophagy in the process of atherosclerosis has drawn increasing attention from researchers. The lysosomal system degrades damages,degenerat- ed,senescent organelles and proteins in the cytoplasm. Basic level of autophagy can protect the cells from environmental stimuli, while excessive autophagy can lead to cell death and do harm to the stability of the plaque. The regulation of macrophage autophagy can affect the development of atherosclerosis. In this paper,the regulation of macrophage signal pathway in the development of athero- sclerosis,the factors that induce autophagy and other factors are reviewed systematically in order to provide a new target for the treat- ment of atherosclerosis.

Objective To investigate the methods and clinical significance of breast cancer treated with photodynamic.Methods From June to December in 2005,photodynamic therapy was used in 12 cases confirmed intramammary lymph node metastasis before operation and 15 cases confirmed chest wall recur- rences by means of lymph node imaging.Results The intramammary lymph node metastasis whose diameter between 0.5~1.0cm measured by lymph node imaging preoperatively completely disappeared when rechecked 3 months postoperatively.Chest wall recurrence regions of breast cancer whose diameter less than 1.0 cm completely remitted.Conclusion Photodynamic therapy is helpful to eliminate the intramammary lymph node metastasis and to cure the postoperative chest wall recurrence of breast cancer.
