Objective To investigate the effect of telephone follow-up on peripherally inserted central cathter(PICC) maintenance. Methods Two hundred and eighty-nine patients with PICC in our hospital from January to December 2010 served as the control group, given conventional health education on intubation, another 305 patients with PICC from January to December 2011 were set as the observation group, where telephone call follow-up was done on the basis of conventional health education on intubation. The two groups were compared in terms of timely catheter maintenance,catheter-related complications, accidental extubation and satisfaction.Result The observation group was significantly better than the control group in terms of timely catheter maintenance, catheter-related complications, accidental extubation and satisfaction (allP<0.05).Conclusion Telephone follow-up helps to improve patients compliance with PICC catheter maintenance, effectively control the complications and improve the quality of life and satisfaction degree.

Objective To observe the changes of osteoblasts and vascularization during osteogenesis by tissue engineering technique under the electron microscope and study the feasibility of improving vascularization of the tissue engineering bone by using the small intestine submucosa(SIS) as the scaffold. Methods The bone mesenchymal stem cells(BMSCs) were isolated by using the gradient centrifuge method.BMSCs were seeded in the SIS.The scaffold-cell constructs were cultured in vitro for two weeks. There were no cells on the SIS as control. They were implanted subcutaneously in the dorsa of the athymic mice. The implants were harvested after(in vivo) incubation for 4,8 and 12 weeks. The changes of osteoblasts and vascularization were observed under the transmission electron microscope and the scanning electron microscope. Results The BMSCs grew quite well.BMSCs differentiated on the surface of the SIS and secreted a great deal of extracellular matrices. The scaffold-cell constructs formed a lot of bone and vessels in vivo. The scaffold degraded after 12 weeks. No osteoblasts but vascularization and fibroblasts were observed as control. Conclusion SIS can be used as the scaffold for constructing tissue engineering bone as it can improve the formation of bone and vessels in vivo.

The methods for aptamer selection have been modified and developed in recent years. Selection efficiency has been increased due to the improvement in separation of bound and unbound nucleic acids; applicability of aptamers has been improved due to the development of primer-free SELEX and the utilization of short oligonucleotide library; more research requirements have been satisfied due to the functional modifications of aptamer selection methods. All the methodological advances enhance the potential value of aptamers in basic researches and clinical applications.

Objective:To study the influence of vitamin C ( VC) on dendritic cells ( DC) ,and detect DC maturation,to provide a feasible method and thought for quickly preparating DC vaccines. Methods:Collected the peripheral blood (about 50 ml) from healthy volunteers, and isolated peripheral blood mononuclear cells with lymphocyte separation medium and obtain DC.With stimulating with different concentrations of VC for (24 h),then washed with PBS,and set up blank control group (V0).The expression of DC surface co-stimulating molecules CD80/86 and HLA-DR, CD40 was detected by flow cytometry.By setting the concentration gradient and time gradient, exciting optimal concentration and stimulating time of VC on DC, and analyzed the reasons of VC promoting DC maturation. Results:VC could effectively stimulate DC,CD80/86 expression had significantly increased contrast to the blank control group (V0).And the experiments show that VC’s best stimulating concentration was 1 mmol/L,and on the third day,the CD80/86 expression rate of VC group was (78.6±4.6) %,and blank control group V0 was (34.1±5.7) %.DC surface HLA-DR expression:VC (56.8± 4.4) %,blank control group V0 (25.4 ±4.7) %,the difference between two groups was statistically significant,P<0.05.CD40 and CD40L expression and results show that VC 2.5 mmol/L group of CD40 expression rate up to (59.3±3.7) %,while V0 group was only (11.1 ±2.4) %,that illustrate VC could significantly regulate CD40 expression on DC surface, but CD40L not reflect.Fluorescence mi-croscope results showed that DC’ s antigen catching ability was also significantly promoted. Conclusion:VC can significantly regulate DC maturity, and may up regulate CD40,thus promoting the express of CD80/86 and HLA-DR.When the concentration is 1 mmol/L,VC expresses the strongest regulation function on DC.

Objective To investigate the high risk factors, pathology and clinical features in fatal pulmonary embolism in the elderly patients with malignant tumor, and to analyze the characters of the embolus and provide experimental data for clinical prevention and treatment offatal pulmonary embolism. Methods Autopsy and clinical data of 19 malignancy cases with fatal pulmonary embolism admitted to Beijing Hospital from 1975 to 2006 were retrospectively analyzed. Results 33.9% of total pulmonary embolism were from malignant diseases (19/56).Saddle and massive pulmonary embolism were in 84.2% of total 19 cases, and micro-embolism cases were in only 15.8%.The 84.2% of embolisms were from pulmonary thromboembolism,15.8% from tumor emboli, and 5.26% from fungi emboli.Pulmonary adenocarcinoma was the most common (36.5%),the second was pancreatic cancer (15.8%).In all the clinical symptoms,78.9% of symptoms were dyspnea,15.8% were syncope.Tachycardia and cyanosis were the common physical signs. Conclusions Malignant tumor is an important risk factor for pulmonary embolism in elderly patients. There are various kinds of emboli in pulmonary embolism in elderly patients with malignant tumor. We should comprehensively consider the causes of pulmonary embolism and give the reasonable and effective treatment to the patients.

Objective To analyze the reproducibility of keratometry and astigmatism measured by the VERION Digital Guidance System and the comparability of VERION with iTrace,Lenstar LS900 and manual keratometer. Methods The keratometry of 62 cataract patients were measured using four different devices. The steep keratometry (Ks),flat keratometry (Kf),astigmatic magnitude, astigmatic axis, cylinder at 0-degree meridian (vector component,J0) and cylinder at 45-degree meridian (vector component,J45) from each machine were recorded and analyzed. The three repeated measurements and the results of VERION system with other three devices were compared to analyze the reproducibility and comparability of VERION system. Results Reproducibility:Intraclass correlation coefficients and Cronbach's alpha values were higher than 0.9 for Ks,Kf,astigmatic magnitude, astigmatic axis,J0 and J45 measured by the VERION system (all P < 0.001).Comparability:The results of Ks and magnitude of astigmatism of VERION were larger than the iTrace (all P < 0.05) in the paired-samples t test. There was no statistical difference for the rest of parameters (all P > 0.05).The Bland-Altman graphs revealed the 95% limits of agreement (LOA) of J0,J45 and the astigmatic axis between VERION and iTrace were (-0.31-0.35) D,(-0.25-0.31) D and-13.5 °-12.3 °,respectively; There was no statistical differences for all parameters except for J45 in the paired-samples t test between the VERION and Lenstar LS900 (all P > 0.05).The Bland-Altman graphs revealed the 95% LOA of J0,J45 and the astigmatic axis were (-0.25-0.31)D,(-0.27-0.36) D and-13.5°-11.0°,respectively;There were statistical differences for the results of Kf and magnitude of astigmatism between the VERION and manual keratometer (all P < 0.05).The Bland-Altman graphs revealed the 95% LOA of J0,J45 and the astigmatic axis between VERION and manual keratometer were (-0.38-0.35) D,(-0.41-0.42) D,-12.6°-16.4°,respectively.Conclusion The VERION system is a reliable system for the measurement of keratometry and astigmatism. The keratometry and astigmatic magnitude of the VERION system have a good agreement with the iTrace,Lenstar LS900 and manual keratometer. However, the astigmatic axis measurements are significantly different among the four devices.
