Objective To inquire into the value of Liechtenstein Hernioplasty in the therapy of oblique or direct hernia.Method Liechtenstein was used in the therapy of 163 cases of oblique hernia (154 cases) and direct hernia (9 cases) from Feb.1998 to Jun.2001 and the clinical data was retrospectively studied.Most of them are of onset hernia except 8 cases of recurrent hernia.In 148 cases,the diameter of hernia ring is less than 2.5 cm and 30.1% of patients are elder than 65.The emergent operation was adopted in 12 cases.Results Off-bed exercises can be admitted at early stage of postoperation with slight pain and no local tension.Total recovery was obtained in 5-10 days after operation.After 6-36 months' follow-up,no mortality and less recurrence was detected.Conclusions There are so many advantages to treat oblique or direct hernia with Liechtenstein, such as minimized trauma, accelerated recovery,low recurrence rate and low cost. The indication of this operation involved all kinds of hernia (onset or recurrence of oblique or direct hernia,emergent operation of incarcerated oblique hernia, etc).

Objective To evaluate lactulose breath hydrogen test in diagnosis of small intestinal bacterial overgrowth in children.Me-thods Thirty-five children,aged 3 to 13 years old with mean (7.72?3.06) years old presented with chronic diarrhea,abdominal pain and flatulence.Among them,18 cases were male and 17 cases were female.Breath hydrogen test was carried out after the administration of a 0.5 g?kg-1 or 10 g lactulose,which was 100 g?L-1 solution.After the ingestion of lactulose,the breath hydrogen was collected in every 20 minutes and up to 3 hours.Small intestinal bacterial overgrowth was diagnosed as positive when the hydrogen concentration increased more than 20 mg?m-3 in a sample.Results Eighteen cases in 35 (51.43%) children were positive and 17 cases were negative.Among these positive cases,8 cases in 18 (22.86%) patients were typically double peaks in breath hydrogen concentrations.Among the causes of small intestinal bacterial overgrowth in children,4 cases with chronic superficial gastritis,4 cases with chronic bacterial enteritis,2 cases with malnutrition and anorexia,1 case with volvulus of stomach,1 case with massive resection of small intestine,1 case with steatorrhea and 5 cases with unknown causes.Four cases of chronic bacterial enteritis were gained remission by antimicrobial agents.The other cases were treated by lac-to-and bifidbacteria probiotics.Seven patients of the clinical systems were controlled,3 cases were improved remarkably and 4 cases were failed.Conclusions The lactulose breath hydrogen test is validated as screening test in the diagnosis of small intestinal bacterial overgrowth in children.Antimicrobial agents and bifidbacteria probiotics is effective in treatment of small intestinal bacterial overgrowth.

Objective The survey is based on status of sexual health and sexual needs for male patients with acute myocardial infarction and Percutaneous Transluminal Coronary Angioplasty (PTCA) plus stent implantation.The survey data provides the basis for guiding patients' health education.Methods General questionnaire,sexual knowledge status questionnaire and sexual health knowledge demand questionnaire were used to carried out the investigation in 214 cases of patients with PTCA plus stent implantation.Collected data entered into a database for statistical analysis.Results The overall score of sexual health of 214 patients was (8.79±2.52) points.There was a statistically significant difference in different education levels,occupation and monthly income.There was no statistically significant difference in different ages.The overall score of knowledge of sexual health demand was (29.40 ± 6.47) points.There was a statistically significant difference in different ages,education levels and occupations.There was no statistically significant difference in different monthly income.Conclusions The sexual knowledge cognitive level of patients with PTCA plus stent implantation is low,but they have higher demand of sexual health knowledge,the medical personnel should strengthen the knowledge of sex education for patients to improve the quality of sex.

Objective:To study the influence of vitamin C ( VC) on dendritic cells ( DC) ,and detect DC maturation,to provide a feasible method and thought for quickly preparating DC vaccines.Methods:Collected the peripheral blood (about 50 ml) from healthy volunteers,and isolated peripheral blood mononuclear cells with lymphocyte separation medium and obtain DC.With stimulating with different concentrations of VC for (24 h),then washed with PBS,and set up blank control group (V0).The expression of DC surface co-stimulating molecules CD80/86 and HLA-DR, CD40 was detected by flow cytometry.By setting the concentration gradient and time gradient, exciting optimal concentration and stimulating time of VC on DC, and analyzed the reasons of VC promoting DC maturation.Results:VC could effectively stimulate DC,CD80/86 expression had significantly increased contrast to the blank control group (V0).And the experiments show that VC’s best stimulating concentration was 1 mmol/L,and on the third day,the CD80/86 expression rate of VC group was (78.6±4.6) %,and blank control group V0 was (34.1±5.7) %.DC surface HLA-DR expression:VC (56.8± 4.4) %,blank control group V0 (25.4 ±4.7) %,the difference between two groups was statistically significant,P<0.05.CD40 and CD40L expression and results show that VC 2.5 mmol/L group of CD40 expression rate up to (59.3±3.7) %,while V0 group was only (11.1 ±2.4) %,that illustrate VC could significantly regulate CD40 expression on DC surface,but CD40L not reflect.Fluorescence mi-croscope results showed that DC’ s antigen catching ability was also significantly promoted.Conclusion:VC can significantly regulate DC maturity,and may up regulate CD40,thus promoting the express of CD80/86 and HLA-DR.When the concentration is 1 mmol/L,VC expresses the strongest regulation function on DC.

Objective To analyze the reproducibility of keratometry and astigmatism measured by the VERION Digital Guidance System and the comparability of VERION with iTrace,Lenstar LS900 and manual keratometer.Methods The keratometry of 62 cataract patients were measured using four different devices.The steep keratometry (Ks),flat keratometry (Kf),astigmatic magnitude,astigmatic axis,cylinder at 0-degree meridian (vector component,J0) and cylinder at 45-degree meridian (vector component,J45) from each machine were recorded and analyzed.The three repeated measurements and the results of VERION system with other three devices were compared to analyze the reproducibility and comparability of VERION system.Results Reproducibility:Intraclass correlation coefficients and Cronbach's alpha values were higher than 0.9 for Ks,Kf,astigmatic magnitude,astigmatic axis,J0 and J45 measured by the VERION system (all P ＜ 0.001).Comparability:The results of Ks and magnitude of astigmatism of VERION were larger than the iTrace (all P ＜ 0.05) in the paired-samples t test.There was no statistical difference for the rest of parameters (all P ＞ 0.05).The Bland-Altman graphs revealed the 95％ limits of agreement (LOA) of J0,J45 and the astigmatic axis between VERION and iTrace were (-0.31-0.35) D,(-0.25-0.31) D and-13.5 °-12.3 °,respectively;There was no statistical differences for all parameters except for J45 in the paired-samples t test between the VERION and Lenstar LS900 (all P ＞ 0.05).The Bland-Altman graphs revealed the 95％ LOA of J0,J45 and the astigmatic axis were (-0.25-0.31)D,(-0.27-0.36) D and-13.5°-11.0°,respectively;There were statistical differences for the results of Kf and magnitude of astigmatism between the VERION and manual keratometer (all P ＜ 0.05).The Bland-Altman graphs revealed the 95％ LOA of J0,J45 and the astigmatic axis between VERION and manual keratometer were (-0.38-0.35) D,(-0.41-0.42) D,-12.6°-16.4°,respectively.Conclusion The VERION system is a reliable system for the measurement of keratometry and astigmatism.The keratometry and astigmatic magnitude of the VERION system have a good agreement with the iTrace,Lenstar LS900 and manual keratometer.However,the astigmatic axis measurements are significantly different among the four devices.

Objective To explore the effect of whole course integrated health education on depressive emotion and self-concept modification in depressive patients and to improve their psychological rehabilitation. Methods Sixties depressive inpatients treated with antidepressive drugs were randomly assigned to receive supportive psychological nursing and general health education (control group 28) or whole course integrated health education (experimental group 32). Hamilton Depression Scale ( HAMD), Tennessee Self-Concept Scale (TSCS) and Symptom Checklist 90(SCL-90) were used before and after intervention to evaluate the clinical effects of two groups. Results Before interventions,the total scores of HAMD,TSCS and SCL-90 were (24.8 ±4. 3) ,(249.68 ±16.23) and(136.39±34.26)in experimental group and (24. 9 ±4. 2) , (248. 74 ± 17. 32) and( 136. 52 ± 35.04) in control group, with no significant difference between two groups (all P ＜ 0.01). After intervention, the total scores of HAMD, TSCS and SCL-90 were (19.7±2.8),(263.55±19.53) and (114.22 ± 18. 75) in experimental group and (22.4 ± 3. 3), (253. 66 ± 18. 37) and (134. 56 ± 32. 64) in control group, with significant difference between two groups (all P＜0.01). Conclusions Whole course integrated health education can improve depressive emotion of patients with depression, develop their positive self-concept and boost their mental health level.

Objective To investigate effect of carnosine on oxygen glucose deprivation/reperfusion ( OGD/RP) induced injury in rat brain slices. Methods Injury of brain slices was determined by TTC methods.The contents of ATP, ADP and AMP were determined by high performance liquid chromatography.Reactive Oxygen species ( ROS) were determined by fluorescence methods.Results Compared with control group, rat hippocampal slices were significantly damaged by OGD/RP, indicated by light color and decreased A490 nm value of TTC staining.Meanwhile the contents of ATP and ADP were significantly decreased, and the content of AMP and ROS were significantly increased, the difference between two group was significant ( P<0.01).Pre-incubation with Carnosine (1000, 200, 40 μg/mL) significantly inhibited the light color and decreased A490 nm value of TTC staining, increased the contents of ATP, ADP and AMP, and decreased the content of ROS, the difference between two group was significant ( P <0.01 ) . Conclusion Carnosine can protect rat hippocampal slices against injury induced by OGD/RP, which may relate to improve the energy metabolism and strengthen the ability of anti-oxidative stress.
