1.Porous tantalum in orthopedic applications:joint replacement and cartilage reconstruction scaffolds

BACKGROUND:Porous tantalum with high porosity, low elastic modulus, high friction coefficient has stable biological characters and good biocompatibility, which is considered as an ideal implant material in orthopedic surgery. OBJECTIVE:To review the application situation of porous tantalum used in artificial joints, spine, bone necrosis, defects of bone and so on. METHODS:A computer-based search of PubMed, Wanfang, and VIP databases was performed for relevant articles published from 1990 to 2014 using the keywords of “porous tantalum, biological character, orthopedic applications” in English and Chinese. RESULTS AND CONCLUSION:Porous tantalum is a kind of good orthopedic implant material, with high porosity, low elastic modulus, high friction coefficient, and good compatibility. Porous tantalum is mostly used in the preparation of integrated acetabular cup, total hip arthroplasty acetabular cup, acetabular reinforcing pad, porous tantalum metal rod, tibial plateau prosthesis, patelar prosthesis and tibial plateau prosthesis, which has achieved good clinical effects.

Objective:To investigate the role of SDF-1/CXCR4 in metastasis of renal cell carcinoma and to observe the intracellular location of different CXCR4 segments in renal carcinoma cells.Methods:The potential nuclear localization sequences of different CXCR4 were discovered by nuclear localization software and experiments.Full length and truncated forms of CXCR4 were fused with green fluorescent protein pEGFP-N1 and their influence on subcellular localization was examined by confocal microscopy after transfecting them into renal carcinoma cell line A498.Results:Analysis with PSORT Ⅱ Prediction revealed that the nuclear localization sequence region of CXCR4 was located between amino acids 146 and 149(RPRK).Expression products of the recombinant plasmids with SDF-1 stimulation,including EGFP-CXCR4(1-510 bp),EGFP-CXCR4(1-765 bp) and wild-type EGFP-CXCR4,were mainly located in the cell nuclei.However,expression product of EGFP-CXCR4(1-267 bp) with SDF-1 stimulation was mainly located in the renal cell cytoplasm.Expression product of wild-type EGFP-CXCR4 full length plasmid without SDF-1 stimulation was mainly located in the cell cytoplasm;these results accorded with the results of bioinformatics analysis.Conclusion:Nuclear localization sequence of CXCR4 is located in the amino acids 90 to 170,which provides a theoretical basis for further clarifying the nuclear localization sequences of CXCR4 in renal cell carcinoma cells and for finding new potential target for inhibiting the metastasis of renal cell carcinoma.

Objective: To analyze the prescription dosage for discharged patients in our hospital and investigate the potential risks.Methods: A retrospective research method was adopted to statistically analyze 55 872 discharge instructions from July to September in 2015,and more attention was paid to the prescriptions with dosage over 4 weeks,expecially the ones with 26-week dosage, and the related influencing factors were studied as well.
5.Effect of different ocular spherical aberration on visual performance after phacoemulsification

AIM: To compare the visual acuity and contrast sensitivity of eyes with different corneal spherical aberration implanted with the same aspherical IOL and evaluate the effect of different ocular spherical aberration on visual performance after phacoemulsification.
METHODS:It was a prospective case series study. Forty-six eyes of thirty-nine age-related cataract patients in our department were included. The patients were divided into 3 groups according to the value of preoperative corneal spherical aberration. Eyes with corneal spherical aberration≤0. 2μm were assigned to group A, those with corneal spherical aberration >0. 2μm and ≤0. 3μm to group B, and those with corneal spherical aberration≥ 0. 3μm to group C. All patients underwent phacoemulcification and recieved AcrySof IQ aspheric IOL. Uncorrected visual acuity ( UCVA ) , best-corrected visual acuity( BCVA) , contrast sensitivity, and total ocular higher - order aberrations for a 6. 0mm pupil were recorded 3mo postoperatively. ANOVA were used to analyze the data.
RESULTS: There were no significant differences in UCVA and BCVA between the 3 groups (P=0. 287, 0. 115). Contrast sensitivity was no statistically significant difference between the 3 groups at any spatial frequency under photopic、 mesopic, and mesopic with glare conditions (P>0. 05). With a 6. 0mm pupil diameter, root mean square values for total ocular higher - order aberrations were lower in groups A and B than that in group C (P=0. 000). The difference of total ocular spherical aberration was statistically significant between the 3 groups (P=0. 000). Coma and trefoil were similar between the groups (P=0. 788,0. 590), with no statistically significant differences.CONCLUSION:Implantation of the same aspherical IOL in eyes with different corneal spherical aberration results in similar visual acuity and contrast sensitivity. Small differences of ocular spherical aberration after phacoemulsification have no effect on visual performance.
6.Construction of recombinant eukaryotic expression plasmid of human thyrotropin receptor extracellular domain encapsulated with cationic liposomes

AlM:To construct recombination eukaryotic expression plasmid of human thyrotropin receptor extracellular domain encapsulated with cationic liposomes.
METHODS:We amplified the target gene of shuttle vector PHMCMVTSHR289, conjugated the target gene and eukaryotic expression plasmid pcDNA3. 1 +, and accredited whether pcDNA3. 1+/TSHR289 was connected or not by enzymatic digestion and sequencing. Cationic liposomes encapsulated the recombination plasmid pcDNA3. 1+/TSHR289.
RESULTS: Recombination plasmid pcDNA3. 1+/TSHR289 digested with enzyme Hindlll and the fragment through 0. 8% gel electrophoresis showed 512bp strip. Recombination plasmid pcDNA3. 1+/TSHR289 were found synonymous mutation through forward ( AAC to AAT ) and reverse sequencing ( GCG to GCT) . The volume ratio of cationic liposomes and recombinant plasmid was 3:1.
CONCLUSlON: lt is successful to construct the recombination plasmid pcDNA3. 1+/TSHR289 by accredit it through enzymatic digestion and sequencing.
7.The value of transperineal prostate biopsy guided by the contrast-enhanced ultrasound and MR diffusion weighted imaging

Objective To explore the value of transperineal prostate biopsy guided by contrast-enhanced ultrasound(CEUS)and MR diffusion weighted imaging (DWI).Methods 128 patients with suspected prostate cancer were analyzed.Both MR diffusion weighted imaging(MRDWI)and contrast-enhanced ultrasound(CEUS) were applied before prostate tissue was biopsied .The suspected lesion was confirmed by specialists of MRI , Ul-trasound and Urological together .The regular six normal sites addition to suspected lesion were biopsied .Biopsy specimens were sent to pathological examination separately with defined puncture site .The sensitivity, specificity and accuracy of MRDWI , CEUS alone and the combination of two methods were analyzed .Results Of the 858 prostatic specimens from 128 patients , 278 specimens were positive , with 172 specimens found by MRDWI and 114 specimens found by CEUS .As for 278 positive specimens , 137 specimens were diagnosed by MRDWI cor-rectly rather than CEUS , and 79 specimens were diagnosed by CEUS correctly rather than MRDWI , 35 specimens were diagnosed by either MRDWI or CEUS and 27 specimens were diagnosed by neither MRDWI nor CEUS .The sensitivity,specificity,accuracy of biopsy guided by MRDWI , CEUS and both of them in diagnosis of prostate cancer were 61.87%,85.00%,77.51%;41.01%,93.97%,76.81% and 90.29%,80.52%,83.68%,respec-tively.The sensitivity and accuracy of biopsy guided by both MRDWI and CEUS were higher than MRDWI or CEUS alone, with statistically significant difference(P<0.05).Conclusions Suspicious lesions may be found more frequently by MRDWI than CEUS , however two methods can complement each other .Sensitivity and accura-cy of biopsy can be improved by the combination of MRDWI and CEUS , which should be recommended in future clinical practice , without increasing the number of needle .
8.Suppression of corneal neovascularization by culture supernatant of human amniotic epithelial cells in vitro

BackgroundCorneal neovascularization (CNV) is a common eye disease.The researches on the pathogenesis and treatment of CNV are focus in Ophthalmology.ObjectiveThis study was to investigate the effects of culture supernatant from human amniotic epithelial cells (AECs) on CNV in vitro and its mechanism.MethodsHuman AECs were obtained from a placenta and cultured in serum-free medium for 48 hours,and the supernatant was collected.The levels of interleukin-1 receptor antagonist (IL-1 Ra) and pigment epithelium-derived factor (PEDF) in the human AECs culture supernatant were detected by enzyme-linked immunosorbent assay (ELISA).Rabbit corneal epithelial cells (CECs) were obtained and cultured in different concentrations of human AECs culture supernatant for 48 hours,serum-free medium was used as the control group.The expression of vascular endothelial growth factor (VEGF) mRNA and basic fibroblast growth factor (bFGF) mRNA in rabbit CECs were measured by quantitative real-time PCR (QRT-PCR).Human umbilical cord vein endothelial cells (UVECs) were cultured in the three mediums above,and the proliferation of human UVECs (absorbance value,A value) was tested by the cell counting kit 8 ( CCK8 ).Migration assay was performed by the wound healing method for the human UVECs.The membrane ultra-structure of human UVECs was examined under the atomic force microscope (AFM).ResultsCultured and passaged human AECs showed a positive response for keratin.The expression of VEGF mRNA (1.00±0.22) and bFGF mRNA (1.00±0.36) in rabbit CECs was suppressed by the human AECs culture supernatant,with a significant reduction in comparison with the serum-free DMEM group (2.98±0.46,2.55±0.48 )(P=0.001,0.002).The A value was significantly lowered in the human AECs culture group for 72 hours compared with the serum-free DMEM group ( 1.941 ± 0.036 versus 2.144 ± 0.059 ) ( P =0.000 ),and the bFGF-induced migration rate of human UVECs was strongly inhibited by the culture supernatant of human AECs in comparison with serum-free DMEM.The plasma membrane of human UVECs cultured with the human AECs culture supernatant was full of bumps,and decreased intercellular connection and cellular pseudopodia were found on the AFM image.The concentration of IL-1Ra was (153.56±0.36)ng/L and that of PEDF was (70.41 ±0.68 )μ,g/L in the human AECs culture supernatant.Nothing was deteched in serum-free DMEM group.Conclusions Human AECs culture supernatant suppressed the expression of VEGF and bFGF in CECs as well as the migration and proliferation of vascular endothelial cells.The effect may be associated with IL-1Ra and PEDF secreted by human AECs.These results suggest that human AECs may be a potential therapy for the inhibition of CNV.
9.Neuropsychological characteristics of selective attention in children with nonverbal learning disabilities.

OBJECTIVETo investigate the neuropsychological characteristics of selective attention such as attention control, working memory and attention persistence of frontal lobe in children with nonverbal learning disabilities (NLD).
METHODSWith Auditory Detection Test (ADT), Wisconsin Card Sorting Test (WCST) and C-WISC, 14 children with NLD and 23 controls were tested and the results of sub-tests of C-WISC were analyzed with factor analysis. ADT was mainly applied to test the ability of auditory discernment and the function of dominance lateralization in the cerebra; WCST was employed to test the function of working memory which was based on the frontal lobe, and, C-WISC, to test the intelligent structure and characteristics.
RESULTSCompared with control group, the correct response rate of ADT in NLD group was much lower (P < 0.01), and the number of incorrect response was much larger (P < 0.01). Children with NLD had deficits of auditory selective attention. Moreover, the number of categories achieved (CA) and perseverative error (PE) of WCST were much lower (P < 0.05), which indicated that children with NLD had the disorders of selective attention and performance function. Factor analysis showed that perceptual organization (PO) related to visual space and freedom from distractibility (FD) related to attention persistence in NLD group were obviously lower than those in control group (P < 0.01, P < 0.05). These findings further supported the above-mentioned results.
CONCLUSIONSChildren with NLD had attention control disorder and working memory disorder mainly in frontal lobe; we suppose that the disorder in right frontal lobe was distinctive.

10.Analysis suspected allergic factors to shenqi fuzheng injection based on prescription sequence analysis and nested case control study.

This study based on twenty 3A grade hospital information system (HIS) database in China, adopt toprescription sequence analysis (PSA) with nested case control study (NCCS) to analysis mainly suspected allergy factor of Shenqi Fuzheng injection (Shenqi Fuzheng). Study design according to start to stop using Shenqi Fuzheng whether using dexamethasone injection will crowd divided into cases group and control group, each case matched 4 controls were selected, two groups according to the ratio of the age and sex matched well. Square test, Fisher exact test, single factor and multiple factor logistic regression were used to analyze data Condition on admission, allergic history, dosage and drug combinations were taken into account in cases of suspected allergic reactions. After analysis in two subgroups we found that the single dose (P = 0.000 2) and the combined use of matrine (P < 0.000 1, OR = 14.312, confidence interval [8.184, 25.029]) had significant effects on the suspected allergic reaction. Study on the existing HIS data and the study method based on screening suspected risk factors for allergic reaction. This study can provide guidance for Shenqi Fuzheng injection safety using in clinical practice, and it can also provides new method for the clinical safety reevaluation of post-marketing Chinese medicine injection.
