Objective To describe the epidemic status of hepatitis B along the Yangzi River area,ill Anhui province.Methods A croSS-sectional seroepidemiological survey was conducted and the samples were collected by stratified cluster sampling.Serological biomarkers to hepatitis B virus were tested using enzyme-linked immunosorbent assay(ELISA)reagents.Results Among 2282 people.the prevalence rates of HBsAg,HBsAb,HBeAg.HBeAb,HBeAb and hepatitis B virus infection were 9.8%,43.5%.1.0%,7.8%,10.3%and 40.6%respectively.The prevalence rate of HBsAg among males was higllel"than that offemales(P<0.05).The vaccination rate was 24.7%,higher in urban than in taral areas.The vaccination rate was high in children younger than ten years old and in students.The prevalence rates of HBsAg and HBV among people who had received vaccines were lower than those who had not.Conclusion The standardized prevalence rates of HBsAg(9.3%)Was high in the area along the Yangzi River in Althai province.The vaccinate rate Was low in the country side.Expanded vaccinate which can obviously reduce the prevalence rate of HBsAg should be enhanced.
3.A meta-analysis of holmium laser enucleation of the prostate for benign prostatic hyperplasia.

OBJECTIVETo evaluate the effectiveness and safety of holmium laser enucleation of the prostate (HoLEP) and transurethral resection of the prostate (TURP)/open prostatectomy (OP) in the treatment of bladder outlet obstruction (BOO) secondary to benign prostatic hyperplasia (BPH).
METHODSWe searched Medline, Cochrane Library, Embase, Wanfang and CBM for randomized controlled trials (RCT) comparing HoLEP with TURP/OP. Comparable data were extracted from eligible studies and pooled for meta-analysis using RevMan5.1.
RESULTSNine RCTs were included in this study, 6 comparing HoLEP with TURP, and the other 3 comparing HoLEP with OP. Meta-analysis showed that, compared with TURP, HoLEP was associated with shorter hospital stay and catheterization time, less hemoglobin loss, longer operative time, and better improvement in international prostate symptom score (IPSS) , peak urinary flow rate (Qmax) and post void residual (PVR) , but the incidences of postoperative urethral stricture and urinary incontinence had no statistically significant difference between the two. Compared with OP, HoLEP showed shorter hospital stay and catheterization time, a lower rate of blood transfusion, longer operative time, and removal of fewer tissues, but the two procedures exhibited no significant differences in either the improvement of IPSS and Qmax or the incidence of urethral stricture.
CONCLUSIONHoLEP is a minimally invasive technique, safe and highly effective for the treatment of BOO secondary to BPH, with its advantages of lower peri-operative morbidity and faster recovery over TURP and OP. However, more high-quality RCTs with larger sample sizes and longer follow-ups need to be carried out to obtain better evidence.

4.Analysis of the infection status and the drug resistance of mycoplasma and chlamydiae in genitourinary tracts of children with suspected nongonococcal urethritis.

OBJECTIVETo investigate the infection and the drug resistance status of mycoplasma and chlamydiae in genitourinary tracts of children with suspected nongonococcal urethritis (NGU) and provide information for clinical rational administration of antimicrobial agents.
METHODSSamples of genitourinary tract secretion from 146 children who were suspected of having nongonococcal urethritis or colpitis were collected and tested for mycoplasma via culture and for chlamydia with antigen detection. Meanwhile, susceptibility test was carried out on the samples which were positive in mycoplasma cultivation. Chlamydia antigen was detected by the polymer conjugate-enhanced (PCE) indirect enzyme immunoassay (EIA) (IDEIA PCE Chlamydia; DAKO). The mycoplasma culture medium was produced by Nanjing Liming Biological Products Co,. Ltd. Antibiotics used for susceptibility test were erythromycin, roxithromycin, josamycin, leucomycin, meleumycin, rovamycin, azithromycin, clarithromycin, cycloate erythromycin, and clindamycin.
RESULTSFifteen samples were positive for Chlamydia trachomatis (Ct) by antigen detection (10.3%), 82 samples were positive in mycoplasma cultivation (56.2%), and among the 82 samples, 58 were positive for Ureaplasma urealyticum (Uu, 39.7%), 9 were positive for Mycoplasma hominis (Mh, 6.2%), and 15 were positive for Uu and Mh (10.3%). Of all the samples, 4 were positive for both Uu and Ct (2.7%). The rates of drug resistance of the 10 commonly used antibiotics were as follows: erythromycin 32.9%, roxithromycin 41.5%, josamycin 19.5%, leucomycin 22.0%, meleumycin 28.0%, rovamycin 30.5%, azithromycin 37.8%, clarithromycin 26.8%, davercin 24.4%, and clindamycin 26.8%, respectively. The results indicated that drug resistance rates of josamycin and leucomycin were the lowest, and the rates of roxithromycin and azithromycin were the highest.
CONCLUSIONSThe infection rates of mycoplasma and chlamydia in children suspected NGU were high. Mycoplasma showed drug resistance to a different degree to 10 common antibiotics. The results of chemosensitivity showed that josamycin had the highest susceptibility rate.

OBJECTIVETo analyse the clinical and angiographic characteristics of spontaneous reperfusion (SR) in AMI, and to evaluate its effect on short-term prognosis.
METHODS112 consecutive AMI patients without intravenous thrombolytic therapy received emergent coronary angiography and primary PCI. The patients were divided into SR group (antegrade TIMI grade 2-3 flow) and non-SR group (antegrade TIMI grade 0-1 flow). The clinical, angiographic and prognostic features of the patients were analyzed.
RESULTS31 patients (27.7%) were in SR group, and there was no significant difference in base-line clinical characteristics between the two groups. Compared with non-SR group, peak values of CK and CK-MB, Ventricular wall motion abnormality and mortality were lower in SR group, ejection fraction was higher in SR group. Logistic regression analysis showed that there was good correlation between SR and peak value of CK, collaterals, ventricular wall motion abnormality and pre-dilation in PCI.
CONCLUSIONSR decreased infarction size, improved heart function and reduced 30-day mortality.

6.Sufentanil reduces emergence agitation in children receiving sevoflurane anesthesia for adenotonsillectomy compared with fentanyl.

BACKGROUNDEmergence agitation is a common problem in pediatric anesthesia, especially after sevoflurane induction and maintenance anesthesia. The purpose of this study was to investigate the effect of sufentanil to reduce emergence agitation after sevoflurane anesthesia in children undergoing adenotonsillectomy compared with fentanyl.
METHODSOne hundred and five children, aged 3 - 11 years, were randomly allocated to receive normal saline (control group), sufentanil 0.2 µg/kg (S2) or fentanyl 2 µg/kg (F2) 1 minute after loss of the eyelash reflex. Anesthesia was induced and maintained with sevoflurane. Time to tracheal extubation, recovery time, Paediatric Anesthesia Emergence Delirium (PAED) scale, and emergence behavior were assessed.
RESULTSThe incidence of severe agitation was significantly lower in S2 and F2 groups vs. the control group, 4/32 and 15/34 vs. 24/34 respectively, (P = 0.002, 0.009, respectively). PAED scales were significantly different among three groups (P = 0.007), and lower in the S2 and F2 groups than in the control group (P = 0.007 and P = 0.025, respectively). And the incidence of severe agitation and the PAED scale score was significantly different between the S2 and F2 groups (P = 0.007, P = 0.019, respectively). Time to tracheal extubation and recovery time were similar in all three groups.
CONCLUSIONSAdministration of sufentanil at 0.2 µg/kg after induction of anesthesia reduced emergence agitation in children receiving sevoflurane anesthesia for adenotonsillectomy compared with fentanyl. This was without delaying the recovery time or causing significant hypotension.

7.Application of 3D Printing Combined with S-ROM Total Hip Arthroplasty in the Treatment of Adult Crowe Ⅳ DDH

Objective To investigate the application of 3D printing combined with S-ROM total hip arthroplasty in the treatment of adult Crowe IV DDH. Methods Between September 2014 and June 2017, 10 patients of adult Crowe IV DDH were selected and divided into 3D group (5 cases) and non-3D group (5 cases) . There were 4 males and 6 females with an average age of 43.6 years (range,27-63years) . Harris scores were recorded in both groups before the operation, and in the group 3D, patients underwent spiral CT scanning to make the hip 3D models. According to the hip 3D models, the sizes of prosthesis and operation schemeswere estimated before the operation ,and the surgery simulation was performed. Then the S-ROM prosthesis was used for total hip arthroplasty. The actual situation of hip joint, the size of prosthesis,time of operation and blood loss were recorded.Harris Score at 1 week post-operationwere recorded again. All patients were followed up related postoperative complications for at least 2 weeks.Results The 3D printing models coordinated with the actual sizes of the hips in the3D group.The time of operation and blood loss were less than the non-3D group. Higher Harris score was observed at 1 week post-operation in the 3D group,compared to non-3D printing group.The follow-up showed that the motion of the hips was improved obviously. Conclusion The Application of 3D Printing combined with S-ROM total hip arthroplasty in the treatment of adult crowe IV DDH can improve efficiency and accuracy of the operation, shorten time of operation , reduce operative blood loss and rate of complications.
8.Epidemiological Investigation on Children and Teenagers with Congenital Heart Disease in three Counties in Zhaotong Area from 2015 to 2016

Objective To make epidemiological investigation on children and teenagers with congenital heart disease (CHD) from October 2015 to November 2016 in three counties in Zhaotong area. Methods The examination was divided into the following two phases: primary screening, secondary screening. Standardized echocardiographic examination was performed for the students with suspected CHD. we combined comprehensive analysis of electrocardiogram, X-ray and cardiac catheterization to make a clear diagnosis for patients. The index of prevalence rate of CHD, the types of the disease, and gender of the patients in 14957 children and teenagers were statistically analyzed. Results Altogether 61 cases of CHD were discovered. The total prevalence of CHD was 4.08‰ (61 of 14957 cases), the prevalence rate in male was 4.55‰, in female was 3.61‰, No difference of the prevalence rate was found between male and female (χ2=0.823, P>0.05);the difference among different age groups had no statistical significance (χ2=4.692, P>0.05);The prevalence rate of CHD in Qiaojia was 5.17‰, in Daguan was 6.36‰, and in Zhenxiong was 3.03‰, There was no significant difference in the prevalence rate among different countries (χ2=5.897, P>0.05). In terms of the types of the disease, atrial septal defect accounted for the first (37.7% ), followed by ventricular septal defect (26.2% ) and patent ductus arteriosus (19.7% ), others types of CHD were rare. Conclusion The prevalence of CHD in investigated area is slightly higher than the average level of the nationwide. The main types of CHD are atrial septal defect, ventricular septal defect and patent ductus arteriosus.

10.Establishment of minimum medical geographic information systems database in China.

OBJECTIVETo establish a minimum medical geographic information systems (GIS) database as a spatial decision supporting system (SDSS), and to use the database into public health practice in China.
METHODSSpatial data collected from different sources were standardized as decimal degree format, including: (1) satellite images covering areas of China; (2) digital maps of China in vector files; (3) diseases database and relevant models.
RESULTSNecessary satellite images for the database have been collected from NOAA AVHRR, Landsat TM, etc., including the normalized difference vegetation index (NDVI) images from AVHRR, earth surface temperature images from AVHRR, GTOPO30 DEM images from USGS and landuse images from USGS. The digital vector files for GIS analysis were collected including political (county, provinces, country) boundaries file, environmental (drainage, land cover, soil type) vector file, population data and climate data; Data on diseases mainly generated from survey or case reporting. Relevant models on transmission of Schistosoma japonicum and Plasmodium vivax, and models of Oncomelania hupensis and Anophores sinansis were developed, and the relevant environmental factors related to incidence of cancers were mapped, to test and verify those database.
CONCLUSIONThe database unified the data from different sources for users. Minimum medical data included in the database could be used in the practice of public health. It is expected that this database be used in a wider range.
