1.Effects of Dopamine and Dobutamine on Endocrine of Premature Infants with Hypotension

Objective To explore the effects of dopamine and dobutamine on thyroid stimulating hormone(TSH),total thyroxine(T4) and growth hormone(GH) in preterm infants with hypotension.Methods Forty-seven preterm infants with hypotension were enrolled in the stu-dy,and 38 preterm infants remained hypotensive after two dose of isotonic saline solution,who were divided into two groups and assigned to receive dopamine or dobutamine.Drug infusion were all started at 4 ?g /(kg?min) and the dose increased by 2 ?g /(kg?min) in a stepwise fashion until either the mean blood pressure normalized or a maximal dose 20 ?g /(kg?min) was reached.Haemodynamic variables and se-rum levels of TSH,T4,and GH were assessed during the first 72 h of treatment and the first 3 d after stopping treatment.Microplate luminometer was used to test TSH,T4,GH levels.Results Necessary cumulative and mean drug doses and maximum infusion required to normalize blood pressure were significantly higher in the dobutamine group than those in the dopamine group(Pa0.05].Conclusions Dopamine and dobutamine both increase the syste-mic blood pressure,with higher drug doses in dobutamine group.Dopamine reduces levels of serum TSH,T4 and GH in preterm infants but such suppression is quickly reversed after treatment is stopped.
2.Causes and treatment of gastrointestinal complications following anterior approach to thoracolumbar fracture and dislocation

Objective To explore the causes and treatment of gastrointestinal complications after anterior approach for thoracolumbar fracture and dislocation.Methods A retrospective analysis was carried out in 153 cases with thoracolumbar fracture and dislocation undergone anterior approach from Jan- uary 1999 to October 2003.Postoperative gastrointestinal complication was seen in 15 cases including sev- en with dynamic intestinal obstructions,three with stress ulcer,three with intestinal bacteria imbalance, one with tuberculosis peritonitis resulted from dissemination of primary tuberculosis,and one with acute relapse of chronic appendicitis.Results All patients were effectively cured by using corresponding methods in regard of different causes.Conclusions(1)Gastrointestinal complications following ante- rior approach for thoracolumbar fracture and dislocation are mainly resulted from following causes,ie,se- rious primary trauma,primary gastrointestinal vegetative nerve injury or that caused by surgical operation, intraoperative contusions of abdominal viscera,postoperative retroperitoneal hematoma,acute lesion of gastric mucous membrane as well as imbalance of intestinal flora.(2)Correct treatment of primary trau- ma,meticulous operation,reasonable utility of antibiotics and appropriate management can effectively prevent and control gastrointestinal complications.
3.Influence of Different Positive End Expiratory Pressure Level on Ventilative Efficiency for Premature Infant with Respiratory Distress

BACKGROUND:At present,new drug and dosage form to treat diabetes mellitus has emerged continually,but it has toxic reaction,side effects,and contraindication or unindication.Especially that it cannot prevent the natural proceeding of diabetes mellitus from aggravation.Based on theory and experience of traditional Chinese medicine,and combining modern methods of pharmacology and pharmaceutical chemistry,it has become one of the hotspots in researches on diabetes mellitus at present to investigate and empolder preparation of pure traditional Chinese medicine that has good effect and little toxicity.OBJECTIVE:To investigate the pharmacological effect of Chinese compound of Qiangyi Jiangtang capsules on diabetic rat models induced by high-fat forage and streptozotocin.DESIGN:A comparative observation of animal trial.SETTING:Staff Room of Pharmacology,Yunnan College of Traditional Chinese Medicine.MATERIALS:Totally 100 2-month-old Wistar rats of common degree,weighing(180±25)g were used.There were 50 males and 50 females,14 rats were randomly selected as the blank control group,and the other 84 rats were used in model establishment.The treatments of the animals in the experiment were accorded with the ethical standard.METHODS:The experiment was carried out in the Staff Room of Pharmacology,Yunnan College of Traditional Chinese Medicine from March to April in 2004.Diabetic models were induced by high-fat forage and streptozotocin.The successful model rats (n=54) were randomly divided into the model group,positive control group and the high,middle and low dosage of Qiangyi Jiangtang capsules groups.The dosages in the low,middle and high dosages treated groups were 0.4,1.2 and 2.0 g/kg respectively. The rats in the blank control group and model group were given isovolume of saline,and those in the positive control group were administrated intragastrically with metformin (0.5 g/kg).After administration,blood glucose,glycosylated hemoglobin and serum fructosamine were determined at 1 month,and the amounts of water drinking and food intake were determined at the 2nd and 4th weeks.MAIN OUTCOME MEASURES:Results of blood glucose,glycosylated hemoglobin and serum fructosamine;amounts of water drinking and food intake.RESULTS:Totally 100 Wistar rats were used,32 of them were excluded due to failure in the model establishment,and the other 68 were involved in the final analysis of results.The blood glucose,glycosylated hemoglobin and serum fructosamine in the high,middle and low dosages of Qiangyi Jiangtang capsules groups were all significantly lower than those in the model group(P<0.01).The amounts of water drinking at the 2nd and 4th weeks in the high,middle and low dosages of Qiangyi Jiangtang capsules groups were significantly lower than those in the model group (P<0.01),and the amounts of food intake were also lower than those in the model group (P<0.05 or 0.01).CONCLUSION: Qiangyi Jiangtang capsules could remarkably decrease the levels of blood glucose, glycosylated hemoglobin and serum fructosamine,and the amounts of water drinking and food intake of rat models of diabetes mellitus.It was proved that traditional Chinese medicine for diabetes mellitus is worthy of further development and research.

6.Effects of Sancaoanqian Decoction on Expression of IL-1?, IL-6 and TNF RⅡ in Experimental Autoimmune Prostatitis Rats

Objective To observe the effects of Sancaoanqian decoction on expression of cytokine IL-1?, IL-6 and TNF RⅡ in experimental autoimmune prostatitis rats, and explore the target protein of cytokine of Chinese herbal compound in treating chronic nonbacterial prostatitis. Methods Seventy-two male Wistar rats were divided randomly into six groups and 12 rats in each group. All groups were made experimental autoimmune prostatitis rat model by injecting SC purified prostate protein with FCA except normal group. The normal and model group were given saline, western medicine group was given indomethacin, large, medium and small doses decoction groups were treated with large, medium and small doses of Sancaoanqian decoction. All rats were sacrificed after 30 days. The mRNA and protein expression of cytokines were tested by methods of immunohistochemistry and fluorescent quantitation RT-PCR. Results Sancaoanqian decoction could reduce the mRNA and protein expression of IL-1?, IL-6 in experimental autoimmune prostatitis rats. There were no significant difference between large and medium doses decoction group, but had a better activity than small dose. The expression of TNF RⅡ were reduced in all doses groups and no difference among three doses of Sancaoanqian decoction. Conclusion Sancaoanqian decoction can regulate the immune function in chronic prostatitis rat model. Down-regulation of cytokines maybe one of important mechanisms in treating chronic prostatitis.

Objective To study the correlation between benign prostatic hyperplasia (BPH) and vesicoureteral reflux (VUR). Methods Intravenous radionuclide cystography (IVRC) was performed on 88 BPH patients and micturition cystourethrography on 30 to find if there was any VUR and the degree of reflux. Results 31 patients had VUR of which there were 6 grade Ⅰ,7 grade Ⅱ,6 grade Ⅲ,6 grade Ⅳ and 6 grade Ⅴ.9 were on the right,6 on the left and 16 bilateral. Conclusions VUR do occur in some BPH patients and IVRC is the better method for its diagnosis and evaluation.
9.Count of Amnionic Fluid Lamellar Body to Predict the Maturity of Fetal Lung

Objective To discuss the value of amnionic fluid lamellar body count(LBC) as a predictor of fetal lung maturity. Methods Amnionic fluid was obtained during the cesarean section of 41 normal term pregnancies. LBC was performed on each specimen, and lecithin-sphingomyelin(L/S) ratio was detected simultaneously. Results (1)Amnionic fluid LBC was (86?43)?10 3 (72.5?10 3~99.4?10 3) in normal term pregnancy. (2) Positive correlation was shown between the amnionic fluid LBC and L/S ratio(r=0.66,P=0.001). Conclusion The LBC is a rapid and accurate method to determine the fetal lung maturity.
10.Establishment of the isolated perfused rat liver model and its modification

Objective To set up the isolated perfused rat liver model. Methods Rat livers were harvested after the cannulation of the portal vein and bile duct. The reperfusion solution was Krebs-Henseleit solution containing bovine albumin serum and sodium taurocholate. The original CZ-1 isolated perfused rat liver system contained two subsystems: recirculating perfusion system and heat-exchange system. Then we modified the original CZ-1 system and omitted the heat-exchange system.The modified CZ-1 system consisted of a thermostatically regulated water bath,a peristaltic pump,a 4-neck round-bottom flask, a flow meter, an in-line manometer, a glass organ chamber, an iron support and a set of recirculating pipe line. Then the livers were connected via the portal vein to the modified CZ-1 system for 120 min. After 120 min reperfusion, bile production was evaluated. Routine HE staining and electron microscopic examination of hepatic tissues were also performed. Results The was not significantly different from that reported by references. Hepatic tissues in reperfusion group were also morphologically normal Conclusion The CZ-1 isolated perfused rat liver system was cost-effective and reliable to use. It was easy to run and is the ideal model for investigation of organ preservation solution.
