1.Study on preparation and in vitro release characteristics of Bitongning Patch

AIM: To prepare Bitongning Patch(Flos Daturae, Moschus and Camphora, etc.) and study its in vitro release characteristics. METHODS: Bitongning Patch was prepared with high polymer hydrophilic material as matrix. The determination method of scopolamine was established by HPCE. The in vitro release was studied according to the requirement of Chinese Pharmacopoeia(2000) and the percutaneous absorption of Bitongning Patch was evaluated in use of the Franz diffusion cell. RESULTS: The release mechanism of Bitongning Patch in vitro coincided with Higuchi Equation. The release rate was 0.4698mg?cm -2?h -1/2. Scopolamine in Patch permeated in vitro at the speed of 0.067mg?cm -2?h -1 by zero-order kinetics. CONCLUSIONS: Bitongning Patch is a skeleton controlled TDDS whose permeation speed is limited by skin.
2.Optimization for formulation matrix proportion of Bitongning Patch by uniform design

Objective To optimize the prescription matrix proportion of Bitongning Patch and observe its percutaneous absorption in vitro. Methods U6(66) Uniform design was performed to investigate the influence of scopolamine and aconitine in the proportion for 24 h cumulative permeation in vitro. The drug permeation behavior through excised mouse skin was studied with improved Franz diffusion cell. Results With the matrix ratio for PVA: 11.02%, PVP: 29.93%, glyceroh 7.42%, azone: 4.95%, and propanediol: 11.06%, respectively, the Patch had the maximal cumulative permeation amount and cumulative permeation curve coincided with the Higuchi Equation. Conclusion The prescription composition optimized by uniform design meets the requirement of skeleton-controlled transdermal delivery system.
3.Evaluation of the left ventricular longitudinal strain in patients with myocardial ischemia by two-dimensional speckle tracking imaging

Objective To assess left ventricular(LV) longitudinal strain in patients with coronary heart disease by two-dimensional speckle tracking imaging (2DSTI),and to explore the clinical value of 2D longitudinal strain in detecting myocardial ischemia. Methods Forty-four patients with coronary heart disease (CHD group) and 28 age-matched subjects (control group) were enrolled into this study. The two-dimensional data were obtained in apical 4-chamble, 2-chamber and long axis view. And the longitudinal strains of every segments, the average longitudinal strain of LV 18 segments (SL18), the average longitudinal strain of 12 segments (SL12,excluded the 6 apical segments) were analyzed. Results In the patients with CHD, the longitudinal strain of ischemia segments and the global LV longitudinal strain were significantly decreased than that of the control subjects. Both in patients with CHD and in control subjects,the longitudinal strains in apical segments were higher than that of middle and basal segments. There was significant difference between SL18 and SL12 ( P=0.027 in CHD group and P =0.003 in control group).Receiver operating curve (ROC) analysis demonstrated that the cutoff point of SL18 to detect myocardial ischemia was - 18.8% (sensitivity 80.2% and specificity 74.1% ) ,and the cutoff point of SL12 to detect myocardial ischemia was - 17.8% ( sensitivity 81.7% and specificity 85.6% ). Conclusions 2D longitudinal strain was sensitive to detect myocardial ischemia, SL12 was better than SL18 in detecting myocardial ischemia. 2DSTI might be useful for identifying patients with severe CHD.
4.Assessment of left ventricular function and infarct size of acute myocardial infarction by automated function imaging

Objective To assess the left ventricular function of patients with acute myocardial infarction (AMI) and its correlation with cardiac troponin T (cTnT) and specific manifestation of electrocardiogram (ECG) by automated function imaging (AFI) of two-dimensional speckle tracking imaging.Methods Forty-six AMI patients who had AMI for the first time and had been treated by primary percutaneous coronary intervention and 30 healthy controls who were age and sex-related to infarct group were involved.The values of cTnT within 24 hours after admission of AMI patients were recorded and the values of ST segment elevation were measured accurately.All the subjects were analyzed for longitudinal peak systolic strain (LPSS) values and the bull' s eyes by AFI.Results Compared to control group, left ventricular ejection fraction (LVEF), global and infarcted LPSS of infarct group were significantly different and the values of ST elevation of infarct group were higher than those of control group.Both global,infarcted segmental LPSS were significant closely correlated to LVEF and cTnT,respectively (P<0.001,all).Both global, infarcted segmental LPSS were correlated to ST elevation (P <0.05,all).Global LPSS had the closest correlation with LVEF (r = -0.565, P<0.001) and so did infarcted LPSS with cTnT (r = 0.432, P <0.01).Conclusions As a procedural simple and rapid diagnostic tool,AFI provides reliable and useful information of the assessment of AMI.Both global and infarcted segmental LPSS have well described left ventricular function of AMI patients.Compared to LVEF, LPSS was more closely correlated to cTnT and ST segment elevation, which meant that LPSS was more sensitive and more closely related to real infarct size and actual involved range of AMI.
5.Assessment of the correlation between time during admission to percutaneous coronary intervention and left ventricular function recovery of acute myocardial infarction by two-dimensional speckle tracking imaging

Objective To assess the correlation between time during admission to percutaneous coronary intervention (PCI) and left ventricular function recovery of acute myocardial infarction (AMI) by two-dimensional speckle tracking imaging (STI).The clinical value of STI in assessing therapeutic effect of AMI treated by PCI and estimation of the prognosis were discussed.Methods Sixty-one AMI patients who had AMI for the first time and had been treated by primary PCI were enrolled.Dynamic images were acquired before PCI,at 7 days after PCI and 30 days after PCI and analyzed by STI.The time during admission to PCI of AMI patients was recorded accurately.Dynamic images were analyzed for longitudinal peak systolic strain (LPSS) values (global,infarcted area) by STI.According to the comparison of left ventricular ejection fraction(LVEF) before PCI and 30 days after PCI,patients were divided into left ventricular function improved group (ΔLVEF≥5 %) and not-improved group,and the values of LPSS and time during admission to PCI were compared between the two group respectively.Results Compared to not-improved group,the time during admission to PCI in improved group was lower ( P <0.001),infarcted segmental LPSS at 7 days after PCI ( P <0.05) and both global ( P <0.001) and infarcted segmental LPSS ( P <0.001) at 30 days after PCI in improved group were higher than those in not-improved group.Linear regression analysis showed that both global and infarcted segmental LPSS were significant correlated to LVEF respectively ( P <0.001,all).Infarcted segmental LPSS at 7 days after PCI were correlated to the time during admission to PCI ( P <0.05).LVEF ( r =0.303,P <0.05),global ( r =0.300,P <0.05)and infarcted segmental LPSS ( r =0.590,P <0.001) at 30 days after PCI were correlated to the time during admission to PCI.Conclusions STI provides reliable and useful clinical information for the assessment of therapeutic effect of AMI treated by PCI and estimation of the prognosis by sensitively presenting the close correlation between time during admission to PCI and left ventricular function recovery of AMI patients.
6.Relationship Between Two Ambulatory Arterial Stiffness Indexes and Early Renal Impairment in Patients of Essential Hypertension

Objective To investigate the relationship between the different ambulatory arterial stiffness index and the markers of renal impairment in order to provide a scientific method for detecting the renal impairment of essential hypertension. Methods Three hundred essential hypertensive patients without overt proteinuria were enrolled. The ABPM was performed and the blood pressure parameters were analyzed in order to estimate the symmetrical ambulatory arterial stiffness index (S-AASI) and ambulatory arterial stiffness index (AASI) . Microproteinuria was measured by urine microalbumin to creatinine (mAlb/Cr)as well as n-acetyl-β-D-glucosaminidase (NAG)to creatinine rate (NAG/Cr). Creatinine clearance (Ccr) and Glomerular filtration rate (eGFR) were estimated from serum creatinine (sCr) . Linear correlations were performed to confirm the independent predictive power of S-AASI and AASI for renal lesion. Results Correlation test showed a significant positively relationship of S-AASI with urine mAlb/Cr (0.708, <0.001), urine NAG/Cr (0.700, <0.001) and sCr (0.229, <0.05) . Ccr (0.601, <0.001) and eGFR (0.309, <0.05) were negatively correlated with S-AASI. On the other hand, AASI was also correlated with urine mAlb/Cr (0.489, <0.001),urine NAG/Cr (0.470, <0.001) and Ccr (0.311, <0.05),but not with the sCr (0.064, >0.05) and eGFR (-0.135, >0.05) . S-AASI seems to get an independent relationship with all of the parameters of renal impairment which could not be detected with AASI. Conclusion This results suggested that S-AASI may be a better approach than AASI to estimate hypertensive renal impairment.
7.Pre-treatment with melatonin inhibits oleic acid-induced acute lung injury in rats

AIM:To assess the protective role of melatonin(MEL)in a rat model of oleic-induced acute lung injury.METHODS:Twenty-four rats were randomly allocated to three groups as follows:saline(NS)injection group,oleic acid(OA)injection group and MEL plus OA injection group,the lavage protein,lung wet-to-dry weight ratio,malondialdehyde(MDA)content,superoxide dismutase(SOD)activity and lung histopathology were examined.RESULTS:(1)Injection 0.15 mL/kg of OA led to a severe acute lung injury(ALI),characterized by significantly increasing in lavage protein,lung coefficient(P<0.01),and by histopathological alterations which presented hemorrhage,edema.thickened alveolar septum and the existence of inflammatory cells in alveolar spaces;(2)Infusion of MEL(20 mg/kg,intraperitoneally for 60 min before the oleic acid)markedly alleviated above-mentioned symptom induced by OA,consistent with decrease of MDA level(P<0.01) and the increase of SOD activty(P<0.01).CONCLUSION:Pre-treatment with MEL can attenuate the OA -induced ALI in rats via cleaning and preventing the formation of free radicals and further lessening the increase of alveolocapillary membrane permeability,these data suggest that MEL may be effective in the prevention of ALI.
8.Analvsis of survev results of human brucellosis in Oinghai province from 2006 to 2010

Objective To analyze the epidemiological features and influencing factors of human brucellosis in Qinghai province,and to provide scientific basis for prevention and control of brucellosis.Methods From 2006 to 2010,select the high incidence areas of brucellosis in Qinghai province and five counties(Henan,Dari,Tianjun,Ping'an and Haiyan counties) included in the “Central Subsidies to Local Public Health Special Fund Human Brucellosis Prevention and Control Projects” for the survey point,as well as high-risk employees from Qinghai Biological Pharmaceutical Factory were investigated.Combined with epidemiological questionnaire investigation [done according to the “National Human Brucellosis Surveillance Program(Trial)”],clinical symptoms and signs,confirmed human brucellosis patient were tested by intradermal allergy test,rose bengal plate agglutination test(RBPT) and standard tube agglutination test (SAT),in accordance with “Diagnostic Criteria and Principles of Management of Brucellosis” (GB 15988-1995) and“Diagnostic Criteria for Brucellosis” (WS 269-2007).Results Of 8368 serum samples detected,347 were RBPT positive,and the positive rate was 4.15%;5346 serum samples were tested by SAT,180 were positive,and the positive rate was 3.37%.In June 2009,112 employees in Qinghai Biological Pharmaceutical Factory were investigated on a follow-up survey,83 were RBPT positive,the positive rate was 74.11%; 58 were SAT positive,the positive rate was 51.79%.Eight of them were new cases and 4 were chronic brucellosis.Twenty five new cases were reported between 2006 and 2010.The peak incidence was from March to July.Most of the cases were herdsmen.ConclusionStrengthening animal quarantine,strengthening public education,and improving protection awareness,can effectively control the disease brucellosis.
9.Change of Space Anterior to the Right Portal Vein in Liver Fibrosis and Cirrhosis:CT Analysis

Purpose To explore the relationship between the change of space anterior to right portal vein and the pathological staging in liver ifbrosis/cirrhosis. Materials and Methods Plain and contrast enhanced CT scan were performed in patients with biopsy proven liver ifbrosis/cirrhosis including S1 in 17 patients, S2 in 13 patients, S3 in 15 patients, S4 in 21 patients and cirrhosis in 22 patients. Twenty subjects were included as control group. The width of anterior space of right portal vein was measured on contrast enhanced CT and correlated with ifbrosis staging. The receiver operating characteristic curve was created for cirrhosis diagnosis. Results The width of anterior space of right portal vein enlarged in patients with S3 ifbrosis to cirrhosis (P<0.05 or P<0.01). It was signiifcantly bigger in group S4 compared to other groups (P<0.01). Spearman rank correlation analysis showed significant positive correlation between the width of anterior space and liver fibrosis staging (r=0.704, P<0.01). ROC curve analysis showed the area under curve (AUC) of 0.897 with the optimum width of ≥10 mm. Conclusion The change in the space anterior to the right portal vein is positively correlated with live ifbrosis staging. CT measurement helps early diagnose and assess the severity of liver ifbrosis and cirrhosis.
10.Design of a controlling switch set based on RS-232.

As a common computer keyboard and a mouse can't meet the requirements of some special occasions, this paper introduces the design of a controlling switch set based on RS-232 for capturing the image at the medical image workstation. Its working principles, circuit, assembly together with its associated software controller are given in detail. The control switch set features easy manufacturing, low cost and a good reliability.
