1.Effects of pioglitazone on serum adipocytokines in polycystic ovary syndrome patients with insulin resistance

Objective To investigate the effects of pioglitazone on serum adipocytokines (leptin, resistin and adiponectin) in polycystic ovary syndrome (PCOS) patients with insulin resistance (IR). Methods Thirty-five PCOS patients with IR were treated with pioglitazone 15mg/d for 12 weeks. The results of ovulation induction were observed. The changes of fasting plasma glucose (FPG), fasting serum insulin (FINS), serum levels of leptin, resistin and adiponectin, follicle-stimulating hormone (FSH), luteinizing hormone (LH), testosterone (T), and blood fat were examined at the baseline and after the therapy by enzyme-linked immunosorbentassay (ELISA) and radioimmunoassay (RIA). Results After 12 weeks' treatment, menstruation and rate of ovulation per cycle were improved in 35 (88.5%) PCOS patients with IR. Waist/hip ratio and F-G score were significantly decreased (P<0.05), and BMI declined with no significant difference (P>0.05). The levels of LH, T, FINS, HOMA-IR, total cholesterol, triglyceride and low density lipoprotein-cholesterol were significantly decreased (P<0.01, P<0.05) after treatment; high density lipoprotein-cholesterol level was significantly increased (P<0.01) after treatment. However, there were no significant differences in FSH and FPG (P>0.05). The levels of serum leptin and resistin were decrease than before (P<0.05), and the level of serum adiponectin was increased (P<0.05). Conclusion Pioglitazone can effectively improve clinical syndromes, insulin sensitivity, glucose and lipid metabolism of PCOS patients with IR. Adipocytokines (leptin, adiponectin and resistin) as regulators of insulin metabolism are involved in the pathogenesis of insulin resistance in PCOS. Pioglitazone treatment can decrease plasma glucose level and improve insulin sensitivity at least partly through improving the profiles of adipocytokines.

Interferon-γ-inducible protein 10 (IP-10),also known as chemokine (C-X-C motif) ligand 10(CXCL10),belongs to the CXC subfamily.IP-10 exerts its function by binding to chemokine (C-X-C motif) receptor 3 or Toll-like receptor 4.IP-10 plays an important role in the occurrence and development of autoimmune diseases.The expression of IP-10 in blood and body fluid is closely associated with the activity of autoimmune disease.IP-10 is expected to become a potential new marker and possess a significant application value in early screening,condition monitoring and therapeutic evaluation of autoimmune diseases.
3.Preliminary exploration and practice on digital oral medicine instructional management platform

In recent years, various network assistant teaching platforms which have flooded into universities have been well received by the majority of teachers and students and educators. Network teach-ing platforms in oral medicine education now, especially the assistant teaching and information management have gradually risen to play a decisive role position. Combined with the characteristics of oral medical un-dergraduate course and the present situation of teaching, a preliminary practical use has been made on the new digital teaching management platform of College of Stomatology, Chongqing Medical University, and the analysis based on its advantages and disadvantages has also been made, in order to make an improvement of oral medicine teaching quality and management efficiency.
4.Clinical observation of a modified anterior fixation technique for thoracolumbar fractures

Objective To modify the anterior fixation technique for thoracolumbar fractures so as to reduce blood loss and improve the success rate of the first vertebral screw implantation. Methods Only surface artery of the injured vertebrae was ligated and the discs adjacent to the burst vertebra excised. Then,the mass autograft ilium was harvested with most cartilage terminal lamina left. The bursted vertebra was resected so that the vertebral canal could be decompressed thoroughly. Structural ilium graft was performed,followed by Z-plate being placed and locked. Results The modified operative technique was used to treat 87 cases,of which 76 cases of fresh thoracolumbar fracture had hemorrhage capacity about 400-800 ml (average 560 ml) as well as operation time for 2.5-3 hours,and 11 cases with old fractures had hemorrhage capacity about 800-1 200 ml (average 1 080 ml) as well as operative time for 3.5-4 hours. Eighty-one cases with kyphosis deformity was completely corrected,four cases had kyphosis deformity of 5?-8? and two lordosis. A total of 39 cases with post-operative paralysis basically recovered,28 partially recovered,nine recovered fractionally and 11 was beyond recovery. No aggravation was found in post-operative nerve syndrome. Conclusions The modified anterior fixation technique is excellent in reducing blood loss,shortening the operation time and increasing the success rate of the primary vertebral screw implantation. [
5.EXPRESSION OF GAMMA INTERFERON-LIKE SUBSTANCE IN ADRENAL PHEOCHROMO-CYTOMA

Expression of gamma interferon (IFN-?)-like substance in adrenal pheochromocytoma was investigated with immunohistochemical technique. In 15 out of the 16 cases of adrenal pheochromocytoma positive IFN-@-like immunoreactive substance was detected in the cytoplasm of the tumor cells, whereas in adrenal tissues adjacent to adrenal tumors in 26 cases (pheochromocytoma 12, adrenal corti-col tumors 12, others 2) only two with pheochromocytoma showed positive IFN-?-like immunoreactive substance in the cytoplasm of zona reticularis cells. The results suggest that IFN-? may be a kind of antigen associated with differentiation of cell.
6.Fasciocutaneous flap pedicle with the perforating branches of posterior tibial artery in repairing leg soft tissue defect

Sixteen patients underwent fasciocutaneous flap pedicle with the perforating branches of posterior tibial artery to repair the skin defects of the middle and lower leg in Department of Orthopaedics,Fuzhou General Hospital of Nanjing Military Area Command of Chinese PLA from February 2003 to February 2007.Based on anatomy,the flap axis ray was designed as the connection of internal condyle of tibia and the middle point between inner malleolus and achilles tendon,rotating along the axis ray adjacent to wound with pedicle having perforating branches of posterior tibial artery.Under guidance of Doppler flowmeter,all skin including flap,pedicle,and surface of perforating branches must be fine.The upper leg skin defect was repaired using anterograde flap,and the middle-lower leg skin defect was repaired using retrograde flap,including lower leg defect in 9 cases,middle leg defect in 3 cases and upper leg defect in 4 cases.The skin defects ranged from 2.0 cm?1.5 cm to 12.5 cm?7.5 cm,and the largest size of flap was about 15.0 cm?10 cm,and the smallest size of flap was about 4.0 cm?2.5 cm.The repairing effect was observed.All flaps survived.Two cases developed distal necrosis of skin,but granulation was found in subcutaneous tissue,of which 1 case healed after changing dressings,and the other recovered after skin grating.All subjects were followed up for over 6 months.All cases could walk naturally but felt pain.Fasciocutaneous flap pedicle with the perforating branches of posterior tibial artery can repair the skin defects of leg soft tissue.The donor site is posterior leg.The pedicel is kept well after injury,and the flap is easy to design.In addition,the blood supply is reliable,and survival rate is high.It is a good surgery to repair leg soft tissue defect.

This thought embodies the Chinese people's cosmology and methodology and exerts a far-reaching influence on various aspects of Chinese culture. The embodiment of Three-Layer thought in the theory and practice of acupuncture and moxibustion from naming of acupoints, principles of treatment, needling instruments, prescription of acupoints as well as needling techniques is elaborated and briefly analyzed. Thus it illustrates the comprehensive application of Three-Layer thought in acupuncture and moxibustion through the history and the significance of Chinese traditional philosophy in the science of acupuncture and moxibustion.
