2.Application of Wood's lamp in the diagnosis of superficial cutaneous fungal infections

Objective To estimate the performance of Wood's lamp examination in the diagnosis of superficial cutaneous fungal infections. Methods Totally, 129 patients, who were diagnosed with superficial cutaneous fungal infections according to clinical medical history and signs, were enrolled in this study. Wood's lamp examination of lesions was carried out. Cutaneous samples were obtained from the patients and subjected to microscopic examination and fungal culture. Results Wood's lamp examination was positive in 84% and 85.7% of patients with tinea versicolor and malassezia folliculitis, respectively; among these patients positive for Wood's lamp examination, 92.9% were positive for fungal culture, and 87.5% for microscopic examination. In patients clinically diagnosed with tinea manus and pedis, tinea corporis or tinea cruris, 8.3% were positive for Wood's lamp examination, while 85.4% were positive for fungal examination. There was a high consistency between Wood's lamp examination and fungal examination in patients with tinea versicolor and malassezia folliculitis, but not in those with tinea manus and pedis, tinea corporis or tinea cruris. Conclusions Wood's lamp examination shows a high specificity and sensitivity and is useful in the diagnosis of tinea versicolor and malassezia folliculitis, but seems unapplicable for the diagnosis of tinea manus and pedis, tinea corporis or tinea cruris.

4.Determination of antibiotic residues in chicken meat using ultra-high-performance liquid chromatography coupled with hybrid quadrupole-orbitrap high-resolution mass spectrometry

Objective:
To optimize the sample pretreatment and establish an ultra-high-performance liquid chromatography coupled with hybrid quadrupole-orbitrap high-resolution mass spectrometry (UPLC-Q-Orbitrap-MS) assay based on the parallel reaction monitoring (PRM) mode for determination of antibiotic residues in chicken meat.
Methods:
Blank matrix-spiked chicken meat samples were extracted with 95% acetonitrile aqueous solution containing Na2EDTA and formic acid. The extraction solutions were cleaned up using different combinations of C18, PSA and GCB fillers, and the combinations with a higher antibiotic recovery rate was screened. The residues of 32 antibiotics were determined using UPLC-Q-Orbitrap-MS based on the PRM mode.
Results:
If the extraction solution was cleaned up using the C18 filler, the largest number of antibiotics with a spiked recovery rate of >80% was seen, with matrix effects of 82.2% to 112.6%. The detection limits of 32 antibiotics were 0.8 to 5.8 μg/kg, with linear correlation coefficients of >0.99, spiked recovery rates of 71.3% to 111.5% and relative standard deviations of 3.2% to 14.2%.
Conclusion
The UPLC-Q-Orbitrap-MS assay is suitable for determination and quantitative analysis of multiple antibiotics in chicken meat.

5.Advance in Modified Constraint-induced Movement Therapy for Upper Extremity Function Recovery after Stroke (review)

Modified constraint-induced movement therapy (mCIMT), which is modified from constraint-induced movement therapy (CIMT), is used in hemiplegics for rehabilitation. It may be used in the selected patients in certain function and age. mCIMT is more flexible than CIMT, which is mainly consisted of intervention movement, intensity, and the time of training and limitation. mCIMT is also used with other rehabilitation methods. There are various assessment for the evaluation of upper extremity motor function. By using Contract and Mo-tor Activity Log, researchers can better guarantee the curative effect.
6.The clinical differential diagnostic value of multiple parameters analysis method for anemia of chronic disease in elderly patients

Objectives To clarify the differential diagnostic value for iron deficiency anemia (IDA)and anemia of chronic disease(ACD)with analysis method of multiple parameters.Methods The serum concentrations of iron parameters (SI,TIBC,TF,SF and sTfR),inflammatory index(IL-1,IL-6,TNF-β),CRP and hepcidin were detected in patients with IDA and ACD by biochemical method and ELISA respectively.Results In the differential diagnosis of IDA versus ACD,the SF (98.6 versus 6.0,P=0)and sTfR/logSF[(13.1-±-4.4) versus (27.6 ± 16.5),P=0.002] had clinical value for the differential diagnosis.Inflammatory factors level was increased in two groups,only the difference in CRP between IDA and ACD showed a statistical significance.The hepcidin had the clinical value in differential diagnosis for single disease diagnosis of IDA and ACD and for differential diagnosis between IDA and ACD [(144.6 ±-1.9) versus (86.5-±-3.1),P< 0.05].Correlation analysis showed that the sTfR/logSF had a significantly negative correlation with SI(P=0.002),a significantly positive correlation with TIBC and TRF,and no significant correlation with inflammatory factors.The hepcidin had significantly positive correlation with IL-6 and TNF-β,and a significantly negative correlation with SI.The positive correlation was observed between sTfR/logSF and Hepcidin(r=0.376,P=0.005).Conclusions Analysis method of multiple parameters could make a differential diagnosis of ACD in the elderly.
7.Construction of a predicting model of uncertainty in illness for the postoperative patients with digestive system tumors undergoing chemotherapy

Objective Construction of a predicting model of uncertainty in illness based on Mishel's theories of uncertainty in illness in a sample of the postoperative patients with digestive system tumors undergoing chemotherapy.Methods By convenient sampling method,MUIS-A,MSAS,SSRS were used to evaluate 484 postoperative patients with digestive system cancer during chemotherapy period,and then AMOS 17.0 software was used to perform analysis.Results The score of uncertainty in illness of postoperative patients with the digestive system cancer undergoing chemotherapy was in the medium level (89.86+11.84) points; Structure provider (social support,economic income) could not only reduce the uncertainty directly,but also had an indirect effect on the uncertainty by stimuli frame (symptoms pattern,event familiarity).Conclusions Structure equation model can be applied to the uncertainty in illness model construction,so as to help medical staff manage uncertainty in illness pertinently.
8.Investigation on B-Learning mode in teaching of burn department

Objective To investigate the application of B -Learning mode in the teaching of department of burns surgery.Methods 234 students were divided into two parts by digital table method randomly.The experimental group was applied by the means of B -learning mode,while the control group was taught by the traditional way.And the results were evaluated.Results The basic theory scores(75.39 ±12.04)points in the students of experimental group were higher than those of the control group(68.27 ±12.38)points.The practice scores(76.42 ±11.89)points in the students of experimental group were higher than those of the control group(65.36 ±12.05)points.Students'professional knowledge scores(79.92 ±12.85)points in the experimental group were higher than those of the control group(71.56 ±13.01)points.Both percentage and score were higher in the experimental group than in the control group in comprehensive ability.Conclusion Blending learning gives students a potential time,environment and recourse to learn.By integrating learning ability and nursing skills improved,B -learning mode plays an important role in improving the quality of burn department internship.
9. Optimization of the extraction process of triterpenoid acids in seeds of Ligustri lucidi fructus by response surface method

OBJECTIVE: To optimize the extraction and purification process of total triterpenoid acid in Ligustri lucidi fructus using response surface method and uniform test. Finally, the content of total triterpenoid acid reached more than 50%. METHODS: The content determination of total triterpenoid acid in Ligustri lucidi fructus was established by HPLC. Use ethanol as extraction solvent and through the single factor investigation about the ethanol concentration, solvent amount, extraction time and extraction times, we determined the number of ethanol extraction. In addition to extraction times, the other three factors were further optimized by response surface method. Then purified by the alcohol extraction-water precipitation method to remove the water-soluble ingredients. Petroleum ether was used to remove the chlorophyll, grease and other small polarity ingredients; using ethanol to remove the alcohol soluble components whose polarity is bigger than fructus ligustri lucidi terpene acids. Design the uniform experiment respectively to determine the amount of petroleum ether, frequency and time, also ethanol concentration and dosage. RESULTS: The content of oleanolic acid and ursolic acid is larger than other compounds in total triterpenoid acid in Ligustri lucidi fructus, and they are isomers. When the results are in the equivalent of oleanolic acid, the optimum extraction process was determined as 80%, 10 times the amount of ethanol, extracting for 2 times, each time 0.5 h. The optimal purification process is 12 times the amount of petroleum ether, purified once, 2 h every time. Then 10 times amount of 60% ethanol to purify for once. CONCLUSION: Optimizing the extraction and purification process of total triterpenoid acid in Ligustri lucidi fructus by using response surface method and uniform experiment is simple, feasible and worth promoting.
10.The application of nutrition support in critical ill patients

Objective To evaluate the efficiency of nutrition support in critical ill patients. Methods A retrospective analysis was made to the efficiency of the nutrition support applied to 233 critical ill patients PN+EEN→EN+per os→per os was employed as the model of nutrition support, and a nasal feeding pump was used for enteral nutrition. Results During the period of nutrition support, in the 233 patients, 26 dead, the mortality was 11.16%. Early enteral nutrition significantly decrease the mortality in critical ill patients. There existed significant differences in complications in three kinds of enteral nutrition may methods. Conclusion It is feasible to give the critical ill patients, it might help to reduced the mortality in ICU patients.
