1.Application of high b-value MR diffusion weighted imaging for malignant and benign breast lesions

ObjectiveTo evaluate the application of high b-value diffusion weighted imaging (DWI) and ADC values for malignant and benign breast lesions.MethodsThere were 165 patients with 171 suspicious malignant breast lesions received low and high b-value ( 500 and 1500 s/mm2 ) DWI scan before dynamic contrast-enhanced MRI (DEC-MRI).Using normal breast as the reference base,the signal intensity and ADC values of the 171 lesions on high-b-value ( b = 1500 s/mm2 ) DWI were retrospectively analyzed If a lesion showed abnormal enhancement on DCE-MRI and high signal intensity on high-b-value (b = 1500 s/mm2 ) DWI,that lesion was defined as positive result on behalf of malignant lesion; the otherwise was defined as negative result on behalf of benign lesion.ADC values were calculated for the 111 lesions with positive results on DWI.And all lesions received a biopsy after MR scan.References to the histopatholngical results,the amounts of positive and negative results on high-b value DWI were recorded.Using 1.13 × 10 -3mm2/s as the cutoff value,the sensitivity and specificity of ADC value for the diagnosis of malignant breast lesion were calculated.Fisher exact tests and Wilcoxon rank sum tests were used forstatistical analyses.ResultsThere are 91 malignant and 80 benign lesions the 171 lesions confirmed by histopathology.On high-b value DWI,139 lesions were visually positive results,including 83 malignant and 56 benign lesions.Other 32 lesions were visually negative results,including 24 benign lesions and 8 malignant lesions (non-mass ductal carcinoma in situ).There are statistically significant difference between malignant and benign lesions ( P < 0.01 ).All invasive carcinoma and mass-forming DCIS were visually positive results,whereas eight non-mass DCIs lesions were visually negative results,the overall sensitivity and specificity were 91.2% (83/91)and 30.0% (24/80),respectively.In the 111 lesions with DWI positive result ( 110 mass lesions and one focal lesion),the mean ADC value of the 63 malignant lesions was (0.73 ±0.24) × 10-3 mm2/s,whereas the mean ADC value of the 48 benign lesions was( 1.19 ±0.42) ×10-3 mm2/s.There was statistically significant difference between benign and malignant lesions (Z =5.818,P <0.01 ).When ADC value of 1.13 × 10-3 mm2/s was chosen as the cutoff value,61 malignant lesions were true positive results,2 mucinous carcinoma were false-negative results,27 benign lesions were true negative results,21 benign lesions were false-positive results,the sensitivity and specificity was 96.8%(61/63) and 56.2% (27/48),respectively.ConclusionsHigh b-value DWI and ADC values were useful for distinguishing of malignant and benign breast lesions,but it should be cautious when it was used for the diagnosis of non-mass breast lesions.

Interferon-γ-inducible protein 10 (IP-10),also known as chemokine (C-X-C motif) ligand 10(CXCL10),belongs to the CXC subfamily.IP-10 exerts its function by binding to chemokine (C-X-C motif) receptor 3 or Toll-like receptor 4.IP-10 plays an important role in the occurrence and development of autoimmune diseases.The expression of IP-10 in blood and body fluid is closely associated with the activity of autoimmune disease.IP-10 is expected to become a potential new marker and possess a significant application value in early screening,condition monitoring and therapeutic evaluation of autoimmune diseases.

8.Imaging Findings of Vertebral Eosinophilic Granuloma(An Analysis of 10 Cases)

Objective To improve the understanding of the radioactive charcteristics of vertebral eosinophilic granuloma.Methods The imaging features (X-ray?CT and MRI) of 10 cases with vertebral eosinophilic granuloma were retrospectively analyzed.Diagnoses of all the patients had been confirmed with pathlolgical examination after operation.Results All the patients had vertebral body damages,7 with osteolytic damages and 3 with cystic damages.Intervertebral crevice became narrowing or disappearing .Some vertebral appendix and paravertebral soft tissue were involved.Conclusion It is out of proportion among the vertebral body damages,the involved vertedral arch,the changes of intervertebral crevice and the mass of paravertebral soft tissue.The remaining bone fragment without sequestrum in the damage ares is considered as the main characteristic expression of vertebral eosinophilic granuloma.
9.Fasciocutaneous flap pedicle with the perforating branches of posterior tibial artery in repairing leg soft tissue defect

Sixteen patients underwent fasciocutaneous flap pedicle with the perforating branches of posterior tibial artery to repair the skin defects of the middle and lower leg in Department of Orthopaedics,Fuzhou General Hospital of Nanjing Military Area Command of Chinese PLA from February 2003 to February 2007.Based on anatomy,the flap axis ray was designed as the connection of internal condyle of tibia and the middle point between inner malleolus and achilles tendon,rotating along the axis ray adjacent to wound with pedicle having perforating branches of posterior tibial artery.Under guidance of Doppler flowmeter,all skin including flap,pedicle,and surface of perforating branches must be fine.The upper leg skin defect was repaired using anterograde flap,and the middle-lower leg skin defect was repaired using retrograde flap,including lower leg defect in 9 cases,middle leg defect in 3 cases and upper leg defect in 4 cases.The skin defects ranged from 2.0 cm?1.5 cm to 12.5 cm?7.5 cm,and the largest size of flap was about 15.0 cm?10 cm,and the smallest size of flap was about 4.0 cm?2.5 cm.The repairing effect was observed.All flaps survived.Two cases developed distal necrosis of skin,but granulation was found in subcutaneous tissue,of which 1 case healed after changing dressings,and the other recovered after skin grating.All subjects were followed up for over 6 months.All cases could walk naturally but felt pain.Fasciocutaneous flap pedicle with the perforating branches of posterior tibial artery can repair the skin defects of leg soft tissue.The donor site is posterior leg.The pedicel is kept well after injury,and the flap is easy to design.In addition,the blood supply is reliable,and survival rate is high.It is a good surgery to repair leg soft tissue defect.
10.Phosphorylated H2AX in predicting sensitivity of hepatic cancer cell line HepG2.215 to chemotherapeutic drugs

Objective:To study the feasibility and reliability of using phosphorylated H2AX(?H2AX)as a predictor for sensitivity of hepatic carcinoma cell HepG2.215 to chemotherapy agents: etoposide, doxorubicin, mitomycin, and cisplatin. Methods: HepG2.215 cells were exposed to etoposide, doxorubicin, mitomycin or cisplatin of 1, 2, 4 and 20 concentration index (CI). Untreated HepG2.215 cells were taken as control. The proportion of HepG2.215 cells expressing ?H2AX was measured by flow cytometry, the number of ?H2AX foci in HepG2.215 cells was measured by immunocytochemistry, and cell proliferation was measured by MTT. The correlation between the number of ?H2AX foci and the percentage of HepG2.215 cells expressing ?H2AX in HepG2.215 cells was analyzed; the correlation of CI with the percentage of HepG2.215 cells expressing ?H2AX or ?H2AX foci and inhibitory rate of cell proliferation was analyzed; and the correlation of inhibitory rate of cell praliferation with the percentage of HepG2.215 cells expressing ?H2AX or ?H2AX foci was also analyzed.
