Objective To explore the relationship among the sense of coherence,empowerment and self-efficacy in peritoneal dialysis patients.Methods General information,the Sense of Coherence Scale,the Peritoneal Dialysis Empowerent Scale-Short Form and the Self-Efficacy for Managing Chronic Disease Scale were used to investigate a total of 160 peritoneal dialysis patients in a third-class hospital in Tianjin from March to June,2015.Results The mean score of the sense of coherence,empowerment,self-efficacy were (62.01±11.94) points,(4.09±0.68) points and (6.08±2.19) points.Both the sense of coherence and empowerment could positively influence the self-efficacy of peritoneal dialysis patients directly (r=0.429-0.571,P ＜ 0.01),and the sense of coherence could also indirectly influence the self-efficacy through the mediation of empowerment (β decreased from 0.429 to 0.172,P ＜ 0.01),the mediation effect accounted for 60.1％ of the total effect.Conclusions Medical staff should take different actions to improve the sense of coherence and empowerment to improve the self-efficacy of peritoneal dialysis patients.

Objective To explore the relationship between 677C/T polymorphism of methy lenetetrahydrofolate reductase (MTHFR) gene and diabetic macrovascular complications (including cerebral infarction and coronary heart disease) in Shanghai.Methods Among 416 Chinese subjects, 216 were in macrovascular disease group 〔the atherosclerosis (AS) group〕 which included cerebral infarction (CI) subgroup consisting of 111 subjects (50 cases with and 61 without diabetes) and coronary heart disease (CHD) subgroup consisting of 105 subjects (48 with and 57 without diabetes); 100 subjects were in diabetes group without macrovascular disease (DM); and 100 normal subjects in control group (C). Genotypes were determined by PCR/restriction enzyme digestion and the relationship between 677C/T polymorphism and diabetic macrovascular complications was studied by population based association analysis. Results The frequencies of MTHFR TT genotype and T allele were significantly higher in macrovascular disease group. Logistic regression analysis showed that MTHFR gene was a potential contributor to AS. Comparison of genotype frequencies between diabetic and non diabetic subgroups in macrovascular disease group revealed no difference. Significant difference of high density lipoprotein cholesterol and diastolic blood pressure between different genotype subgroups was observed within some macrovascular disease groups after stratification by sex and diabetes. Conclusion MTHFR contributes to the development of macroangiopathy (cerebral infarction and coronary heart disease) whether associated with type 2 diabetes or not in Shanghai district.

Objective To observe the efficacy and toxicity of low dose thalidomide combined with VAD regimen contained one-week dexamethasone in treatment on patients with multiple myeloma (MM). Methods 42 patients with MM were diagnosed in our hospital.The VAD regimen contained one-week dexamethasone was vincristine 0.4 mg/d, adramycin 10 mg/d and dexamethasone 40 mg/d for 4 days. At the same time, thalidomide is given 150-200 mg/d. Results The overall response rate (ORR) is 80.95%. Before and after treatment, all the detection indicators in MM patients were statistically different (P＜0.001). There is small adverse reactions. Conclusion Low dose thalidomide combined with VAD regimen contained one-week dexamethasone in treatment of MM has more merits such as less side effects, good tolerance, convenient administration and significant curative effects, so it should be recommended in clinical works.

Objective:To study the curative efficacy of decitabine combine with CAG regimen in the treatment of acute myeloid leukemia and its effects on the serum Interferon-γ (IFN-γ),alpha hydroxybutyrate dehydrogenase (HBDH) and lactate dehydrogenase (LDH) levels.Methods:70 cases of patients with acute myeloid leukemia who were treated from February 2013 to October 2016 in our hospital were selected as research objects.According to the random number table,the patients were divided into the observation group (n=35) and the control group (n=35).Both groups of patients were treated conventional treatment.The control group was treated with CAG scheme,intravenous injection of aclarubicin,20 mg each time,granulocyte colony stimulating factor,300 μg each time,subcutaneous intravenous cytarabine,10～15 mg/m2 each time,while the observation group was treated with intravenous drip of decitabine on the basis of control group,15 mg/m2 each time.Then the therapeutic effect,serum interferon-γ (IFN-γ),alpha hydroxybutyrate dehydrogenase (HBDH),lactate dehydrogenase (LDH) levels before and after treatment,incidence of adverse reactions were compared between two groups.Results:After treatment,the total effective of observation group was significantly higher than that of the control group [91.42％(32/35)vs68.57％(24/35)] (P ＜0.05);the serum IFN-γ,HBDH,LDH levels were significantly higher than those of the control group [(3.21± 1.01)pg/ml vs.(5.13 ± 1.90)pg/mL,(103.62± 26.39)U/L vs.(118.80± 28.60)U/L,(101.36± 27.32)U/L vs.(123.08 ± 30.59)U/L] (P ＜0.05);the incidence rate of adverse reactions was significantly lower than that of the control group [20.00％(7/35) vs.42.85％(15/35)(P ＜0.05)].Conclusion:Decitabine combined with CAG regimen was more effective for acute myeloid leukemia than CAG regimen alone,which might be related to reduce the serum levels of IFN-γ, HBDH and LDH.
