1.Minimally Invasive Stealth Lateral Internal Sphincter Release for Chronic Anal Fissure:a Report of 42 Cases

Objective To investigate the safety and feasibility of minimally invasive stealth lateral release operation in the treatment of chronic anal fissure . Methods Clinical data of 42 patients with chronic anal fissure who underwent minimally invasive stealth lateral release from May 2012 to May 2014 was analyzed retrospectively .We use the homemade fine steel wire to pierce into the sphincter ditch with a distance from the anus of 1 cm, and put the steel wire out above the dentate line .Then the wire was took out from the other edge under the skin through the needle of bobbin .After the needle was fixed by an assistant , the free end of the steel wire was pulled quickly to cut the internal sphincter tissue instantly . Results All the 42 patients were successfully treated and discharged without complications .The recovery time was in 7 -14 d, with an average of 9 d.The healing standards included disappeared symptoms and totally healed wound .All the patients were followed up for 3-6 months, with an average of 4 months.No recurrence was found . Conclusion Minimally invasive stealth lateral release in the treatment of chronic anal fissure is safe and feasible, with many advantages such as simple performance , reliable curative effect, fast recovery time, and less postoperative complications .
2.Preoperative predictors for the nature of lesion in patients with focal masses in the setting of chronic pancreatitis

Objective To establish a predictive model and to find the preoperative predictors for the nature of lesion in the setting of chronic pancreatitis. Methods The 121 patients from 7 tertiary medical centers in Shanghai from July 1998 to April 2007 with focal mass lesions in the setting of chronic pancreatitis were selected as the study population. The final diagnosis had to be confirmed histologically by surgical specimens (n =97) or by follow-up (n = 24). A case control study was conducted; the patients were divided into pancreatic cancer group and chronic pancreatitis group. The age, sex, past history, initial clinical presentations, lab results and imaging exams were collected by reviewing the medical records of these patients. χ~2 test and t test was used for univariate analysis, then the factors with P≤0. 25 were selected for further multivariate analysis, and multivariate logistic regression model was used to estimate odds ratio and 95% CI. Results Of 121 , 21 patients had a final diagnosis of pancreatic cancer and other 90 patients had a final diagnosis of chronic pancreatitis. Abdominal tenderness, direct bilirubin, CA19-9 and CEA were independent predictors of cancer in patients with focal mass lesions. Their odds ratios (95% CI) were 5. 691 (1.468, 22.070) , 1.011 (1.001 , 1.021) , 1.003 (1.001, 1.005) , 101.9 (0.988, 1.051) , respectively. Their P values were 0. 012, 0. 030, 0.003 and 0. 23 , respectively. Conclusions The logistic regression model may accurately predict the nature of lesion in the setting of chronic pancreatitis and may have certain clinical implication.
3.Applicatiuon of integration of PBL and CBL teaching methods in kinesiology experiments

Objective To explore the effect of applying problem-based learning (PBL) and case-based learning (CBL) in the kinesiology experiments. Methods Totally 534-year program undergraduates of Nantong University Medical School were divided into control group (n=26) and experimental group (n=27). The control group received traditional teaching and the experimental group received PBL and CBL teaching. The teaching effect was evaluated by examination and questionnaire. Data were analyzed by SPSS 19.0 and t test was used to compared the average scores of theory and questionnaire, α=0.05. Results The theoretical examination results (89.81 ±1.32) were better than the control group (84.82 ±1.75) (P=0.028). Meanwhile questionnaire survey showed that the experimental group students' evaluation of their teaching methods in promoting learning interest and initiative, combining theory with practice and teamwork con-sciousness were better than the control group students' evaluation of traditional teaching methods (P<0.05). Conclusion The teaching effect of integration of PBL and CBL teaching methods is better than traditional teaching in kinesiology experiments.
4.Progress of clinical laboratory tests and molecular imaging for diagnosis of Alzheimer's disease

With the expansion of aging population, Alzheimer′s disease (AD) is increasingly imposing a substantial burden on the society. Because of irreversible neuronal death in the brain, effective control of AD relies on early diagnosis for timely therapeutic intervention, and this requires efficient laboratory tests and molecular imaging. Current laboratory tests for AD includes measurements of Aβ peptides, Tau, and a number of other recently discovered molecules in the cerebrospinal fluid and the peripheral blood. PET-based imaging of glucose metabolism, amyloid Aβ, Tau neurofibrillary tangles, TSPO protein, and neuronal receptors, has also proven to be useful in clinical finding of AD. This review will focus on the progress in studies of these biomarkers and methodologies for ADdiagnosis.
5.Effects of water extract and ethanol extract from the root of Caragana sinica on hyperuricemia in mice

OBJECTIVE To investigat e the effects of water extract (WCS)and ethanol extract (ECS)from the root of Caragana sinica on hyperuricemia (HUA)in mice. METHODS Kunming mice were randomly divided into normal control group , model group ,allopurinol group (positive control ,5 mg/kg),benzbromarone group (positive control ,7.8 mg/kg),WCS low-dose , medium-dose and high-dose groups (38,75,150 mg/kg),ECS low-dose ,medium-dose and high-dose groups (50,100,200 mg/kg), with 10 mice in each group. Except for the normal control group ,the other mice were given potassium oxazinate intraperitoneally and hypoxanthine intragastrically for consecutive 7 d to establish HUA model. On the third day of modeling ,mice in each administration group were given corresponding drugs intragastrically ,and normal control group and model group were given equal volume of normal saline once a day for 5 consecutive days.The body weight of mice were weighted during administration ;one hour after the last administration ,the organ indexes of liver ,kidney and spleen were calculated ;the contents of serum uric acid (SUA), blood urea nitrogen (BUN)and serum creatinine (SCR);the activity of xanthine oxidase (XOD)in serum and liver tissue were determined. Relative mRNA and protein expressions of XOD in liver tissue ,relative expre ssions of GLUT9,URAT1 and OAT 1 in renal tissue were all detected ;and the pathological changes of renal tissue were observed. RESULTS There were no significant differences in liver index and spleen index in each group (P>0.05). Compared with normal control group , except for allopurinol group , there were no significant differences in the body weight and the contents of BUN and SCR in mice of other administration groups (P>0.05);the renal index and SUA content of mice in the m odel group and allopurinol group were significantly increased (P<0.05);in the model group ,the XOD activity in serum and liver tissue ,the relative mRNA and protein expression of XOD in liver tissue ,the relative expressions of GLUT 9 and URAT 1 protein in renal tissue were significantly increased (P<0.05),and the relative expression of OAT 1 protein in renal tissue was significantly decreased (P< 0.05). Compared with model group ,renal indexes of mice were decreased significantly in WCS and ECS groups (P<0.05),and the pathological damage of renal tissue was significantly improved ;SUA content ,XOD activity in serum and liver tissue ,the relative mRNA and protein expression of XOD in liver tissue ,and the relative expression of URAT 1 protein in renal tissue were decreased significantly in administration groups (P<0.05). The relative expression of GLUT 9 protein in renal tissue of mice in benzbromarone group and ECS high-dose group decreased significantly (P<0.05);relative expression of OAT 1 protein in renal tissue of mice in benzbromarone group ,WCS low-dose and high-dose groups ,ECS low-dose group were increased significantly (P<0.05). CONCLUSIONS WCS and ECS can significantly decrease the contents of SUA in HUA model mice ,and improve pathological state of renal tissue ,the mechanism of which may be associated with inhibiting XOD activity and uric acid reabsorption,and down-regulating protein and mRNA expression of XOD.
