1.The stability of fat emulsion in total nutrient admixture by automated compounding device

Objective:To evaluate the stability of fat emulsion in TNA prepared by automated compounding device and compare it with the manual method.Methods:A total of 20 TNA were prepared by the automated compounding device and the conventional manual method.The TNA were stored at 25℃ for 1 day,4℃ for 7days and again 25℃ for 1 day to observe the stability of fat emulsion by electron scanning microscopy measurement and to measure the pH,osmolality.Results:Time required for TNA prepared by automated compounding device was less than the manual method.At the 1st day,the particle size of fat emulsion in TNA prepared by the automated compounding device was smaller than the manual method;After 7 days storing at 4℃ and 1 day at 25℃,the particle size,pH and osmolality were also stable in automated compounding device group.Conclusion:Automated compounding device reduced time requirements for TNA preparation.The stability and compatibility of TNA prepared by automated compounding device is better than the manual method.

3.Effect of tongxinluo capsule on local blood flow of brain and recovery of neurological function in patients with cerebral infarction: observation with single photon emission computerized tomography

BACKGROUND: Tongxinluo acts on benefiting qi, promoting circulation in collateral, activating blood circulation and resolving stasis, which is well effective in treatment of ischemic cerebral vascular disease. Single-photon emission computed tomography (SPECT) provides reliable reference evidence objectively and sensitively for clinical evaluation of therapeutic effects.OBJECTIVE: SPECT and clinical neurofunctional defect score (NDS)were used to observe the therapeutic effect of tongxinluo capsule on cerebral infarction.DESIGN: Case analysis was designed.SETTING: Internal Department of Neurology of First Affiliated Hospital of Chongqing Medical University.PARTICIPANTS: Totally 22 cases of acute cerebral infarction were admitted in Internal Department of Neurology of First Affiliated Hospital of Chongqing Medical University from March 2002 and July 2003, in which 12 cases were male and 10 cases were female, aged varied from 40 to 78 years, sick duration in range from 3 hours to 3 days.METHODS: Voluntarily, 22 cases of acute cerebral infarction were randomized in tongxinluo capsule group (capsule group)(11 cases) and the control (11 cases). In the control, danshen tablet was administrated,4 tablet/time, 3 times/d. In capsule group, tongxinluo capsule group was administrated, 4 pills/time, 3 times/d. The drugs were administrated for 15 days in two groups. Before and after treatment, SPECT ROI technique was used to assay ROI average radioactive counts on the affected and healthy sides to understand local blood flowing volume of brain in two groups. Neurofunctional defect evaluation was done before and after treatment on each patient.in two groups.RESULTS: Totally 22 cases of cerebral infarction all entered result analcontrol after treatment [(49.182±5.344 5), (28.364±4.610 3) score P < 0.001].the affected side of the control and treatment group before treatment was decreased. Fifteen days after treatment, the volume in original decreased flowing area of brain was improved after taking the capsule (66.536±18.676,ter administration of the capsule reflected in SPECT radionuclide, but it was not improved remarkably in the control.CONCLUSION: Tongxinluo capsule improves definitely cerebral blood flow in patients with cerebral infarction. With improvement of blood flowing in brain, the neurofunctional defect score is promoted clinically, too. It is explained that the capsule promotes the recovery of nerve function.

Objective To evaluate the effects of nutritional education by "knowledge,attitude and behavior" on health improvement in patients with type 2 diabetes.Methods One hundred and sixteen patients with type 2 diabetes were selected with nutritional education intervention by "knowledge,attitude and behavior",and another 100 diabetic patients as control group with regular education for six months,to follow up their changes in awareness of diabetes-related nutritional knowledge and biochemical examinations. 6.Polymerase Chain Reaction-Sequence Special Primers in Detecting Gene Polymorphism of Pediatrics Diseases

Objective To establish an approach of polymerase chain reaction-sequence special primers(PCR-SSP) to perform polymorphism analysis for multiple genes simultaneously,in order to be used in clinical analysis.Methods The optimized PCR-SSP approach was used to analyze the polymorphism of the following genes:mutation of-308A/G and-238G/A in tumor necrosis factor-?((TNF-?)),-174G/C in interleukin-6(IL-6).Results Polymorphism analysis of multiple genes and many clinical samples could be simultaneously performed with one PCR program,showing clear genotype,quick and accurate genotype.Conclusion The optimized(PCR-SSP) is suitable for polymorphism analysis of the large-sample polygenic mononcleeotide point mutation,and it can be widely applied in clinical test for low cost,quickness and accuracy.
7.Relationship Between Blood Level of Galectin-3 and the Severity of Coronary Lesions in Patients With Coronary Artery Disease

Objective: To investigate the relationship between blood level of Galectin-3 and the severity of coronary lesions in patients with coronary artery disease (CAD). Methods: A total of 158 consecutive subjects received coronary artery angiography (CAG) in Yantai Yu huangding hospital were studied and they were divided into 2 groups: Control group,n=38 individuals with normal coronary artery, CAD group,n=120 patients with at least 1 coronary branch stenosis ≥ 50%. CAD group was further divided into 2 sets of subgroups:①By the number of branches involved, as Single vessel disease, n=42, Double vessels disease including LM,n=40 and Triple vessel disease,n=38.②By the quartile of Gensini score as 1st quartile group, the patients with Gensini score ≤ 18.5, 2nd quartile group,18.5 < Gensini score ≤ 45.0, 3rd quartile group, 45.0 < Gensini score ≤ 71.5 and 4th quartile group, Gensini score>71.5,n=30 in each subgroup. Blood levels of Galectin-3 were examined, and the severity of coronary lesions was evaluated by both branch numbers and Gensini score analysis. Results: Compared with Control group, the blood level of Galectin-3 was higher in CAD group,10.66 (5.81, 16.17) ng/ml vs 18.3(1.14, 2.52) ng/mlP<0.01; with the more branches of coronary lesions involved, the blood levels of Galectin-3 increased accordingly, all P <0.01. With the elevation of Gensini score, the levels of Galectin-3increased accordingly, except for the difference between the 3rd quartile group and 4th quartile group, all P <0.01.With adjusted other factors, blood levels of Galectin-3 were positively related to the number of coronary branchlesions (r =0.52, P <0.01) and Gensini score levels (r =0.17, P =0.04).Conclusion: Blood level of Galectin-3 is positively related to the severity of coronary lesions whichimplies that Galectin-3 may have potential detrimental effect on the occurrence and development of coronaryatherosclerosis.
8.Preoperative albumin level and surgical prognostic risk for digestive tract diseases

Objectives:To evaluate the correlation of preoperative albumin level(PAL) and surgical prognostic risk for digestive tract diseases. Methods:This is a retrospective study of 1009 surgical patients who were undergoing elective esophageal,gastric,pancreaticoduodenal,or colon surgery between January 1998 and December 2003.Patients were subdivided into 7 groups based on the first PAL by different 5 g/L in hospital,whose incidence of complications,mortality,postoperative stay,intensive care unit(ICU) stay,and resumption of oral intake were statistically analyzed. Results:Incidence of complications and mortality increased significantly as PAL decreased(P35 g/L.Patients with PAL≤35 g/L undergoing esophageal or pancreatic procedures sustained a significantly higher complication rate and mortality than those of colonic or gastric surgery.The delay of postoperative stay,ICU stay,and resumption of oral intake were related to these complication rates.Esophageal and pancreatic procedures had the longest stay days and colon procedures had the shortest in patients with PAL≤35 g/L. Conclusions:Esophageal or pancreatic procedures performed in patients with PAL≤35 g/L who could have had surgery result in higher risk than colon or gastric surgery.PAL and surgical site identify surgical risk for digestive tract diseases.
9.Isolation and differentiation of neural stem cells from neonatal rats in vitro

BACKGROUND: Neural stem cells are always derived from fetal rats and adult rats, and it is complex to isolate the cells by cell culture.OBJECTIVE: To study a convenient and effective method for the isolation and the culture of neural stem cells in neonatal rats.DESIGN, TIME AND SETTING: An observation study based on cells was carried out in the Chongqing Medical University (Chongqing, China) from October 2006 to March 2007.MATERIALS: Wistar neonatal rats of 1-3 days old.METHODS: Subsequent to trypsin digestion, primary culture of the cells was performed in serum-free suspension culture medium. Then the cells were induced to incubate in DMEM/F12 containing 0.10 volume fraction of fetal bovine serum. MAIN OUTCOME MEASURES: Phase contrast microscopy was employed to observe the growth of neural stem cells and the morphology of the differentiated cells. Neural stem cells and the differentiated neurons were identified using indirect immunofluorescence cytochemistry, as well as expression of gilal fibrillary acidic protein. Moreover, the proliferation of the BrdU-labeled neural stem cells was also investigated.RESULTS: The neural stem cells isolated from neonatal rat brains had the potential of serial passage and proliferation, besides, they express neuroepithelial stem cell protein (nestin) and differentiate into neurons, astrocytes and oligodendrocytes.CONCLUSION: Neural stem cells can be harvested from neonatal rat brains at a large scale, and they maintain their undifferentiated features and have the capacity of self-renewal and pluripotentiality.
10.Clinicopathological characteristics of inflammatory myofibroblastic tumor: analysis of 173 cases

The clinicopathological data of 173 cases of inflammatory myofibroblastic tumor (IMT)were retrospectively analyzed.Among them 125 were males and 48 females with a mean age of 47.9 y (5-78 y).The lesions of 117 cases were located in lungs,41 cases in eyes,8 in ileocecum,2 in liver,2 in spleen,1 in abdominal wall,1 in maxillary sinus and 1 in face.The average diameter of lesions were 3.5 cm,ranging from 1.0 cm to 7.0 cm.The clinical manifestations were not specific,depending on the locations of tumor.The imaging examinations were helpful for diagnosis.The prcsence of slender-spindled myofibroblasts and proliferation of fibroblast cells,with infiltration of chronic inflammatory cells were the basic histopathological features of the disease.Combined with the histological characteristics and immunohistochemical staining IMT can be differentiated from other spindle cell tumors.Appropriate surgical resection is the main treatment for IMT.
