OBJECTIVE:To observe the association between the ER expression and the sensitivity of TC chemotherapy in locally advanced breast cancer patients.METHODS:The expression of ER in 57 locally advanced breast cancer patients was detected by EnVision method,and the sensitivity to TC chemotherapy in ER negative(-)vs.ER positive(+)patients were evaluated.RESULTS:There were significant differences between the ER negative(-)patients and the ER positive(+)patients in the sensitivity to TC chemotherapy(86.36%(19/22)vs.60.00%(21/35))(P
2.Volatile oil of Magnolia biondii inhibits expressions of P-selectin protein in serum and renal tissue of rats with diabetic nephropathy

OBJECTIVE: To investigate the protection mechanism of volatile oil of Magnolia biondii Pamp. (VOMBP) against diabetic nephropathy in rats by observing its effects on level of soluble P-selectin (sP-selectin) in serum and expression of P-selectin in renal tissue. METHODS: Fifty SD rats were randomly divided into normal control group, untreated group, and low-, medium- and high-dose VOMBP-treated group. Diabetic nephropathy was induced in rats by intraperitoneal injection of 1% streptozotocin. Before and the 1st day, 4th, 8th and 12th week after the induction, random blood glucose (RBG) and 24-hour urinary micro-albumin were detected in different groups. At the 12th week, the rats were sacrificed to collect the blood samples and renal tissues. The contents of blood urea nitrogen (BUN) and serum creatinine (SCr) were detected and the pathological change in renal tissues was observed by light microscope; the level of sP-selectin was detected by enzyme-linked immunosorbent assay and the expression of P-selectin protein in renal tissues was measured by immunohistochemical method. RESULTS: Compared with the normal control group, RBG, 24-hour urinary micro-albumin, the contents of BUN, sP-selectin in serum and expression of P-selectin protein in renal tissue in the untreated group were significantly increased (P<0.01), the content of SCr was significantly decreased (P<0.01), and pathological change of renal tissues was also obvious. Compared with the untreated group, 24-hour urinary micro-albumin, the level of sP-selectin in serum and expression P-selectin protein in renal tissue of the three VOMBP-treated groups were all significantly decreased (P<0.01), and pathological change was lessened too. However, there were no significant differences among the three VOMBP-treated groups (P>0.05). CONCLUSION: VOMBP can protect the kidney in rats with diabetic nephropathy by inhibiting the expressions of P-selectin protein in serum and in renal tissue.
3.Correlation analysis of the rheumatoid arthritis and the meteorological factors

Objective To explore the relation between rheumatoid arthritis (RA) and the meteorological factors.Methods All RA cases in the group were followed for 12 months,patients were follow-up for every two weeks.In each followed-up,all RA cases were evaluated by the disease activity score-28 (DAS28) and the improved Jamieson questionnaire,and the correlation of the DAS28 and the average values of meteorolo-gical factors (temperature,humidity,atmospheric pressure,relative humidity,wind speed) by Pearson correlation coefficient.The moderate-severe activity cases were evaluated by chi-square test of different seasons.Results One hundred and eighty-eight cases completed the observation,One hundred and sixty-four cases were in moderate disease activities (x2=9.42,P=0.022).Sixty-eight cases were distributed in winter,which was significantly higher than other three seasons.The average temperature and wind speed was negatively related to the DAS28 (r=-0.743,P<0.01; r=-0.576,P=0.003).Conclusion The activity of RA has the characteristics of seasonal changes,and is easily being aggravated in winter.Many factors may influence RA,including temperature and wind speed.Creating an appropriate micro-climate environment is beneficial for RA patients to reduce the disease activity.
4.Application of robotic system in gastrointestinal surgery.

Robotic system helps surgeons in performing surgery. Currently Da Vinci system is the most popular. Da Vinci system has been used for the stomach and bowel diseases in 27 cases(18 cases of stomach and 9 cases of colon and rectum) in the Zhongshan Hospital, Fudan University. Accurate preoperative staging is crucial, and Da Vinci system is advantageous in lymph node dissection, preservation of nerve plexus, and complete resection of mesorectum. Adoption of gastrointestinal tract reconstruction technique should depend on the operation and experience in surgery. Though Da Vinci system has limitations and the cost is high, it is believed to be the future trend.

5.Protective effect of arctigenin in GK rats combined with hypertension macroangiopathy.

To study the protective effect of Arctigenin in goto-kakizaki (GK) rats combined with hypertension macroangiopathy. Six-week-old GK rats were divided randomly according to blood glucose level into four groups: the model group and low, middle and high dose arctigenin groups (12.5, 25, 50 mg x kg(-1)), with Wistar rats as the normal group. All of GK rats were given high-glucose and high-fat diet. After 16 weeks, GK rats were orally administrated with 10 mg x kg(-1) x d(-1) N-Ω-nitro-L-arginine methyl ester for eight weeks. During the modeling, all of arctigenin groups were orally administrated with different dose of arctigenin twice a day; The model group and the normal group were given solvents. At the beginning, mid-term and end of the experiment, blood glucose was measured. At the end of the experiment, efforts were made to detect blood pressure, collect abdominal aortic blood after anesthesia, fix thoracic aorta after bloodletting to make paraffin sections, observe morphological characteristics and detect the expression of VEGF by immunohistochemistry. According to the results, the blood glucose rose in all GK rats, with no significant difference between the drug group and the model group. At the end of the experiment, the blood pressure significantly increased in GK rats, indicating that Arctigenin could notably reduce the blood pressure in GK rats in a dose-dependent manner. The blood routine test showed increases in both the total white blood cell count and differential blood count, MPV and PDW, abnormal blood platelet parameters and decrease in PLT in GK rats, suggesting that Arctigenin could remarkably reduce the total white blood cell count and differential blood count, MPV and PDW. The thoracic aortic morphological observation revealed obvious endangium lesions in GK rats, demonstrating that Arctigenin could ameliorate the lesion extent. VEGF immumohistochemical staining showed a higher VEGF expression in the model group but lower expression in Arctigenin groups. In conclusion, Arctigenin had a protective effect on aorta in GK rats. Its mechanism may be related to blood pressure lowering, anti-inflammation, improvement in blood platelet function and reduction of VEGF expression.

6.Spinal cord plasticity and the effect of step training on functional recovery after incomplete spinal cord injury

Objective To explore neural plasticity around an injured region of the spinal cord and the effects of step training on functional recovery after incomplete spinal cord injury (SCI). Methods Adult female Sprague-Dawley rats ( n = 24) were induced with spinal cord contusion at T10 and divided into a step training group and a control group ( 12 rats in each). Training started from the 7th day post-injury and lasted for (20 ± 10)min per day, 5 days per week, for 9 weeks. Treadmill speeds were 3 m/min at the beginning, and adjusted daily according to each rat's tolerance up to 11 m/min or more. The functional recovery was measured weekly with the open-field locomotor rating scale of Basso, Beattie and Bresnahan (BBB score). The expression of glial fibrillary acidic protein (GFAP), neurofilament protein (NF) and growth-associated protein-43 (GAP-43) in the spinal cord around the injured region were detected. Results After 70 days of step training, the average BBB score of the step training group reached ( 12.86 ± 0.94 ), significantly higher than that of the control group ( 10.71 ± 0.95 ). The expression of NF and GAP-43 around the injured region increased significantly more in the step training group than among the controls. Conclusions Step training can promote functional recovery and neural plasticity in rats after incomplete SCI.
8.Antibacterials in Patients Accepted Operation in Grass-roots Hospitals:Their Application and Cost Analysis

OBJECTIVE To investigate the application of antibacterials in patients accepted operation in grass-roots(hospitals),so to standardize the application of antibacterials and cut down the medical cost.METHODS Full-time administrators for nosocomial infection investigated the application of antibacterials in patients who accepted(operation) in Sep 2004,and filled in the questionnaires.RESULTS In 1 383 cases of 11 hospitals the application rate of antibacterials was 98.63%;in which 86.50% were for prophylactic usage and 13.50% for therapeutic usage;(29.90%) for single antibiotics treatment and 50.15% for bigeminy,18.70% were for trigeminy.Time of(application) differentiated(6.90,7.00,6.60d) fromⅠto Ⅲ kinds of operation.Per capita cost of antibacterials was $956.50(47.60%).CONCLUSIONS High cost of antibacterials results from such factors as multiple kinds,long time and(combined) application.
9.Diagnosis value of immunophenotype and karyotypes in the de novo chronic lymphocytic leukemia

Objective To investigate the diagnosis value of immunophenotype and karyotypes in newly diagnosed chronic lymphocytic leukemia (CLL).Methods To retrospect the flow cytometry (FCM) immunophenotype and karyotypes characteristics in newly diagnosed 70 CLL cases.Results In all cases,the positive rates of CD19,CD20,CD5,CD23,CD22 were 100 %,88.5 % (54/61),77.1% (54/70),67.6 % (23/34)and 51.9 % (27/52),respectively.And 6 were misdiagnosed,2 was CD+5CD+19(+),but CD20,CD22 were strongly positive,final diagnosed as mantle cell lymphoma (MCL) by FISH t(11;14) examination and CyclinDl; CD+5CD+19(-) CLL were 16 cases (22.9 %),but 4 were misdiagnosed,the misdiagnosis rate was 25 %,significantly higher than that of CD+5CD+19(-) CLL (P =0.030).59 cases were examined by conventional cytogcnetic (CC),and 13 cases were with abnormal karyotypes,positive rate was 22.0 %,with complex karyotypes in 5 cases (8.5 %); 10 cases combined with FISH abnormalities karyotype examination rate was 60 %.Conclusion Typical CLL immunophenotypic characteristics were CD5,CD19,CD23 co-expression,and CD-5 CLL with higher misdiagnosis rate,combined with CD20 (,) CD22 expression and karyotype analysis helps to CLL and other B lymphoid proliferative diseases (B-LPD) identification.Conventional cytogenetic detection combined with FISH scan can improve the recognition ability of abnormal chromosome.
10.Apoptosis and oxidative injury of donor islets during isolation and purification

Objective To observe the changes of islet cell apoptosis and oxidation-antioxidation before the transplantation, and to explore the pathways of islet protection. Methods Fifteen human pancreases were perfused with the Hanks solution containing collagenase, then digested and isolated. During the procedure, islet cell apoptosis was detected by TUNEL, SOD and MDA in the pancreas were measured by colorimetric method, and the morphologic changes were observed by H-E staining and dithizone staining. Results In the procedure of human islet isolation, especially in the stage of digestion, the apoptosis of human islet cells occurred. In the stages of perfusion and digestion, the MDA contents reached the high levels (6. 18 ± 2. 38 and 9. 21 ± 2. 75 umol/mg protein respectively),and the structures of the islets and tissues around the islets were damaged. Conclusion In the stages of perfusion and digestion, apoptosis of islet cells can be caused by oxidation. It suggests that antioxidation is a pathway for protection of islets before transplantation.
