2.Expression of Fas,caspase-3 protein and change of apoptosis in neonatal rats with white matter damage

Objective To explore the effect of Fas and caspase-3 on apoptosis in the white matter damage(WMD) neonatal rats.Methods The pups(45 in each group) were perfused at 30 min,1 h,4 h,12 h,1 d,3 d,7 d,14 d,21 d of recovery from hypoxia-ischemia(HI).Immunohistochemical technique was applied to investigate the changes in the expression of Fas and caspase-3 in periventricular white matter tissues.Apoptotic cells were detected in these tissues by TUNEL.Results Apoptosis index(AI) in the experimental group increased significantly at 4 h and reached the peak at 3 d after HI.No expression of Fas was found in the control group.The expression of Fas in the experimental group appeared at 30 min after HI,increased at 1 h,reached the peak at 12 h and lasted till 3 d.Caspase-3 in the experiment group was up-regulated,peaked at 1 d,demonstrating significant differences at 1 h,4 h,12 h,1 d,3 d and 7 d compared with the control group(P
3.Reconstruction for knee dislocation with multiple ligaments injury at stage I.

OBJECTIVETo investigate clinical outcomes of tendon allograft reconstruction with arthroscopy minimally invasive technique at stage I for the treatment of knee dislocation with multiple ligaments injury.
METHODSForty-eight patients with knee dislocation were reconstructed anterior and posterior ligament under arthroscopy at stage I from January 2008 to January 2012, and repaired ligaments injury of knee joint by minimally invasive technique. There were 38 males and 10 females aged from 20 to 59 years old with an average of 35.6 years old; 22 cases on the left side and 26 cases on the right side; the time from injury to operation ranged from 2 d to 2 weeks. Two cases combined with anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), medial collateral ligament (MCL) and posterolateral complex injuries, 36 cases combined with ACL, PCL, and MCL injuries, 10 cases combined with ACL, PCL and PLC injuries; 4 cases combined with peroneal nerve injury. Lysholm scoring were used to compared the cases before operation and final following-up to evaluate knee function.
RESULTSAll patients were followed up from 12 to 30 months with an average of (18.2 ± 6.3) months. Activity and stability of joint were obviously improved. Lysholm score were improved from 40.3 ± 4.1 before operation to 87.0 ± 6.4 at final following-up.
CONCLUSIONReconstruction with arthroscopy minimally invasive technique at stage I for the treatment of knee dislocation with multiple ligaments injury could recover stability of joint better,reserve joint function. Preoperative training and postoperative individualized rehabilitation treatment is the key point of recover knee joint function.

4.Small partial laryngectomy without tracheotomy for T1-2 stage glottic carcinoma.

OBJECTIVE:
To investigate the feasibility and clinical characteristics of small partial laryngectomy without tracheotomy for T1-2 stage glottic carcinoma.
METHOD:
Forty-five patients with laryngeal squamaous cell carcinoma in T1-2 stage received small partial laryngectomy without tracheotomy.
RESULT:
All patients were primarily healed and were hospitalized for an average of 11.5 days post-operatively. In all patients, the function of respiration and the reflection of cough were normal, and laryngeal obstruction did not happen. The only postoperative complication was subcutaneous emphysema noted in 29 patients. Among them, subcutaneous emphysema extincted after 4-6 days in 26 patiens, only 3 patiens suffered from delayed healing because the subcutaneous emphysema extincted after 2 weeks. Mild subcutaneous emphysema did not affect the function of respiration and deglutition, healing of wound, and psychology of patients. All patients had been followed-up for 1-13 years. Only 2 patients died of tumor recurrence or metastasis. The function of respiration and deglutition were normal in the living patients, and no implanting metastasis on surface of trachea were found.
CONCLUSION
The theoretical foundation of small partial laryngectomy without tracheotomy for T1-2 stage glottic carcinoma has been well established. This surgical technique is feasible, safe and effective. It can significantly improve clinical outcome of T1-2 stage glottic carcinoma with minimal invasiveness. Furthermore, it can obviously abate the surgical, physiological and psychological trauma on patients.

5.Questionnaire Survey and Analysis of Cultivation Mode of Autonomous Learning Ability in TCM Formula

Objective To observe the effects of cultivation mode of autonomous learning ability in TCM formula on students’ learning capacity and comprehensive quality. Methods On the basis of inheriting the traditional teaching mode, classroom questioning, class discussion, problem-based learned teaching, simulated prescription exercises, autonomous teaching and training, the mid-term oral test, and autonomous proposition consisted of autonomous learning ability training mode diversification. In the teaching process, teachers' teaching was the primary, and was interspersed with cultivation mode of autonomous learning ability. At the end of the teaching activities, questionnaire survey method was adopted to evaluate the autonomous learning ability training mode in the teaching of TCM formula. Results 81.30%-97.74% students believed that this mode is helpful to improve their learning ability and comprehensive quality; 75.87%-98.49% students believed that it is necessary to carry out the mode in the teaching activities; 69.53% students believed that conducting mid-term examination in the form of oral teat is feasible. Conclusion The cultivation mode of autonomous learning ability for TCM formula can stimulate the learning enthusiasm of the students from various angles, raise interest in learning, arouse students’ learning enthusiasm and initiative, and further cultivate autonomous learning ability and clinical thinking ability, which can provide references for the teaching reform of TCM formula.

7.Expression of bcl-2 protein and apoptosis of neurocytes in neonatal rats with brain white matter damage.

OBJECTIVESome research has shown that the brain white matter damage is closely related to apoptosis of pre-oligodendrocytes. The relationship of bcl-2 protein, a protein of anti-apoptosis, with brain white matter damage in neonatal rats is rarely reported. This study examined the changes of bcl-2 protein expression following brain white matter damage in neonatal rats.
METHODSNinety 2-day-old Sprague-Dawley (SD) rats were randomly divided into 2 groups: experimental group (n=45) and control group (n=45). Brain white matter damage was induced by ligation of the right common artery, followed by 6% hypoxia exposure in the rats from the experimental group. The rats of the control group were sham-operated, without hypoxia-ischemia treatment. The expression of bcl-2 protein in the periventricular white matter and the callositas was detected by immunohistochemical technique. Apoptosis of neurocytes in these tissues was detected by TUNEL.
RESULTSThe apoptosis index of neurocytes in the experimental group was up-regulated at 4, 12 and 24 hrs and at 3 and 7 days, peaking at 3 days after white matter damage, compared with the control group (P < 0.05). The expression of bcl-2 protein in the experimental group began to increase at 1 hr, reached a peak at 12 hrs and remained a higher level until 3 days after white matter damage compared with that observed in the control group (P < 0.05).
CONCLUSIONSThe expression of bcl-2 protein increased at the early stage of white matter damage in neonatal rats. The peak of apoptosis lagged behind that of the bcl-2 protein expression, which suggests that bcl-2 protein may have protective effects against neuronal apoptosis.

8.Cefotaxime-agar medium for laboratory detection of extended-spectrum and AmpC ?-lactamases in Enterobacteriaceae

Objective To develop an easy,rapid and reproducible cefotaxime-agar medium(CTX- AM)for phenotypic detection of extended-spectrum ?-lactamases(ESBLs)and AmpC ?-lactamases (AmpCs)in Enterobacteriaceae.Methods The surface of a cefotaxime(CTX,0.5 ?g/ml)-Mueller- Hinton agar and ceftizoxime(CAZ,1 ?g/ml)-Mueller-Hinton agar plate was inoculated with a lawn of E. coli ATCC 25922 according to the standard disk diffusion method,respectively.Immediately prior to use.blank and clavulanic acid(10 ?g),cloxacillin(300 ?g),clavulanic acid/cloxacillin(10/300 ?g) disk were rehydrated with 10 ?l of saline and several colonies of each test organism were applied to disks. Then the results of CTX-AM method to interpret based on a zone of growth around the periphery of disks.A total 58 of ESBL and AmpC producing and non-producing isolates of Enterobacteriaceae,as identified by the double-disk enhancement test(DDET)and the three-dimensional extract method(TDEM).were used to evaluate the CTX-AM method.Positive control(E.cloacae 029M,K.pneumoniae ATCC 700603)and negative control(E.coli ATCC 25922)strains were included.Results The results of CTX-AM method were similar to the DDET and TDEM method for detecting ESBLs and AmpC production in Enterobacteriaceae,respectively.But inhibitor-resistant ?-lactamase(IR-BLs)and other ?-lactamases were not detected by DDET method.Conclusions The new method described here allows for testing of ESBL and AmpCs on a single plate.It is easy to perform and interpret,and also cost-effective,clinical laboratories may use this technique routinely to detect the oresence of ESBL and AmoCs.

10.The impact of heart rate on image quality and radiation dose of coronary angiography using 640-slice dynamic volume CT

Objective To analyze retrospectively the impact of different heart rates on image quality and radiation dose of coronary angiography using 640-slice dynamic volume CT.Methods A total of 461 patients with suspected coronary artery disease or referred to health check underwent coronary angiography with 640-slice dynamic volume CT.Two groups were created according to their heart rates:Group A had heart rate < 65 beats per minute (n=337)and Group B had heart rate between 65 to 122 beats per minute (n=124).Image quality was assessed by analyzing the 1 5 segments of the main coronary branches using 3-grade scale (grade I-good,grade Ⅱ-acceptable,grade Ⅲ-poor).Effective radiation dose was also evaluated.Results Patients in both A group and B group performed successful CT coronary angiography with a total of 6 91 5 coronary segments.Among them,coronary segments that could be evaluated reliably accounted for 94.5% (6 535/6 91 5)while 5.5% (380/6 91 5)were too small (≤1.5 mm)to be assessed. For the image quality,Group A and Group B had grade I in 90.5% (305/337)vs 74.2% (92 /124),grade Ⅱ in 9.5% (32/337)vs 21.0% (26 / 124)and grade Ⅲ in 0 % (0 / 337 )vs 4.8% (6 / 124),respectively.Image quality was significantly different be-tween Group A and Group B (P <0.001).In addition,32 patients (9.5%)in Group A had slight cardiac motion artifacts but with-out affecting image quality,whereas 26 patients (21.0%)in Group B had higher degree of cardiac motion artifacts thus graded as grade Ⅱ.Stair-step artifacts were not found in all patients.The effective radiation dose was higher in Group B than in Group A by 32.05%(7.91±0.34 mSv vs 5.99±0.17 mSv).Conclusion Coronary angiography using 640-slice dynamic volume CT can guarantee excellent image quality when heart rate < 65 beats per minute.Although the image quality would decrease in some extent it is still diagnostic when heart rate is between 65 to 122(include 65 and 122)beats per minute.
