2.Changes of Serum and Cerebrospinal Fluid Neuron-Specific Enolase,S-100? and Myelin Basic Protein in Children with Epilepsy after Single Episodes

Objective To detect the possibility of brain damage in the epileptic children with single episodes.Methods The serum and cerebrospinal fluid(CSF)levels of neuron-specific enolase(NSE)in 20 cases with single episodes within 24 hours after seizures and 38 cases of controls were determined respectively by electrochemiluminescence,and the levels of S-100? protein(S-100?) and myelin basic protein(MBP)were determined by enzyme-linked immunosorbent assay.Results The levels of NSE in serum and CSF in epileptic group were(15.01?5.14) ?g/L and(7.84?2.62) ?g/L,respectively,and controls were(10.33?2.48) ?g/L,(3.95?1.58) ?g/L.The levels of S-100? in serum and CSF in epileptic group were(0.39?0.15) ?g/L and(0.59?0.20) ?g/L,respectively,and controls were(0.11?0.05) ?g/L,(0.29?0.19) ?g/L.The levels of MBP in serum and CSF in epileptic group were(0.23?0.09) ?g/L and(0.33?0.07) ?g/L,respectively,and controls were(0.23?0.06) ?g/L,(0.31?0.07) ?g/L.The serum and CSF levels of NSE,S-100? in epileptic group within 24 hours after seizures were significantly higher than those in control group(all P0.05).Conclusions The levels of NSE and S-100? in serum and CSF can reach higher levels within 24 hours after seizures,neuronal damage may result from single episodes.
3.Clinical study of late-course hyperfractionated radiotherapy in the treatment of local advanced non-small cell lung cancer

Purpose:To evaluate the clinical effect of late-course hyperfractionated (LCH) radiotherapy and conventionally fractionated (CF) radiotherapy for local advanced non-small cell lung cancer (NSCLC). Methods:From August 1995 to April 1998, 74 patients with inoperable stage Ⅲ NSCLC were randomized into two groups: CF and LCH. 37 patients in CF group received a fraction of 2 Gy daily, 5 days per week, to the total dose of 65~70 Gy /6.5~7 weeks. 37 patients in LCH group were treated with the same fractionation as CF group to the dose of 40 Gy, and then followed by LCH radiotherapy: 1.2Gy, twice daily, with the interval of 6 hours between fractions, to the total dose of 64 Gy/6 weeks. Objective To investigate the effect of stellate ganglion block ~SGB) on formalin-induced nociceptive behavior of rabbits, the expression of brain-derived neurotrophic factor ~BDNF) of dorsal root ganglia ~DRG) and the concentration of serum interleukin-8 ~IL-8) in rabbits. Methods Twenty-four rabbits were used in this study. Catheters were inserted closely to right stellate ganglia of the animals, which were randomly divided into control group ~A), SGB group ~B) and pain model group ~C). Group B and C were administered with an injection of 0.5ml of 3% formalin into the right front paw subcutaneously, group A 0.5ml saline by the same way. 0.5ml of (0.25)% bupivacaine was administered via the catheter in group B at 10 minutes before injection, while group A and C were given 0.5ml of saline at the same time. Nociceptive behavior was observed for 60 minutes after injection, and assessed by weighted pain score. Serum IL-8 was measured radioimmunologically at 10 minutes ~T_0) before injection, 10 ~T_1), 60 ~T_2) and 120 minutes ~T_3) after injection. The expression of BDNF in the right DRG of cervical 7, 8 and thoracic 1 was investigated immunohistochemically at 120 minutes after injection. Results Formalin-induced nociceptive behavior in phase Ⅱ was relieved following SGB. 5.Arthroscopic hybrid suture technique for repairing 20 bucket-handle tear of inside meniscus

Objective To investigate the arthroscopic inside-out repairing technique(hybrid suture technique) for bucket-handle tear of inside meniscus and present the short-term clinical outcome. Methods Twenty patients with 20 repairable inside meniscus tears were enrolled. With arthroscopic technique,the midbody portion and anterior portion of the lesion were repaired using standard inside-out technique with vertical and interlacing tibial and femoral side arrangement. For the most posterior portion, all-inside suture technique with two posterior portals were performed. For isolated red-white zone tears,fibrin clot was implanted into the repaired region for enhancing the healing pro-ACL injuries were reconstructed simultaneously. Results Twenty patients with 20 meniscal repairs were available for 5-36 (19.2 ± 2.3)months follow-up. Eighteen cases were asymptomatic at the latest clinical evaluation,and joint space tenderness in 2 cases. Twenty cases received MRI evaluation during follow-up, showing that 15 menisci were completely healed, 5 partially healed. Postoperative Lysholm scores of the knee joint function [82-97(90.3 ±1.5) points] were significantly improved compared with their preoperative scores [45-69 (56.7 ± 2.0)points] (P = 0.024).Conclusion For meniscal tears involving red-red and red-white zones,arthroscopic hybrid suture technique provides stable strength along the whole length of injured region, and consequently achieves satisfactory clinical outcomes.
7.Minimally invasive aortic valve replacement surgery and early follow-up results

Objective The purpose of this study was to evaluate the safety and clinical oulcomes of aortic valve replacement (AVR) performed with minimally invasive technique.Methods From June 2010 to October 2011,20 cases of minimally invasive AVR were performed.The mean age was (47.60±12.28) ;12 males and 8 females.All patients are ventilated with a double-lumen endotracheal tube,through the 3nd anterior intercostals space with a 5 -6cmskin incision,right femoral artery and vein cannulation are used to establish CPB,direct aortic cross-clamped by Chitwood sliding clamp through the right 4th intercostals space,and completed the aortic valve replacement.Results Mean length of incision was (4.73±0.54)cm.Mean duration of cardiopulmonary bypass was (124±39.83)min,crossclamp time was (97.21±33.17) min.Median intubation time was (13.55±3.87)hours.Median duration of intensive care and postoperative hospital stay was (16.34±3.82)hours and (6.63±1.45) days,respectively.Hospital mortality was 0.There was no perivalvular leakage,Conclusion Minimally invasive aortic replacement with a modified Port-Access approach is feasible,small incisions,more cosmetic,shorter length of bospital stay and less need for blood transfusion are attainable.

9.Clinical observation of Shu-acupuncture method in Nei Jing (Classic of Internal Medicine) for shoulder and arm pain

Objective: To observe the clinical effects of Shu-acupuncture method in Nei Jing (Classic of Internal Medicine) in the treatment of shoulder and arm pain. Methods: A total of 90 patients with shoulder and arm pain were randomly divided into an observation group and a control group, 45 cases in each group. The treatment of Shu-acupuncture method in Nei Jing (Classic of Internal Medicine) was adopted in the observation group, routine acupuncture was used in the control group. The two groups were treated once every day, with 5 treatments as one course, and a 2-day rest between two courses. After 3 courses, pain was assessed by visual analog scale (VAS), and the clinical effects were compared between the two groups. Results: After the treatment, VAS scores were significantly changed in both groups (both P<0.01). The VAS score was lower in the observation group than that in the control group, with a statistical difference between the two groups (P<0.05). The total effective rate was 100% in the observation group, versus 91.1% in the control group, the difference was statistically significant (P<0.05). Conclusion: The therapeutic effect of Shu-acupuncture method in Nei Jing (Classic of Internal Medicine) is better than that of routine acupuncture in treating shoulder and arm pain.
10.Treatment of neck type cervical spondylopathy with Zheng's Jin Gou Diao Yu (gold-hook-fishing) acupuncture method: a randomized controlled trial

Objective: To observe the clinical effect of Zheng's Jin Gou Diao Yu (gold-hook-fishing) acupuncture method versus ordinary acupuncture on superficial fascia for treating neck type cervical spondylopathy in Kyrgyz. Methods:A total of 64 Kyrgyz patients conforming to the diagnostic criteria of neck type cervical spondylopathy were included. The patients were randomized into a Zheng's Jin Gou Diao Yu (gold-hook-fishing) acupuncture group and a conventional acupuncture group, with 32 cases in each group. Patients in the Zheng's Jin Gou Diao Yu (gold-hook-fishing) acupuncture group were treated with Zheng's Jin Gou Diao Yu (gold-hook-fishing) acupuncture method to stimulate the superficial fascia, the stimulation sites were primarily located at bilateral sides of the cervical vertebra as well as the trigger points in shoulder-neck region; patients in the conventional acupuncture group were punctured at the same acupoints, with twirling reducing method, and the depth of insertion was determined by the treated region. Patients in both groups received treatment every day for a succession of 5 d as a course, with a 2-day interval between 2 courses, and the whole treatment lasted for 3 courses. After 3 courses of treatment, the McGill pain questionnaire (MPQ) and neck disability index (NDI) were measured to compare the clinical effect between the two groups. Results: After treatment, scores of MPQ and NDI scale dropped when compared with those before treatment, and the differences showed statistical significance (allP<0.05); scores of MPQ and NDI in the Jin Gou Diao Yu (gold-hook-fishing) acupuncture group were substantially lower than those in the conventional acupuncture group, and the differences showed statistical significance (allP<0.05). The total effective rate was 96.9% in the Jin Gou Diao Yu (gold-hook-fishing) acupuncture group and the cured rate was 78.1%, which were higher than 81.3% and 40.6% in the conventional acupuncture group, and the differences showed statistical significance (bothP<0.05). Conclusion: Zheng's Jin Gou Diao Yu (gold-hook-fishing) acupuncture method by stimulating superficial fascia to treat cervical spondylopathy in Kyrgyz can produce a better clinical effect than conventional acupuncture treatment, and is effective in improving pain and stiffness in patients, and thus is worth clinical popularization.
