Objective To understand the death level and main death causes among residents in the Wansheng Economic De‐velopment Zone to provide the basis for comprehensive prevention and control .Methods According to the ICD‐10 international classification of diseases ,the death causes were classified and coded for statistically analyzing the crude mortality rate ,standardized mortality rate and the death causes constituents in this surveyed population .Results The crude mortality rate of residents in Wan‐sheng Economic Development Zone during 2014 was 650 .57/100 000 and the standardized mortality rate was 413 .35/100 000 .The sequence of death causes was circulatory system disease ,respiratory system disease ,tumor ,injury and poisoning and digestive sys‐tem diseases .The death number for these 5 kinds of death causes accounted for 93 .21% of the total death number .The top five of potential life loss years were injury and poisoning ,tumor ,circulatory system disease ,respiratory system disease and digestive system diseases .The rates of potential years of life loss were 13 .76‰ ,10 .19‰ ,8 .35‰ ,4 .84‰ and 2 .69‰ respectively .Conclusion Chronic noncommunicable diseases ,injury and poisoning become the main causes of death in the Wansheng Economic Development Zone and disease burden .

AIM: To evaluate the clinical effect and the rotational stability of AcrySof Toric intraocular lens ( IOL ) implantation to correct preexisting corneal astigmatism in cataract surgery.
METHODS: Twenty-three patients ( 28 eyes ) were enrolled from the department of ophthalmology in the first Affiliated Hospital of Nanjing Medical University. All patients underwent similar phacoemulsification procedure combined with AcrySof Toric IOL implantation from June 2012 to December 2013. The uncorrected visual acuity ( UCVA ) , best corrected visual acuity ( BCVA ) , anticipated residual astigmatism, postoperative residual astigmatism, and Toric IOL axis were detected and measured.
RESULTS:Three months after operation, the UCVA of all eyes were 0. 75±0. 16 and the BCVA were 0. 84±0. 15, there was no significant difference between UCVA and BCVA ( t = 1. 036, P>0. 05 ). The anticipated residual astigmatism was ( 0. 28±0. 12 ) D. The actual residual astigmatism after 3mo of the operation was (0. 42±0. 17) D. There was no significant difference between anticipated and actual residual astigmatism (t=1. 259, P>0. 05). The mean axis rotation of Toric IOL was 3. 02o±1.56o (0o-7o) after 1d of operation and 3. 28o±1. 85o (0o-7o) after 3mo. Among all the eyes, 25 eyes ( 89%) rotated <5o, in 3 eyes (11%) rotated 5o-7o.
CONCLUSION: The AcrySof Toric IOL implantation shows good effectiveness, predictability and stability in correcting pre-existing astigmatism in cataract patients.

Objective To observe the effect of calycosin on cisplatin?induced inhibition of human gastric BGC823 cells. Methods BGC823 cells were treated with different drugs:saline,cisplatin,calycosin and cisplatin combined with calycosin. MTT assays were used to detect the prolif?eration rate of BGC823 cell. Then the protein level and RNA level of cyclin D1,CDK4 and CDK6 were detected by Western blotting and Real?time PCR. Results The proliferation inhibition rates of BGC823 cells treated with 20μg/mL cisplatin,10μg/mL meperoflavone,and combina?tion of the 2 drugs were 56.44%± 2.08%,9.52%± 2.77%and 74.44%± 0.82%,respectively. The inhibition rate of the combination of drugs was significantly higher than that of the single drug treatment group(P<0.05). In addition,we found that calycosin can significantly enhance the inhi?bition of Cyclin D1,CDK4 and CDK6 by cisplatin in protein level and RNA level. Conclusion Calycosin can significantly increase the inhibitory rate of cisplatin on BGC823 cell proliferation,and the combination of the two drugs can reduce the side effects of cisplatin.

A retrospective study was conducted based on the clinical data of 15 choledocholith patients after Billroth-Ⅱgastroenterestomy who were treated with endoscopic retrograde cholangio-pancreatography (ERCP) through colonoscopy or duodenoscopy.It showed that choledocholith removed by colonoscopy for patients with Billroth-Ⅱ gastroenterestomy can achieve comparable therapeutic effect,but this method can reduce the operation difficulty,shorten the operation time and significantly increase the success rate.

OBJECTIVE: To investigate the feasibility and results of laryngeal functional preservation surgery for piriform sinus carcinoma with paraglottic space involvement. METHOD: Fourteen cases of piriform sinus carcinoma with paraglottic space involvement were reviewed. Laryngoplasty was performed after tumor was resected, bilater al selective neck dissections were performed at the same stage. Postoperative chemoradiotherapy was applied to 11 patients while the other 3 patients did not receive chemotherapy nor radiotherapy. The follow-up time ranged from 6 months to 5 years, and the mean follow-up time was three and a half years. RESULT: Twelve cases healed primarily while 2 cases experienced delayed heal because of pharyngeal fistula. Postoperative swallow bucking occurred in all patients, the longest gastric feeding time was 2 months. For the patients underwent postoperative chemoradiotherapy, the tumor did not recur in ten cases during the 3 years follow-up, while recurred in another case and the patient died without further treatment. Among those 3 cases without postoperative radiotherapy or chemotherapy, the tumor recurred in 2 cases during the following 6 months and chemoradiotherapy was applied, no recurrence presented in one patient, another one lost of follow-up. For the patient with recurred tumor 8 months after surgery, total laryngectomy was employed with postoperative chemoradiotherapy, the case died of tumor recurrence one and half year after the first surgery. The 3-year survival rate was 78.6%. For the vocal cord at lesion side, the mobility was noticeable in 6 patients, and visible anatomic landmark was lost in the other 8 patients. CONCLUSION Laryngeal function preservated surgery could be performed in selected cases of piriform sinus carcinoma with para-glottic space involvement in order to achieve better clinical results and quality of life.

Objective To evaluate the effect of morphine on the growth of subcutaneous tumor of human gastric cancer cells in nude mice.Methods Thirty SPF male BALB/C nude mice,aged 4-5 weeks,weighing 15-20 g,in which the model of subcutaneous tumor of human gastric cancer cell line MGC-803 was established,were randomly divided into 3 groups (n=10 each) using a random number table:control group (group C),normal saline group (group N),and morphine group (group M).The mice in group C received no treatment.Morphine 20 mg/kg was intraperitoneally injected once a day for 14 consecutive days in group M,while normal saline 1.5 ml/kg was given instead of morphine in group N.The caliper was used to measure the tumor size every 2 days starting from 3 days after beginning of administration,and the relative tumor volume was calculated.The nude mice were sacrificed on 15th day,and the tumor tissues were obtained for determination of nuclear factor-kappa B activity and Bcl-2 and Bax protein and mRNA expression by real-time reverse transcriptase polymerase chain reaction and Western blot.Results Compared with group C,the relative tumor volume was significantly decreased,the activity of nuclear factor-kappa B in tumor tissues was significantly decreased,the expression of Bcl-2 protein and mRNA was significantly down-regulated,and the expression of Bax protein and mRNA was significantly up-regulated at each time point in group M (P＜0.05),and no significant change was found in each parameter mentioned above in group N (P＞0.05).Conclusion Morphine can inhibit the growth of subcutaneous tumor of human gastric cancer cells in nude mice,and the mechanism is associated with promotion of apoptosis in tumor cells.

BACKGROUND:The position of median and radial nerves corresponding to elbow is usual y estimated by macroscopical measurements, but there are big data errors when the elbow is bent. OBJECTIVE:To measure and analyze the impacts of elbows in different positions on median and radial nerves under X-ray. METHODS:Ten fresh frozen upper limb specimens were selected. The distance of median and radial nerves to the osseous marks inside the elbow was measured when the elbow was in extension, extension-half pronation, bent at flexion 90 degrees and bent at 90 degrees with the injection of normal saline into the joint capsule, respectively. RESULTS AND CONCLUSION:The positions of median and radial nerves were not influenced when the elbow was in extension or in extension-half pronation (P>0.05). The distance of median and radial nerves to the osseous mark of the elbow was significantly lengthened when the elbow was moved from from extension to was bent at 90 degrees (P<0.05). The intra-articular injection of normal saline when the elbow flexion 90 degrees significantly elongated the distance of median and radial nerves to the osseous mark of elbow (P<0.05). These results suggest that the elbow in different positions can induce the displacement of median and radial nerves. In addition, the distance between median, radial nerves and the osseous marks of the elbow is obviously lengthened when the elbow is in flexion with intra-articular injection of normal saline.

Objective To explore the clinical effect of early cognitive rehabilitation on cognitive,physical function and quality of life in the patients transferred from intensive care unit (ICU).Methods A total of 120 cases of patients who were transferred from ICU to general wards was randomly divided into the control group and the observation group with 60 cases in each group.The control group was given symptomatic treatment according to their original disease,without cognitive intervention.On the basis of the control group,the observation group was treated with cognitive rehabilitation training,2 times / week,2 h/ times,12 weeks of treatment.The cognitive function,physical function and quality of life of two groups were compared with the memory and executive screening scale (MES),the daily activity scale (ADL),and the concise Health Survey (SF-36) before and after treatment.Results Compared to before treatment,after treatment,MES,SF-36 (in addition to bodily pain) and ADL scale project score in the observation group was significantly higher,SF-36 bodily pain scores decreased significantly,all the differences were statistically significant (P ＜ 0.05);Compared to the control group,after treatment,the MES,SF-36 (in addition to bodily pain) and ADL scale scores in the observation group of were significantly increased,the bodily pain score of SF-36 in the observation group compared with the control group decreased significantly,all the differences were statistically significant (P ＜ 0.05).Conclusions Early cognitive rehabilitation can significantly improve the cognitive,physical function and quality of life in the patients transferred from ICU,and it is worthy of clinical reference.
