2.Logistic regression analysis on fatal risk factors to patients with sepsis

Objective To study the fatal risk factors to the patients with sepsis.Methods The data of 49 patients with sepsis were analyzed,who were consecutively admitted to intensive care unit(ICU)of Peking Union Medical College Hospital within six months.Patients' general states,clinical information,human leucocyte antigen-DR(HLA-DR)in blood monocytes and plasma level of diamine oxidase(DAO)were recorded.Simple logistic regression was done to analyze all the risk factors,and statistically significant variables were selected and used in multivariate and unconditioned Logistic regression analysis.The primary end point was hospital mortality.Results The subjects involved in present study were consisted of 49 patients with sepsis admitted to ICU of the Peking Union Medical College Hospital from Aug.2003 to Jan.2004,averagely aged 60.8?16.9 years;including 32 males(65.3%)and 17 females(34.7%);with APACHEⅡscores 18.6?8.0 and SOFA scores 5.9?3.8.Twenty-one out of the 49 patients(42.9%)were from non-operation department,and the other 28 patients(57.1%)were from operation department.ICU duration was 14.4?12.8 days;eighteen of the 49 septic patients died during hospitalization,with the 28-days mortality of 36.7%.Positive blood cultures(OR=5.237,95%CI 1.041~26.34,P=0.045),decreased platelet count and blood HLA-DR(OR=0.943,95%CI 0.901~0.98,P=0.01),and increased plasma levels of DAO(OR=2.235,95%CI 1.101~4.53,P=0.026)were obviously associated with mortality.The positive blood cultures,decreased blood HLA-DR and increased plasma levels of DAO were independent fatal risk factors for the patients with sepsis.Conclusions The positive blood cultures,decreased blood HLA-DR and increased plasma levels of DAO are high risk for the patients with sepsis at admission.

4.Corneal high order aberrations in adults with myopia:correlations with myopic degree and keratometry

?AIM: To investigate the corneal high order aberrations in adults with myopia and to study its correlations with myopic degree and keratometry.?METHODS: A total of 63 patients ( 126 eyes ) from the ophthalmology department of our hospital were included in this study during May 2014 to June 2015. Visual acuity, intraocular pressure ( IOP ) , diopter, slit lamp examination, central corneal thickness ( CCT ) , funduscopic examination, corneal topography and corneal total higher order aberrations ( THOAs ) were recorded. Right eye was selected. According to the spherical equivalence ( SE) , 63 eyes were divided into two groups, the mild to moderate myopia group (M-M group, SE<-6. 00D) and the high myopia group (H group, SE≥-6. 00D). The correlations of corneal THOAs with diopter, keratometry, IOP and CCT were analyzed. Kolmogorov-Smirnov Test was used for normality test. Independent-samples t test was used for comparison between two groups. Pearson correlation analysis was for comparison between two continuous variables.?RESULTS: Z44 was found a statistic difference between the M-M group and H group (t=2. 51,P=0. 02). IOP was negatively correlated with Z5-3(r= -0. 52,P=0. 01), and CCT was positively correlated with Z4-4(r=0. 43,P=0. 03) in H group. There was no correlation between higher order aberrations and SE in M-M group ( all P>0. 05 ) Total coma aberration ( TCA ) was negatively correlated with SE (r=-0. 57,P=0. 002) and spherical degree (Sph, r=-0. 55,P=0. 003) in H group. Z40 was positively correlated with average K (AK) (M-M group:r=0. 40,P=0. 02. H group:r=0. 55, P=0. 03), K steep (Ks) (M-M group:r=0.39, P=0.02. H group:r=0.51, P=0.01) and K flat (Kf) ( M-M group:r=0. 40, P=0. 02. H group:r=0. 57, P=0. 01) in both groups. Z53was negatively correlated with Ks(r=-0.4, P=0. 04) in H group.?CONCLUSION: The THOAs parameters are influenced by diopter and keratometry. TCA is negatively correlated with SE and Sph in patients with high myopia, and spherical aberration is positively correlated with keratometry.

Objective To explore the effect and mechanism of wortmannin on the human gastric carcinoma cells. Methods SGC7901 cells were treated with 10nmol/L,30nmol/L and 60nmol/L of wortmannin,a specific inhibitor of AKT,for different time periods. Cell viability was estimated by MTT assay. Western blot was used to detect the level of NF-?B neucleoprotein and reverse transcriptasepolymerase chain reaction(RT-PCR) was used to determine transcription of NF-?B mRNA. Results All three different concentrations of wortmannin could inhibit the growth of SGC7901 cells,and the depression effect obviously depended on time and drug dose(P0.05). The mechanisms of immunosuppression of TWH were studied. The results indicated that TWH was able to reduce the antibody forming cells of splenocytes in mice and to suppress directly the proliferation of B cells to lipopolysaceharide ( LPS ) and to decrease the production of IL-2. Furthermore, TWH was able to produce immu-nosuppressive effect by activiting Ts cells.
8.CT in the Differential Diagnosis of Atypical Renal Angiomyolipoma and Renal Carcinoma

Objective To investigate the CT appearances of atypical renal angiomyolipoma to raise the CT diagnosing level and search for the differential points from renal angiomyolipoma. Methods Twenty-one patients with atypical renal angiomyolipoma and 24 patients with renal carcinoma underwent CT scan and enhanced scanning. Results CT results showed hyperintense in 12 of all 21 lesions of atypical renal angiomyolipoma,with iso-intensity in 6,hypointense in 3,and levering-cortex-up sign in 9 cases. The tumor body showed homogeneous enhancement on CT enhanced scanning and prolonged enhancement was noted in 7 cases. CT showed hyperintense in 3 of all 24 lesions of renal carcinoma,with mixed intensity in 5,hypointensity in 16,calcification in 2 cases. The majority of the tumor body showed heterogeneous enhancement on CT enhanced scanning with cortical hyperintensity and medullary low-intensity,and with contrast appeared fast wash-in and wash-out. Of all cases,8 presented with pseudo-capsule,7 with tumor embolus formation in the renal vein and inferior vena,10 with retroperitoneal lymphadenectasis,and 3 with intrahepatic metastasis. Conclusion CT scanning proves to play critical role in differentiating the atypical renal angiomyolipoma from renal carcinoma.

10.Quantitation of IFN-? mRNA in duck PBMC and its application

Objective IFN-? is a pleiotropic cytokine with potent immunomodulatory effects and antiviral activity. To study the mechanism of IFN-? clearing duck hepatitis B virus (DHBV) in ducks, it is essential to establish a method to quantify expression of DuIFN-? in immune response. In the present study,a semi-quantitative competitive RT-PCR was developed to quantify expression of duck IFN-?(DuIFN-?) mRNA by PBMCs. Methods Based on ?-actin consensus sequence, fishing the ?-actin gene as house keeping gene from duck PBMC by RT-PCR. A competitive internal control was constructed and the competitive RT-PCR system could be used to quantify the transcription of DuIFN-? mRNA. Results After duck PBMCs were stimulated in vitro with PHA, the peak of DuIFN-? expression was at 24-36h. Then RT-PCR method was applied to detect DuIFN-? mRNA transcription by PBMCs from DHBV infected ducks immunized with DuIFN-? plasmid plus DNA vaccine or DNA vaccine alone. Results showed that expression of DuIFN-? in ducks co-immunized with DuIFN-? plasmid were higher than other groups immunized without DuIFN-? plasmid as adjuvant. Conclusions The results indicated that DuIFN-? gene could be a useful adjuvant to develop vaccines. The semi-quantitation of DuIFN-? mRNA by competitive RT-PCR provides the basis for future study of the mechanism of IFN-? in duck hepatitis B virus persistent infection.
