3.Analysis of diagnosis and treatment of 89 patients with primary hyperparathyroidism

Objective To summarize the clinical manifestations,diagnosis and treatment of the primary hyperparathyroidism (PHPT).Methods The clinical data of 89 patients with PHPT admitted during 1991 to 2013 were retrospectively analyzed.Results The mean age of the 89 patients was (44.7 ± 15.8) years (13-86 years),male to female ratio was 1 ∶ 2.1.79% of the 89 patients were symptomatic.62% of the 89 patients had been misdiagnosed.52% had been misdiagnosed as kidney stones or bone diseases (fracture,tumor,and metabolic bone disease).The remaining subjects had been misdiagnosed as peptic ulcer,rheumatoid arthritis or pancreatitis.The median serum parathyroid hormone (PTH) level in 88 patients was 697.3 pg/ml (26.4-5 000.0 pg/ml,n =88),the mean serum calcium level was (3.03 ± 0.56) mmol/L (n =88).The 24 h urine calcium was (10.02 ± 4.73)mmol/24 h (n =36).The sensitivity of localization of the parathyroid lesions with technetium-99m methoxy-isobutylisonitrile (99mTc-MIBI) was 96%,being higher than ultrasound (71%),CT (71%),and MRI (72%) (P<0.05,respectively).79 cases had been treated surgically.The median serum PTH level was 748.2 pg/ml (46.1-5 000.0 pg/ml) before surgery,79.9 pg/ml (11.35-838.0 pg/ml) 15 minutes after surgical removal of the lesions (P<0.05 vs preoperative) and 22.9 pg/ml (2.3-1 203.0 pg/ml) on the first postoperative day (P<0.05 vs preoperative).Serum calcium was (3.03 ± 0.56) mmol/L before surgery,(2.25 ± 0.37) mmol/L on the first postoperative day (P<0.05 vs preoperative) and (2.08 ± 0.36) mmol/L on the second postoperative day (P< 0.05 vs preoperative).After the operation,42 cases(53%) had perioral and limb numbness,2 cases (3%) had hoarseness of voice and bucking while drinking,1 case(1%) had tetany.All these symptoms were improved after calcium supplementation.67 patients (85%) had been diagnosed as parathyroid adenoma pathologically,7 patients (9%) as hyperplasia,3 patients (4%)as cysts,and 2 patients (3%) as carcinoma.Among 67 cases of adenomas,62 cases had a single parathyroid adenoma,1 case with multiple parathyroid adenomas,and 4 cases with ectopic parathyroid adenomas.The ectopic lesions were located below the thyroid,in the rib cage,left mediastinum,and above the sternum.Conclusions The majority of domestic PHPT was symptomatic.Delayed diagnosis of PHPT is still very frequent.The clinical presentations of PHPT are variable,determination of serum calcium and PTH level simultaneously is the keypoint for early identification and diagnosis of PHPT.The clinician should be familiar with the various clinical manifestations of PHPT and thus enhance understanding of pathogenesis of the disease.99mTc-MIBI scintigraphy remained the first choice of preoperative imaging localization of the parathyroid lesions,followed by ultrasound,CT,and MRI.Surgical removal of the lesions is effective and safe.

Ultrasonography is a simple and reliable non-invasive method in tumor vascularity detection of breast neoplasms. Tumor vascularity morphometric and topological characteristic in breast neoplasms provides an important basis for differentiation between benign and malignant lesions. Studies indicate that visual blood vessel shape and direction, velocity, properties of blood flow can be satisfactorily shown by color Doppler flow imaging(CDFI). But CDFI is confined by the angle between acoustic beam and blood flow. Power Doppler ultrasound (PDU) can overcome the difficulties mentioned above, but can not offer the information of blood velocities and motion error. The intratumoral flow features can clearly and visually shown by three-dimensional ultrasonography (3DUS) and contrast- enhanced ultrasound (CEUS). 3 DUS and CEUS will be new potential diagnosis techniques in vascularity estimation of breast neoplasms.

7.Role of carvedilol on energy metabolic reversion and rem odeling of pressure overload-induced left ventricular hypertrophy in rats

AIM: To study changes of medium chain acyl- Co A dehydrogenase (MCAD), muscle carnitine palmitoyltransferase I (M-CPT-I) and co lligin mRNA/protein expression, to elucidate molecular mechanism of the recapit ulation of fetal energy metabolism and ventricular remodeling and the effects o f carvedilol during the development of pressure overload-induced left ventricula r hypertrophy in rats. METHODS: Male Wistar rats of hypertrophy induced by constrictio n of abdominal aorta (CAA) were randomized into 2 groups (n=12, each group): 4-week group (CAA 4 weeks group) and 12-week carvedilol intervention gro up (CAR group). Additi onal rats (n=12) underwent abdominal cavity incision without ligation to ser ve as age-matched sham operated controls (SH). Hemodynamics, ventricular remodel ing parameters and free fatty acid (FFA) both in blood serum and myocardium we re measured. RT-PCR analysis of the expression of mRNA of M-CPT-I, MCAD and col lagen binding protein (colligin) were investigated. The protein expression of co lligin was analyzed by Western blotting in the experimental animals and sham op eration.RESULTS: LVM/BW and MAP in CAA group were increased more signif icantly than in sham group. There were progressive increases in FAA both in bloo d serum and myocardium in CAA group than in sham group, accompanied with downreg ulation of gene expressions of M-CPT-I and MCAD and colligin mRNA/ protein upre gulation in LV in CAA group, while changes of all of these parameters in CAR gro up were attenuated. CONCLUSIONS: (1) The down-regulated expression of cardiac FAO en zyme genes (M-CPT-I and MCAD) in the hypertrophied heart may be responsible for "the recapitulation of fetal energy metabolism" during the development of pres sure overload-induced left ventricular hypertrophy in rats. (2) Carvedilol atten uates the reversion of the metabolic gene expression back towards fetal type. (3 ) Carvedilol is effective in regressing the left ventricular remodeling by inhi biting colligin protein expression. A molecular mechanism by which carvedilol ma y confer cardioprotective effects in heart failure may be, in part, via preservi ng o f the adult metabolic gene regulation and regressing left ventricular remodeling .

AIM: To study changes of expression of muscle LIM protein(MLP) mRNA and protein during the progression of left ventricular hypertrophy induced by coarctation of abdominal aorta in rats and to explore if there is disruption of cytoskeleton protein during hypertrophic growth in left ventricle. METHODS: Using a model of hypertrophy induced by coarctation of abdominal aorta(CAA) in male Wistar rats, hemodynamics, ventricular hypertrophic index, expression of MLP mRNA and protein were investigated in the experimental animals at 1, 4, 8, 16 weeks after operation and sham operation. RESULTS: Left ventricular shows significantly hypertrophy 4 weeks after operation ( P0.05 ). CONCLUSIONS: MLP changed at transcription level and there was no disruption of cytoskeleton during hypertrophic growth of left ventricle without sever heart failure. As the actin-base cytoskeleton protein, MLP plays a critical role in the maintenance of cardiac myocyte cytoskeleton and cardiac systolic function.

The mammalian fetal heart relies primarily on glucose and pyruvate as substrates for ATP production, and it is rapidly transformed to fatty acid ?-oxidation (FAO) postnatally. However, energy metabolic pathways revert to a fetal pattern, when cardiac hypertrophy and heart failure developed. This return was called recapitulation, ultimately it is maladaptive for the body. The process of FAO was performed under a precise regulating system, nuclear transcription factors such as PPAR?, Sp1/3, Coup-TF(chicken ovalbumin upstream promoter transcription factor) all take part in the regulation of genes transcription of FAO enzymes. It was identified that the reguction of PPAR? activity in hypertrophic myocardium due to pressure overload might result in down-regulation of gene expression of FAO enzyme. The mechanism involved in reinduction of a fetal gene transcription participated in the regulation of myocardial energy metabolism in the development of cardiac hypertrophy resulting from pressure overload.

Objective To investigate alterations in epigenetic regulation in B cells in subacute cutaneous lupus erythematosus (SCLE).Methods Peripheral blood mononuclear cells (PBMC) were isolated from the peripheral venous blood of SCLE patients and healthy donors with density gradient centrifugation.B cells were isolated with CD19 micro beads and protocols provided by the manufacturer.Total RNAs were isolated with RNA kits.The expressions of silence information regulator 1 (Sirt1) mRNA were investigated with quantitative real time polymerase chain reaction (PCR).Results The levels of Sirt1 mRNA were significantly increased in SCLE B cells relative to healthy controls (P < 0.01).Conclusions Histone modifications appear abnormal in B cells in SCLE.
