1.Bioinformatic Analysis of Differentially Expressed Genes in Peripheral Blood of Human Immunodeficiency Virus/Tubercle Bacilli Co-infected Patients.

Objective To analyze the differentially expressed genes in peripheral blood of human immunodeficiency(HIV)/tubercle bacilli co-infected patients and explore the biological regulatory mechanism and network of key proteins,so as to provide new evidence for early diagnosis and clinical treatment of HIV/TB co-infected patients. Methods Microarray gene chip data of HIV/TB co-infected patients were downloaded from public databases GEO and imported into the analysis software GEO,STRING,PANTHER,and GenClip. The gene expression profiles,protein interaction networks,processes of molecular biology,and gene functions were analyzed. Results The expression profiles of 15 529 genes between the two groups of patients were similar,and gene expression profiles from 44 subjects were highly correlated. The 251 differentially expressed genes had good diagnostic capabilities in the differential diagnosis of HIV/TB infection. RPLP1 might be a key gene in the diagnosis of HIV/TB infection. The differentially expressed genes and positive regulators showed certain functions such as external stimuli,signal transduction pathways in cells,migration of neutrophils,and immunological and other relevant functionalities. Meanwhile,they may also be involved in free radical-related apoptosis,inflammation,and activation pathways. Conclusions A total of 251 differentially expressed genes are found to be able to distinguish simple HIV infection from HIV/TB infection. Protein-protein interaction network of top 40 differential expression genes includes RPLP1 gene,which is possibly associated with HIV/TB co-infection and may be involved in and the positive regulation of external stimuli,signal transduction pathways in cells,migration of neutrophils,and immunological functions. These findings may provide certain evidence for the diagnosis and treatment of HIV/TB infection.
2.Multi-slice spiral CT features of crush pulmonary parenchymal injuries in the massive Wenchuan earthquake

Objective To evaluate the multi-slice spiral CT (MSCT) features of crush pulmonary parenchymal injuries in the massive Wenchuan earthquake. Methods One hundred and seventeen patients with crush pulmonary parenchymal injuries in the massive Wenchuan earthquake were reviewed, the location, extent and MSCT appearances of the injuries and associated thoracic injuries were evaluated. The MSCT appearances of the injuries on 1 d, 2--5 d and 6--14 d were compared using Chi-square test. Results One hundred and thirteen patients had pulmonary contusion and bilaterally in 70 cases, 7 had pulmonary laceration. A total of two hundreds and eighty-three lung lobes were involved and 3 or more lobes involved in 47 patients. Ill-defined patchy opacity was detected in 75 patients, associated ground-glass opacity (GGO) in 24 cases. Lung laceration presented as pneumatocele in 4 cases and demarcated hematoma in 3 cases. Coexisted rib fractures were detected in 98 patients and flail chests were in 29 patients. Forty-three patients had coexisted fractures of scapula, clavicle and (or) sternum. Ninety-nine patients had coexisted pleural injuries. Conclusions Crush pulmonary parenchymal injuries in the Wenchuan earthquake are severe. The bilateral, multifocal and multiple lobes involved injuries with a high incidence of coexisted other severe thoracic injuries are the noticeable features.
3.Association of lumbar disc herniation with the interaction between environmental factors and Fas/FasL/Caspase-9 apoptosis-associated gene polymorphisms

Objective To investigate the etiologic roles of apoptosis-associated genes,environmental factors and their interactions in lumbar disc herniation (LDH).Methods A case-control trial was conducted.We recruited 128 outpatients with LDH as case group and 132 normal people matched by age and gender as control group.Peripheral venous blood samples were collected and DNA was extracted from leukocytes.By using a modified Brucker Autoflex MALDI-TOF mass spectrometer,we analyzed 3 genes with 9 polymorphic sites,namely,Fas-1377G/A rs2234767,Fas-670G/A rs1800682,Fas rs2147420,Fas rs2296603,Fas rs7901 656,Fas rs1 57101 9,FasL-844C/T rs7631 10,CASP-9-1263A > G rs4645978,and CASP-9-712C > T rs4645981.The correlations between polymorphism of Fas,FasL and CASP-9 genes and the risk of LDH were evaluated by non-conditional Logistic regression model.Multiple Logistic regression model was performed to assess the interaction between apoptosis-associated genes and environment factors,such as lumbar vertebral loads,bed type,spare-time exercises and spare-time activities. Results There were preferable balances in case and control groups in age and gender without significant differences.However,the two groups differed significantly (P <0.05)in lumbar vertebral loads,body mass index, bed type, spare-time exercises and spare-time activities. Correlation analysis showed remarkable correlations between LDH and the polymorphisms of FasL-844C/T (rs7631 10)and CASP-9-1263A>G (rs4645978),and FasL-844C/T TT and CASP-9-1263A>G GG genotypes might be the high risk genotypes of LDH.The gene-environment interaction analysis revealed that super-multiplicative and sub-multiplicative interactions respectively between FasL-844TT genotype and lumbar vertebral loads (3-4 level),and between CASP-9-rs4645978 GG and lumbar vertebral loads (3-4 level).Conclusion FasL,CASP-9 genes and lumbar vertebral loads and their interactions play important roles in the pathogenesis of LDH.It suggests that the risk of LDH may be codetermined by environmental factors and inherited susceptibility genes,and that the mechanisms of interactions vary in different genotypes and the same or different environmental factors.
4.Characteristics of pneumoconiosis on multi-detector row CT and its' anatomic-pathologic basis.

In order to investigate the fine distinction of the tomographic images and the dominant anatomic distributions, we carefully reviewed and analyzed the features and predominant anatomic distribution of forty-nine pneumoconiosis patients with confirmed diagnosis on multi-detector row CT (MDCT). It was found that the round and small opacity p and irregular small opacity were mostly shown in the MDCT features of pneumoconiosis, while the large opacity and progressive massive fibrosis (PMF) were less frequently depicted in the MDCT. Distributions of round and small opacity and irregular small opacity in the six lung lobes were significantly different (P < 0.01). The most common p opacity was significantly seen in the upper and lower left lungs as well as in the upper right lung's opacity was in the upper left lung as well as lower left and right lungs. The large opacity commonly distributed in upper left and right lungs, while the PMF was often shown in both of two lungs. The results demonstrated that the MDCT could be an effective modality for detecting tiny lesions and anatomic distribution of pneumoconiosis, and it would be helpful for early diagnosis and accurate staging of the pneumoconiosis disease.

5.16-Detector row CT features and related anatomic-pathology basis of idiopathic pulmonary fibrosis.

The objective of this study was to elucidata the correlation of 16-detector row CT features with the complications and anatomic-pathology of the disease in patients with idiopathic pulmonary fibrosis (IPF). The 16-detector row CT appearances and predominant anatomic distribution were analyzed in 38 cases of IPF. Of the 14 patients (36.8%) with honeycomb lung, 10 (71.4%) had honeycomb appearances predominantly distributed in the peripheral zone of the middle and lower lobes, particularly in the basal segments of the lower lobes. 11 patients (78.6%) had the clinical duration of honeycomb lung for more than 3 years. The interlobular interstitial thickening was seen in 16 cases (42.1%), the interlobular septa thickening in 20 cases (52.6%); and the thickened bronchus-vascular bundle in 9 cases (23.7%). The above-mentioned three kinds of CT features observably distributed in the peripheral zone of the middle and lower lobes, in the basal segments of the lower lobes, and in the lower lobes and middle lobe of right lung in the middle of the zone, respectively. In 15 patients (39.5%), ground-glass opacity was predominantly distributed in the peripheral zone of lung and in the basal segments of the lower lobes; and the clinical duration lasted less than 1 year was seen in 10 cases (66.7%). These results indicated that some characteristic manifestations with pathologic variation and predominant anatomic distribution of IDF can be shown on 16-detector row CT.

6.Association of occupational heat and noise exposure with hypertension

Objective:
To explore the influence of heat and noise exposure on the risk of hypertension in workers,and to provide evidence for prevention of hypertension in occupational population.
Methods:
Cluster sampling method was used to recruit 2 130 steel workers from four factories(north iron-making,long steel,cold steel and power department)of some steel company. A cross-sectional study was conducted to collect physical examination results,heat and noise exposure. Logistic regression models were used to analyze the effects of noise,heat and their interaction on hypertension.
Results:
Totally 1 971 valid questionnaires were collected,and the response rate was 92.54%. There were 654 workers suffering from hypertension,with a prevalence rate of 33.18%. There were 1 006 workers exposed to noise,accounting for 51.04%;and 853 workers exposed to heat,accounting for 43.28%. The prevalence rates of hypertension in workers exposed to noise and heat were higher than those in workers without the above exposure(P<0.05). The prevalence rates of hypertension increased with the increase of age and working years(P<0.05). The results of multivariate logistic regression analysis showed that noise(OR=1.711,95%CI:1.380-2.121),heat(OR=1.721,95%CI:1.394-2.125)were risk factors for hypertension;the interaction between noise and heat on hypertension was not statistically significant(P>0.05),while the interaction between noise and age(OR=1.086,95%CI:1.057-1.115),between heat and age(OR=1.087,95%CI:1.059-1.115)both increased the risk of hypertension.
Conclusion
Occupational exposure to heat and noise are risk factors for hypertension. These two occupational hazards also interact with age,which can increase the risk of hypertension in workers.
7.Efficacy analysis of the single versus combination drug therapies for benign prostatic hyperplasia with overactive bladder

Objective To explore the efficacy of the single versus combination drug therapies for benign prostatic hyperplasia(BPH) combined with overactive bladder(OAB).Methods A total of 471 outpatients with BPH and OAB meeting the inclusion/exclusion criteria were enrolled in this prospective cohort study from March 2012 to October 2015.Patients were divided into two groups:(1) the single alpha-blocker treatment group (prostate volume < 30 ml),and (2) the 5 alpha reductase inhibitors(5-ARIs) plus alpha-blocker combination treatment group(prostate volume ≥ 30 ml).The 318 patients were treated with alpha blockers for 4 weeks,and then received a continuing alpha-blocker treatment for 8 weeks if IPSS score changes were less than 30% (i.e.single alpha-blocker treatment group).And 153 patients were treated with 5-ARIs for 12 weeks,then received 5-ARIs plus alpha-blocker combination treatment for another 4 weeks(a total of 16 weeks)if IPSS score changes were less than 30 % (i.e.combination treatment group).The improvements of post-voiding residual(PVR),PV,maximum urinary flow rate(Qmax),international prostate symptom score(IPSS),overactive bladder symptom score (OABSS),quality of life (QOL),urine storage period symptom score (USPSS) and voiding symptom score(VSS)were compared between the two groups.Results The values of IPSS,OABSS,QOL,USPSS and VSS index in the two groups were improved after treatment as compared with pre-treatment(all P≤0.05).Patients in combination treatment group had little improvement in PVR and Qmax after treatment.The OAB symptom remission rates of BPH patients with OAB in single alpha-blocker treatment group were 70.5% (206/292)and 78.6% (165/210)after 4 and 12 weeks of treatment respectively.The OAB symptoms remission rates of BPH patients with OAB in combination treatment group were 54.5 % (64/122) and 67.1% (53/79) after 12 and 16 weeks of treatment respectively.Conclusions Both single alpha-blocker treatment and alpha-blocker plus 5ARIs combination treatment,which identification was based on prostate volume,have good effects on BPH patients with OAB.The single alpha-blocker treatment can improve PVR and Qmax,and the alpha-blockers plus 5ARIs combination treatment can improve the prostate volume in BPH patients with OAB.
8.Total flavonoids of Pterocarya hupehensis Skan inhibit migration and in-vasion of fibroblast-like synoviocytes by regulating PI3K/AKT signaling pathway

AIM:To observe how total flavonoids of Pterocarya hupehensis Skan(PHSTF)affects the migra-tion and invasion of human rheumatoid fibroblast-like synoviocyte line MH7A.METHODS:The MH7A cells were divided into control group(without any treatment),low-,medium-and high-dose(6.25,12.5 and 25 mg/L,respectively)PHSTF groups,phosphatidylinositol 3-kinase(PI3K)inhibitor 740Y-P(10 μmol/L)group,and 740Y-P(10 μmol/L)+high-dose(25 mg/L)PHSTF group.The viability of the MH7A cells was determined by CCK-8 assay,while the migration and inva-sion were assessed by scratch and Transwell assays.The protein levels of matrix metalloproteinase-2(MMP-2),MMP-9,PI3K,p-PI3K,AKT and p-AKT were detected by Western blot.RESULTS:Compared with control group,the cell via-bility in PHSTF treatment groups was reduced(P<0.05),the cell wound healing area was significantly decreased(P<0.01),migratory and invasive cells in the lower chamber were significantly reduced(P<0.01),and the protein expres-sion of MMP-2 and MMP-9 and the ratios of p-PI3K/PI3K and pAKT/AKT were decreased(P<0.01).Compared with high-dose PHSTF group,the addition of PI3K/AKT pathway agonist 740Y-P significantly increased the migration and invasion ability of MH7A cells(P<0.01)and elevated the protein expression of MMP-2 and MMP-9 and the ratios of p-PI3K/PI3K and pAKT/AKT(P<0.01)under the treatment with PHSTF.CONCLUSION:Total flavonoids of Pterocarya hupehensis Skan could inhibit the migration and invasion of MH7A cells by regulating the PI3K/AKT signaling pathway.
9.Clinical Investigation of Key Parameter Range of AMG Muscle Relaxant Monitor.

The accelerometry(AMG) muscle relaxant monitor is the most widely used quantitative muscle relaxant monitor to assess the degree of neuromuscular at present. In this study, the ulnar nerve was stimulated by using train of four stimulation(TOF) mode of the AMG muscle relaxant monitor, and the movement of the adductor pollicis muscle was monitored. In this way, the distribution range of key parameters (acceleration peak value, response time, and TOF ratio) of the adductor pollicis muscle during the use of muscle relaxant in clinical practice is analyzed and will provide a practical basis for the development and improvement of the muscle relaxant monitor.
