1.Association between left ventricular hypertrophy and peritoneal transport properties in peritoneal dialysis patients

Objective To investigate the association between left ventricular hypertrophy and peritoneal transport properties in Peritoneal Dialysis patients.Methods Sixty-nine Continuous Ambulatory Peritoneal Dialysis (CAPD)patients were enrolled in current study.All patients underwent echocardiography for left ventricular mass index (LVMI).Transport status was categorized as high transport,high average transport,low average transport and low transport based on modified peritoneal equilibration test (PET).The data collected included hemoglobin,albumin,blood urea nitrogen,creatinine,urea clearance (Kt/V) and creatinine clearance rate (Ccr),dialysis vintage,systolic blood pressure,diastolic blood pressure.Results Patients with high transport status were 41 cases,more than those with low transport status(28 cases).The dialysis age of high transport,high average transport,low average transport and low transport vintage were (39.2 ± 21.8),(26.6 ±15.6),(28.6 ± 14.4),(45.7 ± 35.0) years old respectively,and the difference was significant (F =4.128,P < 0.05).The dialysis age in the higher transport group was longer than that in high average transport group and low average transport group (P < 0.05).LVMI has significant positive correlations with D/Pcr at 4th hour,SBP and DBP (r =0.339,0.351,0.316,P < 0.01) and the negative correlation with albmin (r =-0.292,P <0.05).Left ventricular hypertrophy(LVH) in all patients was 63.8%,ant it was higher in high transport group than that low average transport and low transport group (x2 =5.455,5.091,P < 0.05) Conclusion High transport status is the most common in CAPD patients.There is high incident rate of LVH in this population.LVH has significant positive correlations with D/Pcr,higher SBP,DBP,and lower albumin.
2.Comparison of multi-slice spiral CT portography and endoscopy in the diagnosis of esophageal and gastric varices in liver cirrhosis

Objective To study the correlation of multi-slice CT portography (MSCTP) and digestive endoscopy in the diagnosis and evaluation of esophageal and gastric varices (EGV) caused by cirrhosis.Methods A total of 92 patients with cirrhosis were enrolled in the prospective study.All the patients were examined by endoscopy and 64-slice spiral CT scan in 4 weeks.The types,grading of EGV were observed by endoscopy and MSCTP,and Kappa conformance test was applied with the endoscopic findings as gold standard.The sensitivity,specificity,consistency,and Youden index were evaluated for the diagnosis of sophageal and gastric varices by MSCTP.Results Sixty-five patients were diagnosed to have EGV by endoscopy and 27 were negative.The positive patients included 45 patients of GOV1,19 of GOV2 and 1 patient of IGV1.MSCTP diagnosed 67 cases of EGV and 25 patients of negative results.The positive patients included 46 of GOV1,18 of GOV2 and 3 of IGV1.Two patients of IGV1 varicose veins without positive findings on endoscopy were diagnosed by using MDCTP,which revealed isolated varicose veins under the gastric mucosa.There was high consistency between MSCTP and EGV in the diagnosis of EGV (Kappa =0.732,P < 0.01).The sensitivity of MSCTP was 93.8%,specificity was 77.8%,consistency was 89.1%,and Youden index 71.6%.There was high consistency between MSCTP and EGV in the classification of EGV (Kappa values were 0.743 and 0.763,P < 0.01).Conclusions There is high consistency between MSCTP and digestive endoscopic in the diagnosis and classification of EGV in cirrhosis.MSCTP is superior to endoscopy in the detection of gastric varices.
3.Effect of fleabane injection on serum D-dimer, fibrinogen and hypersensitive C-reactive protein in patients with IgA nephropathy

Objective To observe the effect of fleabane injection on serum level of D-dimer, fibrinogen and hypersensitive C-reactive protein in patients with IgA nephropathy and to explore the mechanism of fleabane injection for treating IgA nephropathy. Methods 29 patients with IgA nephropathy were given fleabane injection together with routine treatment. Another group of 28 patients with IgA nephropathy were only treated with routine treatment as con-trol. Determinations of 24 hours urine protein output(24HPQ), serum level of D-dimer(D-D), Fibrinogen(Fib) and hypersensitive C-reactive protein (hsCRP) were carried out pre- and post-study. Results Significant decrease in 24HPQ , serum level of D-D, Fib and hsCRP were observed in both treatment group and control group(P <0. 01 ~0. 05), but more significant in treatment group as compared with control group ( P < 0. 05). Conclusion Flea-bane Injection plus routine treatment could significantly decrease 24HPQ in patients with IgA nephropathy, and this maybe contribute to regulation of D-D, Fib and hsCRP by fleabane injection.
4.The effects of TCM YiShen decoction on the expression of PAI-1 in kidney tissue of mice with IgA nephropathy

Objective To explore mRNA and protein expression of PAl-1 in kidney tissue of mice with IgA nephropathy (IgAN) and the effects of YiShen Decoction. Methods The IgAN model was built by the method of oral intake of bovine serum albumin(BSA)together with the injection of staphylococcus enterotoxin B(SEB)through caudal vein. The mRNA expression of PAI-1 was measured by fluorescent quantitative polymerase chain reaction (FQ-PER)and the expression of PAI-1 protein was measured by immunohistochemistry. Results No significant difference of mRNA and protein expression of PAI-1 was found between the low concentration and high concentration Yishen Decoction group. But the secretion and mRNA expression of PAI-1 in the low and hiigh concentration Yishen Decoction group was decreased more than that in the IgAN model group. Conclusion The abnormal expression of PAI-1 mRNA and protein played an important role in the onset and development of IgAN. The TCM Yishen Decoction could reduce the abnormal expression of the mRNA and protein of PAI-1 in kidney tissue of mice with IgA nephropathy.
5.TriVex system for the treatment of lower extremity varicose veins

Objective To evaluate the TriVex system in the treatment of varicose veins of lower extremities,focusing on postoperative complications and management.Methods Clinical data of 108 patients (146 legs) of varicose veins of the lower extremity undergoing TriVex procedure were retrospectively analyzed.Deep veinons patency was verified in all patients by preoperative sonography.Above knee stripping of the great saphenous vein was done first when necessary.The below knee phlebectomy of the side branches was done with the new system (Trivex System/Smith and Nephew).Postoperative patients were followed-up,and results were evaluated.Results Procedure was successful in all cases.98 cases were followed up for 1 ~ 24 months.The mean operation time per leg was (34±8) minutes.Complications were as following:26 legs (17.8%) developed postoperative hematoma which was healed by conservative therapy including two cases in which the tension seroma,which was successfully managed by puncture aspiration.Transien skin numbness or paraesthesia developed in 13.0% (19/146).Subcutaneous induration in 11.6%(17/146) cases.Residual varicose and recurrence in 3.4% (5/146).Incision related complications developed in 4.8% (7/149) cases.Conclusion Transilluminated powered phlebectomy (TriVex) is a safe and effective cosmetic procedure for less invasive varicose vein surgery.
6.Infection Episode and Related Risk Factors in Continuous Hemodialysis Patients: A Survey

OBJECTIVE To investigate the infection episode and related risk factors in continuous hemodialysis patients. METHODS The relationship among infection and etiologies of infection,nutritional status,pathogens and causes of chronic renal failure(CRF) were retrospectively analyzed in 180 continuous hemodialysis patients. RESULTS Totally 113 times infections were observed among the 86 inpatients under continuous hemodialysis.The main infectious site in hemodialysis patients was lungs.Thirty eight times were positive in 50 times of etiologic detection,Gram-negative germ was the most common(60.3%).Hemoglobin and serum albumin decreased obviously in infectious patients.Diabetes and systemic lupus erythematosus patients were more susceptible to infection.The hepatitis virus infections rate in hemodialysis patients was relatively high. CONCLUSIONS There is higher infections rate in continuous hemodialysis patients.Diabetes and systemic lupus erythematosus patients are more susceptible to infection.Anemia,lower serum albumin,old age and bad compliance are the susceptible factors.
7.Correlation Study Between Serum Level of LncRNA KCNQ1OT1 Overlapping Transcript and Coronary Artery Lesion in Relevant Patients

Objective:To investigate the serum expression of long non-coding RNA overlapping transcription content KCNQ1OT1 (KCNQ1) gene in patients with coronary artery disease (CAD) and its clinical significance.
Methods:A total of 196 patients treated in our hospital were divided into 2 groups:CAD group and Control group, the patients were without CAD. n=98 in each group. Expression levels of serum KCNQ1OT1 and P53 were measured by quantitative RT-PCR;the relationship between KCNQ1OT1, P53 and clinical features in relevant patients were analyzed.
Results:Compared with Control group, CAD group had increased expression of serum KCNQ1OT1 and decreased expression of P53, both P<0.05. Correlation analysis revealed that the expression of KCNQ1OT1 was negatively related to P53 (r=-0.856, P<0.001);multi Logistic regression analysis indicated that serum KCNQ1OT1 was independently related to CAD (P<0.05).
Conclusion:CAD patients had obviously increased serum level of KCNQ1OT1;KCNQ1OT1 was independently related to CAD occurrence.
8.Injection of NKG5SV gene to inhibit growth and metastasis of hepatocellular carcinoma.

OBJECTIVETo study the injection of NKG5SV gene to inhibit growth and metastasis of hepatocellular carcinoma (HCC).
METHODSNKG5SV gene was inserted into retroviral vector pLXSN by normal methods. LacZ gene was used as control. LCI-D20 tumor together with saline, pLXSN-LacZ DNA or pLXSN-NKG5SV was subcutaneously inoculated to the nude mice. Tumor formation rate and tumor size were noted 35 days after inoculation. LCI-D20 tumor was inoculated subcutaneously. Saline, pLXSN-LacZ DNA or pLXSN-NKG5SV was intratumorally injected respectively 10 days after inoculation. Tumor growth was observed 35 days after inoculation. Liver cancer was resected 22 days after intrahepatic inoculation. Saline, pLXSN-LacZ DNA or pLXSN-NKG5SV was respectively injected at incisal margin or intraspleen. Mice were killed 35 days after inoculation to observe tumor recurrence at incisal margin, intrahepatic metastasis and extrahepatic metastasis.
RESULTSTumor formation rate and tumor diameter(cm) were 1.76 +/- 0.11, 1.51 +/- 0.34, 0.33 +/- 0.04 in the control group, LacZ group, NKG5SV group respectively when tumor and different cDNA were inoculated together. Tumor diameter(cm) and weight(g) were 0.87 +/- 0.08, 0.83 +/- 0.05, 0.26 +/- 0.04; 0.43 +/- 0.06, 0.38 +/- 0.04, 0.08 +/- 0.06 in the control group, LacZ group, NKG5SV group respectively when different cDNA were injected into the LCI-D20 tumor. Sites with extrahepatic metastasis nidi, incisal margin recurrence tumor size(cm), intrahepatic metastasis nidi, metastasis involved hepatic lobes in the control group, LacZ group, NKG5SV group were 4.25 +/- 1.48, 4.25 +/- 1.04, 0.63 +/- 0.51; 1.51 +/- 0.27, 1.35 +/- 0.17, 0.81 +/- 0.17; 2.50 +/- 1.41, 2.38 +/- 1.06, 1.25 +/- 0.71; 2.13 +/- 0.99, 2.00 +/- 0.75, 1.38 +/- 0.74 respectively when NK cells were injected at incise margin. They were 4.38 +/- 1.85, 4.25 +/- 1.48, 1.00 +/- 0.75; 1.13 +/- 0.23, 0.97 +/- 0.29, 0.76 +/- 0.16; 2.50 +/- 1.41, 2.05 +/- 1.12, 0; 2.13 +/- 0.83, 1.75 +/- 0.88, 0 respectively when NK cell were injected intrasplenicly.
CONCLUSIONSNKG5SV gene can inhibit HCC growth and postoperative metastasis and recurrence.

9.Clinical significance of thromboelastography in elderly hypertensive patients

Objective To explore the clinical significance of thromboelastography (TEG) in detecting coagulation status in elderly hypertensive patients.Methods A total of 75elderly patients with hypertension, were selected as the elderly hypertensive group.Meanwhile, 60cases of senile cerebral infarction patients and 47cases of normal elderly were selected as the senile cerebral infarction group and the normal elderly group respectively.The values of R, MA, α-Angle, K, CI were measured by TEG-5000thrombus elastometer and then compared in the three groups.Meanwhile, the values of platelet inhibition induced by AA and ADP were also detected in the elderly hypertensive group.The PLT and WBC of the elderly hypertensive group were detected by automatic five class blood cell analyzer.The correlation between TEG parameters and PLT and WBC in elderly hypertensive group were analyzed.Results Compared with the normal elderly group, the MA and CI of the elderly hypertensive group were significantly increased (P<0.05), theα-Angle and K were significantly decreased (P<0.05), the R value was not significantly different (P>0.05), while in the senile cerebral infarction group, the R was significantly shortened (P<0.05), the CI was significantly increased (P<0.05) and the K value was significantly decreased (P<0.05).Compared with the elderly hypertensive group, the R value of the aged cerebral infarction group was shortened (P<0.05).In the elderly hypertensive group, PLT were negatively correlated with MA, α-Angle and CI, negatively correlated with K value, and not related to R value.WBC was positively correlated with MA, α-Angle and CI, negatively correlated with K value, and low correlation with R value.The proportion of patients ADP inhibition rate≥30%was 41.94% (13/31), while the proportion of patients AA inhibition rate≥50%was 20.00% (8/40).Conclusion The detection of TEG is of great significance in helping the clinical diagnosis, treatment guidance and disease monitoring of elderly hypertension, and can prevent the occurrence of complications of hypertension in the elderly hypertensive patients.
10.A comparative study of research hotspots and trends in digital transformation of higher education in China and abroad

Objective:To compare research hotspots in digital transformation of higher education in China and abroad and analyze the research trends through the bibliometric method and text analysis method, and to offer advice and suggestions for digital transformation of higher education in China.Methods:The relevant literature in Web of Science and China National Knowledge Infrastructure from the establishment of the databases to April 1, 2023 were retrieved. Citespace and VOSviewer were used to visually compare the research hotspots and trends in digital transformation of higher education in China and abroad from the perspectives of number of published papers, journal, author, country, cooperation network, keyword co-occurrence and clustering, and keyword burst.Results:The results showed that both international and domestic researchers paid attention to the practical significance and development model of digital transformation of higher education in the context of rapid development of science and technology and the post-pandemic era. International researchers emphasized the advantages and disadvantages of digital transformation from the aspects of connotation, practice logic, and theoretical framework, while Chinese researchers focused on clarifying the principles related to digital transformation, summarizing the experience of other countries, and exploring the development path. The application of educational technology and the digital literacy of teachers and students had become research hotspots in China and abroad.Conclusion:The research on digital transformation of higher education has gradually shifted from theoretical system to practice effect. In the future, with the continuous deepening of theoretical research, how to improve the effectiveness of digital transformation of higher education is a research direction worthy of attention.
