Objective To retrospectively analyze the clinical distribution and changes in antimicrobial resistance profiles of Staphylococcus aureus (S. aureus). Methods We collected clinical specimens of S. aureus from The First Hospital of China Medical University. The Vitek-2 and BD Phoenix 100 were performed for bacterial identification and drug sensitivity tests, and WHONET 5.6 was used to analyze the data. Results From 2007 to 2016,there were 3 377 unrepeatable strains of S. aureus, including 1 705 that were methicillin resistant S. aureus (MRSA). The isolation rate of S. aureus was 9.4 % and of these,50.5 % were MRSA. There were 776 S. aureus specimens from outpatients or the emergency department, including 16.8 % MRSA,and 2 011 S. aureus from inpatient departments, including 60.2 % MRSA. The main sources of specimens were sputum (41.8 %),pus (17.9 %),and body secretions (17.5 %). The average resistance rates of MRSA for erythromycin, ofloxacin, ciprofloxacin, gentamycin, and tetracycline were higher than 75.0 %. The average resistance rate of methicillin sensitive S. aureus (MSSA) for erythromycin was up to 76.8 %,and for tetracycline, gentamycin, ciprofloxacin, and ofloxacin, were less than 25.0 %. In 10 years, the average resistance rates of MRSA and MSSA for 11 kinds of common antibiotics had no obvious change. Conclusion The constituent rate of MRSA was high in The First Hospital of China Medical University, especially from the areas that were not sterile, suggesting that clinicians should pay attention to the identification of infection and sources for MRSA,which were from such areas. Hospital infection control should be focused on at the same time,in order to reduce the incidence of MRSA.

Objective To explore the risk factors of early neurological deterioration(END)in patients with acute cerebral infarction(ACI)based on the characteristics of computed tomography angiography(CTA)images of head and neck plaques. Methods A total of 218 ACI patients were selected, and according to whether END occurred within 72 h after onset, they were divided into an END group(54 cases)and non END group(164 cases). The clinical data and imaging indicators were compared and analyzed between the two groups.Multivariate logistic regression analysis was used to analyze risk factors, and a nomograph prediction model was constructed and evaluated. Results National Institutes of Health Stroke Scale(NIHSS)score at admission ≥4 points, glycated hemoglobin(HbA1c)≥7.7%,severe or above ipsilateral carotid artery stenosis, and unstable plaque(soft plaque and mixed plaque)were the independent risk factors in END patients(P<0.05).The result of nomograph prediction model showed that its discrimination, accuracy and effectiveness were better. Conclusion NIHSS score at admission,HbA1c level, ipsilateral carotid artery stenosis degree, and plaque type are all independent influencing factors for the occurrence of END.

Objective To summarize the isolation and drug resistance rate of Escherichia coliin The First Hospital of China Medical University over the past 10 years,in order to provide evidence for the efficacies of clinical anti-infection treatments. Methods The data was collected from Escherichia coli isolated from patients treated at The First Hospital of China Medical University between 2013 and 2022.VITEK 2 and VITEK MS were used for bacterial identification,VITEK2 and KB method were used for drug sensi-tivity testing, and WHONET 5.6 software was used for analysis. Results From 2013 to 2022,6 845 strains were isolated, including 80.5%from inpatients and 19.5%from emergency and outpatients. The specimens were most commonly found in the urine(57.8%),blood(15.0%),secretions(9.2%),and drainage fluid(8.1%).The isolation rate of extended-spectrumβ-lactamase(ESBL)producing Escherichia coli was 57.2%(54.3%to 61.5%).The drug resistance rate of Escherichia coli to carbapenems was low,at only 1.2%(0.2%to 2.6%).Conclusion Escherichia coli remains an important pathogen in clinical infections, with varying degrees of resist-ance to multiple antibiotics, and the resistance rate is increasing.Clinical physicians should pay sufficient attention to this issue.

Objective To analyze pathogen distribution and drug resistance in a tertiary hospital in Shenyang in 2022 and provide evi-dence-based guidance for this and other hospitals to formulate antibacterial drug application strategies. Methods In 2022,bacterial iso-lates collected from patients in a tertiary hospital in Shenyang were identified and subjected to drug sensitivity tests based on the require-ments of the national clinical laboratory operation procedures. The data were analyzed using Whonet 5.6 software. Results In total,4968 pathogenic strains were isolated from this hospital in 2022.The top five isolates were Escherichia coli,Klebsiella pneumoniae,Acineto-bacter baumannii,Enterococcus faecalis, and Pseudomonas aeruginosa. The resistance rates of Escherichia coliand Klebsiella pneumo-niaeto carbapenems were 1.9%and 17.7%,respectively.The resistance rates of Enterobacter cloacaeto imipenem and meropenem were 26.7%and 25.0%,respectively.The isolation rate of methicillin-resistant Staphylococcus aureus(MRSA)was 15.6%.The resistance rates of vancomycin-resistant Enterococcusand linezolid-resistant Enterococcus faecaliswere 7.1%and 11.6%,respectively.The resist-ance rates of tropical yeasts to fluconazole was>20%.Conclusion The distribution and drug resistance of pathogens in this hospital differed from those in other regions of China. The drug resistance of strains in this hospital should be closely monitored to understand the distribution and drug resistance of pathogens in a timely manner and to provide guidance for the diagnosis and treatment of infections.

Immunotherapy, as an emerging treatment method, has been proven to improve the prognosis of patients with relapsed/refractory B-cell acute lymphoblastic leukemia (B-ALL) and has good application prospects. Immunotherapy, including chimeric antigen receptor T cell immunotherapy (CAR-T) and monoclonal antibodies, has shown great potential for application, and has been approved for marketing. This article summarizes the application of the above two therapies in the treatment of relapsed/ refractory B-ALL, and concludes that CAR-T is a kind of personalized immunotherapy, and the selection of ideal targets is an important part of its action. Currently, the ideal targets in clinical studies include CD19, CD22 and CD19/CD22. Monoclonal antibodies, including blinatumomab and inotuzumab ozogamicin, have shown superior therapeutic efficacy for relapsed/refractory B- ALL. Immunotherapy has shown superior therapeutic effects compared to conventional chemotherapy, expanding the selection of treatment options for relapsed/refractory B-ALL.
