1.Laparoscopic ultrasonography assisted laparoscopic ureterlithotomy

ObjectiveTo evaluate the role of laparoscopic ultrasonography (LUS) in laparoscopic ureterolithotomy.MethodsLUS was utilized in laparoscopic ureterolithotomy for 7 cases of ureterolith to locate the ureter and stone.ResultsLUS demonstrated the relationship of the ureter to the adjacent structures and guided the dissection of the ureter in all the 7 patients and precisely localized the position of calculi.The stone could be touched by atraumatic clamps in 5 cases.LUS-demonstrated localizations of the ureter and calculi were verified on laparoscopic ureterotomy in the other 2.The mean operating time for LUS was 14 min (ranging from 9 to 20 min ).ConclusionsLUS is a real adjuvant means for locating the ureter and stone.
2.Cytokine secretion, angiogenesis and cardiomyocyte apoptosis after transplantation of autologous bone marrow mesenchymal stem cells into myocardial infarction regions

AIM: The autologous bone marrow mesenchymal stem cell (BMSC) transplantation for treating acute myocardial infarction (AMI) via coronary artery in clinic has made remarkable and encouraging progress. The experiment aimed to study cytokine secretion in autologous BMSC transplantation for myocardial infarction dogs and its influence on angiogenesis and cardiomyocyte apoptosis. METHODS: Experiments were performed at the Animal Laboratory and the Central Laboratory, First Affiliated Hospital of Anhui Medical University between December 2005 and May 2007. 10 mL of bone marrow was sterilely aspirated from the posterosuperior iliac spine of 36 healthy mongrel dogs. BMSCs were isolated and purified by Percoll density gradient centrifugation and cultured in vitro by adherence method. After being co-cultured with 5-azacytidine for two passages, these cells were labeled by 5-bromodeoxyuridine (Brdu) for preparation. Dog models of AMI were established in a model control group and a cell transplantation group randomly. 2 mL of cell suspension of autologous BMSCs were implanted into four different regions in the acute myocardial site via topical injection in the cell transplantation group, while the same dose of DMEM was injected into the corresponding regions in the model control group. Myocardial infraction tissues were measured after transplantation. RESULTS: Flow cytometry demonstrated that the positive rate of CD34 and CD11b on third passage of BMSC cell surface was below 5%, while the positive rate of CD44 and CD105 was above 90%. 3.Diagnostic Ability of Laparoscopic Ultrasonography(LUS) for Urinary System Injury Induced by Laparoscopic Surgery in Animal Experimental Study

Objective To appraise the diagnostic capability of laparoscopic ultrasonography (LUS) for urinary system injury induced by laparoscopic surgery in goats. Methods Animal models of different types of urinary system injury at different sites were established in five goats. One goat without injury was set as a negative control. LUS was used to examine the animals following randomization and single-blind principles. Results The sensitivity of LUS was 76.9% (10/13),specificity was 100%,and the false-negative rate was 23.1% (3/13). No false-positive results were detected in this study. The sham injury was accurately detected through LUS. The scan time of LUS ranged from 19 to 25 min with a mean of 21 min. Conclusions LUS is of value in the diagnosis of urinary system injury.
5.Endourological treatment of multiple complex bladder pathological changes with holmium laser

Objective To evaluate the effectiveness of transurethral holmium laser treatment for multiple complex bladder pathological changes.Methods Clinical data of 507 cases of transurethral holmium laser resection for multiple complex bladder pathological changes under endoscope from 2001 to 2005 were retrospectively reviewed.A total of 17 types of bladder pathological changes were encountered,and 2 or more types of pathological changes occurred simultaneously in 111 cases,including 36 cases of 3 types of coexisting changes.Results Primary operation was performed in 505 patients,and two-stage operation was performed in 2 patients.The operation time was 5~35 min(mean,20 min).No obturator nerve reflex and other complications were seen.The operation was accomplished in 3 patients with cardiac pacemaker and 3 patients with coronary stent.Follow-up examinations in all the cases for 12~24 months revealed no relapse.Conclusions Transurethral holmium laser resection for multiple complex bladder pathological changes is safe and reliable.During the procedure,the degree of resection depth in the bladder wall can be precisely controlled.It can be used as the first choice for minimally invasive treatment of multiple complex bladder pathological changes.
6.Clinicopathologic feature and PRA,PRB expressions of 17 cases uterine malignant mixed mesodermal tumor and their significannce

Background and purpose:MMMT is known as a rare malignancy in gynecological tumor. Because of its difficulty in diagnosis and treatment, the prognosis is extremely poor. This study was to discuss the clinicopathologic feature and PRA,PRB expressions of uterine MMMT. Methods:We analysed clinicopathologic data and the expressions of PRA,PRB by immunohistochemical staining on 17 cases of uterine MMMT, and 11 patients were followed up. Results:①The patients usually presented with abnormal vaginal bleeding with no specifi city clinically. ②The morphological changes were various and complicated, including epithelial and mesenchymal components and a variety of inter-permeated and transitional tissue elements.③The positive rate of PRA in stageⅠ and stageⅡ were 66.7% and 40%, and PRB in stage Ⅰ and stageⅡwere 55.6% and 20%, respectively. ④The mean survival time in stageⅠ,Ⅱ and Ⅲ were 43.8 months (32-59), 34.25 months (19~41) and 5 months, respectively. Conclusion:The diagnosis of uterine MMMT was mainly based on tissue morphology; the development of uterine MMMT might be related with the loss of PRA and PRB ; the clinical stage and the expression of PRA and PRB might be the prognostic factors for uterine MMMT.
7.Investigation and influencing factors analysis on the professional identity of nursing students

Objective To investigate the status of professional identity of nursing undergraduates in different grades and to search for their different professional identity level, so as to establish a pertinent method on nursing education for promoting nursing undergraduates′professional identity. Methods By stratified sampling, a total of 286 nursing undergraduates from four grades in Dalian Medical University were surveyed by general information questionnaire and the Professional Identification Scale for Nurses. Results The average total scores of the professional identity of nursing undergraduates were (107.29±15.01) points. There was significant difference in the total score and the five dimensions of professional identity,F=5.625, 5.258, 7.459, 4.182, 6.891, 7.299, P<0.01. Professional identity score was on the rise along with the grade growth, but the score of grade two was significant lower than the other three grades, and the score showed an increase in grade three; grade, whether profession was consistent with their own interests or not, the reasons of choosing the profession were the important factors affecting professional identity of nursing undergraduates. Conclusions The professional identity score of nursing undergraduates is in medium level and remains to be improved. It is suggested that nursing educators should take corresponding measures according to different grades, own interest and physical and mental characteristics of the students, so as to improve their professional identity and lay the foundation for stability and sustainable development of the nursing team in the future.
8.Clinical analysis and countermeasure research of bleeding during percutaneous nephrolithotomy by endoscopy and holmium laser

Objective To provide guidance for clinical prevention and treatment of bleeding during percutaneous nephrolithotomy( PC-NL) . Methods The clinical data of 1 012 patients with intraoperative and postoperative bleeding during percutaneous nephrolithotomy in our urology department were collected,hemorrhoea occurred on 36 cases,the occurrence rate was 3. 56%. The incidence,correlation with cal-culi,diabetes mellitus,examination item,technical operation were analyzed and compared. Results The incidence was 5. 52% for patients with complicated calculi. The incidence of delayed massive haemorrhage has been increased postoperatively in the diabetes mellitus patients. This incidence was 1. 81% for patients with preoperative examination. Along with the extension of time in carrying out technology,PCNL asso-ciated bleeding incidence decreased year by year. Conclusion The occurrence of haemorrhage associated with PCNL could be decreased by correctly handling complicated calculi,preoperative examination,keeping blood glucose homeostasis and improving the manipulation ability of operator.
9.Detection and antibiotic sensitivity test of Klebsiella peneumoniae in Ailurus fulgens

To confirm the etiology of a dead case for a 6 year-old female Ailurus fulgens,one strain of the predominant bacteria from pathologic tissues(heart,liver,spleen,lung and other samples) of the dead Ailurus fulgens were examined and isolated.The isolate was named R1 and no other bacteria were isolated.The bacterial etiological examination(morphological characteristics,biochemical characteristics and 16S rDNA gene detection)of R1 showed that it was identifed as K.peneumoniae.Artificial infection to mice about R1 was also conducted in this study.R1 had strong pathogenicity to mice and the LD50 is 6.5 × 104 CFU/mL.Moreover,the clinical and pathological features of the dead mice were consistent with that of the Ailurus fulgens.To find effective therapeutic drugs of curing other Ailurus fulgens,antibiotic sensitivity test of R1 was conducted,and the results revealed that R1 was highly sensitive to cefotaxime et al,moderately sensitive to amikacin and resistant to penicillin.These data showed that K.peneumoniae was bacterial pathogen leading to death of the Ailurus fulgens and it had strong resistance to penicillins,macrolides and virginiamycin and it had broad drug resistance spectrum.However,R1 is sensitive to cephalosporins and aminoglycoside antibiotics.
10.Application of a Pressure-measuring and Stone-extracting Ureteral Access Sheath in Flexible Ureteroscopy For Renal Stones

Objective To investigate the application value of a pressure-measuring and stone-extracting ureteral access sheath (UAS) used together with negative pressure suction system during flexible ureteroscopy in the treatment of renal caculus.Methods A retrospective analysis was performed on the 98 cases of renal caculus who received flexible ureteroscopic holmium laser lithotripsy (FURS) in the Department of Urology of the Kunming General Hospital of Chengdu Military Command from November 2014 to September 2015,and the enrolled cases were divided into two groups:group A and group B.The new UAS was used in group A combined with vacuum suction during lithotripsy,and simple manometry type UAS was used in group B.The infusion pump flow rate of 0.2 L/min was set in the operation,the perfusion pressure limit was set to 100 mmHg,the suction pressure of group A was 10 kPa.Perfusion flow rate and pressure was adjusted in a timely manner based on the clear vision and the internal pressure force on the kidney.Then we recorded and compared the internal pressure and variation trend of renal and perfusion time of two groups.Results Two groups of patients were successfully completed surgery.The renal pressures in group A were adjusted duly and were lower than the that in group B in the whole process of operation.The average renal pressure in group A was 10.88 ± 5.91 1mmHg.The average renal pressure in group B was 20.10 ± 7.68 mmHg,the pressure control was difficult and unsteady.Conclusion The new type of UAS in the flexible ureteroscopic holmium laser lithotripsy (FURS) can make the renal pressure in surgery controllable and is safe for the FURS.
