1.Discussion on the main pathogenesis in traditional Chinese medicine and etiology about primary osteoporosis.

Osteoporosis is characterized by reduced bone mass and impaired micro-architectural structure, leading to an increased susceptibility to fractures. It is a complex, multifactorial disorder resulting from genetic factors, environmental factors and gene-environment interactions. Currently there are three opinions on the main pathogenesis of primary osteoporosis in traditional Chinese medicine: kidney deficiency, spleen deficiency, and spleen-kidney deficiency, in which disagreement remains. In this paper, the authors combine the modern etiology of osteoporosis to explain scientific connotation of the three opinions, aiming to comprehend the pathogenesis of primary osteoporosis and strengthen the communication between traditional Chinese medicine and Western medicine, and trying to evaluate the clinical curative effect on osteoporosis.
2.Clinical application of an improved ruler for fixing stereotactic head frame

Objective To study a simple, measurable and objective method for fixing stereotactic head frame. Methods A self-made stereotactic head frame ruler was used in the localization in operations of 29 cases of Parkinson's disease and 1 case of intractable cancer pain. Results Of the 30 cases,significant effects were obtained in 29 coses(96.7%) and curative effects in 1 case (3.3%). Conclusions The improved ruler can be fully applicable to the ASA-602S model stereotactic frame, being a simple, measurable and objective method for fixing stereotactic head frame and applying CT scanning orientation.
3.Effect of chloroform extract of Zingiber mioga Roscoe on human dermal microvascular endothelial cell expression of cell adhesion molecules and adhesivity to lymphocytes

Objective To investigate the effect of chloroform extract of Zingiber mioga Roscoe on human dermal microvascular endothelial cell (HDMEC) expression of cell adhesion molecules and adhesivity to lymphocytes.Methods HDMECs were isolated from human foreskin tissue and subjected to subculture.After several passages,some HDMECs were treated with chloroform extract from Zingiber mioga Roscoe (CFMG) of 200 mg/L for 30 minutes before or after 24-hour stimulation with tumor necrosis factor (TNF)-α or interleukin (IL)-1α.Subsequently,enzyme-linked immunosorbent assay was performed to determine the expression levels of intercellular adhesion molecule-1 (ICAM-1),vascular cell adhesion molecule-1 (VCAM-1) and E-selectin on HDMECs.Both T lymphocytes isolated from venous blood of healthy adults and Ramos Burkitt's lymphoma B cells were labelled with chromium-51 and incubated with the HDMECs for four hours followed by the determination of radiation intensity of lymphocytes adhering to HDMECs using a gamma-counter.Results Compared with 0.2% dimethyl sulfoxide (DMSO),CFMG slightly down-regulated the expression of ICAM-1,VCAM-1 and E-selectin on resting HDMECs (all P > 0.05).In comparison with culture medium,TNF-α enhanced the expressions of ICAM-1,VCAM-1 and E-selectin significantly (all P < 0.01),and IL-1α elevated the expressions of ICAM-1 and E-selectin markedly(both P < 0.01) as well as the expression of VCAM-1 slightly (P > 0.05).The upregulating effects of TNF-α and IL-1α on the expressions of adhesion molecules were notably suppressed by the CFMG treatment prior to the stimulation with TNF-α and IL-1α(all P < 0.01),but not affected by that after the stimulation (all P > 0.05).The adhesivity of HDMECs to T lymphocytes and Ramos cells was slightly decreased by CFMG treatment compared with 0.2% DMSO (both P > 0.05),but was markedly increased by TNF-α and IL-1α compared with the culture medium (both P < 0.01).Conclusions CFMG may play an antiinflammatory role via blocking the up-regulating effect of pre-inflammatory cytokines such as TNF-α and IL-1α on the expression of adherence molecules on HDMECs and,hence,inhibiting inflammatory cell infiltration in tissue.
4.Relationship of the variation of leptin receptor gene at 3 057 nucleotide to type 2 diabetes mellitus, obesity and plasma lipid levels

Objective To explore the relationship between the variation of leptin receptor (LR) gene and type 2 diabetes mellitus (DM), obesity and plasma lipid levels. Methods The variation of LR gene exon 20 was detected by polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP) and their relations to type 2 DM and obesity were analyzed. Results The variant frequency at 3 057 nucleotide of G to A transversion was 82.09% for total, 87.33% for DM group, 75.42% for control group. The frequency of AA genotype in DM cases was higher than that in controls, but that of GA genotype was lower, and that of GG in cases did not indicate. According to BMI and WHR level, there was no obvious relationship between gene variations and obesity. There were significant differences between AA genotype and GA genotype about the serum levels of TG, HDL and SBP, DBP. Conclusion At nucleotide 3 057 in LR exon 20, a G to A transversion was found, which may be the susceptibility marker of DM in Chongqing population. An allele was associated with high blood lipid and increased blood pressure.

AIM: To observe the effect of Shenmai injection,a chinese medicine,on apoptosis of cardiac myocytes after hypoxia.METHODS: Cardiac myocytes were separated from neonate rat heart and cultivated in vitro.Hypoxia condition was induced by mixture of 95%N2 and 5%CO2.Cells were exposed to hypoxia for 6 h or 12 h and treated with Shenmai injection(5 mL/L) from 24 h before hypoxia until the end of hypoxia.First,apoptosis was detected with Annexin V-FITC and PI staining by flowcytometry.Then,the activity of cardiac myocyte mitochondria was observed by MTT method.Mitochondria membrane potential and the activity of caspase 3,7 were also measured by laser scan microscopy and multi-detection microplate reader,respectively.RESULTS: The apoptotic cells became more and more with prolonged hypoxia.Shenmai injection enhanced mitochondria activity,kept membrane potential,inhibited the activation of caspase3,7 and then decreased apoptotic cells(P

AIM: To study the effect of Tribulus terrestris L. saponin (TTLS) on apoptosis and changes in cytosolic calcium concentration induced by hypoxia/re-oxygenation in rat cortical neurons. METHODS: Rat cortical neurons in primary culture were used, and a apoptosis model was induced by hypoxia/reoxygenation. LDH releasing rate was detected by spectrophotometry. The apoptosis rate of cortical neurons was analyzed quantitatively by flow cytometry with Annexin V-FITC and PI staining. Intracellular free Ca2+([Ca2+]i) was observed with a confocal laser-scanning microscope and determined by mean fluorescent value with Fluo-3 fluometry. AIM: The purpose of this study was to investigate the expression of myristoylated alanine-rich C kinase substrate (MARCKS) and p-MARCKS during cerebral ischemia. METHODS: The multi-cerebral infarction model was established by method of Kaneko. The stroke symptoms and signs scoring system were used to evaluate the animal model. Microscope and electronic microscope were used to watch the structure of rat brain. The expression of MARCKS and p-MARCKS was measured by Western blotting. The expression pattern of these proteins was further analyzed for their distribution at cellular level by immunohistochemistry staining of the tissues. 8.Effect of Xinmailing Solution on neuronal cell injury induced by glutamate

AIM: To detect the effects of Xinmailing Solution and MK-801 on injury of neuronal cell induced by glutamate. METHODS: Cultured neuronal cell injuried by glutamate was prepared and the content of malondialdehyde and nitrite in cell supernatant was measured. Morphology changes were also observed with discrepancy microscope at the same time. RESULTS: Xinmailing Solution and MK-801 attenuated cell injury induced by glutamate,and inhibited increase in malondialdehyde and nitrite in cell supernatant. CONCLUSION: Xinmailing Solution had a protective effect on neuronal cell at cell level.
9.Sorafenib in the treatment of advanced renal cell carcinoma (analysis of 33 cases)

Objective To investigate the efficacy and toxicity of sorafenib in the treatment of advanced renal cell carcinoma. Methods From May 2007 to JUN 2009, 33 patients with advanced renal cell carcinoma were given oral sorafenib 400-600 mg twice daily. There were 23 males and 10 females in the study group. The pathological diagnosis of the primary tumors was clear cell carcinoma in 29 patients, papillary renal cell carcinoma in 2 patients, chromophobe renal cell carcinoma in 1 patient and chromophobe renal cell carcinoma mixed with clear renal cell carcinoma in 1 patient. Fifteen patients had multiple organ metastases and 18 patients had single organ metastasis. The median follow-up time was 29 weeks. Results Four (12%) patients achieved partial remission, 2 (6%) patients achieved progression disease, the remaining 27 (82%) patients achieved stable disease. Complete remission was not observed in the group. Two of the partial remission patients benefited on bone metastases. Common toxicities were skin reaction (85%), diarrhea (46%), erythra (42%), alopecia (36%), oral ulcer (18%) and hypertension (9%). Conclusions Sorafenib could be effective in controlling tumor growth. The overall effectiveness was 12%, the disease control proportion was 94% in this group and its toxicity was relatively minor and well tolerated.
10.Analysis of Therapeutic Effect of X-knife on Intracranial Tumors in 44 Cases

Objective to investigate therapeutic methods and effect of X-knife for on intracranial diseases. Method Recent effect of 44pqtients with cranial diseases by X-knife was observed. Radiological follow-up was performed on 40 cases with mean 5.65 months of followup time. Result 92.5% of tumors were controlled locally, stability and recovery rate was 90.0%. The local control of metastatic tumors of brain was higher, but most patients with metastatic tumors died of primary lesion. New metastatic lesions appeared in patients without panencephalic radiotherapy in 1~5months. Tumors of pineal region were sensitive to X-knife. Conclusion X-knife has a definite effect on intracranial diseases. For patients with tumors of pineal region complicated by serious hydrocephalus, shunting should be conducted before X-knife treatment. For patients with mild or morderate hydrocephalus, X-knife chould be utilized only under correct interventions such as dehydration. The local control rate of intracranial metastatic tumors was high, but survival time postoperation depended on panencephalic radiotherapy or control of primafry lision. For tumors with diameter> 3cm, pituitary tumors, brains stem tumors and tumors in cerebellopontine angle region repeated X-knife were suggested, which could improve cure rate and decrease complications.
