Objectives The purpose of this study is to observe the effects of valsartan on hepapetic fibrosis. Methods Thirty male Sprague-Dawley rats were randomly divided into three groups: valsartan -prevetive group (A), modle group of hepatic fibrosis (B)and valsar-tan-treating group (C). The model of hepatic fibrosis in rats was induced by intraperitoneai injection of dimethylnitrosamine (DMN) for 4 weeks(2ml/kg everyday, three times a week). Valsartan (10mg/kg everyday) was given together with injection of DMN per intrngastric (Ig) in group A for 8 weeks. After stop injection of DMN, the S valsartan(10mg/kg, everyday)was given per Ig in group C for 4 weeks. After modeling, normal saline were given per Ig everyday in group B. At the end of eighth week, the histomorphylogic structure of the liver was ob-served with light microscope. Immunohistoebemical staining was used to evaluate the expression of a-SMA. Results In group B, there was a large necrotic area and a number of pesudolobes appeared in the liver tissue. In group A, there were normal hepatic cords. In the group C, there was fibrosis interval formation and portal area expansion and fibrotie intervals extending to the lobule. The quantitative analysis of Mas-son staining showed that the collagen quantities in group B was higher than that of other group(P<0.01). The collagen quantities in group A was lower than that of group C(P<0.05). The results of immanohistochemical staining showed that the expression of a-SMA in group B was strong positive, middle positive in group C, and weak positive in group A (P<0.05). Conclusion The valsartan has preventive and treatment effects on hepatic fibrosis in rats of hepatic fibrosis model induced by DMN, and the preventive effect of valsartan is better than its treatment effect. The valsartan can ameliorate the hver cirrhosis by partly suppressing the activation of HSC.

BACKGROUND: Alzheimer disease is a senile degenerative disease of nervous system. Neuron apoptosis is regarded as one of possible reasons, and neuron culture in vitro is a common method to research the mechanism of apoptosis.OBJECTIVE: To observe the influences of nitric oxide-donor, sodium nitroprusside (SNP), on the expression of gene cpp32 in the cultured hippocarnpal neurons in vitro.DESIGN: A randomized controlled animal experiment. SETTING: Institute of Biochemistry and Molecular Biology in Guangdong Medical College.MATERIALS: The experiment was carried out in the Institute of Biochemistry and Molecular Biology, Guangdong Medical College, and newborn (< 24 hours) Sprague-Dawley rats were used.METHODS: The hippocampl neurons of rats were primarily cultured, and then treated with SNP of different terminal concentrations (0, 25, 50, 100,200, 400 μmol/L) for 24 hours, and the expressions of mRNA and protein were analyzed with RT-PCR and Western blot respectively. The hippocampl neurons of rats were treated with SNP of different terminal concentrations (0, 25, 50, 100, 200, 400 μmol/L) for 12 hours, and the activity of CPP32 enzyme was detected with CPP32 activity detected kit.MAIN OUTCOME MEASURES: The expressions cpp32 mRNA and CPP32 protein and activity of CPP32 were detected.RESULTS: The cpp32 mRNA expression was unchanged as the increasing dose of SNP, but the pro-CPP32 was activated and the activity of CPP32 was increased significantly at 50 μmol/L SNP which was 3.02 times of that in the control group, and reached to the maximal value at 100 μmol/L which was 3.47 times of that in the control group.CONCLUSION: SNP cannot increase the cpp32 mRNA expression, but can increase degradation of pro-CPP32 and activate CPP32.

Objective:To discussed the diagnostic value of magnetic resonance cholangiopancreatography (MRCP), and the use of laparoscopic surgery in management of patients with choledochal cyst with extrahepatic bile duct anomaly. Methods:Of 330 consecutive patients who underwent laparoscopic choledochectomy at Guangzhou Women and Children's Medical Center from January 2010 to September 2018, there were 23 patients with extrahepatic bile duct anomaly. The data of these patients were retrospectively analyzed. There were 4 males and 19 females, with an average age of 3.2 (range 0.3~9.0) years. According to whether the extrahepatic bile duct anomaly was diagnosed by preoperative MRCP, these patients were divided into the preoperative MRCP diagnosis group and the preoperative MRCP undiagnosed group. The impact of MRCP in diagnosing bile duct anomaly to prevent bile duct injury, on operation time, hospital stay and the types of extrahepatic bile duct anomaly on outcomes of laparoscopic treatment were analyzed. Results:All the 23 patients with choledochal cysts complicated by extrahepatic bile duct anomaly were confirmed at surgery. The incidence of extrahepatic bile duct anomaly was 6.97% (23/330). There were 47.8% of type II AHD (11/23); 36.8% of type III AHD (7/23); 4.3% of type IV AHD (1/23); 17.4% (4/23) of the type with communication with accessory bile duct (CABD). The preoperative MRCP diagnosis group consisted of 14 patients, while the preoperative MRCP non-diagnosis group consisted of 9 patients, including 2 patients without MRCP. The diagnostic rate of MRCP in preoperative diagnosis of extrahepatic bile duct anomaly was 66.7%(14/21). The preoperative MRCP undiagnosed group, when compared with the preoperatives MRCP undiagnosed group, had a significantly higher bile duct injury rate [preoperative MRCP diagnosis group 7.1%(1/14), preoperative MRCP non-diagnosis group 55.6%(5/9)], and a significantly longer operation time [preoperative MRCP diagnosis group(232.6±10.0) min, preoperative MRCP undiagnosed group (278.9±22.45)min], (all P<0.05). Laparoscopic surgery was completed in 22 of 23 patients. One patient was converted to open surgery. AHD reconstruction was needed in 11 patients with type II AHD and 1 patient with type IV AHD. Seven patients with type III AHD did not require any surgical intervention for the anomaly. The 4 patients with CABD underwent simple ligation. Postoperative chyloperitoneum developed in 1 patient, who successfully responded to conservative treatment. Postoperative recovery was uneventful in the remaining patients. At a median follow-up of 2 years (range 1 to 5 years), no further complications, including intrahepatic bile duct dilatation and hepatic atrophy were detected. Conclusions:MRCP was effective in preoperative diagnosis of choledochal cysts with extrahepatic bile duct anomaly. It helped to decrease intraoperation injuries to bile duct anomalies. MRCP was also useful in classifying patients with extrahepatic bile duct anomaly to better preoperatively planning of surgical treatment strategies. Laparoscopic surgery could be completed in the majority of these patients with good postoperative results.

Objective To investigate the indications and results of long channel technique with pedicle inner preputial flap urethroplasty for middle hypospadias repair. Methods During September 2007 to April 2010,28 cases of middle hypospadias were include in this study. The average age was 2.0 years (1.5 -6.0 years). The orifice of urethra was opened in the shaft of penis, and the distance between the urethral meatus and the center of glans was 25 mm ( 16 - 37 mm).A pars-meatus skin incision was made, which was dissected deep to the urethral plate. The urethral plate was transected, and a long channel between the urethral plate and the corpora cavernosa was created. The neourethra was made from the inner prepuce, and transposed to the ventral tunnel through a tunnel between buck's fascia and albuginea of ventrolateral corpora cavernosa. The neourethra was anastomosed with the proximal urethra. The buck's and dartos fascia along the skin incision were brought together and sutured individually, covering the proximal neo-urethra and the anastomosis.Induced penile erection confirmed that 13 cases were with mild penile curvature, and the other 15 case were without penile curvature. The average length of the defected urethra and tunnel was 38 mm (30 -42 mm) and 33 mm (26 -38 mm). ResultsSuccess was achieved in all cases without fistula or urethral stricture formation with the average follow-up of 20 months (6 -31 months),and penile curvature was completely corrected. The urethra was opened in the apex of the glans with normal-looking circumcised penis. ConclusionsLong channel technique with pedicle inner preputial flap urethroplasty can provide another option for repairing middle hypospadias without penile curvature or with mild penile curvature, especially for young children and those with small penis. This technique is simple, and the result is satisfied.

Objective:
To investigate the proportion of achieving the blood lipid control target and its influencing factors among residents at a high risk of atherosclerotic cardiovascular disease (ASCVD), so as to provide insights into management of blood lipid among residents at a high risk of ASCVD.
Methods:
Residents at a high risk of ASCVD and at ages of 35 to 70 years were sampled using a multi-stage cluster sampling method from 6 counties (districts) in Shaoxing City from May to July 2021. The residents' demographics, smoking, alcohol consumption and medical history of chronic diseases were collected using questionnaires, the height, weight, waist circumference (WC) and blood pressure were measured, and the total cholesterol (TC), triacylglycerol (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C) and fasting blood glucose were detected. The proportion of blood lipids achieving the control target was analyzed, and factors affecting the proportion of blood lipids achieving the control target were identified using a multivariable logistic regression model.
Results:
A total of 1 695 individuals at a high risk of ASCVD were enrolled, including 940 men (55.46%) and 755 women (44.54%), with a mean age of (62.56±6.08) years. There were 285 participants that achieved the target of blood lipid control (16.81%). Multivariable logistic regression analysis identified gender (male, OR=1.962, 95%CI: 1.396-2.758), age (OR=1.037, 95%CI: 1.013-1.061), WC (OR=0.979, 95%CI: 0.964-0.995), diastolic blood pressure (OR=0.981, 95%CI: 0.967-0.994), smoking (OR=1.485, 95%CI: 1.034-2.133), alcohol consumption (OR=0.684, 95%CI: 0.498-0.941), hypertension (OR=1.428, 95%CI: 1.006-2.207), administration of hypoglycemic drugs (OR=2.326, 95%CI: 1.720-3.144) as factors affecting the achievement of the target for blood lipid control among residents at a high risk of ASCVD.
Conclusions
Individuals at a high risk of ASCVD with higher WC, higher diastolic blood pressure and alcohol consumption are less likely to achieve the target for blood lipid control, while male individuals with older age, hypertension and administration of hypogcemic drugs are more likely to achieve the target for blood lipid control.

BACKGROUND:Bone marrow mesenchymal stem cel s transplanted into rats exposed to silica dust can home to the injured lung, but the homing effects via different ways are stil unclear.
OBJECTIVE:To comparatively observe the distribution of bone marrow mesenchymal stem cel s transplanted via different ways into rats exposed to silica dust.
METHODS:Bone marrow mesenchymal stem cel s of donor rats were isolated through whole bone marrow adherent method and transfected by Lv-eGFP. Receptor rats were exposed to silica dust through windpipe injection and randomly divided into intravenous injection and intratracheal injection groups. Then, transfected bone marrow mesenchymal stem cel s were injected via the vein and trachea into acceptor rats. The acceptor rats were kil ed at weeks 1, 2, 3, 4 after transplantation to take the lung, heart, liver, spleen, kidney, and brain tissue that were made into frozen sections and observed under fluorescence microscopy. The intensity of green fluorescence (absorbance value) was analyzed using image analysis software.
RESULTS AND CONCLUSION:Strong, wide and lasting green fluorescence was both observed in the lung tissue of intravenous injection and intratracheal injection groups, which was especial y remarkable around the bronchus and blood vessels. The fluorescence intensities of both two groups were slightly decreased with time, but there was no difference between the two groups (P>0.05). The fluorescence in the other organs of both two groups was also observed at early stage. It was stronger and wider in the liver, spleen and heart, while fainter and less in the kidney and brain, and reduced with time in al the organs. Fluorescence could be observed few and faint only in the liver and spleen at late stage, and could hardly be seen in the brain. The fluorescence intensities of the liver, spleen, heart, kidney and brain had no significant difference between the two groups at the same time (P>0.05), but the fluorescence intensity in the brain at the 1st week showed significant difference between the two groups (P<0.05). These findings indicate that the intravenous injection and intratracheal injection of bone marrow mesenchymal stem cel s have similar homing effects in rats exposed to silica dust.

BACKGROUND:Bone marrow mesenchymal stem cels transplantationvia the trachea can relieve the lung injury of rats exposed to silica dust, but their distribution and migrationin vivois stil unclear. OBJECTIVE:To investigate the distribution and homing of bone marrow mesenchymal stem cels transplanted via the trachea into rats exposed to silica dust. METHODS: Bone marrow mesenchymal stem cels from Sprague-Dawley rats were isolated through bone marrow adherent method and transfected with lentivirus carrying enhanced green fluorescent protein gene (Lv-eGFP). Trypan blue staining and cel counting kit-8 were applied to assay the viability and proliferation of the transfected and untransfected cels. Sprague-Dawley rats, SPF level, were randomized into control group and silica dust exposure group. Rats in the two groups were respectively injectedvia the trachea with 1 mL of sterile silica dust suspensions (40 g/L) and 1 mL of normal saline. At 2 days after modeling, 2.2×106 transfected bone marrow mesenchymal stem cels were injected via the trachea into the rats of control group and silica dust exposure group. Rats were kiled at weeks 1, 2, 3, 4 after transplantation, and the distribution and intensity of green fluorescence in the lung, heart, liver, spleen, kidney, and brain tissue were observed under the fluorescence microscopy by frozen sections and analyzed using imaging analysis software. RESULTS AND CONCLUSION: When the multiplicity of infection was 50, there were no significant differences between the viability and proliferation activity of the transfected and untransfected cels (P > 0.05). After transplantation of transfected bone marrow mesenchymal stem cels, strong green fluorescence was observed widely in the lung, especialy around the bronchus and blood vessels, and stil obvious at the 4th week. The fluorescence of other organs also could be observed at the 1st week. It was strong and wide in the liver, spleen and heart, while faint and less in the kidney and brain, and al reduced with time. It shows bone marrow mesenchymal stem cels transplantedvia the trachea into rats exposed to silica dust can be homing to the injured lung of rats.
