This study was aimed to establish an effective method for the simultaneous content determination of strychnine and brucine in rat plasma. The Wistar rat plasma samples were processed by liquid-liquid extraction and separated on a UPLC BEH C18 column. The mobile phase was acetonitrile - acid water (17?83).The acid water was 0.02 mol·L-1.The potassium dihydrogen phosphate and 0.014 8 mol·L-1 sodium heptane sulfonate was mixed with equal amount. The 10% phosphonic acid was used to adjust the system to the pH of 2.8.The detection wavelength was set at 260 nm.The results showed that foreign materials in the Wistar rat plasma did not interfere with the content determination of samples. The calibration curve of strychnine was in good linearity within the range of 0.067 84-3.392 00μg·mL-1.The calibration curve of brucine was in good linearity with the range of 0.052 48-2.624 00μg·mL-1.The intra-and inter-day precisions were less than 10%.The mean extraction recoveries were more than 85%.The stability results met the requirements for biopharmaceutical analysis. It was concluded that the UPLC detection method was sensitive and precise, which can be used in the pharmacokinetics study of strychnine and brucine in Tong-Mai(TM) pill.

Objective:To investigate the effect of pidotimod in reducing pulmonary infection in patients with lung cancer undergoing chemotherapy. Methods:One hundred and twenty patients with lung cancer in the Fifth People′s Hospital of Dalian City from July 2017 to July 2018 were selected. The patients were divided into control group and pidotimod group by random digits table method with 60 cases each. The patients were treated with standard two drugs chemotherapy containing platinum drug according to the pathological type, and the patients in pidotimod group were combined with pidotimod. The number of pulmonary infections during chemotherapy, number of completed scheduled chemotherapy and adverse reaction were observed. The correlation between pulmonary infection and pidotimod was analyzed by multivariate orderly Logistic regression. Results:The incidence of pulmonary infection in pidotimod group was significantly lower than that in control group: 18.33% (11/60) vs. 40.00% (24/60), and there was statistical difference ( χ2 = 6.845, P<0.01). The rate of completed scheduled chemotherapy in pidotimod group was significantly higher than that in control group: 55.00% (33/60) vs. 36.67% (22/60), and there was statistical difference ( χ2 = 4.062, P<0.05). Multivariate orderly Logistic regression analysis result showed that pidotimod could reduce the risk of pulmonary infection ( OR = 0.210, 95% CI 0.072 to 0.606, P = 0.004), and help to complete the scheduled chemotherapy ( OR = 2.323, 95% CI 1.080 to 5.003, P = 0.031). In pidotimod group, no obvious adverse reaction related to pidotimod application was detected, and chemotherapy was not affected. Conclusions:Application of pidotimod can reduce the chance of pulmonary infection in patients with lung cancer undergoing chemotherapy and help patients complete scheduled chemotherapy.

Objective To evaluatc the efficacy and safcty of recombinant endostatin (Endostar)combined with concurrent radio-chemotherapy (CRCT) in patients with unresectable stage Ⅲ non-small cell lung cancer (NSCLC).Methods From March 2009 to November 2011,47 patients received threedimensional conformal radiotherapy of 60-66 Gy in 30-33 fractions over 6-7 weeks And concurrent chemotherapy of docetaxel 65 mg/m2 and cisplatin 65 mg/m2.Endostar was administered once a week before and on week 2,4,6 during CRCT at a dose level of 7.5 mg/m2/d.Tumor response was evaluated with thoracic CT scans performed 4 weeks after completion of treatment in accordance with RECIST 1.1 criteria.Acute toxicities were evaluated in accordance with CTCAE 3.0.Results Forty-four patients completed treatment and toxicity evaluation,42 patients completed evaluation of efficacy.Five patients achieved complete response,29 partial response,3 stable disease, and 5 progressive disease,2 were net assessed.Overall response rate was 77%.One-year overall survival rate was 81%,and one-year progression-free survival rate was 51%.Twelve patients died,2 died of treatment related toxicities,8 of cancer, and 2 of unknown causes.Nineteen patients developed grade 3/4 neutrocytopenia, grade 3 acute esophagitis and pneumonitis were observed in 4 and 4 patients, respectively, and 1 patient died of pneumonitis. No patient developed cardiovascular toxicities and hemorrhage. Conclusions Endostar combined with CRCT for unresectable stage Ⅲ NSCLC was safe and the short term outcomes were promising.Further investigations are warranted.

Objective To explore the histological basis of diffusion tensor tractography (DTT)fiber tracer images of benign prostatic hyperplasia (BPH)and prostate cancer (PCa).Methods Sixty-eight patients with PCa and sixty patients with BPH confirmed by pathology were recruited for this study.Conventional MRI and DTI examinations were performed. The original DTI data were processed at the AW4.5 workstation. After pos-t processed,FA and ADC values were recorded,DTT was plotted.Fiber bundle travel was observed and scored by two senior radiologists. The BPH and PCa specimens were stained with Masson fibers, and the diameter and angle of the fiber bundles were recorded. Results The FA values of BPH and PCa were 0.22±0.05 and 0.47±0.01 ,respectively.ADC values of BPH and PCa were (1.31 ±0.27)×10-3 mm2/s and (0.6 1 ±0.09)× 10-3 mm2/s,respectively.The differences in ADC and FA values among BPH and PCa were statistically significant (P<0.05).DTT showeded that the fibers of BPH were dense and long, while the fibers of PCa were messy and short. The diameter and angle of collagen fibers and smooth muscle fibers for BPH were both larger than those for PCa,but only the difference between the diameter of smooth muscle fibers was statistically significant (P<0.05).Conclusion There are significant differences between BPH and PCa in DTT images, which is consistent with the characteristics of fiber structure.DTI fiber bundle can reflect the differences of fiber structure between BPH and PCa.

Objective:To observe the clinical efficacy of combining repeated transcranial magnetic stimulation (rTMS) at low frequency with xing nao kai qiao acupuncture in the treatment of upper limb motor dysfunction after a stroke. Methods:Forty stroke survivors with upper limb motor dysfunction were randomly divided into a control group and a combination group, each of 20. All received basic neurological medication, conventional rehabilitation treatment, and xing nao kai qiao acupuncture. The combination group additionally received rTMS. Both groups were treated once a day, 5 days a week for 2 weeks consecutively. Before and after the treatment, both groups were evaluated using the Brunnstrom motor function stages, modified Ashworth spasticity grades, Fugl-Meyer upper limb motor function (FMA-UE) scoring, the modified Barthel index (MBI), and motor evoked potential (MEP) latency and amplitude. Results:The upper-limb and hand motor functioning of both groups improved significantly compared with before the treatment. The hand motor function staging of the combination group was then significantly better than among the control group. The average FMA-UE and MBI scores of both groups improved significantly, with significantly greater improvement in the combination group than in the control group. MEP latency and amplitude also improved significantly in both groups, with the average MEP latency and amplitude of the combination group superior to the control group′s averages. Conclusion:Supplementing xing nao kai qiao acupuncture with low-frequency rTMS can significantly improve the motor functioning, spasticity, and skill in the activities of daily living of stroke survivors with upper limb motor dysfunction.
