1.Clinic study on treatment of cardiac arrhythmia by fastigial nucleus stimulation

Objective:To study the curative effect and reliability of fastigial nucleus stimulation (FNS)in treating nonmalignant arrhythmia. Methods:In a random period,FNS was used to treat 45 patients,who consulted doctors for repetitive palpitation,had cardiac arrhythmia (supraventricular premature beats,supraventricular runs,ventricular premature beats)confirmed by 24 h Holter monitoring electrocardiogram,and whose state of illness did not improve after being treated by one or several antiarrhythmic drugs. After those patients were treated by FNS 40 minutes a time,two times a week,for 15 times,the changes of 24 h Holter monitoring electrocardiogram were observed. Results:Before and after treatment by FNS,the numbers of SVPB were (3 358.5?5 044.0)/24 h and (1 142.8?2 722.3)/24 h respectively;those ofVPBwere(10 469.7?9 464.9)/24 h and(4 379.4?8 143.3)/24 h respectively;normalized low frequencies(LFnorm)were(35.28?7.04)nU and(31.82?7.35)nU respectively;normalized high frequencies(HFnorm)were (24.32?4.18)nU and (26.84?4.66)nU respectively. Objective To investigate the effect of vitamin D on pulmonary function in patients with chronic obstructive pulmona-ry disease,and to explore its possible mechanism.Methods Patients with chronic obstructive pulmonary disease admitted in the hospital were randomly divided into 2 groups,one group of patients were given regular COPD treatment (control group,30 cases), the other group of patients were given vitamin D on the basis of conventional therapy(experimental group,30 cases).Pulmonary function test and C reaction protein (CRP)determination in the 2 groups were performed,the contents of PaO2 ,PaCO2 were also determined.Results Compared with the control group,lung function improvement in experimental group were more obvious,and the the number of acute attack significantly reduced(P <0.05).The concentration of CRP in experimental group was significantly lower after treatment than before treatment(P <0.05),while the concentration of CRP in control group didn′t change significantly (P >0.05).Conclusion Vitamin D contributes to the improvement of pulmonary function in patients with COPD,and its mecha-nism might relate to the reduction of inflammatory reaction.
3.Current status of physical activities among preschool children and relationship of activity type with bone mass

Objective To investigate physical activities of preschool children by gender and to explore the effects of activity type on bone indexes.Methods During 2009 and 2010,397 preschool children of 3-5 years old were randomly selected from 4 kindergartens in Chengdu Province of China.Ultrasound bone analyzer was used to assess children's bone mass.A physical activity questionnaire was completed by parents to evaluate physical activities at leisure time.Student's t test and least square regression were used for data analysis.Results Concerning activity types,boys spent more leisure time on running and Wushu than girls did (t values were 1.94 and 2.84,respectively ; both P < 0.05).However,girls spent more time on dancing (0.78 h),jumping rope (0.08 h) and manual labour (0.22 h) each day (t values were-9.50,-3.43 and-1.92,respectively; all P < 0.05).The weekly total exercise time and energy consumption per unit of body weight of girls vs.boys were 7.29 vs.6.51 h and 127.57 vs.113.85 kJ (t values were 2.63 and 2.04,respectively ; both P < 0.05).About per day time on sleeping and per week time on watching television,there were no significant difference between boys and girls (t =0.180,0.520;P >0.05).But boys spent more time on electronic game and computer than girls (t =0.760,2.510;P < 0.05).The normalized correlation coefficient for bone mass and moderate physical activities or jumping was 0.184 and 0.275,respectively (both P < 0.05).Conclusions Our data suggest that preschool children's bone volume may be positively correlated with moderate physical activities and jumping activities.Introducing some moderate physical activities or activity appliance,toys and playing fields as well as increasing professional training might be helpful.
4.Analysis of clinical feature of 152 cases of infectious mononucleosis

Objective To explore and observe the clinical characteristics and laboratory testing results of children with infectious mononuclear cells syndrome (IM),in order to improve diagnostic level.Methods 152 children with IM were selected,and the clinical manifestations,laboratory testing were retrospectively analyzed.Results There were a variety of clinical symptoms,the main clinical manifestations included fever 138 cases(90.8%),swollen lymph nodes 145 cases(95.4%),angina 112 cases (73.7%),hepatosplenomegaly 28 cases(18.4%),double eyelid edema 25 cases(16.4%),rash 7 cases(4.6%).Laboratory-testing of white blood cell count > 10 × 109/L in 125 cases (82.2 %),atypicallymphocyte proportion were more than or equal to 10% in 48 cases (31.5 %),liver function damage in 58 cases (38.2%),myocardial damage in 38 cases (25%),abnormal urine analysis in 30 cases (19.7%),thrombocytopenia 2 cases (3.4%).Conclusion Clinical symptoms of IM children are diversity,in some cases the clinical sympotoms are not typical,to improve the understanding of this disease can reduce the rates of missed diagnosis and misdiagnosis.
5.Study on the related factors of post-encephalitic epilepsy

Objective To analyze retrospectively the risk factors and predictors of post-encephalitic epilepsy (PEE) and refractory epilepsy in patients with encephalitis.Methods In a hospital based study,the patients with encephalitis were reviewed retrospectively between the January of 1995 and December of 2010.Related factors were evaluated including age,sex,seizure types,neuroimaging,electroencephalogram(EEG) in intermittent period,clinical symptoms,consciousness level,initial seizure and steroid hormone therapy,etc.Results 237 patients with encephalitis were enrolled,whose median age was 26.3 (range 15-57) years old.PEE occurred in 103 (43.46%) patients; and 67 of whom had partial seizure.Significant risk factors for PEE included age (OR =3.72,95% CI 2.70-5.25,P =0.018),disturbance of consciousness level(OR =5.37,95% CI 2.43-13.03,P =0.012),cortical lesion in imaging (OR =11.42,95% CI 5.94-31.27,P =0.000),spike discharges in EEG (OR =18.04,95% CI 7.30-48.38,P =0.000) and initial seizures in acute phase (OR =32.68,95% CI 9.62-97.59,P =0.000).The refractory epilepsy occurred in 6t patients.The significant risk factors of refractory PEE included focal seizures(OR =4.09,95% CI 2.14-9.10,P =0.021),status epilepticus (OR =4.48,95% CI 1.89-8.07,P =0.017) and poor controlled seizure (OR =6.17,95% CI 3.52-11.34,P =0.001) during acute phase,multifocal spikes discharge in EEG(OR =5.53,95% CI 2.91-10.07,P =0.006),cortical lesion in neuroimaging(OR =2.33,95% CI 1.37-7.72,P =O.028),however,early steroid hormone therapy (OR =2.19,95% CI 1.11-4.87,P =0.037) and longer time to initial seizure (OR =4.40,95% CI 3.19-11.62,P =0.014) could significantly reduced the incidence of refractory epilepsy in PEE patients.Conclusion Our data indicated that PEE occur in 43.46% patients especially in younger patients with disturbance of consciousness level,cortical lesion in imaging,spike discharges in EEG and initial seizures in acute phase.And the risk factors for refractory PEE are also discussed.
6.The clinical-electroencephalographic features of the 6 patients with idiopathic generalized epilepsy in adults with phantom absences

Objective To investigate the clinical and electroencephalogram (EEG) features of patients with idiopathic generalized epilepsy in adults with phantom absences.Methods Six patients were referred to the clinic of epilepsy from April,2007 to December,2011.They all had clinical assessment,EEG,or video EEG confirming absences seizure.Results Six patients showed the following similar clinical-EEG features:(1) mild ictal impairment of consciousness associated with generalized 3.0-3.5 Hz spike and slow wave discharges; (2) late-onset generalized tonic-clonic seizures; (3) absence status epilepticus with or without secondary generalized tonic-clonic seizures; (4) generalized discharges were mostly seen in three types in the awaking stage:fragmented discharge (<4 s),brief discharge (4-10s) and long-time discharge (> 10 s).None of the patients had myoclonic jerks or photosensitivity.One patient' s mother had a history of generalized tonic clonic seizures.One patient had a history of children absence epilepsy and one patient had a history of febrile convulsion in the age of 1-3.Conclusion Idiopathic generalized epilepsy with phantom absences has distinct clinical and EEG features and may become a new idiopathic generalized epilepsy syndrome in adults.
7.Serum omega-3 polyunsaturated fatty acid and insulin resistance in type 2 diabetes mellitus and non-alcoholic fatty liver disease

Objective To investigate the relationship between serum omega-3 polyunsaturated fatty acid (ω-3PUFA) and insulin resistance(IR) in patients with type 2 diabetes mellitus and non-alcoholic fatty liver disease (NAFLD). Methods This trial involved 51 patients of type 2 diabetes mellitus with NAFLD (G4 group), 50 patients of type 2 diabetes alone(G3 group), 45 patients of NAFLD alone (G2 group) and 42 healthy control subjects (G1 group). Serum ω-3PUFA profile was analyzed with capillary gas chromatography. Insulin resistance was assessed by homeostasis model assessment (HOMA-IR). ALT, AST, γ-glutamyltransferase(GGT) and serum lipids were measured. Results The levels of HOMA-IR were higher in G4 group than those in G3, G2 and G1 group(4. 90 ± 2. 54 vs 2. 38 ± 1.23, 2. 20 ± 1.15, 1.13± 0.42;P<0.05). The level of ALT, AST, GGT, TC, TG, LDL-C were higher in G4 group than those in G3, G2 and G1 group(P < 0. 05). The level of ω-3PUFA was signifieantly lower in G4 group than those in G3, G2 and G1 group(5.68 ±2.02 vs 7. 17 ±2. 38, 6.97±2.32, 10.08±2.76;P <0.05). ω-3PUFA cocentration was negatively correlated with HOMA-IR, TC, TG and LDL-C (r = -0.491, -0. 376, - 0. 462, - 0. 408, P < 0. 05). Conclusions Serum ω-3 PUFA is significantly decreased in patients with type 2 diabetes mellitus and NAFLD. Serum ω-3PUFA is negatively correlated with insulin resistance. ω-3PUEA plays a very important role in the development of diabetes mellitus and NAFLD.

The association of the serum phospholipid fatty acid components with high-sensitivity C reactive protein in patients with metabolic syndrome and type 2 diabetes mellitus was investigated.Saturated fatty acid in patients with metabolic syndrome and type 2 diabetes mellitus was increased (45.98±1.55 vs 42.05±2.04,P<0.01),while polyunsaturated fatty acid was decreased(42.38±1.93 vs 46.60±2.06,P<0.01),and high sensitivity C reactive protein was significantly increased[(6.36± 1.28 vs 0.79±0.45) mg/L,P<0.01)].
9.Evaluation of left ventricular systolic function in different layers myocardium in patients with hypertrophic cardiomyopathy by two-dimensional speckle tracking echocardiography

Objective To assess the systolic function in left ventricular subendocardial myocardium , midmyocardium and subepicardial myocardium in patients with hypertrophic cardiomyopathy (HCM ) by two‐dimensional speckle tracking echocardiography .Methods A total of 32 patients with HCM and 32 healthy adults were included in the study .All standard measurements were obtained from the left ventricular short‐axis views at the levels of mitral annulus ,papillary muscle and apex ,and apical four‐chamber ,two‐chamber ,and long‐axis views .Two‐dimensional speckle tracking imaging was employed .The global peak systolic radial strain ,peak systolic longitudinal strain and circumferential strain of each layer of the left ventricle were measured .Results In patients with HCM and the control group ,the peak systolic longitudinal strain and circumferential strain reduced from subendocardial myocardium to subepicardial myocardium .The global peak systolic radial strain in patients with HCM was significant lower than that in the control group . And in patients with HCM ,peak systolic longitudinal strain in subendocardial myocardium ,midmyocardium and subepicardial myocardium was significant lower than those in the control group .Conclusions Peak systolic longitudinal strain and circumferential strain are reduced from subendocardial myocardium to subepicardial myocardium .Myocardial systolic function is impaired in patients with HCM .In patients with HCM ,longitudinal strain of each layer is lower ,but circumferential strain is mainly reduced in midmyocardium and subepicardial myocardium compared with that in the control group .
10.Detection of the viscoelasticity of relaxed myocardium in vivo by intervention ultrasound indentation system

Objective To explore the viscoelasticity of relaxed myocardium in vivo through indentation method with an intervention ultrasound indentation system (IUIS). Methods Old myocardial infarction (OMI) models of canine were established by ligating left anterior descending branch of coronary artery for 3 months. The indentation creep tests were used respectively in OMI group and shame group (n=8, each) by IUIS in middle and advanced diastole stage in vivo. Test data were processed with three-parameter solid viscoelasticity model, and the viscoelastic parameters, such as instantaneous elastic modulus (E1), relaxation modulus (E∞), creep elastic modulus (E2) and viscous damping coefficient (η) in normal and infarcted myocardium were obtained and compared. Results All the parameters of E1, E∞, E2 and η increased obviously in OMI group than in sham group shown as follows: 27.81±6.74kPa vs. 6.78±2.43kPa; 17.87±3.59kPa vs. 4.52±1.56kPa; 49.54±14.35kPa vs. 16.82±12.37kPa and 1.97±0.78Pa.s vs. 0.66±0.40Pa.s. The differences were statistically significant (P<0.05). Conclusions IUIS is a feasible method to assess the viscoelasticity of relaxed myocardium in vivo. Three-parameter viscoelasticity model can be used to describe creep properties of relaxed myocardium. Both elastic modulus and viscosity resistance have increased in infarcted myocardium.
