1.The analysis of clinical features and prognosis factors in patients with severe fever with thrombocytopenia syndrome

Objective To explore the influence of clinical indicators in patients with thrombocytopenia syndrome (SFTS) with severe fever on disease development and prognosis.Methods The SFTS patients who were admitted to the Department of Infection Diseases of Wuhan Union Hospital between April 1, 2015 and October 30, 2015 were included.Among 164 patients with SFTS, there were 136 cases recovered and 28 cases died.Clinical date including clinical manifestations and laboratory test index were collected and retrospectively analyzed.The correlation between the prognosis and clinical indicators were analyzed with the Spearman correlation analysis, the discriminatory power of clinical dates were tested with the area under the receiver-operating characteristic curve.Results The average age of died patients in the study was (64.5±9.1), which increased significantly(t=-3.609,P<0.01) compared with recovered patients with (56.9±10.2).Laboratory results suggest all patients had a drop in white blood cell and platelet count;almost all patients had a drop in serum calcium levels, and a rise in aspartate aminotransferase, alanine aminotransferase, lactate dehydrogenase, creatine kinase (CK), activated partial thromboplastin time (APTT), D-dimmer levels.The nucleic acid quantification of SFTSV suggests death patients (5.83±0.98) lg TCID50/mL was significantly higher than the recovered patients (3.96±1.08) lg TCID50/mL, with statistical difference (t=-8.49, P<0.01).Conclusions Age, the nucleic acid quantification of SFTSV, CK and APTT are factors can be used to predict prognosis of SFTS.
2.Observation of immunoglobulin A secreting cells and secretory immunoglobulin A in jejunum of mice infected with plerocercoids in snakes

Objective To observe the immunoglobulin A secreting cells (IgASCs) and secretory IgA (sIgA) level in jejunum of mice infected with plerocercoids,and to understand the roles of resistance to invasion processes of the plerocercoids.Methods A total of 100 Kunming mice (half males and half females) were chosen,the weight was 20-25 g,they were randomly divided into control and experiment groups according to their body weight via the random number table method,50 per group.Mice of experiment group were fed each with 5 plerocercoids in snakes,and mice of control group were not infected,testing time and methods were the same in the two groups.Ten mice were randomly sacrificed from one group on days 1,7,14,28 and 56 after infection,to collect empty intestinal juice and jejunal segment.The immunohistochemical method was used to examine the quantity of IgASCs in jejunal mucosa,while enzyme-linked immunosorbent assay (ELISA) was applied to test the level of slgA of jejunal fluid.Results The IgASCs were distributed in lamina propria of the jejunal mucosa,and the percentage of positive IgASCs reached the peak value [(64.24 ± 0.60)%] at d 1 after infection in experiment group,then decreased,and it was lower than control group at d 14 [(41.98 ± 0.42)% vs (43.52 ± 0.94)%,t =-4.727,P < 0.01].The sIgA level reached the peak value [(22.05 ± 1.43) mg/L] at d 7 after infection in experiment group,then decreased,and there was no statistical significant difference between control and experiment groups at d 56 [(20.00 ± 0.42) mg/L vs (21.26 ± 2.59) mg/L,t =1.516,P > 0.05].There was a positive correlation between the percentage of positive IgASCs in the jejunal mucosa and sIgA level in the jejunal fluid at d 7 after infection (r =0.663,P < 0.01),and there was a negative correlation between them at d 14 after infection (r =-0.542,P < 0.05).Conclusion The plerocercoids infection might induce high level expressions of IgASCs and sIgA,they show positive correlation at d 7 after infection.

Objective:To study the immunoregulatory activity and mechanisms of Chinese medicinal herb Rhizoma Menipermi extracts.Compare the biological activity of Rhizoma Mednispermi extract with water (RMW),ethanol (RME) and Rhizoma Menispermi Polysaccharide (RMP).Methods:The chemical composition of Rhizoma Menipermi extract was separated with water,ethanol extraction and distillation methods.The effect of Rhizoma Menipermi extracts on lymphocytes and the effect on the metabolism and phagocytosis of mouse macrophages were studied by MTT colorimetric method and neutrol red method.Results:RMP can stimulate the proliferation of mouse spleen cells and human lymphocytes and enhance the metabolism and phagocytosis of mouse macrophages.RMW and RME have inhibitory effect on mouse spleen cells and human lymphocytes and mouse in vitro,and inhibited the metabolism and phagocytosis of mouse macrophages.Conclusion:Diffrent Rhizoma Menipermi composition showed diffrent effects on lymphocytes,RMW and RMP can be used for the supplementary treatment of cancer. 4.Effect of Repetitive Transcranial Magnetic Stimulation on Motor and Ambulation Function for Incomplete Spinal Cord Injury

Objective To assess the effect of repetitive transcranial magnetic stimulation (rTMS) on motor and ambulatory function in incomplete spinal cord injury (SCI) patients. Methods 18 incomplete SCI patients (AIS D) were randomized to treatment group (n=10) and control group (n=8). The treatment group received rTMS while the control group received sham stimulation for 2 weeks. All the patients received routine rehabilitation. They were assessed with Lower Extremity Motor Score (LEMS), 10 m Walking Test for Walking Speed, modified Ashworth scale (MAS), Walking Index for SCI Scale II(WISCI II), and Spinal Cord Independence Measure (SCIM) before and after treatment, and followed up for 2 weeks after treatment. Results The treatment group significantly improved in LEMS, walking speed, and SCIM after treatment and during follow up (P<0.05), while the control group improved only in SCIM (P<0.05). There was more significant improvement in LEMS in the treatment group than in the control group (P<0.05) after treantment and during follow up. There was no difference between two groups in MAS, walking speed, WISCI II and SCIM. Conclusion rTMS can further improve the motor of lowere limbs for incomplete SCI patients.
5.Study on mechanism of the anti-tumor activity of Acanthopanax gracilistylus.

OBJECTIVETo study the mechanism of anti-tumor activity of Acanthopanax gracilistylus extract (Age).
METHODSThe tumor cells proliferation was detected by using (3H)-TdR incorporation method, and the effects of Age on cell cycle of tumor cells, retinoblastoma (Rb) protein and cyclin-dependent kinases (Cdk) were analyzed by flow cytometry and Western blotting assay, respectively.
RESULTSIt was indicated by cytoactivity test in vitro that Age only had effect in inhibiting the proliferation of tumor cells, it couldn't lead to death of cells. Under action of Age, the proliferation of tumor cells was halted at G0/G1 stage of cell cycle, and showed no direct cytotoxic effect by Age. Age could induce lowering of the expression of Rb, Cdk2 and Cdk4, cause halt of tumor cell proliferation.
CONCLUSIONThe tumor inhibitory effect of Age is realized by way of regulating the activity of cell cycle controlling enzymes to suspend the proliferation of tumor cells.

Novel coronavirus Omicron variant infection can cause severe illness and even death in certain populations. Omicron variant infection may lead to systemic inflammatory response, coagulation disorder, multi-organ dysfunction and other pathophysiological changes, which are different from other Novel coronavirus variants to a certain extent, so therapeutic strategies should not be the same. The National Medical Center for Major Public Health Events invited experts in fields of infectious diseases, respiratory medicine, intensive care, pediatrics and fever clinic to develop this quick guideline based on the current best evidence and extensive clinical practices. This quick guideline aims to standardize the diagnosis and treatment of novel coronavirus Omicron infection, and to improve the disease management abilities of clinicians.
7.Reform and exploration of teaching methods for eight-year program medical students' early exposure to clinical practice

Objective:To explore the effect of early clinical clerkship training in the early exposure to clinical practice of eight-year program medical students.Methods:Experimental control method was used in this study. A total of 120 eight-year program medical students in the third year of Batch 2014 to 2016 from the Union Hospital, Tongji Medical College, Huazhong University of Science and Technology were selected as the research objects. The students of each grade were randomly selected by computer and divided into experimental group and control group, with 20 students in each group. Before early clinical exposure, the experimental group received 16 class hours of early clinical clerkship training, while the control group did not receive early clinical clerkship training. After the early clinical exposure, the clinical diagnosis and treatment ability of the two groups of students was compared. SPSS 24.0 software was used for t test. Results:The scores of medical history inquiry of experimental group and control group were [(17.45±1.96) points and (15.95±1.93) points; (18.30±1.03) points and (16.75±1.86) points; (17.95±1.36) points and (16.40±1.60) points, respectively]. The physical examination scores were [(17.75±1.65) points and (16.05±1.64) points; (17.85±1.18) points and (16.80±1.47) points; (18.25±1.16) points and (16.85±1.63) points, respectively]. The clinical judgment scores were [(18.15±1.42) points and (16.35±2.41) points; (18.20±1.24) points and (16.65±1.53) points; (18.35±1.35) points and (16.25±1.83) points, respectively]. Diagnosis and treatment scheme scores were [(17.15±1.57) points and (14.55±2.56) points; (17.30±1.42) points and (15.90±1.48) points; (17.80±1.06) points and (16.35±1.87) points, respectively]. The scores of communication skills were [(17.95±1.15) points and (17.00±1.19) points; (18.55±0.83) points and (17.45±1.50) points; (18.45±1.00) points and (17.45±1.23) points, respectively], with statistically significant differences (all P<0.05). Conclusion:The application of early clinical clerkship training in the early exposure to clinical practice of eight-year program medical students can improve the quality of students' clerkship.
