1.Determination of Ginsenoside Rg_1 in Yushangling Capsules by HPLC

Objective To established a method for the content determination of ginsenoside Rg1 in Yushangling Capsules by HPLC. Methods HPLC was used with the C18 column. The mobile phase was acetonitrile-0.5% phosphoric acid (24∶76). The flow rate was 1.0 mL/min, the column temperature was 25 ℃, and the detection wave was set at 205 nm. Results The calibration curve was linear in the range of 0.05~0.80 ?g. The regression equation was Y =3629375.5X+2517.1, r =0.9998. The average recovery was 100.48% and RSD was 1.59%. Conclusion The method is simple, accurate and suitable for the content determination of ginsenoside Rg1 in Yushangling Capsules.
2.Influence of Mixed Decoction of Cortex Phellodendri,Radix Glycyrrhizae and Ramulus Loranthi on Ex-traction Rate of Berberine

OBJECTIVE:To study the infuence of mixed decoction of Cortex Phellodendri,Radix Glycyrrhizae and Ramu?lus Loranthi on extraction rate of berberine.METHODS:The extraction amount from Cortex Phellodendri,Radix Glycyrrhizae and Ramulus Loranthi mixed decoction was determined by HPLC.The HPLC conditions were:Hypersil BDS C 18 column;mobile phase,acetonitrile-33mmol/L KH 2 PO 4 -triethylamine(20∶72∶0.1);detecting wavelength,345nm;column temperature25℃;flow fate1.0ml/min.RESULTS:Decoction of Cortex Phellodendri,Radix Glycyrrhizae and Ramulus Loranthi in combi?nation obviously decreased the extraction rate of berberine.CONCLUSION:Cortex Phellodendri should be extracted separately in formulating extract technic for preparations containing above-mentioned herbs.
3.Stability of Shenkang Injection Respectively in Five Different Solutions

To explore the stability of Shenkang injection respectively in five different solutions to choose the best com-patibility program and improve the rational drug use. Methods:The pH value, insoluble particles and the content of protocatechuic al-dehyde in Shenkang injection were determined after mixed with five solutions. Results:In the 6-hour mixing, the change order of proto-catechuic aldehyde content was 0. 9% sodium chloride injection> 5% fructose injection> 10% invert sugar injection > 5% dextrose injection> 10% glucose injection, especially in 0. 9% sodium chloride injection, the content was decreased by 20%. The number of insoluble particles was increased after the 2-hour mixing except in 10% invert sugar injection solution, and the increase in the four so-lutions was beyond the requirement in the 2010 edition of Chinese Pharmacopoeia, and the number of insoluble particles in 0. 9% sodi-um chloride injection was the highest with the fastest change. During the experimental process, the pH value was stable. Conclusion:The five solutions show influence on stability of Shenkang injection in varying degrees. Shenkang injection in 0. 9% sodium chloride in-jection is the least stable, which should be used with caution in clinics, while invert sugar injection and glucose injection can be the best choice for Shenkang injection and the mixed solution should be used up in 2h.
4.Determination of Blood Concentration of Pioglitazone Hydrochloride by HPLC

OBJECTIVE:To establish the method of determining the blood concentration of pioglitazone hydrochloride. 5.Antibacterial Activity in vitro of Xiyanping Injection Combined with 7 Kinds of Antibiotics

Objective: To study the antibacterial activity in vitro of Xiyanping injection combined with cefoxitin, cefuroxime, ceftriaxone, cefmenoxime, amikacin, levofloxacin and meropenem against staphylococcus aureus, escherichia coli, klebsiella pneumoniae, pseudomonas aeruginosa and acinetobacter bauman.Methods: Minimum inhibitory concentration (MIC) of the 7 kinds of antimicrobial agents used individually or combined with Xiyanping injection on germs were determined respectively on 96 plates by using the agar dilution method and the checkerboard method.Fractional inhibitory concentration index (FICI) was calculated to evaluate the antibacterial activity of Xiyanping injection combined with the 7 kinds of antimicrobial agents in vitro.Results: MIC of Xiyanping injection combined with cefoxitin was the same as that of cefoxitin used individually on escherichia coli.MIC of levofloxacin combined with Xiyanping injection showed no difference with that of levofloxacin used individually on klebsiella pneumoniae.Besides, Xiyanping injection could increase the MIC of the other antimicrobial agents on germs.Conclusion: Xiyanping injection combined with antimicrobial agents on staphylococcus aureus, escherichia coli, klebsiella pneumoniae shows antagonistic and irrelevant relationship.
6.The value of aspirin challenge tests in the diagnosis of non-steroidal anti-inflammatory drugs-exacerbated respiratory disease

Objective:To investigate the value of aspirin challenge tests in the diagnosis of non-steroidal anti-inflammatory drugs-exacerbated respiratory disease (NERD).Methods:Fifty patients (22 males and 28 females; aged 16-61 years) who were diagnosed with chronic rhinosinusitis with nasal polyps (CRSwNP) with/without asthma, and underwent NERD standardized diagnosis in the Allergy Centre of West China Hospital, Sichuan University from December 2021 to November 2022 were included in the study. The first step was asking about the history of exacerbation respiratory symptoms after intake of any non-steroidal anti-inflammatory drug, including aspirin; the second step was performing intranasal aspirin challenge (IAC); and the third step was performing oral aspirin challenge (OAC). The diagnosis of NERD was made if any of the above steps was positive, and the subsequent steps were not performed, otherwise the diagnosis was made to OAC. If OAC was negative, the diagnosis was non-NERD. All patients completed the sino-nasal outcome test 22 (SNOT 22) score, Lund-Kennedy score by nasal endoscopic, allergen skin prick test, blood routine and serum total IgE test. SPSS version 20.0 was used for statistical analysis.Results:The diagnosis of NRED was confirmed in 27 patients (27/50, 54%). Seven (7/50, 14%) of them were diagnosed by clinical history and 20 (20/50, 40%) were diagnosed by aspirin challenge tests, of which 17 (17/20, 85%) were positive to IAC and 3 (3/20, 15%) to OAC. Of the 43 patients who underwent IAC testing, only 2 (2/43, 5%) developed asthma attacks during challenge. Comparing the clinical characteristics of patients in NERD and non-NERD group, there were significant differences between the two groups in gender ( P=0.001), hyposmia ( P=0.003), history of repeated CRSwNP surgeries ( P=0.028), comorbid asthma ( P=0.013), SNOT-22 score ( P=0.004) and the percentage of peripheral blood eosinophil ( P=0.043). Conclusions:Patients may be underdiagnosed if the diagnosis of NERD is made only by medical history, and it is necessary to carry out aspirin challenge tests. IAC is an important means to diagnose NERD with high accuracy and good safety. However, If IAC is negative, further OAC is required.
7.Clinical analysis of 36 cases of neonatal-onset genetic epilepsy

Objective:To study the genetic profiles and clinical characteristics of neonatal-onset genetic epilepsy.Methods:From July 2016 to May 2021, patients with neonatal-onset genetic epilepsy admitted to our hospital and received second-generation genetic sequencing were enrolled in this study. According to the types of genetic variations, the patients were assigned into ion channel group and non-ion channel group. Clinical characteristics, treatments and prognosis of the two groups were compared.Results:A total of 36 patients with identified genetic variations were enrolled, involving 15 epilepsy-related genes. KCNQ2, SCN2A and STXBP1 were the most common pathogenic genes. 20 cases (55.6%) were in the ion channel group and 16 cases (44.4%) in the non-ion channel group. No significant differences existed in their general status, seizure types, EEG characteristics, treatments and outcomes between the two groups ( P>0.05). Among all 36 cases, the age of onset ranged from 10 min to 24 d after birth and 28 cases (78.8%) developed epilepsy within 1 week after birth. Developmental and epileptic encephalopathies were diagnosed in 20 patients. 7 patients were diagnosed with self-limited neonatal epilepsy, 2 were pyridoxine dependence, 2 were Zellweger syndrome and 1 case of self-limited familial neonatal-infantile epilepsy, Turner type mental retardation with epilepsy, PURA syndrome, Rett syndrome and 22q11.2 deletion syndrome, each. The patients received antiepileptic drugs including phenobarbital, levetiracetam, oxcarbazepine, topiramate, valproic acid, benzodiazepines (nizepam/clonazepam /clobazam/midazolam), lacosamide and lamotrigine. 5 patients died after giving up treatment. 31 patients were followed up for 6 to 50 months. 22 cases (71.0%) were controlled at 1- to 35-month-old including 21 cases (56.7%) with developmental delay. 6 cases (19.4%) had ineffective seizure control and 3 cases (9.7%) showed reduced seizures, all with varying degrees of developmental delay. Conclusions:Neonatal-onset epilepsy is correlated with multiple genes. KCNQ2, SCN2A, STXBP1 are the common pathogenic genes with multiple variants of KCNQ2 gene. Most patients have seizures within 1 week after birth. More than half of patients have ion channel related gene variations. Sodium channel blockers have certain effects as treatment.
8.Association between Yang Deficiency Syndrome and the End-point Events of Diabetic Kidney Disease: A Retrospective Cohort Study

ObjectiveTo investigate the impact of yang deficiency syndrome on the progression to end-point events of diabetic kidney disease (DKD). MethodsA retrospective study among patients with stage Ⅳ DKD admitted to Dongzhimen Hospital of Beijing University of Chinese Medicine from September 1st, 2016 to September 30th, 2021 was conducted. Data on the patients' general information, clinical indicators including duration of diabetes, duration of proteinuria, history of smoking and drinking, hemoglobin (HGB), fasting blood glucose (FBG), albumin (ALB), serum creatinine (Scr), urea nitrogen (BUN), uric acid (UA), cholesterol (TC) , triglycerides (TG), low-density lipoprotein (LDL), 24-hour urine protein quantification (24h-UTP) and estimated glomerular filtration rate (eGFR), and TCM syndromes including symptoms, tongue and pulse, and syndrome scores were collected. The patients were divided into exposure group (yang-deficiency group) and non-exposure group (non-yang-deficiency group). The general information, clinical indicators and incidence rates of end-point events were compared, and the impact of yang deficiency syndrome on the end-point events of stage Ⅳ DKD was analyzed. Survival analysis was performed using Kaplan-Meier method, and multivariate Cox proportional risk models were used to identify independent predictors of end-point events. ResultsA total of 160 patients with stage Ⅳ DKD were included in the study, including 43 cases of yang deficiency syndrome and 117 cases of non-yang deficiency syndrome. Compared to those in the non-yang deficiency group, the waist circumference, BUN and the incidence of end-point events in the yang deficiency group were significantly higher (P<0.05 or P<0.01). Spearman correlation analysis showed that yang deficiency syndrome was positively correlated with incidence of end-point events of stage Ⅳ DKD (r = 0.167, P = 0.035). Furthermore, 24h-UTP and BUN levels were also positively correlated with end-point events in stage Ⅳ DKD patients (P<0.01), while ALB and HGB levels were negatively correlated (P<0.01). Kaplan-Meier survival curves showed that yang deficiency syndrome was associated with an increased risk of end-point events (Log Rank P = 0.011). Moreover, 24h-UTP levels ≥3500 mg, BUN level ≥8 mmol/L, ALB level <30 g and HGB level <11 g were all associated with the increase of the risk of end-point events (P<0.05 or P<0.01). Multivariate Cox regression analysis showed that yang deficiency syndrome was an independent risk factor for patients with stage Ⅳ DKD to progress into end-point events (HR = 2.36, 1.32 to 4.21; P = 0.004), as well as 24h-UTP ≥ 3500 mg, BUN ≥ 8 mmol/L, HGB<11 g and ALB<30 g (P<0.05 or P<0.01). ConclusionsFor stage Ⅳ DKD, patients with yang deficiency syndrome are more likely to have end-point events, which is an independent risk factor for the progression into end-point events.
9.Effect of lathyrol derivatives on non-small cell lung cancer and the possible mechanism.

OBJECTIVES:
Non-small cell lung cancer (NSCLC) accounts for 85% of all lung cancer, with highmorbidity and mortality rate. Nove drug development for NSCLC is urgently needed.This study aims to investigate the activity of lathyrol derivatives and the mechanism for its inhibitory effect on the growth of NSCLC cells.
METHODS:
Three lathyrol derivatives were synthesized from lathyrol and their structures were verified by nuclear magnetic resonance. MTT assay was used to detect the effects of the lathyrol derivatives on the proliferation activity of NSCLC cells (A549 and H1299 cells), and the compound with the best activity was selected for subsequent experiments. Colony forming assay, wound-healing assay, and transwell assay were applied to detect in vitro cell proliferation, migration and invasion ability in A549 and H1299 cells, respectively. Quantitative real-time RT-PCR and Western blotting were performed to detect mRNA and protein levels of E-cadherin, N-cadherin, β-catenin, and MMP2 in A549 cells, respectively.
RESULTS:
Three lathyrol derivatives inhibited the growth of A549 and H1299 cells in a dose-dependent manner, and they showed a weak inhibitory effect on normal cells Beas-2B and 16HBE, indicating that they possessed certain selective toxic effects. Therefore, C-5 benzoylated lathyrol with the best activity was selected as the ideal drug for the subsequent experiments. Compared with the control group, the number and size of cell clusters in the treatment group of A549 and H1299 cells were significantly decreased, the relative mobility were significantly decreased, and the number of invaded cells were significantly decreased (all P<0.05), indicating that the in vitro cell proliferation, migration and invasion ability were decreased. The mRNA levels of integrin α2, integrin β1, MMP2, MMP9, β-catenin, and N-cadherin were decreased, while the expression of E-cadherin was increased (all P<0.05). The protein levels of N-cadherin, β-catenin, MMP2, and integrin αV were decreased, while the expression of E-cadherin was increased (all P<0.05).
CONCLUSIONS
The lathyrol derivatives synthesized in this study possess good inhibitory activity against NSCLC. Among them, C-5 benzoylated lathyrol significantly inhibits the proliferation, migration, and invasion ability of NSCLC cells in vitro through regulating the process of epithelial-mesenchymal transition.

10.Value of allergen nasal provocation test in assessment of the efficacy of house dust mites specific immunotherapy.

Objective:To investigate the value of nasal provocation test(NPT) in evaluating the efficacy of allergen immunotherapy(AIT) in patients with dust mite induced allergic rhinitis(AR). Methods:A total of 83 patients with dust mite induced AR with/without asthma were included. Symptom score(SS), daily medication score(DMS), combined symptom and medication score(CSMS), rhinoconjunctivitis quality of life questionnaire(RQLQ), NPT and skin prick test(SPT) were assessed before and after 1 year AIT. Results:There were statistical differences in SS(P<0.000 1), DMS(P<0.000 1), CSMS(P<0.000 1), and RQLQ(P<0.000 1) after 1 year of AIT compared with pre-treatment. The effective rate of CSMS was 73.49%, and the effective rate of NPT was 42.17%. CSMS was consistent with NPT in efficacy assessment(Kappa=0.437, P<0.001); while in 54 patients with pre-treatment NPT concentrations other than the original concentration, CMSM and NPT showed better consistence(Kappa=0.895, P<0.001). Among the 48 patients with ineffective NPT assessment in the first year, 25 patients completed the second-year follow-up, and 12 patients(48.00%) showed effective in NPT. However, 10 out of 12 patients(83.33%) with NPT concentration other than original solution pre-treatment showed effective NPT at the second year. Conclusion:NPT can be used as one of the indicators for efficacy evaluation for dust mite induced AR patients, especially for patients with positive NPT induced at lower concentrations before treatment.
