1.Expression and significance of BCR-ABL mRNA in chronic myeloid leukemia patients by qPCR

Objective To investigate the early prediction and monitoring value of real time fluorescence quantitative PCR(qPCR) detection of BCR-ABL mRNA on chronic myelogenous leukemia(CML)patients with minimal residual disease(MRD)after alloge-neic hematopoietic stem cell transplantation(allo-HSCT).Methods From January 2013 to January 2016 in People's Hospital of Yantian District,School of Medicine in Shenzhen University and Shenzhen Hospital of Peking University,67 cases of outpatient and hospitalized patients with chronic myelogenous leukemia in the Department of Hematology were selected as the observation group, and 50 healthy people who took physical examination in the People's Hospital of Yantian District at the same period of time were se-lected as the control group.BCR-ABL mRNA levels before and after allogeneic hematopoietic stem cell transplantation in patients with chronic myelocytic leukemia were detected by qPCR.Results The BCR-ABL mRNA results in the control group were nega-tive.Before transplantationin the levels of BCR-ABL mRNA in patients with CML in accelerated and blastic phase were higher than that in patients in chronic phase(P<0.05);After transplantation the BCR-ABL/ABL ratio of qPCR detected at all time points in the first 36 months was significantly lower than that before the transplantation,and the difference was statistically significant(P<0.05);qPCR can still detect the level of BCR-ABL/ABL expression after 36 months of transplantation.Conclusion qPCR is accu-rate and reliable,and has important clinical application value for the clinical diagnosis of chronic myeloid leukemia and minimal re-sidual disease detection.
2.Diagnosis value of ADC values combination with MRI signs on breast lump lesions with the type of TICⅡ

Objective To explore the significance of ADC value combined with MRI signs on identifying benign and malignant breast lump lesions with the type of TICⅡ.Methods 187 patients with breast lump lesions of TICⅡ,which were confirmed by surgical pathology,proceeded MRI.The ADC value,early-phase enhancement rate,length of lesions,lobulated appearance and spiculation, inverted nipple,thickening of the skin and the length of ipsilateral axillary lymph nodes were analyzed and recorded.T-tests was used to analyzed the measurement data,χ2test was used to analysis the attribute data.The ROC curves were used to evaluate the diagnostic efficiency of ADC value and MRI signs.Results The ADC value was (1.418±0.299)×10-3mm2/s and(0.860±0.142)× 10-3mm2/s (P<0.01)for breast benign and malignant lesions respectively,while the early-phase enhancement rate were (170.387± 74.580)% and (160.778±39.786)%(P=0.258)respectively.When ADC values were used for differential diagnosis of breast benign and malignant lesions with a threshold of 1.017×10-3mm2/s,the area under the sensitivity and specificity were 89.0% and 98.0% The sensitivity and specificity of lobulated appearance,spiculation,inverted nipple and thickening of the skin were 92.6% and 33.3%, 88.2% and 88.2%,20.5% and 94.1%,35.3% and 88.2%,respectively.When the 1.0 cm was used as the threshold of the length of ipsilateral axillary lymph,its sensitivity and specificity were 47.8% and 80.4%.The ROC curve of early-phase enhancement rate showed no diagnostic capability(P>0.05).Conclusion ADC value combined with MRI features are helpful to improve the sensitivity and specificity in breast lesions with the type of TICⅡ.
3.Design and application of a prone and side lying double-purpose inflatable mattress for acute respiratory distress syndrome

Acute respiratory distress syndrome (ARDS) is a leading cause of high mortality in critically ill patients in intensive care units (ICU). Prone ventilation is one of the important treatment methods for ARDS, but due to the severity of the patients' conditions, their physical weakness, and poor cooperation, it is difficult for them to adjust to the prone position on their own. This requires a large number of medical staff to adjust the patient to a prone position and implement prone ventilation for than 12 hours daily to achieve therapeutic effects. Clinical implementation often faces challenges such as a shortage of staff, patient discomfort, difficulty in changing positions prolonged time leading to bedsores, and lack of auxiliary equipment. It is particularly challenging for ARDS patients, especially those in critical condition, to adjust their on their own. Therefore, healthcare workers from the department of emergency of Yingtan 184 Hospital designed a prone and side lying double-purpose inflatable mattress for ARDS, and obtained the National Utility Model Patent of China (patent number: ZL 2023 2 1249425.7). The prone and side lying dual-purpose inflatable mattress mainly consists of a main mattress, a forehead pad, a prone position pad, and bilateral side lying position pads. The prone position pad and the lateral position pad on both sides are relatively independent, and can be inflated separately through their inflation nozzles to meet the ventilation needs of ARDS patients in prone or lateral positions, making it easy to adjust the patient's position. This mattress is reasonably designed, simple in structure, easy to operate, comfortable, and practical. It has transformation, promotion, and application value.
