1.Intervention and intervention of cisplatin combined with gemcitabine or vinorelbine in the treatment of patients with non-small cell lung cancer

Objective To investigate the effect of psychological intervention on patients with non-small cell lung cancer treated with cisplatin or gemcitabine or Changchun vinorelbine. Methods 64 cases of patients with non-small cell lung cancer after surgery, nursing for patients in the group given basic treatment, routine care control group, the observation group based on routine nursing and psychological nursing effects were compared between the two groups. Results No significant difference of anxiety and depression in the patients of the two groups before nursing, after grouping nursing, the observation group improved significantly; compared two groups of patients with quality of life score, visible two group before the intervention had no significant difference after intervention group was clearly observed after the patients in the observation group were higher than that of the control group, comparison between groups were indicates that the difference is obvious (P<0.05). Conclusion Chemotherapy in non-small cell lung cancer after surgery for patients with psychological intervention, compared to conventional nursing, can improve the psychological status of patients with better, improve the quality of life of patients, so it is worthy of reference in clinical use.
2.Clinical application of double incision in phacoemulsification combined with trabeculectomy

Objective Conducting phacoemulsification combined with trabeculectomy through double incision to treat glaucoma and cat-aract,and observing the visual acuity,intraocular pressure before and after the operation and complications of this therapy. Methods Totally 60 patients (78 eyes) with glaucoma and cataract from January 2010 to December 2013 in our hospital were observed. 32 patients (40 eyes) who were given phacoemulsification combined with trabeculectomy through double incision were regarded as the observation group,and the other 28 patients (38 eyes) who were merely given phacoemulsification were regarded as the control group. The visual acuity,intraocular pres-sure and anterior chamber of the two groups were observed and recorded,and they were followed up for 36 months. Results Visual acuity and intraocular pressure of the two groups were improved after operation,but the observation group achieved a better result which showed a significant difference (P< 0. 05). Anterior chamber inflammation occurred in patients of the observation group,and 2 cases were of anterior chamber exudation. The control group,by contrast,has less anterior chamber inflammation. There was no hyphema,shallow anterior chamber, choroidal detachment occurred in both of the two groups. Conclusion Using phacoemulsification combined with trabeculectomy through double incision to treat glaucoma and cataract could receive a good recovery of visual acuity and intraocular pressure. Although,more severe anterior chamber inflammation occurred after operation compared with the control group,it can be alleviated soon after medication. This meth-od is a good therapy which can reduce pain and economic burden of patients from multiple operations. But it requires a strict selection of ap-propriate case.
3.Causes and nursing strategy of gastroesophageal reflux in 31 patients with cervical vertebra fracture and paraplegia

In order to explore the causes and nursing care of gastroesophageal reflux in patients with ceryical vertebra frac-ture and paraplegia,the clinical data of 79 patients were retrospectively analyzed. Among which,31 patients suffered from gas-troesophngeal reflux. The main causes of gastroesephageal reflux were improper body position,gastrointestinal dysfunction,con-sciousness disorders,lnappropriate nasngastric feeding,drng adverse reaction,inappropriate feeding time,lack of knowledge in nurse aids and family members. It is suggested to take proper body position,assess gastrointestinal functions,implement naso-gastric feeding correctly,observe patient carefully to detect gastroesophageal reflux as early as possible,as well as provide health education for the nurse aids and family members to prevent gastroesophageal reflux and complications.
4.Application of new type exercise activity in nursing of general surgery patients after abdominal surgery

Objective To apply the new type exercise activity and make a guidance,to discuss the application effect of new type exercise activity in nursing of general surgery patients after abdominal surgery.Methods 120 patients with abdominal surgery were divided into the experimental group(58 cases)and the control group (62 cases )according to the mantissa of hospital number.The patients in the control group received routine nursing care.In addition,the patients in the experimental group received systematic new exercise activity.Postoperative rehabilitation progress and complications were compared between two groups.Results The recovery process of the experimental group was faster than the control group,the first time of getting out of bed,anal exsufflation time and wound healing time were shorter than those of the control group.The incidence of complications of the experimental group was lower than the control group.Conclusions The new type exercise activity can speed up the recovery process of general surgery patients after abdominal surgery and reduce the incidence of oostooerative complications.

Objective To investigate effect of diabetes mellitus factor on attenuation of myocardial ischemia-reperfusion injury by sufentanil postconditioning in rats.Methods Diabetes mellitus was established by intraperitoneal streptozotocin 50 mg/kg and was confirmed by blood glucose ≥ 16.7 mmol/L.Thirty healthy adult male SD rats,weighing 250-300 g which diabetes mellitus model was successfully established were randomly divided into 3 groups ( n =10 each):diabetic mellitus sham operation group (group DM-S),diabetic mellitus ischemia-reperfusion group (group DM-IR) and diabetic meltitus sufentanil postconditioning group (group DM-SP).Another 30nondiabetic mellitus rats were also randomly divided into 3 group ( n =10 each):nondiabetic mellitus sham operation group (group NDM-S),nondiabetic mellitus ischemia-reperfusion group (group NDM-IR) and nondiabetic mellitus sufentanil postconditioning group (group NDM-SP).Myocardial ischemia-reperfusion was induced by 30 min occlusion of left anterior descending branch of coronary artery followed by reperfusion.Sufentanil postconditioning was induced by iv injection of sufentanil 1.0 μg/kg at 5 min before reperfusion.MAP,SBP,HR and RPP (HR × SBP) were recorded before ischemia,30 min of ischemia and 120 min of reperfusion.Arterial blood samples were collected at 120 min of reperfusion for measurement of plasma cardiac troponin 1 (cTnI) concentration.The animals were then sacrificed for determination of total areas of right and left ventricles ( LV + RV),infarct area (IS),area at risk (AAR) and IS/AAR.Results MAP and RPP were significantly lower,while plasma cTnI concentration was higher during myocardial ischemia reperfusion in diabetic and nondiabetic rats.Sufentanil postconditioning significantly decreased IS,IS/AAR and plasma cTnI concentration in nondiabetic rats during myocardial ischemia reperfusion but had no effect on IS,IS/AAR and plasma cTnI concentration in diabetic rats.The IS,IS/AAR and plasma cTnI concentration were significantly higher in DM-SP group than in NDM-SP gnoup ( P < 0.05).Conclusion Diabetes mellitus factor can negate sufentanil postconditioning induced protection against myocardial ischemia-reperfusion injury in rats.
7.Influence of drip velocity of nitrate on blood pressure of patients with coronary heart disease

Objective To discuss the relationship of drip velocity of nitrate on blood pressure while treating coronary disease,in order to provide appropriate drip velocity for clinical treatment.Methods 155 patients with coronary disease using nitrate to lower blood pressure were selected.They were divided into the nitro glycerin group(85 cases) and the isosorbide mononitrate group( 70 cases) according to difference of medication.The velocity of drugs was adjusted on basis of blood pressure changes.The blood pressure changes at different drip velocities were observed and compared.Results The systolic pressure and the diastolic pressure between two groups showed no difference at 20 drops/min,but the results were the opposite at 30 drops/min.The systolic pressure and the diastolic pressure in the nitro glycerin group showed evident changes at different drip velocities,but in the isosorbide mononitrate group,these changes were not so significant.9 patients in the nitro glycerin group had headache during treatment,no headache occurred in the isosorbide mononitrate group.Conclusions Intravenous use of nitrate at a velocity of 20 drops/min is relative secure.The risk of hypotension will increase if the medication speed increases.lsosorbide mononitrate has little influence on blood pressure.
8.Role of different opioid receptors in sufentanil postconditioning-induced attenuation of myocardial ischemia-reperfusion injury in rats

Objective To evaluate the role of different opioid receptors in sufentanil postconditioning-induced attenuation of myocardial ischemia-reperfusion (I/R) injury in rats.Methods Fifty adult male Sprague-Dawley rats,aged 4-6 months,weighing 200-330 g,were randomly divided into 9 groups:I/R group (n =7),ischemic postconditioning (IPC) group (n =7),sufentanil postconditiong (SP) group (n =6),κ-opioid receptor antagonist nor-binaltorphimine (nor-BNI) + SP group (group BNI + SP,n =5),δ-opioid receptor antagonist naltrindole + SP group (group NTD + SP,n =5) and μ-opioid receptor antagonist CTOP + SP group (group CTOP +SP,n =5).Myocardial I/R was produced by 30 min occlusion of left anterior descending branch of coronary artery followed by 120 min reperfusion.IPC was induced by 3 episodes of 10 s reperfusion followed by 10 s ischemia at the end of 30 min myocardial ischemia.Sufentanil 1 μg/kg was injected intravenously at 5 min before reperfusion in SP group.In groups BNI + SP,NTD + SP and CTOP + SP,nor-BNI 5 mg/kg,naltrindole 5 mg/kg and CTOP 1 mg/kg were injected intravenously,respectively,before SP and the corresponding doses of nor-BNI,naltrindole and CTOP were injected intravenously,respectively,at 5 min before reperfusion.Arterial blood samples were collected at 120 min of reperfusion for measurement of the plasma cardiac tropnin I (cTnI) concentration.The rats were then sacrificed and hearts removed for determination of myocardial infarct size (IS) and area at risk (AAR).IS/AAR was calculated.Results Compared with I/R group,the plasma cTnI concentration and IS/AAR were significantly decreased in groups IPC,SP,BNI + SP and NTD + SP (P < 0.05),and no significant change was found in the plasma cTnI concentration and IS/AAR in groups CTOP + SP,NTD,BNI and CTOP (P > 0.05).Compared with SP group,IS/AAR was increased in NTD + SP and BNI + SP groups and the plasma cTnI concentration and IS/AAR were significantly increased in CTOP + SP group (P < 0.05).Conclusion The μ-,κ-and δ-opioid receptors mediate sufentanil postconditioning-induced attenuation of myocardial I/R injury in rats.
9.Effect of the curcumin on expression of IL-2 and IL-6 of hippocampus in pentyle-netetrazol-induced epilepsy in rats

Objective To investigate the mechanism of anti-epileptic effect of the curcumin .Methods The SD rats were injected intraperitoneally with pentylenetetrazol kindling 25 .0 mg/kg to induce a rat epilepsy model .All of the treatments were performed once a day continuously for 28 days .The rats in blank group and model group received 5 ml of normal saline .The rats in the high and low curcumin group were given 200 mg/kg and 100 mg/kg of curcumin once a day ,respectively .The rats in the sodium valproate (VPA) group were given 400 mg/kg of VPA once a day by gavage .After treatment ,the seizures level was recorded by using the Racine′s six point grading scale ,and the expression of IL-2 and IL-6 of hippocampus were detected by the enzyme linked immunoassay (ELISA) .Results The seizures level was reduced by curcumin in epileptic rats .The ex-pressions of IL-2 and IL-6 of the model group were significantly higher than those of the blank group (P<0 .05) ,while those rats of the anti-epileptic groups ,including high dose group and low dose group ,were lower than those rats of the model group (P<0 .05) .When compared with the curcumin low dose group ,the expression of IL-2 and IL-6 of curcumin high dose group is lower (P< 0 .05) .There was no significant difference between the high dose curcumin group and VPA group (P>0 .05) . Conclusion The curcumin can reduce the seizure level in rats ,it shows some anti-epileptic effets and dose-dependently ,which may be through down-regulating the expression of IL-2 and IL-6 in hippocampus .
10.Investigation and analysis of the current situation of medical equipment allocation in 103 tertiary public hospitals

Objective:To investigate the current situation of medical equipment allocation in 103 tertiary public hospitals,analyze and compare the proportion of medical equipment in different regional hospitals in our country,and provide basis for scientific and reasonable allocation of medical equipment.Methods:A questionnaire was designed by literature research and expert consultation,and an electronic questionnaire was released through the public account of"China Medical Equipment"and the platform of"Good Medical Workers"to analyze the correlation between the number of medical equipment configuration,the proportion of domestic products,the region where the hospital was located and the number of hospital beds.Results:A total of 103 valid questionnaires were collected,involving 103 tertiary public hospitals in East China,North China,Central China,South China,Southwest,Northwest and Northeast China,of which 18%were in North China,37%were in East China and 9%were in South China.In North China,South China and East China,the average number of medical equipment in the tertiary public hospitals in the region was 9785 units per hospital,9245 units per hospital and 8153 units per hospital,and the number of the expensive equipment was relatively small,which had no correlation with the number of beds.There were statistically significant differences between North China and East China in the number of beds,the number of pieces of equipment per 100 beds and the amount of equipment per 100 beds(t=-2.582,2.939,4.653,P<0.05).The number of life support equipment configuration was significantly positively correlated with the number of beds,and the domestic share,especially the domestic share of life support equipment,increased.Conclusion:The analysis of medical equipment allocation in public hospitals in China can provide data support for hospitals to better adapt to the adjustment of the new expensive medical equipment catalogue,provide reference for the demonstration and promotion of large medical equipment allocation and localization in tertiary hospitals,and provide reference for relevant health management departments.
