1.Influence of drip velocity of nitrate on blood pressure of patients with coronary heart disease

Objective To discuss the relationship of drip velocity of nitrate on blood pressure while treating coronary disease,in order to provide appropriate drip velocity for clinical treatment.Methods 155 patients with coronary disease using nitrate to lower blood pressure were selected.They were divided into the nitro glycerin group(85 cases) and the isosorbide mononitrate group( 70 cases) according to difference of medication.The velocity of drugs was adjusted on basis of blood pressure changes.The blood pressure changes at different drip velocities were observed and compared.Results The systolic pressure and the diastolic pressure between two groups showed no difference at 20 drops/min,but the results were the opposite at 30 drops/min.The systolic pressure and the diastolic pressure in the nitro glycerin group showed evident changes at different drip velocities,but in the isosorbide mononitrate group,these changes were not so significant.9 patients in the nitro glycerin group had headache during treatment,no headache occurred in the isosorbide mononitrate group.Conclusions Intravenous use of nitrate at a velocity of 20 drops/min is relative secure.The risk of hypotension will increase if the medication speed increases.lsosorbide mononitrate has little influence on blood pressure.
2.Prenatal diagnosis and pregnancy outcomes in 42 fetuses with pleural effusion

Objective To investigate the value of prenatal diagnosis in identifying the etiology and predicting the prognosis of fetal pleural effusion (FPE).Methods Forty-two cases of FPE were recruited in this study from January 2012 to September 2016.Ultrasound scan and genetic tests were performed on all fetuses.Seven fetuses with severe FPE were given pleurocentesis.Pregnancy outcomes of all the fetuses were followed up.Results FPE was commonly accompanied with other abnormalities,such as ascites,hydrops,hydramnion,hygroma colli,abnormal posturing,joint contractures,arrhythmia and micromandible.Chromosomal abnormality was detected in 11 fetuses (26.2%),of which ten were further confirmed by karyotype analysis,including six with 45,X,three trisomy 21 and one trisomy 18,and one was detected with a 9.83 Mb uniparental disomy (UPD) located at 12q24.21q24.31 by gene chip.One fetus was diagnosed with--SEA/--SEA thalassemia.All of the 12 families decided to terminate the pregnancies after genetic counseling.Among the other 30 fetuses,seven with severe FPE and normal karyotype underwent pleurocentesis.Five of the seven cases were with favorable outcomes,one with progressive hydrops was aborted and one neonate with severe hydrops died after birth.Spontaneous regression of FPE with good outcome was found in two cases.Parents of the other 21 fetuses chose to terminate the pregnancies.Conclusions Prenatal diagnosis is important to identify the etiology and predict the outcome of FPE.Chromosomal abnormality is a relatively common cause of FPE,and 45,X and trisomy 21 are the most common abnormalities.Intrauterine intervention is beneficial for FPE without chromosomal or other definite genetic abnormalities.Genetic test may be of great value for pregnant counseling.
3.Effect of benzoapyrene on DNA damage and expression of genes involved in nucleotide excision repair in lung cancer cells.

OBJECTIVETo investigate the effect of benzo[a] pyrene(BaP) on DNA damage and expression of genes involved in nucleotide excision repair[xeroderma pigmentosum group B, C, G(XPB, XPC, XPG) and excision repair cross-complementing 1 (ERCC1)] in lung cancer A549 cells.
METHODSCell survival was measured using MIT metabolic viability assay. Single cell gel assay was applied to determine the DNA damage and repair. The level of gene expression was measured by reverse transcription-polymerase chain reaction.
RESULTSThe cell survival decreased from 95.0% to 70.0% after 24 h treatment with BaP of varying concentration ranging 0.625-20.000 mumol/L. The cell survival decreased to 87.0% and 73.0% respectively after 12 h and 24 h treatment with 10 mumol/L BaP, with DNA damage gradually elevated. At 12 h after 24 h treatment, the cell survival further decreased to 59.0% and DNA damage became most serious. At 24 h after 24 h treatment, cell survival recovered to 71.0%, and damaged DNA was repaired gradually. XPB and XPC gene expression increased to 4.5-fold and 11.2-fold respectively compared with basal level at 24 h treatment or 12 h after 24 h treatment with 10 mumol/L BaP respectively. However, ERCC1 and XPG gene expression was inhibited in 24 h treatment period, then recovered gradually after treatment.
CONCLUSIONBenzo[a]pyrene could lead to DNA damage and expression level change of genes involved in nucleotide excision repair in lung cancer A549 cells.

4.Effects research of hope theory used in clinical rehabilitation nursing of the patients with depression

Objective To explore the intervention effects of carrying out psychological nursing by hope theory on depression and the mediating effect of psychological capital for the intervention effect. Methods A total of 100 patients with depression who were treated in the Department of Psychiatry of Nanchang 334 Hospital from July to September in 2016, were divided into experimental group (50 patients)and control group(50 patients), and the two groups were given setraline systemic therapy and psychiatry routine nursing; however, the patients in control group received routine health education, on that basis, patients of experimental group were added hope psychological intervention. The intervention time of the two groups lasted for eight weeks. Before and after intervention, the rehabilitation effects of the two groups were compared using PCQ,HHI and HAMD. Results Before intervention,the total score of PCQ, HHI and HAMD in the two groups were not significantly statistical difference (P> 0.05); the total score of PCQ and HHI in experimental group before intervention were higher than that after intervention (P<0.01),but the total score of HAMD in experimental group before intervention was lower than that after intervention(P< 0.01). In the control group, the scores of PCQ and HHI had no statistical significance with that before intervention, but the score of HAMD score was lower than that before intervention (P< 0.01). The two groups were compared after intervention, and the total score and changing difference values of PCQ, HHI and HAMD in experimental group were better than that in control group(P<0.01). The Bootstrap test showed that psychological played mediating effect in hope psychological nursing improved depression effect, and the values of the mediating effect was 0.246, which ratios to the total effect was 0.338, and 95% CI of the mediating effect in hope psychological nursing to depression was 0.189-0.305. Conclusions Using hope theory carrying out psychological nursing for patients with depression can improve effectively depression emotion, and psychological capital is the mediating variable of hope psychological nursing improved depression effect.
5.Path analysis on influence of disability acceptance and resilience on stigma in laryngeal carcinoma patients with permanent tracheostomy

Objective To explore the influence of disability acceptance and resilience on stigma in laryngeal carcinoma patients with permanent tracheostomy and analyze the impact path. Methods From June 2016 to August 2018, this study selected 264 laryngeal carcinoma patients with permanent tracheostomy of Inpatient and Outpatient Department in Guangxi Medical University Affiliated Tumor Hospital as subjects by convenience sampling. All patients were investigated with the Social Impact Scale (SIS), Acceptance of Disability Scale (AODS) and the 10-item Connor-Davidson Resilience Scale (CD-RISC10). The correlations were analyzed and structure equation model was used to path analysis. A total of 264 questionnaire were sent out and 256 valid questionnaires were collected. Results Among those patients with permanent tracheostomy, the total scores of stigma, disability acceptance and resilience were (61.50±9.27), (76.88±9.34) and (26.49±4.05) respectively. Correlation analysis showed that the scores of disability acceptance and resilience had negative correlations with stigma with statistical differences (P< 0.01). Structure equation model analysis showed that disability acceptance and resilience of laryngeal carcinoma patients with permanenttracheostomy had direct effect on stigma (β=-0.43,-0.28), the indirect effect of disability acceptance on stigma by resilience was -0.06. Conclusions Laryngeal carcinoma patients with permanenttracheostomy have a high level of stigma, the disability acceptance and resilience has significant roles on stigma.
