1.Construction of nursing quality evaluation index system of psychiatric closed wards in Sichuan Province

BackgroundIn Sichuan Province, most healthcare institutions providing mental health services have adopted self-developed evaluation indicators for the quality of nursing care in psychiatric closed wards, lacking unified standards. This results in insufficient authority and homogeneity, which is unfavorable for the standardized assessment and continuous improvement of nursing quality. ObjectiveTo construct a standardized evaluation indicator system for nursing quality of psychiatric closed wards in Sichuan Province, so as to provide references for nursing quality management and assessment. MethodsBased on bio-psycho-social medical model and guided by "Structure-Process-Outcome" quality evaluation framework, preliminary evaluation indicators for nursing quality in psychiatric closed wards were developed through literature analysis, research team discussions and clinical experience. Through two rounds of Delphi expert consultation, the indicators were revised and finalized. ResultsThe response rates for two rounds of Delphi expert consultation questionnaire were 100%. The expert authority coefficients were 0.845 and 0.864, respectively, and the Kendall's coordination coefficients ranged from 0.119 to 0.210 (P<0.01). Ultimately, a nursing quality evaluation index system for psychiatric closed wards was established, comprising 3 first-level indicators, 9 second-level indicators and 46 third-level indicators. ConclusionThe nursing quality evaluation indicators for psychiatric closed wards constructed based on the Delphi method can provide references for nursing quality management and evaluation in such wards. 2.Influence of nursing students empathy on moral courage: the path of moral sensitivity

BackgroundNurses' moral courage is a special type of altruistic behavior, which is considered to be a key factor to improve patient safety. Empathy level can positively predict altruistic behavior, and moral sensitivity exerts an obvious mediation role in the positive prediction. At present, there is no relevant research to explore the path role of moral sensitivity in the relationship between empathy and moral courage. ObjectiveTo verify the mediation role of moral sensitivity in the relationship between empathy and moral courage among nursing students, so as to provide references for increasing the moral courage among nursing students. MethodsFrom June 1 to June 30, 2022, a total of 602 nursing students were recruited via convenience sampling from a tertiary grade A hospital in Mianyang, and participants were assessed using Interpersonal Reactivity Index-Chinese (IRI-C), Moral Sensitivity Questionnaire-Revised Chinese Version (MSQ-R-CV) and Nurses' Moral Courage Scale (NMCS). Pearson correlation analysis was used to assess the correlation among above scales. The Process macro plug-in for SPSS was used to test the mediation effect of moral sensitivity on the relationship between empathy and moral courage. ResultsA total of 602 valid questionnaires were collected, with an effective recovery rate of 96.78%. IRI-C score of nursing students was positively correlated with MSQ-R-CV score and NMCS score (r=0.269, 0.146, P<0.01). MSQ-R-CV score was positively correlated with NMCS score (r=0.722, P<0.01). The mediation effect model was established with IRI-C score as independent variable, MSQ-R-CV score as intermediary variable, and NMCS score as dependent variable. Analysis showed that the direct effect of empathy on moral courage among nursing students was not significant (direct effect value of -0.052, 95% CI: -0.178~0.008), while analysis yielded a significant indirect effect of empathy on moral courage via moral sensitivity (indirect effect value of 0.189, 95% CI: 0.209~0.407). ConclusionThe moral sensitivity of nursing students plays a significant mediation role in the relationship between empathy and moral courage. This article aims to provide a review of the domestic and foreign studies on the quality indicators for evaluating the psychiatric nursing care in recent years. According to Donabedian model, the research progress of quality indicators for evaluating the psychiatric nursing care were described from the three dimensions of structure quality, process quality and outcome quality. This paper summarizes the status of the research, points out the shortcomings of existing research, and provides references for the future research on the quality indicators for evaluating the psychiatric nursing care.
4.Impact of personalized psychological intervention based upon acceptance and commitment therapy on pediatric liver transplantation donors and spouses

Objective:To explore the effect of personalized psychological intervention based upon acceptance and commitment therapy on paternal donors and spouses after pediatric liver transplantation.Methods:From July 2017 to November 2019, a total of 160 parental donors and spouses undergoing pediatric liver transplantation were selected as research objects.Through a random number table, 40 donors and 40 liver donor spouses were assigned into control and experimental groups.The control group received routine psychological intervention while the experimental group had personalized psychological intervention with acceptance and commitment therapy based upon conventional psychological intervention.Symptom checklist-90, numeric rating scale, chronic pain acceptance questionnaire-8, length of stay, hospitalization cost, complication rate, initial postoperative ambulation time and hospital satisfaction were employed as evaluation tools.Results:After interventions, SCL-90 score, pain assessment score and initial ambulation time of donor group were significantly lower than those of control group( P<0.05). Hospitalization satisfaction and chronic pain acceptance were significantly higher than those of control group( P<0.05). SCL-90 score was significantly lower in liver spouse observation group than that in control group and hospitalization satisfaction higher than control group( P<0.05). Conclusions:Personalized psychological intervention based upon acceptance and commitment therapy can help parental donors and spouses to improve their mental health, help donors to recover sooner and boost hospitalization satisfaction.
5.Investigation on the status of traditional Chinese medicine nursing knowledge-attitude-practice of newly recruited nurses in traditional Chinese medicine hospital

Objective:To investigate the current status of the traditional Chinese medicine (TCM) nursing knowledge-attitude-practice of newly recruited nurses in TCM hospitals, so as to provide evidence for improving the standardized training of TCM nursing among newly-recruited nurses.Methods:From March to April in 2020, a total of 800 newly-recruited nurses from 6 TCM hospitals above the municipal level in Hunan Province were selected as research objects by convenient sampling method. The investigation was conducted by TCM Nursing Knowledge-Attitude-Practice Questionnaire. A total of 800 questionnaires were distributed and 768 valid questionnaires were collected, with an effective recovery rate of 96%.Results:The total score of the Knowledge-Attitude-Practice Questionnaire of the newly recruited nurses of the TCM hospital was (191.61±41.80) ; the scores of knowledge and attitude dimension were (73.93±17.21) and (78.17±15.81) , at a middle level; the score of practice dimension was (39.51±8.78) , at a low level. There were significant differences in the total score of the questionnaire among the new nurses with different hospital levels, working years, whether they graduated from a college of TCM and whether they had received the training of TCM nursing knowledge-attitude-practice ( P<0.01) . Conclusions:The newly recruited nurses in TCM hospitals have a high level of TCM nursing knowledge and a positive attitude towards TCM nursing. However, in clinical work, TCM nursing has poor application ability and insufficient use of clinical skills. In-depth study of TCM nursing knowledge should be strengthened, focusing on the cultivation of TCM nursing thinking. It is also important to improve laws, regulations and related rules, and to conduct training according to the different characteristics of new nurses.
6. A qualitative study on psychological experience of pediatric living donor liver transplantation donors

Objective:
To understand the psychological experience of the pediatric living donor liver transplantation who is the child`s parent, identify the problems and gains that donors face in the event of donor liver, help donors to solve their difficulties to promote early recovery of donors and children, and help potential donors better understand the surgical.
Methods:
The phenomenological methodology in qualitative research was used in the study, and the Colaizzi 7-step data analysis method was used for analysis the interview recording.
Results:
Total of four themes are summarized. Preoperative psychological path: excitement, impatience, anxiety. Postperative growth: grateful, regain hope, hard work and proactive. Postnatal negative emotions: fear and jealousy. Family and social support.
Conclusion
Medical staff should strengthen psychological care and health education, use narrative care and other methods to mobilize the donor′s positive psychological experience, while providing perioperative quality care, strengthen social and family support, and help donors to restore physical and mental health.
7.Clinical value of ultrasound-guided percutaneous precise puncture and catheter drainage for cervical abscess

Objective To investigate the efficacy and feasibility of ultrasound-guided percutaneous catheter drainage for cervical abscess.Methods The clinical data of 18 patients with cervical abscess treated by percutaneous puncture and catheterization under ultrasound guidance were analyzed retrospectively.Results All of the 18 patients were successfully treated on one round.None of the 18 patients was nerve injury,main vessels injury or dead during the operation.Conclusions Ultrasound-guided percutaneous catheter drainage of neck abscess is safe and effective,and has high clinical application value.
8.Modified strategy of anesthesia for laparoscopic radical resection of rectal carcinoma: ultrasound-guided anterior quadratus lumborum block combined with general anesthesia

Objective To evaluate the efficacy of ultrasound-guided anterior quadratus lumborum block combined with general anesthesia for laparoscopic radical resection of rectal carcinoma. Methods A total of 80 patients of both sexes, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ, aged 40-64 yr, scheduled for elective laparoscopic radical resection of rectal carcinoma, were divided into 2 groups ( n=40 each) using a random number table method: anterior quadratus lumborum block combined with general anesthesia group ( group QG) and general anesthesia group ( group G) . In group QG, anteri-or quadratus lumborum block was performed with 0. 33% ropivacaine 25 ml and dexamethasone 5 mg under ultrasound guidance before operation, and the same procedure was performed on the other side. Combined intravenous-inhalational anesthesia was applied, propofol 3-5μg∕ml and remifentanil 3-5 ng∕ml were given by target-controlled infusion, and cisatracurium was intermittently injected in two groups. Patient-controlled intravenous analgesia with sufentanil 2μg∕kg was used for postoperative analgesia. The analgesic pump was set up to deliver a 2 ml bolus dose with a 15-min lockout interval. Bruggrmann comfort scale ( BCS) scores were recorded at 1, 6, 12, 24 and 48 h after operation ( T1-5 ) . Tramadol was used for rescue analgesic after operation. The consumption of remifentanil and sufentanil, requirement for tramadol, occurrence of adverse reactions and patients' satisfaction with postoperative analgesia were recorded. The emergence time, first ambulation time, time to first flatus∕poo and length of hospital stay were also recorded. The develop-ment of anterior quadratus lumborum block-related complications was recorded. Results Compared with group G, BCS scores were significantly increased at T4,5 , the consumption of remifentanil, requirement for tramadol and incidence of nausea and vomiting were decreased, patients' satisfaction with postoperative an-algesia was increased, and the emergence time, first ambulation time, time to first flatus∕poo and length of hospital stay were shortened in group QG (P<0. 05). Conclusion Ultrasound-guided anterior quadratus lumborum block combined with general anesthesia can reduce the consumption of opioids in the perioperative period and is helpful in improving outcomes when used for laparoscopic radical resection of rectal carcinoma.
9.The protective effect of dexmedetomidine atdifferent doses combined with ulinastatin in lung resection pa-tients with one lung ventilation

Objective To observe the protective effect of dexmedetomidine(Dex)at different doses com-bined with ulinastatin in lung resection patients with one lung ventilation. Methods 80 patients having undergone unilateral lung resection were divided into four groups randomly:control group(C group)and groups Dex 1-3,20 cases in each group.One lung ventilation(OLV)was used in all the groups during operation.The patients in groups Dex 1-3 were treated with 0.5,1.0,2.0 μg/kg combined with ulinastatin,and the C group with amount of normal sa-line instead.The comparisons were done among the four groups in terms of SOD,L-6,IL-10,serum malondialde-hyde(MDA)concentration at 5 min after endotracheal intubation(T0),30 min(T1),60 min(T2),120 min (T3)as well as the levels of FVC,FEVl and FEVl/FVC at 24 h,48 h and 72 h after surgery. Results In the groups C and Dex 1,SOD decreased at T1-4 and IL-6,MDA and IL-10 at T2-4 rose.SOD decreased at T2-4 in groups Dex 2-3 and MDA,IL-6 and IL-10 rose.Compared with group C,the levels of SOD and IL-10 at T2-4 in groups Dex 1-2 and at T1-4 in group Dex 3 rose. In groups Dex 1-3,postoperative FVC,FEVl and FEVl/FVC rose.Compared with group Dex 1 or 2 respectively,SOD and IL-10 at T2-4 in group Dex 3 significantly rose,but MDA and IL-6 significantly declined;FVC,FEVl and FEVl/FVC significantly rose 48 h and 72 h after surgery (P<0.05). Conclusion Dex combined with ulinastatin has a protective effect for patients with one lung ventila-tion after lung resection,with the best-suggested dose of 1.0 μg/kg.
10.Effect of ultrasound-guided transverse abdominal plane block on postoperative analgesia in patients undergoing orthotopic liver transplantation under general anesthesia

Objective To evaluate the effect of ultrasound-guided transverse abdominal plane block (TAPB) on postoperative analgesia in patients undergoing orthotopic liver transplantation under general anesthesia.Methods Forty American Society of Anesthesiologists physical status Ⅲ-V patients,with body mass index of 18-24 kg/m2,aged 18-64 yr,undergoing elective modified piggy-back orthotopic liver transplantation,were divided into 2 groups (n =20 each) by a random number table method:TAPB combined with general anesthesia group (TAPB-GA group) and general anesthesia group (GA group).In TAPB-GA group,two-point TAPB was performed below bilateral costal margins under ultrasound guidance after induction of general anesthesia,and a mixture of 0.33% ropivacaine 15 ml plus 0.5% dexamethasone 0.5 ml was injected into each point.The equal volume of normal saline was injected into each point instead in group GA.Patient-controlled intravenous analgesia was performed with sufentanil 2 μg/kg after operation in both groups.Sufentanil 5 μg was intravenously injected as rescue analgesic,and the visual analog scale score was mainrained ≤3 within 48 h after operation.The intraoperative consumption of remifentanil and extubation time after operation were recorded.The requirement for sufentanil as rescue analgesic and development of nausea and vomiting,itching and respiratory depression were recorded within 48 h after surgery.Results Compared with group GA,the intraoperative consumption of remifentanil and requirement for sufentanil as rescue analgesic within 48 h after surgery were significantly reduced,the time of extubation was shortened,and the incidence of nausea and vomiting,itching and respiratory depression was decreased in group TAPB-GA (P< 0.05).Conclusion Ultrasound-guided TAPB can provide better efficacy of postoperative analgesia with fewer adverse reactions in patients undergoing orthotopic liver transplantation under general anesthesia.
