Objective To identify clinicopathologic factors which predict survival following hepatectomy in HBV-related cirrhotic patients with early hepatocellular carcinoma (HCC).Methods A database was used to identify patients with histologically confirmed early HCC (≤5 cm,no nodal involvement,metastases,or major vascular invasion) who underwent surgical resection (excluding ablation or transplantation).Among 20 700 patients with HCC who were diagnosed at the Eastern Hepatobiliary Surgery Hospital from April 2005 to November 2010,537 (2.6％) patients with early HCC were studied retrospectively.Prognostic factors were evaluated using the Kaplan-Meier curves,Cox proportional hazards models and the receiver operating characteristic (ROC) curves.Results The study included 537 patients.The median tumor size was 2.9 cm,and 33％ of patients had tumors ≤2 cm.Most HCC lesions were solitary (63％) and had no evidence of vascular invasion (64％).Following surgery,the overall median and 5-year survival were 45 months and 33％ respectively.After adjusting for demographic factors and histological grade,tumor size ＞2 cm (hazard ratio [HR]:1.56),multifocal tumors (HR:1.34),and vascular invasion (HR:2.03) remained independent predictors of poor survival (all P ＜ 0.05).Based on these findings,a prognostic scoring system was developed that allotted 1 point each for these factors.Patients with early HCC could be stratified into 4 distinct prognostic groups (median and 5-year survival,respectively):0 points (97 months,96％),1 point (85 months,76％),2 points (76 months,54％),3 points (56 months,39％) (P ＜0.01).Conclusions The present study emphasized the importance of pathologic staging even in patients with small HCC.Anatomical resection of HCC should be the preferred surgical procedure in cirrhotic patients.

Objective:To discuss the role of clinical pharmacists in perioperative drug treatment for a patient after renal transplan-tation. Methods:Clinical pharmacists participated in the drug therapy for a patient after renal transplantation. Pharmaceutical care and health education were performed for the patient in the aspects of immunosuppression, anti-hepatitis B virus, gastric mucosa protection, diarrhea, acute rejection and the other treatment programs. Results:The participation of clinical pharmacists in the clinical treatment for the patient improved therapeutic scheme and promoted therapeutic efficacy. Conclusion:The participation of clinical pharmacists in clinical drug therapy for patients can provide individualized service to improve reasonable, effective and economical drug use.

Long-term treatment with an agonist of peroxisome proliferator-activated receptor (PPAR)-γ is associated with bone fractures in the clinical practice. However, the mechanisms underlying the fractures are not fully understood. This study was aimed to examine the effect of rosiglitazone (an agonist of PPAR-γ) of different doses on the proliferation, differentiation, and transforming growth factor beta 1 (TGF-β1)-induced expression of connective tissue growth factor (CTGF) in primary rat osteoblasts in vitro. Osteoblasts were isolated from newly born SD rats and treated with different doses of rosiglitazone (0-20 μmol/L). The proliferation and differentiation of osteoblasts were measured by MTT assay and NPP assay, respectively. The expression of CTGF was determined by RT-PCR and Western blotting. The results showed that most isolated osteoblasts displayed strong alkaline phosphatase (ALP) activity and treatment with different doses of rosiglitazone did not affect their proliferation, but significantly inhibited the differentiation of osteoblasts in a dose-dependent manner. Moreover, treatment with different doses of rosiglitazone significantly reduced the TGF-β1-induced CTGF mRNA transcription and protein expression in a dose-dependent manner in rat osteoblasts. It was concluded that the activation of PPAR-γ may inhibit the differentiation of osteoblasts by reducing the TGF-β1-induced CTGF expression in vitro.

The on-line monitor for the changes in the content of baicalin in Scutellariae Radix formula particles during the extraction process was conducted by using near infrared spectroscopy (NIR). High performance liquid chromatography (HPLC) was used as a reference method. Kennard-Stone (KS) was used to divide sample sets, so as to compare different pretreatment methods. The synergy interval partial least squares (SiPLS) was used to screen out modeling wave band to establish partial least-squares models. The relative error method was applied to predict forecast set samples of Scutellariae Radix in three extraction phases. The results showed that the model established by Savitzky-Golay smoothing with 11 points (SG11 points) was the best, with the root mean square with cross validation (RMSECV), root mean square error of correction (RMSEC) and root mean square error of prediction (RMSEP) of baicalin of 0.092 7, 0.134 4 and 0.114 8, respectively, the determination coefficient R2 of greater than 0.99, and the relative error of baicalin content of less than 5%. This indicates that the on-line near infrared reflectance spectroscopy could be applied in on-line monitor and quality control of the extraction process of Scutellariae Radix formula particles.

Objective To make up an exercise prescription based on traditional Chinese medical training (EP-TCMT) for patients with stable chronic obstructive pulmonary disease (COPD).Methods Eighty-five pa- tients with stable COPD were randomly divided into a control group (CG group),a traditional Chinese medicine group ( TC group) and an exercise prescription group ( EP group).The patients in the TC and EP groups were giv- en intensive training for 8 weeks.Their 6 rain walk distance (6MWD) and Borg scale scores were assessed before and after the treatment.Results The 6MWD in the TC group increased from 337.68?59.18 m to 386.14?76.71 m,while those in the EP group improved from 348.00?55.94 m to 425.17?53.22 m.The Borg scale scores in the TC group decreased from 3.14?1.94 to 2.32?1.25,while those in the EP group declined from 3.45?1.84 to 1.72?0.70.Conclusion Making up EP-TCMTs is feasible.Additional treatment was found to improve exercise tolerance and decrease dyspnea in COPD patients.Exercise therapy based on traditional Chinese methods is easy and safe.

Objective: To investigate the effects of traditional Chinese medicine Xuebijing injection (血必净注射液) on tissue tumor necrosis factor-?(TNF-?) expression and blood coagulation parameters in septic rats.Methods: Wistar rats were subjected to sepsis induced by cecal ligation and puncture(CLP).Ninety-six healthy rats were randomly divided into four groups: normal group,sham operation group,CLP model group,and Xuebijing-treated group.The two latter groups were given respectively intravenous injection of normal saline or Xuebijing injection with the dose of 4 ml/kg at 0.5,12,24,36,48 and 60 hours after the establishment of CLP model.Eight rats were sacrificed at 2,8,24,48 and 72 hours postCLP in the two latter groups.Prothrombin time(PT),activated partial thromboplastin time (APTT),and fibrinogen(Fbg) levels were determined.Tissue TNF-? protein levels in liver and lung tissues were also measured at various intervals.Results: TNF-? protein levels in liver and lung tissues were significantly increased at 2 hours after establishment of CLP model compared with those of the normal group(both P
