1.Effects of types of gingival flap suture on postoperative quality of life after removal of impacted mandibular wisdom teeth

Objective To evaluate effects of different types of gingival flap suture on the quality of life in patients after removal of mandibular third molar. Methods A randomized, controlled clinical trial design was adopted. Sixty-two patients in oral surgery department, Stomatology Hospital of Tianjin Medical University were selected. Patients were required removal of bilateral symmetry impacted mandibular third molars, and enclosed suture (primary suture) was used in one side, open drainage suture (secondary suture) was used in the other side. The postoperative symptom severity scale (PoSSe scale) was used to evaluate the quality of life in patients after removal of mandibular third molar. The evaluation was completed 7 days after surgery, and the scale was compared between two groups. Results A total of 57 cases of clinical data were enrolled in the final analysis. The operative time for primary suture group was (17.7±4.6) min, and the operative time for secondary su?ture group was (16.3 ± 5.1) min, the difference was not statistically significant (t=1.539, P>0.05). The PoSSe score was sig?nificantly higher in primary suture group (26.348 ± 12.414) than that of secondary suture group (21.633 ± 11.382, P<0.05). Conclusion Secondary suture reduces the influence of teeth removal on quality of life in patients through relieving pain and swelling, and which is suitable for clinical application.
2.Chinese herbal medicine and therapeutic exercise for relieving spasticity in hemiplegic patients

Objective To observe the effectiveness of Chinese herbal medicine plus movement therapy in treating upper limb spasticity in hemiplegic patients. Methods Sixty-six patients were divided randomly into two groups. Thirty-five patients in the therapy group were treated with Chinese herbal medicine and movement therapy, and 31 patients in the control group were treated with movement therapy alone. The modified Ashworth scale and sur- face electromyography were applied to evaluate spasticity. Results The total effectiveness rates were 94.3% in the therapy group and 64.5% in the control group. After treatment, the average integrated electromyogram (IEMG) val- ue in the therapy group was lower than that in the control group. The difference in IEMG values before and after treat- ment in the therapy group was significantly larger than in the control group. Conclusion Chinese herbal medicine plus movement therapy is more effective than movement therapy alone in relieving spasticity.

4.Quantifying the effect of antiplatelet therapy by whole blood platelet aggregometry

Objective To study the impedance detected by the whole blood electrical impedance aggregometry in evaluating the effect of antiplatelet therapy.Methods The imprecision of the two different assays(the whole blood electrical impedance aggregometry and the plasma light transmission aggregometry) was compared.Thirty healthy volunteers and 60 patients with coronary artery disease under antiplatelet treatment were enrolled in the present study,whose anti-aggregated blood were collected for the measurement of electrical impedance in the presence of ADP,arachidonic acid and collagen,respectively.Finally,we analysed the agreement between the results of the two different assays which were used to measured the antiplatelet effect of aspirin and clopidgrel.Results The imprecision analysis showed the CV,(the whole blood electrical impedance aggregometry was 4.8%,superior to the plasma light transmission aggregometry (CV = 7.7%).In healthy control group,the mean impedances were(9.6?3.4)?、(10.4?2.8)? and (13.5?2.2)? in the presence of ADP,arachidonic acid and collagen as in the patients taking aspirin 100 mg/day,the 95% patients' results were 0 ?.Furthermore,the mean impedance of patients taking aspirin 100 mg/day and/or clopidogrel 75 mg/day was significantly lower than that of healthy volunteers(ADP as agonist,t = 2.391,P

Objective:Adopting RP-HPLC method,to study and establish the fingerprint of smilacis rhizome.Methods: YMC-Pack ODS-A(5 ?m,4.6 mm?250 mm) chromatographic column,acetonitrile-0.05% phosphoric acid solution gradient elution were applied;with flow rate of 0.8 ml/min and UV detector at 290 nm.Results: Technology investigation indicated that the analytical method this study established has a better reproducibility.The similarity of smilacis rhizoma fingerprints from different sources is better;smilacis rhizome and its three confusions fingerprints are apparently different.Conclusion: HPLC fingerprint analysis can be a method for quality control of medical material of smilacis rhizoma.

Objective To detect the correlation of serum p53 antibody and p53 protein expression of cancer tissue in patients with colorectal cancer (CRC), and to evaluate if the serum p53 antibody might reflect the mutation of p53 gene in CRC. Methods One hundred and thirty-two patients with CRC and 36 patients with non-tumor disease were included in this prospective study. Objective To evaluate the correlation between RPA or the polymorphism of CYP 2C19 and the incidence of ischemic events and the influence on the clinical prognosis .Methods A case-control study was used.A total of 202 patients [male 66%,(63 ±11) years] with ACS on aspirin and clopidogrel treatment were recruited , whose RPA were measured by whole blood aggregometry ( WBA ) , and their CYP2C19 polymorphism were also tested .Their clinical ischemic events were recorded in the mean follow-up period of 16 months.The RPA cut-off values for antiplatelet low-responsiveness were defined by the receiver operator characteristic curve ( ROC); the relationships of clinical outcomes with RPA and CYP 2C19 were assessed by the Kaplan-Meier survival analysis.Results CYP2C19*2 (681G>A) present in 52.5% of recruited patients and*3 (636G>A) present in12.9%.RPA induced by adenosine diphosphate ( ADP) showed significant difference among CYP 2C19*2 or *3 heterozygotes, CYP2C19*2 or *3 homozygotes and noncarriers (χ2 =9.318, P=0.009);whereas, RPA induced by arachidonic acid (AA) (χ2 =2.441, P=0.295) and the incidence of ischemic events (χ2 =0.513, P=0.774) were not.During follow-up, 18 (9%) patients experienced clinical ischemic episodes , and their RPA were higher than patients without ischemic episodes [(8.6 ±4.8) Ωvs (5.2 ±3.7) Ω, P =0.013; (8.6 ±6.8) Ωvs (1.6 ±3.7) Ω, P <0.001].Moreover, employing 6.5 Ω(induced by ADP) and 2.5 Ω(induced by AA) as cutoff values,RPA showed optimal negative predictive values (97%, 96%) and poor positive predictive values (16%,29%).Survival analysis showed, statistically, patients with clopidogrel low-responsiveness had higher riskof ischemic episodes than patients with clopidogrel responsiveness (HR =2.86, χ2 =11.27,P =0.0008);however, patients with aspirin low responsiveness (HR =1.77, χ2 =1.74, P =0.19) or patients withCYP2C19*2 or *3 (HR =0.89, χ2 =0.12, P =0.73) did not.Conclusion Clopidogrel lowresponsiveness is associated with the occurrence of clinical ischemic events ; however, patients withCYP2C19 function reduced genetypes do not show higher risk of ischemic episodes though it presented slighlyhigher RPA.
8.Ameliorative effect of metformin on insulin resistance induced by olanzapine gavage in rats

Objective · To investigate the effect of metformin on insulin resistance (IR)-related metabolic parameters in olanzapine-bearing rats.Methods · Thirty female SD rats were randomly divided into metformin intervention group, olanzapine group and control group. During the first 6 weeks,5 mg /(kg·d) olanzapine was given to the two test groups . The control group was given the same amount of saline. From the 7th week, the intervention group began to combine with metformin 500 mg / (kg·d), while the olanzapine group combined with the same amount of saline, continuing for 4 weeks.At the end of 6th week and 10th week, intraperitoneal glucose tolerance and homeostasis model assessment IR index were assessed. Results · The area under the glucose tolerance curve (P=0.040) and the IR index (P=0.000) were significantly higher for the intervention group and the olanzapine group than the control group at the end of 6th week. At the 10th weekend, the glucose tolerance (P=0.015) and IR index in the intervention group were significantly lower than those in the olanzapine group (P=0.001). Conclusion · Metformin may rectify the impaired glucose tolerance and improve IR induced by olanzapine partly.
9.Rapid assessment of children's active trachoma in rural area of Naxi district and Jiangyang district of Luzhou in China during 10 years

Background Trachoma is a serious blinding eye disease.At present,the incidence rate of trachoma in China has been greatly reduced with the unremitting efforts of World Health Orgnization (WHO) and Chinese government.However,in some economically underdeveloped areas,there is still active trachoma.Ohjective This study aimed to survey the prevalence of children's active trachoma in rural area of Naxi district and Jiangyang district of Luzhou city and assess the prevention and treatment outcome of trachoma during 10 years.Methods A cross-sectional study was performed in Naxi district and Jiangyang district of Luzhou city,the economic underdeveloping and water-starved regions in the year 2000.The active trachoma was screened in 214 school-age children with the male 113 and female 101 based on simplified trachoma grading system and trachoma rapid assessment survey standards and requirements of WHO.The symptoms of active trachoma were divided into trachomatous follicular (TF),trachomatous inflammation intense (TI),trachomatous scarring (TS),trachoma trichiasis (TT) and corneal opacities (CO).The epidemiological survey was re-assessed in 100 school-age children (with the male 58 and female 42) in these regions in the year 2013 to evaluate the prevalence change of children active trachoma during 10 years.Results In the year 2000,214 children were screened in 4 villages of Naxi district and Jiangyang district,and the prevalence rate of TF was 49.07% (105/214) in the year 2000.The prevalence rate of TF was 49.05% (52/106) in the Naxi district and 49.07% (53/108) in the Jiangyang district,showing no significant difference between them (x2 =0.01,P =0.99).In the 100 school-age children who received survey in Naxi district and Jiangyang district in the year 2013,no TF,TF,TI,TS,TT and CO was found,showing no active trachoma.Conclusions The active trachoma has been controlled in rural areas of Luzhou city following the prevention and treatment of trachoma and the improvement of economic status in past decade.
10.Impact of CYP2C19 polymorphisms on eradication of Helicobacter pylori using triple therapy with esomeprazole

Objective To prospectively compare the efficacy of omeprazole and esomeprazole on H. pylori eradication related to cytochrome P450 (CYP) 2C19 polymorphism in Chinese Han poputafion. Methods A total of 240 patients with peptic ulcer were randomly assigned to receive either EAC (amoxicillin, clarithromyein and esomeprazole) treatment or OAC (amoxicillin, clarithromycin and orneprazole) treatment for one week with 120 each. Then, the patients were sustained treated with esomeprazol or omeprazole for 3 weeks. The endoscopic evaluation was performed 2 weeks after treatment. At the end of the treatment, a carbon-13 (13C) urea breath test (13C-UBT) was performed to determine H. pylori status. Polymerase chain reaction and restriction fragment length polymorphism were used to detect CYP2C19 genotypes including extensive metabolizer (EM), poor metabolizer (PM), homozygous (homEM) and hetEM. Results Two hundred and twenty five of 240 patients completed the study. H. pylori eradication was 79.2% with OAC regimen and 88.3% with EAC regimen by intention to-treat (ITT) analyses without difference (P>0.05). Whereas H. pylori eradication was 87.20/00 with OAC regimen and 91.4% with EAC regimen by per-protocol (PP) analyses without difference (P>0. 05). However, both ITT and PP analysis showed that there was significant difference in H. pylori eradication between EAC regimen (91.9% and 97.1%) and OAC regimen (71.8% and 77.8%) in patients with homEM genotype (P= 0.037 and P=0.028). Two weeks after treatment, the percentage of ulcer healing was 79.2% or 81.9% in EAC group and 69.2% or 76.1% in OAC group by ITT or PP analysis, respectively (P> 0.05). Low side effects were noted in EAC or OAC groups (3.3% vs. 7.5%,P>0.05). Conclusion The high eradication will be achieved by esomeprazole-based triple regimen which is superior to omeprazole-based triple regimen in treatment of patients with homEM genotype.
