1.Clinical significance of B-mode ultrasound found bladder trabeculation in BPH patients

Objective To study the role of B-mode ultrasound found bladder trabeculation in evaluating the degree of bladder outlet obstruction (BOO) and the bladder function in benign prostatic hyperplasia (BPH) patients.Methods Conducted prospective research to determine differences in clinical data and urodynamic parameters between BPH patients with and without bladder trabeculation diagnosed by abdominal ultrasound.Results Thirty-six BPH patients with bladder trabeculation were compared with 68 BPH patients without bladder trabeculation.The mean age was (73.7 ± 10.1 ) years for the patients with bladder trabeculation and ( 69.6 ± 6.2 ) years without bladder trabeculation, IPSS was 24.4 ± 6.6 and 22.8 ± 8.3 respectively, in which no significant differences were found ( P > 0.05 ).The detrusor pressure at maximum flow rate was ( 131.7 ± 57.3 ) cm H20 and (92.1 ± 47.8 )cm H2O ) respectively.The linearized passive urethral resistance relation was 4.6 :± 1.1 and 3.5 ± 1.5 respectively, showing a significant difference ( P <.05 ) between the two groups.Seventy-two point two percent (26/36) of the patients with bladder trabeculation had a low compliance bladder, among whom 23.1% (6/26) of the patients had bilateral hydronephrosis with renal insufficiency.The percentage for the control group was 42.6% (29/68) and 10.3% (3/29) respectively (P < 0.05).Conclusions The bladder trabeculation found by B-ultrasound in BPH patients suggests the existence of BOO and a high risk of bilateral hydronephrosis.Bladder trabeculation in patients without urinary retention suggests they are in compensation status.Relief of the obstruction is helpful to recovery of bladder function and the reduction of complications.
2.Clinical analysis of low does oral Desmopressin in treatment of nocturia in elder women

Objective To investigate the efficacy and safety of low dose of oral desmopressin in elderly women with nocturia. Methods Eligible female patients with nocturia older than 60 years were included in this study.A total of 97 patients were randomly divided into 2 groups.Care was taken to match the patients of the two groups by age and clinical criteria.Control group (n=48 ) received liquid restriction during nighttime.Experimental group (n=49) received 0.1 mg desmopressin at bedtime and liquid restriction for 8 weeks.Patients were required to visit the outpatient clinic from the first visit,and after 4 and 8 weeks of treatment.Patients maintained flow volume charts and used diaries to record voiding data throughout the study.At each visit,all patients were evaluated by blood biochemical routine test,mean nocturnal urine volume,mean number of nocturia,mean duration of the first sleep period and sleep quality.At baseline,all the patient's blood sodium,liver and kidney function were normal. Results After 4 weeks of treatment with desmopressin,28 patients (57.1%) had less than 2 voids.15 patients (31.3%) in the control group had less than 2 voids.After 8 weeks,35 patients (71.4%) with desmopressin had less than 2 voids.16 patients (33.3%) in the control group had less than 2 voids.Compared with control group,nocturia cure rate in experimental group was significantly higher after 4 weeks and 8 weeks (P < 0.05).After 8weeks,desmopressin significantly decreased mean nocturnal urine output from 590 ± 70 ml to 376 ± 50 ml (P < 0.05).Mean nocturnal urine output in the control group was not significantly decreased from 600 ± 90 ml to 550 ± 60ml (P >0.05) ; Mean number of nocturia before and after receiving desmopressin were 2.9 and 1.6 respectively which differed significantly (P < 0.05).Mean number of nocturia before and after in control group were 2.8 and 2.3 respectively with no significant difference (P > 0.05).The mean duration of the first sleep period increased by 73% (from 2.2 to 3.8 h) in the desmopressin group,compared with an increase of 19% (from 2.1 to 2.5 h) in the control group (P < 0.05).39 (79.6%) patients in demopressin group were satisfied with sleep quality compared with only 15 (31.3%) patients in control group were satisfied (P < 0.05).No serious complications were found during the medication.All the patient's blood sodium,liver and kidney function remained normal during treatment. Conclusions Low does oral administration of desmopressin could be an effective and well-tolerated treatment for nocturnal polyuria in elderly women.
3.Value of regional cerebral oxygen saturation and anesthesia depth in predicting postoperative cognitive dysfunction in patients with non-macrovascular surgery

Objective:To explore the value of regional cerebral oxygen saturation (rScO 2) and anesthesia depth monitoring in predicting postoperative cognitive dysfunction (POCD) in patients with non-macrovascular surgery. Methods:A retrospective analysis of 147 patients with non-macrovascular surgery under general anesthesia admitted to the First Affiliated Hospital of Xi'an Jiaotong University from August 2017 to June 2019 was performed and divided into the POCD group ( n=37) and the non-POCD group ( n=110) according to the presence/absence of postoperative POCD. The changes of bispectral index (BIS) and rScO 2 in patients before anesthesia induction (T 0), endotracheal intubation (T 1), 2 hours after operation (T 2), after operation (T 3), and at extubation (T 4) were recorded, and the predictive value for the occurrence of POCD was analyzed by receiver operating characteristic (ROC) curve. Results:There was no statistically significant difference in anesthesia time, operation time and operation type between the two groups ( P>0.05). There was no significant difference in BIS and rScO 2 levels between the two groups at T 0, T 1 and T 4 ( P>0.05). BIS and rScO 2 levels in the POCD group at T 2 and T 3 were lower than those in the non-POCD ( P<0.05). Both BIS and rScO 2 of the two groups reached the lowest value at T 2, and the reduction rate of rScO 2 in the POCD group was higher than that in the non-POCD group [(31.84±3.27)% vs (14.81±2.52)%, P<0.05]. The ROC curve of BIS-T 2, rScO 2-T 2, BIS-T 3, rScO 2-T 3, rScO 2 reduction from the baseline value to predict POCD in patients with non-macrovascular surgery was plotted, and the AUCs were 0.514, 0.617, 0.505, 0.633, 0.724, respectively. The highest AUC value of 0.808 was found for combined detection at T 2 (rScO 2 and BIS). Conclusions:The combined detection of intraoperative regional cerebral oxygen saturation and anesthesia depth monitoring is of good clinical application value in predicting postoperative cognitive dysfunction in patients with non-macrovascular surgery.
4.Impact of micrometastasis in pathologically negative lymph node on staging and prognosis of non-small cell lung cancers.

OBJECTIVETo study the influence of micrometastasis in lymph node on staging and prognosis of non-small-cell lung cancer (NSCLC).
METHODSIn 39 NSCLC patients, micrometastasis in pathologically negative lymph nodes were tested through immunohistochemical cytokeratin (CK) analysis and the relationship between CK(+) and staging, survival were analyzed.
RESULTSIn these 39 patients, the survival of CK(+) and CK(-) patients were 32 months and 48 months respectively (P = 0.0178). Multivariate analysis of Cox regression model showed: clinical stage (P = 0.0288) and relapse or metastasis (P = 0.0053) affected the prognosis while micrometastasis in lymphnodes (P = 0.7740) did not.
CONCLUSIONThe detection of micrometastasis in the lymphnodes may serve as a supplement to the present staging system for lung cancer. Even though the prognosis of patients with micrometastasis being poorer than those without, micrometastasis in the lymph nodes should not be regarded as an independent prognostic factor.

5.Value of Hiquid-based cytology and colposcopy in screening for cervical lesion among urban community women of Beijing

ObjectiveTo study the value of liquid-based cytology and colposcopy in screening for cervical lesion among urban community women of Beijing.MethodsA total of 795 women aged 20~54 years with sexual activity living in Zhanlanlu Community of Beijing were screened for cervical lesion Cervcal specimen was collected for thin-layer,liquid-based cytology test (LCT) from each of the participants in gynecologic examination.Colposcopy and biopsy were performed for the women with positive LCT.ResultsForty-five of 795 (5.7%) women were positive for LCT[≥ASC-US (atypical squamouscell of undetermined significance)],with 33 of ASC-us,eight of low-grade squamous intraepithelial lesion (LSIL),three of high-grade squamous intraepithelial lesion (HSIL),one of atypical glandular cells (AGC).Five of 45 women with positive LCT refused to accept colposcopy.Among 40 women with colposcopy and biopsy,chronic cervicitis was diagnosed in 11(27.5%),cervical condyloma acurninatum in 14(35.0%),cervical intraepithelial neoplasia (CIN) 1 in seven(17.5%),CIN 2 in three (7.5%),CIN 3 in four(10.0%),and early invasive cervical cancer in one(2.5%).In 750 women with negative LCT,cervical condyloma acuminature was diagnosed in two(0.3%),CIN 1 in five(0.7%)and low-grade glandular intraepithelial lesion (LGIL) bin one(0.1%).Sensitivity and specificity of LCT screening for cervical lesion(≥CIN 1)were 71.4%and 94.2%,respectively,with positive and negative predictive values of 37.5%and 99.2%.respectively,and those screening for cervical lesion more than CIN 2 were 100.0%, 96.0%,20.5%and 100.0%,respectively.ConclusionsMore attention should be paid to early screening for cervical lesion in urban community women.LCT combined with colposcopy and biopsy provide very helpful information in screening for early cervical cancer.
6.Genetic diagnosis of polycystic kidney disease using single cell fluorescent polymerase chain reaction.

OBJECTIVETo explore the feasibility of using single cell fluorescent polymerase chain reaction (PCR) in the preimplantation genetic diagnosis (PGD).
METHODSingle buccal cell genetic analysis was performed with fluorescent PCR of linked microsatellite D16S423, followed by electrophoresis on ABI 3730.
RESULTThe amplification success rate, allele dropout rate, and diagnostic accuracy rate of the single cell fluorescent PCR were 93.3%, 10.7%, and 89.3%, respectively.
CONCLUSIONSingle cell fluorescent PCR is a stable and reliable approach for the PGD.

7.Preliminary analysis of bitter substances in spica of Prunella vulgaris.

Volatile oil components and the contents and types of amino acid in spica of Prunella vulgaris were analysed by GC-MS and amino acid analyzer. Esters, fatty acids, aromatic hydrocarbon, ketone and several alcohol compounds were identified by mass spectrum comparison. In these ingredients, beta-ionone smelled aroma of cedar, raspberry, nerolidol showed weak sweet soft orange blossom flavor, neroli tasted sweet and fresh, nerolidol tasted sweet with light aroma of wood, hexadecanal showed a weak aroma of flowers and wax, alpha-sinensal had rich and fresh sweet orange flavor. To some extent, these types of aromatic substances can affect the taste of herbal tea or decoction made of Spica Prunellae. Among amino acids detected, natural amino acids accounted for a larger proportion, and those natural amino acids showed bitterness, slight bitterness, sourness (freshness), sweetness, slight sweetness, sourness (slight freshness). The results indicated that bitter and slightly bitter amino acids have the greatest impacts on the sense of Spica Prunellae.

8.Determination of two polyacetylenes in Herba Lobeliae Chinensis by HPLC analysis.

OBJECTIVETo develop an HPLC method for determination of two polyacetylenes, lobetyolin and lobetyolinin, in Herba Lobeliae Chinensis.
METHODC18 column was used with the mobile phase consisted of acetonitrile and water. Linear gradient elution from 10% to 40% acetonitrile in 25 min was applied, at the flow rate of 1.0 mL x min(-1), the detection wavelength was at 267 nm.
RESULTLower contents of lobetyolin and lobtyolinin were found in collected samples of Herba Lobeliae Chinensis. The highest amounts of lobetyolin and lobetyolinin were found to be 0.461 and 0.436 mg x g(-1) in a sample procured from Hong Kong. However, there were no lobetyolin and lobetyolinin in some of the samples.
CONCLUSIONA simple and effective HPLC method to analyze the two polyacetylenes in Herba Lobeliae Chinensis was established. It could be applied for the quality control of this herb.

9.Correlation between patient-based questionnaires and computer tomography staging in chronic rhinosinusitis

Objective To investigate the relationship between the patient-based questionnaires and the computed tomography (CT) staging in patients with chronic rhinosinusitis (CRS). Methods Quantitative data of 121 preoperative recruits with CRS were collected by using the Lund-Mackay CT staging system, a visual analogue scale ( VAS), sino-nasal outcome test-20 ( SNOT-20), and the medical outcome study short-form 36 items (SF-36). The patients were classified into several subgroups according to whether CRS was associated with nasal polyps (NP) or not, sex, duration of disease, and educational background.Correlation between the patient-based questionnaires and the CT staging were analyzed in the total cohort patients and subgroups. Results In the total cohort patients, there were significant correlations between SNOT-20 and SF-36 ( r = -0. 561, P < 0. 01 ), SNOT-20 and VAS ( r = 0. 743, P < 0. 0l ), and SF-36 and VAS ( r = - 0. 504, P < 0. 01 ), however, the CT staging did not correlate with the patient-based questionnaires (P > 0. 05 ). Significant but weak correlations were found between the CT staging and the patient-based questionnaires in the C RS with NP subgroup (CT vs SNOT-20, r = 0. 318, P = 0. 005; CT vs SF-36, r = - 0. 358, P = 0. 002; CT vs VAS, r = 0. 358, P = 0. 002). Compared between CRS with NP and without NP subgroup, there were statistic differences on the Lund-Mackay CT stage and the SNOT-20 and VAS scores (t value was 3.249, -2.409, -2.957, respectively, all P<0.05). Conclusions The patient-based questionnaires correlate well with each other, but CT staging correlated significantly but weakly with the patient-based questionnaires only in the CRS with NP subgroup. Nasal polyps do not appear to be responsible for the adverse effects of CRS on quality of life.
10.Researching the character and stenosis of coronary calcified plaques by using 16-slice spiral CT.

OBJECTIVETo explore the characters of coronary calcified plaques by using 16-slice spiral CT and determine their stenosis degree according to the results of catheter coronary angiography.
METHODSTwenty patients who had received 16-slice spiral CT coronary angiography and conventional coronary angiography (CAG) were found to be with calcified plaques. The characters of these plaques, including the diameter of calcified plaques and lumen diameter of the exact artery segment, were retrospectively analyzed. The stenosis degree of the corresponding segment was judged in accordance with the results of CAG.
RESULTSTotally 84 calcified plaques were observed in 16-slice spiral CT images in these 20 patients. Among them there were 16 small nodules (diameter: < 0.15 cm), 56 purely calcified plaques (diameter: > or = 0. 15 cm), and 12 complex plaques with calcify component. There was no obvious stenosis in artery segments with little calcified nodules. The stenosis degree of most segments with purely calcified plaques (75%) was less than 50%. The stenosis degree had no significant correlation with the size of plaques (P > 0.05). However, the stenosis degree of complex plaques had much closer relationship with the characters of mixed plaques.
CONCLUSIONSMost coronary segments with calcified plaques have slight stenosis. Their stenosis degree is not related with the size of plaques. The stenosis degree of complex plaques has a closer relationship with the characters of mixed plaques.
