1.Expression of serum tumor necrosis factor-аlpha and interleukin-1beta in a vascular dementia rat model undergoing total flavonoids of hippophae

BACKGROUND:Total flavonoids of hippophae have different degrees of regulation effect on the immune system. However, there is no available evidence addressing the effect of total flavonoids of hippophae on vascular dementia inflammatory factor. OBJECTIVE:To observe the effect of total flavonoids of hippophae on serum tumor necrosis factor-αand interleukin-1βlevels in a rat model of vascular dementia, and to discuss its interventional mechanism for vascular dementia. METHODS:A total of 72 Wistar rats were randomly divided into six groups:sham operation, model, control, high-dose, medium-dose, and low-dose of total flavonoids of hippophae groups. Except for the sham operation group, rats in the other five groups were applied to establish vascular dementia models using the improved“vascular occlusion”method. Sham operation group was only subjected to the isolation of bilateral common carotid artery, without ligation. At 1 day after modeling, rats in control group was intragastrical y given Ginkgo biloba extracts 48 mg/kg?d, while rats in total flavonoids of hippophae groups were given 200, 100, 50 mg/kg?d of total flavonoids of hippophae. Model group and sham operation group received equal volume of saline. After 30 days of administration, rats were detected in Morris water maze. Serum tumor necrosis factor-αand interleukin-1βlevels of rats in different groups were detected after intervention. RESULTS AND CONCLUSION:Compared with sham operation group, the vascular dementia model rats had a disordered learning and memory ability, and increased levels of serum tumor necrosis factor-αand interleukin-1β(P<0.05). After intervention of Ginkgo biloba extracts and total flavonoids of hippophae, the learning and memory abilities were improved, and serum tumor necrosis factor-αand interleukin-1βlevels were decreased (P<0.05). Total flavonoids of hippophae can reduce serum tumor necrosis factor-αand interleukin-1βlevels in vascular dementia rats. This may be the main mechanism of treating vascular dementia.
2. Prospective cohort study of body height and cancer incidence among adult men and women in Shanghai

Objective: To investigate the association between body height and risk of cancer incidence among adult men and women in Shanghai. Methods: Baseline information on body height and other factors relevant for cancer was obtained from Women's (1996-2000) and Men's Health Studies (2002-2006) in Shanghai. Participants with missing data on body height, or those lost to follow-up in one year after the study enrollment were excluded, leaving 74 709 women and 61 161 men for analysis. There were 4 031 incident cancers for women and 2 047 for men confirmed at the end of 2009. COX regression model was used to evaluate the HR s ( hazard ratios) and 95% CI (confidence interval) between height and cancers. Results: After adjustment for some potential confounding factors including age, weight, physical activity, energy intake, occupation, income and reproductive factors (for women only), HR s and 95% CI s for cancer incidence per 1 standard deviation (6 cm for both men and women) increment in body height were 1.04 (95% CI : 1.00-1.08) for total cancers, 1.18 (95% CI : 1.05-1.33) for stomach cancer, 1.34 (95% CI : 1.06-1.70) for kidney cancer and 1.05 (95% CI : 0.99-1.12) for breast cancer in women; and 1.06 (95% CI : 1.00-1.11) for total cancers, 1.11 (95% CI : 1.00-1.25) for lung cancer, 1.34 (95% CI : 1.02-1.75) for pancreatic cancer, 1.23 (95% CI : 1.01-1.49) for rectal cancer and 0.78 (95% CI : 0.66-0.94) for prostate cancer in men. Conclusion: A possible link between body height and cancer incidence in men and women is suggested. The association between body height and cancer incidence varies by cancer type. From October 1969 through December 1981, 3483 esoghageal carcinomas without superficial lymphatic metastasis were treated by 60 Co in our hospital. The radiation dose was over 50Gy and follow-up was over ten years. Results: No significant difference of survival rates was found in various cancer locations in the esophagus combined with the data between our hospital and Xingtai Tumor Hospital, the 5-year survival rates for lower third esophageal cancer were 20.7%~36.4% in surgery group and only 4.3% in radiation group. 4.Study on characteristics of pharmacological effects of traditional Chinese medicines distributing along kidney meridian based on medicinal property combination.

To study the characteristics of pharmacological effects of property combinations of traditional Chinese medicines (TCMs) distributing in the stomach meridian based on medicinal property combination, in order to further define the association relationship between properties of TCMs and their pharmacological effects, and build a bridge for the interpenetration and combination between the medicinal property theory of TCMs and their pharmacological effects. On the basis of the studies on the medicinal property theory of TCMs distributing along the kidney meridian and their pharmacological effects, efforts were made to collect relevant data for medicinal properties and pharmacological effects and mine the characteristics of pharmacological effects that were corresponding to relevant medicinal property combination by processing materials related to medicinal properties and pharmacological effects with the association rules method. According to the analysis, TCMs distributing along the kidney meridian with different medicinal property combinations were significantly differentiated in the pharmacological effects, but shared identical pharmacological effects, such as immunological enhancement. In this study, TCMs distributing along the kidney meridian with different medicinal property combinations were taken as the carriers to closely integrate the traditional Chinese medicine theory with the modem study achievements, lay a solid foundation for further developing and enriching the traditional Chinese medical property theory, while providing a new perspective on the development of modem medicine.

6.Clinical study of dexmedetomidine combined with propofol on double-balloon enteroscopy

Objective To explore the efficacy and safety of dexmedetomidine combined with propofol on double-balloon enteroscopy.Methods Forty cases of patients ASA Ⅰ-Ⅱ grade who underwent double-balloon enteroscopy were divided into dexmedetomidine combined with propofol group (combine group) and propofol group by random digits table with 20 cases each group.Combine group was given load 0.7 μg/kg (intravenous infusion for 10 min) before induction and 0.2 μg/ (kg·h) continuous infusion during the surgery.All the patients were used propofol by target concentration with 2.5-4.0 mg/L for target-controlled infusion heart rate (H R),mean arterial pressure (MAP),respiratory rate (RR),peripheral oxygen saturation (SpO2) were recorded before examination(T0),eye lash reflex time(T1),during the perform (T2),the end of check (T3),sedation score,induction time,recovery time,total propofol amount,rates of adverse reaction and satisfaition were recorded.Results The induction time,recovery time was no statistically significant difference between two groups (P > 0.05).The RR,SpO2 was no statistically significant difference between two groups at different time points (P >0.05).The HR at T1,T2 in combine group was significantly lower than that at T0 and in propofol group same period[(65.8 ± 7.3),(68.6 ± 8.2) times/min vs.(84.6 ± 7.1) and (77.6 ± 7.2),(89.6 ± 8.4) times/min,P < 0.05].The MAP at T1 in combine group was significantly higher than that at T0 and in propofol group same period [(88.9 ± 9.4) mm Hg (1 mm Hg =0.133 kPa) vs.(81.3 ± 4.5) and (73.5 ± 6.8) mm Hg,P < 0.05],at T2 was significantly lower than that in propofol group [(80.6±6.6) mm Hgvs.(88.5 ±7.6) mm Hg,P<0.05].Sedation score 1 score 18 cases,2 scores 2 cases in combine group; 1 score 3 cases,2 scores 17 cases in propofol group,the difference was statistically significant (P < 0.05).Combine group apnea two cases,moving,choking three cases,propofol group were eight cases and seven cases,the difference was statistically significant (P < 0.05).The total propofol amount in combine group was significantly lower than that in propofol group [(421 ± 76) mg vs.(638 ± 89) mg,P < 0.05].Conclusion Dexmedetomidine composite target controlled infusion of propofol used for double-balloon enteroscopy can produce good narcotic analgesic which is safe and effective anesthesia.
7.Effect of high concentration sevoflurane wash-in in induction of anesthesia on cardiac function in young infants

Objective To evaluate the effect of the high concentration of sevoflurane wash-in in induction of anesthesia on the cardiac function in young infants.Methods ASA Ⅰ young infants,aged 28-60 days,undergoing resection of hemangioma on body surface under general anesthesia,were studied.According to the principle of randomized block design,8 blocks were designed based on the body weight (3.0-3.4 kg,3.5-3.9 g,4.0-4.4 kg,4.5-4.9 kg,5.0-5.4 kg,5.5-5.9 kg,6.0-6.4 kg,6.5-6.9 kg),with 3 infants in each block.The infants were randomly divided into 3 groups (n =8 each):anesthesia induction with 6%,7% and 8% sevoflurane groups (groups S1-3).In groups S1-3,the infants inhaled 6 %,7 % and 8 % sevoflurane respectively for induction of anesthesia.Transthoracic echocardiographic examinations were performed before sevoflurane inhalation (baseline,T1) and at 4 min of sevoflurane inhalation (T2) to record the stroke volume (SV),left ventricle end-diastolic volume (LVEDV),ejection fraction (EF),stroke volume index (SI),heart rate (HR),cardiac output (CO),cardiac index (CI),peak early diastolic velocity of mitral annulus (Ea),peak late diastolic velocity of mitral annulus (Aa),peak systolic velocity of mitral annulus (Sa),and Tei index.The mitral annulus poteroseptal Ea/Aa (Ea/Aa) was calculated.Results Compared with the baseline value,SV,EF,SI,Ea/Aa and Tei index were significandy increased,HR,Sa,Ea and Aa were significantly decreased (P < 0.05),and no significant change was found in LVEDV,CO and CI at 4 min of sevoflurane inhalation in groups S1-3 (P > 0.05).Tei index was significantly higher in groups S2.3 than in group S1 (P < 0.05).There was no significant difference in the parameters of cardiac function between group S2 and group S3 (P > 0.05).Conclusion 6 %,7 % and 8 % sevoflurane wash-in in induction of anesthesia exert no effect on the cardiac function in young infants,and the 3 concentrations can be recommended for clinical use.

In comparison with the clinical PET scanner,PET scanner for small animals puts forward a higher demand in spacial resolution and sensitivity. This article describes the development of the PET scanner for small animals and compares the characteristics of several kinds of contemporary typical PET scanners for small animals in different aspects based on the development of detectors. Meanwhile,it also introduces the development of the detectors designed for measuring depth of interaction (DTC) to reduce parallax error.
10.Evaluation the Value of Left Ventricular Rotation and Torsion in Pre-apical Hypertrophic Cardiomyopathy Diagnosis Using Two-dimensional Speckle Tracking Imaging

Objective To discuss the diagnostic value of two-dimensional speckle tracking imaging in the evaluation of left ventricular rotation and torsion in pre-apical hypertrophic cardiomyopathy(PAHCM)patients. Methods A total of 26 patients with PAHCM,26 patients with hyperten-sive left ventricular hypertrophy(HLVH)and 26 healthy volunteers were recruited for the study. Two-dimensional echocardiographs were performed carefully. Two-dimensional images were obtained from the left ventricular apical four-chamber section,two-chamber section and two short-axis sec-tions,including mitral valve and apical levels. The peak subendocardial rotation(endo-rot),epicardial rotation(epi-rot),transmural torsion(mural-tor)and bulk rotation(bulk-rot)of each short-axis section were measured and analyzed using QLAB 9.1 software. Results In the mitral level, there was no statistically significant difference in endo-rot,epi-rot,mural-tor and bulk-rot between the PAHCM group and normal control group (P>0.05). There was statistically significant difference in endo-rot,epi-rot,mural-tor and bulk-rot between the PAHCM group and HLVH group (all P<0.05). There was statistically significant difference in endo-rot,epi-rot,mural-tor and bulk-rot between the HLVH group and normal control group(all P<0.05). In the apical level,there was no statistically significant difference in epi-rot among three groups(P>0.05). There was statisti-cally significant difference in endo-rot,mural-tor and bulk-rot between each two groups(P<0.05). There was statistically significant difference in G-tor between each two groups(P<0.05). The receiver operating characteristic curve analysis showed that the accuracy of differential diagnosis of G-tor PAHCM was high(the area under the receiver operating characteristic curve is 0.89),the sensitivity was 73.08%,and the specificity was 92.31%. Conclusion Two-dimensional speckle tracking imaging could accurately and quantitatively measure the left ventricular rotation and tor-sion in PAHCM patients. The G-tor could accurately screen and identify between PAHCM and HLVH.
