2.Establishing of Good Extracting Practice of the Traditional Chinese Medicine and Facilitating the Stan-dardization of Chinese Botanical Extracts

OBJECTIVE:In order to enhance the quality control of Chinese botanical extracts and to facilitate the entry of Chinese botanical drugs and traditional Chinese medicine into the international market.METHODS:The market overview of the botanical extracts was expounded,problems existed in the quality control of the Chinese botanical extracts were analyzed,and the significance and key points in establishing Good Extracting Practice(GEP)of the traditional Chinese medicine were discussed.RESULTS&CONCLUSION:The quality control of the Chinese botanical extracts must be emphasized,which means to establish the feasible quality standard;the production process should be optimized and the corresponding‘Standard Operating Procedure’should be formed;advanced technology and facilities should be adopted to improve the content of hi-tech,the toxic substances should be strictly controlled;the quality of the raw material should be guaranteed,the connection between which and the‘Good Agricultural Practice for Chinese Crude Drugs'should be strengthened.

OBJECTIVE:To provide references for the expanding of Chinese traditional medicine enterprises in USA market.METHODS:The management mode of the botanical preparations in the USA and the related laws and regulations were overviewed from the perspectives of dietary replenisher and drug areas.RESULTS&CONSCLUSION:For the entry of Chi?nese traditional medicine into the USA market,a realistic way of behavior for the time being is to adopt a step-by-step pol?icy,i.e.first of all,a research of the standard botanical extracts should be conducted before entering into the USA market in the status of dietary replenishers,then the clinical research application was raised after marketing in USA market for several years,eventually,new drug application was raised.
4.Opportunities and Challenges for the Expanding of Traditional Chinese Medicine in European Union Herbal Drug Market

OBJECTIVE:To facilitate a better understanding of the European Union(EU)herbal drugs market for Chinese TCM enterprises and to promote the internationalization of Chinese traditional medicines.METHODS:The opportunities and challenges confronted by TCM enterprises in their expanding of their products in the EU herbal medicine market were analyzed based on the knowledge about market size,market segmentation,consumption patters and developmental tendencies of the EU herbal medicine market.RESULTS&CONCLUSIONS:There were both opportunities and challenges for the expanding of traditional Chinese medicines in the EU herbal medicine market,yet on the whole,the opportunities were more than the challenge.Chinese enterprises should strive to further expand the EU herbal medicine market.

European Society of Hypertension/European Society of Cardiology jointly published new guidelines on diagnosis and treatment of hypertension in 2007.It reflected the the latest developments on comprehensive assessment,treatment modalities and strategies,as well as therapeutic approach for special populations.In addition,the new guidelines updated evaluation.It also stressed the importance of an early,faster and more stringent treatment and aggressive combination therapy.More impartantly,it requested prevention and treatment earlier.It is of great importance for the guidelines to guide the current diagnosis and treatment of hypertension.

7.Clinical outcomes of laparoscopic treatment of tubal adhesion and tubal distal occlusion

Objective To investigate the clinical pregnancy outcomes of laparoscopic treatment for different degrees of tubal adhesion and tubal distal occlusion.Methods Clinical information of 41 infertile patients from April 2001 to December 2005 was reviewed.According to the extent of tubal and pelvic lesion,the patients were classified as stage Ⅰ in 10 patients,stage Ⅱ in 10,stage Ⅲ in 17,and stage Ⅳ in 6.After a diagnosis was made by hysterosalpinggography(HSG),all the patients received a salpingostomy and adhesionlysis under laparoscope.Results No abnormal findings were detected by hysteroscopy and bilateral tubes were patent after operation in all the 41 patients.The 10 patients at stage Ⅰ were followed up for 12~21 months.Four of them got pregnant in six months and 1 got pregnant in 1 year because of intermittent separation,all the 5 patients being intrauterine pregnancy.The 8 patients at stage Ⅱ were followed up for 12~26 months.Three patients got pregnant,including intrauterine pregnancy in 1 and ectopic pregnancy in 2.The pregnancy occurred at 15,16,and 26 months after operation,respectively.The 17 patients at stage Ⅲ were followed up for 3~48 months.Four had pregnancy,at 3,4,12,and 14 months after operation,respectively,including intrauterine pregnancy in 1 and ectopic pregnancy in 3.The 6 patients at stage Ⅳ were followed up for 12~36 months and no pregnancy was observed.Among the 12 patients with pregnancy,8 got pregnant within 12 months and 3 at 13~18 months after operation,the pregnancy rate within 18 months being 91.7%(11/12). Conclusions Clinical pregnancy outcomes are related with the degree of tubal lesion and adnexal adhesion.Patients at stage Ⅰ~Ⅱ have better pregnancy outcomes than patients at stage Ⅲ~Ⅳ.

Objective To study the clinical application of laparoscopy combined with hysteroscopy to female infertility.Methods From January 2007 to March 2008,145 cases of female infertility were examined and treated by laparoscopy combined with hysteroscopy in our hospital.We analyzed the clinical records of the patients to determine the value of the method.Results By using laparoscopy combined with hysteroscopy,136 cases of pelvic disease(93.8%,136/145) were detected in this series.The most common causes of infertility were pelvic inflammation(58.6%,85/145) and endometriosis(29.0%,42/145).The incidence rate of pelvic inflammation in secondary infertility patients was significantly higher than in those with primary infertility(P
9.Investigation and Analysis of the Cognition of Physicians to the Prescription Handling of Commonly Used TCM Decoction Pieces in a Third Grade Class A Hospital of TCM from Beijing

OBJECTIVE:To provide reference to promote the proper use of TCM decoction pieces.METHODS:According to Beijing TCM Decoction Piece Dispensing Regulation,stratified random sampling method was adopted to select a group of physicians in our hospital and conduct questionnaire survey about their knowledge of the prescription handling of 8 commonly used TCM decoction pieces including Scutellaria baicalensis,Angelica sinensis,Fritillary,Phellodendron chinense,Aconitum carmichaeli,Eucommia ulmoides,Ziziphus jujuba,Rhei Radix et Rhizoma.Then,the survey data was collected and analyzed.RESULTS:A total of 81 questionnaires were given out and all were returned with effective recovery rate of 100%.In terms of professional titles,associate chief physicians showed the lowest average error rates in prescription handling cognition (27.5 %),compared with physicians (35.8%) and attending doctors (37.0%).They made most mistakes about Fritillary and E.ulmoides.To the contrary,both physicians and attending doctors made most mistakes about E.ulmoides.In the scope of working life,physicians from different groups showed similar average error rates in prescription handing recognition (31.3%-38.8%).All of them made most mistakes about P.chinense and E.ulmoides.As for major,those who study western internal medicine shared larger average error rates (43.8%) than those who study internal medicine of TCM (36.8%),traumatology and orthopedics of TCM (31.8%) and acupuncture (36.5%).Each major showed the highest error rates in E.ulmoides.The order of 8 TCM decoction pieces from the most mistakes to the least mistakes made by physicians was E.ulmoides,P.chinense,Fritillary,Rhei Radix et Rhizoma,S.baicalensis,A.carmichaeli,Z.jujuba and A.sinensis.CONCLUSIONS:Physicians with different professional titles,working life and majors show deficiency in knowledge about the prescription handling of 8 commonly used TCM decoction pieces.Generally,physicians show the worst understanding of E.ulmoides.
10.The effect of allo-human bone marrow mesenchymal stem cells on interleukin-1, tumor necrosis factor-α, transforming grouth factor-β1 of patients with rheumatoid arthritis

ObjectiveTo study the effect of allo-human bone marrow mesenchymal stemcells (bMSCs) on the secretion of interleukin(IL)-1, tumor necrosis factor(TNF)-α and transforming grouth factor (TGF)-β of patients with rheumatoid arthritis (RA) in vitro. MethodsBMSCs were isolated from bone marrow of healthy volunteers and purified by density gradient centrifugation and cultured in vitro. The mononuclear cells from the peripheral blood of patients with RA and healthy controls were isolated respectively.bMSCs and mononuclear cells were co-cultured in vitro and the density of IL-1, TNF-α and TGF-β3 in the co-culture system were detected by ELISA. ANOVA and Pearson correlation were used for statistical analysis.ResultsMononuclear cells from peripheral blood of patients with RA were co-cultured with bMSCs for seven days. There were an decreased density ofIL-1[(38.4±0.5) vs(6.2±1.0) ng/L], TNF-α[(29.4±1.3) vs (4.6±1.2) ng/L]and an increased density of TGF-β[(2.6±1.0) vs (22.5±2.2) ng/L]in the co-culture system (P<0.05). But on the other hand, for healthy volunteers there were no significant change in the density of IL1[(4.4±1.1) ng/L]and TNF-α[(5.0±1.7) ng/L]in the coculture group, as compared with the mononuclear cell group[(4.4±1.3) vs(5.3±1.7) ng/L](P>0.05). There was an increased density of TGF-β in the coculture system[(4.8±1.4) vs(10.5±1.2) ng/L](P<0.05). IL-1 was positively correlated to TNF-αt (r=0.896,P=0.000), TNF-β1 was nagative correlation with 1L-1 and TNF-α (r=-0.356,P=0.019; r=-0.380,P=0.000).ConclusionHuman bone marrow MSCs have modulatory effects on main cytokines of patients with RA in vitro. bMSCs could down-regulate the levels of IL-1 and TNF, but up-regulate the density of TGF-β. These immune-modulatory effects are not MHC restricted. The results of this study have provided evidence for the development of effective therapy for RA.
