Objective:To evaluate the effects of losartan、fosinopril and captopril on the excretion rate of urinary albumin (UAER) and the clearance of endogenous creatinine (Cr-C) in elderly patients. Methods:In this study,162 elderly patients with stage of the 2—3 class hypertension were included,The patients were divided into 3 groups, that were losartan group and the fosinopril group and captopril group, the anti-hypertensive effects and UAER and Cr-C were determined before and after the treatment. Results:The hypertensive effect rates were 94% in losartan group and 93%、93% in fosinopril group and captopril group respectively.Decrease of UAER was observed in 89% of patient in losartan group and 59%、50% in fosinopril group、captopril group.Cr-C were improved in 94%、78% of patient in losartan group and in fosinopril group and 63% in captopril group. Conclusions:Losartan has better effects on UAER and Cr-C than captopril in elderly hypertensive patients.

Objective To formulate countermeasures of improving nurses′knowledge of malignant hyperthermia by investigating theatre nurses. Methods A self-designed questionnaire was used to investigate 120 theatre nurses for awareness, demand and access about malignant hyperthermia. Results Average correct response rate on the awareness of theatre nurses about malignant hyperthermia was (17.14±9.68)%.There are differences in different education, title and seniority(F=8.002,6.509,2.643,P<0.05).100.0%(120/120)theatre nurses are interested in studying knowledge related to malignant hyperthermia, 94.2%(113/120) theatre nurses are willing to learn knowledge related to malignant hyperthermia. Conclusions The awareness of theatre nurses about malignant hyperthermia is low. It is necessary to strengthen the training of theatre nurses about the knowledge of malignant hyperthermia. A variety of forms can be adopted.

This study was aimed to explain the relationship between chemical component and therapy objective and to identify effective component group (ECG). The objective of this study is to build a new method to identify the ECG. Traditional Chinese medicine (TCM) formula and one of its treated diseases were selected as the re-search object. The entity grammar systems (EGS) were used as the theoretical framework. A grammatical method for ECG identification was constructed. The component-disease relationship was inferred and the ECG was identi-fied with these inference results. In this paper, 16 compounds which act to 12 proteins in Type 1 diabetes melli-tus (T1D) disease network were identified from Bai-Hu Tang plus Xiao-Ke Fang and 9 chemical compounds were determined as the candidate effective components group. The results indicated that this method can be used to identify ECG and provide a new way to elucidate the molecular mechanism of TCM formula.

ObjectiveTo investigate the therapentic effect and safety by use of Metoprolol to control patients ,heart rate in off-pump coronary artery bypass graft (OPCAB). MethodsAnalyse the dates of 40 OPCABG patients in anesthetic stage. ResultsIn all of the cases, when patients were treated with venous injection of Metoprolol, the heat rate obviously decreased 1 minutes later( P < 0.01 ). the heat rate decreased significant to( 69.2 ± 10. 8)times/min 3 minutes later, Compared to the heat rate ( 85.1 ± 13.5) times/min of pretherapy there was statistical significance. the heat rate decreased to nadir(63.0 ± 6.2) times/min 10 minutes later. While it raised gradnally 30 minutes later, but patients'heat rate nulli-exceed. it of pretherapy. On post-treatment of 3min、5min、 10min、30min, The MAP nulli-genic marked change. ConclusionMetoprolol could control effectively patients, heat rate in OPCAB and keep hemodynamics stable.

Objective To compare pharmacodynamics of vecuronium administered according to body surface area and body weight during general anesthesia. Methods Forty ASA Ⅰ or Ⅱ patients, aged 18-64 yr,weighing 40-85 kg,undergoing general anesthesia, were randomly divided into 2 group ( n =20 each ).The patients received vecuronium 2× ED95 based on body weight (group W) or based on body surface area (group S).Anesthesia was induced with propofol 2 mg/kg,fentanyl 3 μg/kg and vecuronium 0.1 mg/kg ( group W) or 2.824 mg/m2 (group S).The patients was tracheal intubated and mechanically ventilated when the maximal depression of T1 was achieved.PET CO2 was maintained at 35-45 mm Hg and BIS value was maintained at 40-50.The intubation condition was evaluated using Cooper's score. The onset time, maximal depression of T1,duration of clinical action, recovery index, duration of pharmacological action and amount of vecuronium consumed were recorded. The coefficient of variation for all the indexes mentioned above was calculated. Results There was no significant difference in the coefficient of variation for intubation condition, onset time, duration of clinical action, recovery index, and duration of pharmacological action between the two groups ( P > 0.05).Compared with group W,the coefficient of variation for the maximal depression of T1 and amount of vecuronium consumed were significantly decreased in group S ( P < 0.05).Conclusion Vecuronium 2 × ED95 administered according to body surface area can reduce the individual variation in pharmacodynamics during general anesthesia.

Chinese Journal of Internal Medicine 2016;55(8):613-618
Objective To summarize the clinical features of different racial patients with celiac disease (CD) and analyze the disease prevalence, diagnosis and treatment in Chinese population. Methods All the patients were diagnosed as CD and enrolled in Beijing United Family Hospital between January 2005 and July 2015.Clinical data including nationality, age, symptoms, endoscopic and pathological findings, outcome were collected and compared in patients from different countries. Results A total of 87 patients were enrolled including 63 Caucasians,18 Asian patients and 6 Middle East patients. The peak age of disease onset was 40-60 years old.Patients with typical symptoms such as chronic diarrhea and weight loss only accounted for 20.7% (18/87) and 9.2% (8/87) respectively. Some patients presented with nonspecific symptoms such as abdominal pain and bloating [32.2% (28/87)],even constipation [5.7% (5/87)].13.8% (12/87) patients were previously diagnosed as irritable bowel syndrome. The incidence of abdominal pain, bloating, diarrhea and constipation between Asians and Caucasians had no statistical significance (P > 0.05);but the proportions of weight loss, growth retardation, iron deficiency anemia and dermatitis herpetiformis in Asian group were significantly higher than that in Caucasian group (P < 0.05).IgA type of anti-gliadin antibody (AGA),endomysium antibody (EMA) and tissue transglutaminase antibody (tTGA) were dominant autoimmune antibodies in patients with CD,which accounted for 58.6% (51/87),44.8% (39/87) and 36.8% (32/87) respectively. The endoscopy showed that the lesion of CD was mainly located in small intestine, with reducing severity from the proximal to the distal small intestine. The lesions of duodenal bulb and descending duodenum appeared more significant in Asian group.Accordingly pathological intestinal atrophy and the degree of intraepithelial lymphocytosis were more severe in Asian patients. All 87 cases took the gluten-free diet (GFD).Eighty-one cases received serological follow up and 8 with endoscopic intestinal biopsy. The celiac disease antibodies in 47 patients turned negative from 6-9 months after GFD treatment, while 34 patients turned negative from 12-18 months after GFD.All patients reported disease remission to some extent. After 1 year GFD treatment, the pathology of endoscopic intestinal biopsy in 8 patients showed significant improvement of villous atrophy and lymphocyte infiltration. Conclusions CD patients with typical clinical manifestations are not the majority.Serological celiac disease antibodies (AGA,EMA and tTGA) have a high diagnostic value.GFD treatment is effective on majority of celiac patients.Clinical manifestations, endoscopy, intestinal pathology, and response to GFD in Chinese patients are not the same as Caucasians.Clinicians need to pay attention to the differential diagnosis.
