1.Clinical observation on effect of huoxue jieyu decoction in treating depression after craniotomy: a report of 45 cases.

OBJECTIVETo observe the therapeutic effect of Huoxue Jieyu decoction (HJD) on patients with depression after craniotomy.
METHODSNinety patients were randomly divided into 2 groups according to the sequence of their consultation. The 45 patients in the treated group were treated with conventional symptomatic treatment plus oral administration of HJD, and the 38 patients in the control group were treated in the same way but with chlorimipramine (anafranil) instead of HJD. Hamilton depression (HAMD) scores before and after treatment, occurrence of adverse effects and long-term therapeutic effect of treatment were observed.
RESULTSThe score of HAMD was insignificantly different in 2 groups before and after treatment (P >0.05), but the occurrence of adverse effects between them was significantly different (P < 0.01).
CONCLUSIONThe therapeutic effect of HJD on depression was similar to that of anafranil, but HJD has less adverse effect and more stabilized long-term effect.

2.Establishment of HPLC Fingerprint ,Chemical Pattern Recognition Analysis and Content Determination of the Leaves of Toricellia angulata from Different Regions

OBJECTIVE:To provide reference for the quality control of the leaves of Toricellia angulata . METHODS :HPLC method was adopted. The determination was performed on Agela Promosil C 18 column with 0.2% phosphoric acid solution-acetonitrile(gradient elution )as mobile phase at the flow rate of 1.0 mL/min. The detection wavelength was set at 210 nm,and column temperature was 35 ℃. The sample size was 10 μL. HPLC fingerprint of 10 batches of the leaves of T. angulata was established and similarity evaluation was conducted by using Similarity Evaluation System of TCM Chromatographic Fingerprint(2004 edition). The chromatographic peak was identified by comparing with the chromatogram of reference substance. Cluster analysis ,PCA and PLS-DA were used to identify chemical patterns ,and the quality differential markers were screened. The contents of hyperoside and isoquercitrin were determined by the same HPLC. RESULTS :The similarities of HPLC fingerprint of 10 batches of the leaves of T. angulata with control fingerprint were 0.923-0.983. A total of 11 common peaks were identified ,and the peaks 4 and 5 were hyperoside and isoquercitrin ,respectively. Results of cluster analysis ,PCA and PLS-DA showed that 10 batches of leaves of T. angulata could be divided into two categories ,Y10 was clustered into one category ,and others were clustered into one category. PLS-DA analysis showed that 6 common peaks (peaks 4,3,10,2,6 and 11) with variable importance projection (VIP)greater than 1 were selected. Average contents of hyperoside and isoquercitrin in 10 batches of the leaves of T. angulata were 0.47-6.97,0.21-1.87 mg/g,respectively. CONCLUSIONS :Established HPLC fingerprint and the method for content determination are stable and reliable ,and can be used for the quality control of the leaves of T. angulata from different areas. Six quality differential markers including hyperoside in the leaves of T. angulata from different areas are qnyz202034) preliminarily screened.
3.Three-step sequential therapy on pathomorphology of bronchial lung tis-sues of asthmatic rat airway remodeling after hormone intervention

Objective To observe the effect of three-step sequential therapy on the pathomorphology of the bronchial lung tissues of rat asthmatic model with airway remodeling after hormone intervention using light microscope and electron microscope.Methods The airway remodeling of asthmatic rats model was induced by hormone intervention to simulate hormone withdrawal (HW)of steroid dependent asthma (SDA)patients.Altogether 150 rats randomly divided into five groups (each n =30)of airway remode-ling group,dexamethasone intervention group,pulmicort group,three-step sequential therapy group and normal control group.Morphological changes of the bronchial lung tissues of rats of all groups were ob-served under light microscope and electron microscope,and thicknesses of the bronchial walls and the bronchial smooth muscle and the number of the cell nucleus in the bronchial smooth muscle were meas-ured.Results Using HE staining,the lung tissues of rats except for those in normal control group were infiltrated by inflammation cells with different degrees,but the rats of three-step sequential therapy group were observed significantly slight inflammatory reaction.Using Masson trichromatic staining,massive col-lagen fiber in pulmonary interstitial of airway remodeling group deposited,while deposition increased dur-ing hormone withdrawal in dexamethasone intervention group,and significantly less deposition was ob-served in both three-step sequential therapy group and pulmicort group.Before HW,the thickness of the bronchial walls in three-step sequential therapy group decreased significantly compared with that in airway remodeling group(P <0.01);during and after HW,the thickness of the bronchial walls in three-step se-quential therapy group decreased compared with dexamethasone intervention group and airway remodeling group(P <0.01,P <0.05).Before HW,there is no difference (P >0.05)of bronchial smooth muscle thickness observed in all the medication groups;during and after HW,bronchial smooth muscle thickness decreased compared with dexamethasone intervention group and airway remodeling group(P <0.01,P <0.05).The number of cell nucleus in the bronchial smooth muscle of three-step sequential therapy group decreased significantly,regardless of HW,compared with dexamethasone intervention group and airway remodeling group(P <0.01).Using electron microscope,airway epithelial cilia falling off was remark-ably alleviated during HW in three-step sequential therapy group and pulmicort group,while that in dexa-methasone intervention group during or after HW was the same as the airway remodeling group. Conclusion Three-step sequential therapy could inhibit the airway remodeling by the way of relieving inflammatory changes of the bronchial lung pathomorphology of rat asthmatic model.
4.The surgical treatment strategy and perioperative management of patients with heart valve disease complicated with severe chronic heart failure

Objective To summarize the individualized selection of surgical treatment strategies and the key points of perioperative management for patients with heart valve disease complicated with severe chronic heart failure. Methods The clinical characteristics of 5 male patients with valvular heart disease complicated with severe chronic heart failure (CHF) were analyzed retrospectively from June 2017 to October 2018 in Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology, with an average age of 60.21 years. Results Five patients were given angiotensin receptor and neprilysin inhibitor (ARNI)-based anti-heart failure treatment after admission. The operation mode of these patients was decided to be valve replacement under cardiopulmonary bypass after individualized evaluation of patients’ improving symptoms. Three patients were treated with intra-aortic balloon pump (IABP) and continuous renal replacement therapy (CRRT) early after operation to assist patients in improving cardiac function. Five patients recovered oral anti-heart failure after awakening. All patients were discharged smoothly 2 weeks after operation. Conclusion Individualized evaluation is needed for the choice of operation timing and mode, standardized preoperative treatment for heart failure, shortening the aortic blocking time during cardiopulmonary bypass, and early application of left ventricular adjuvant drugs or instruments are all important measures to help patients recover smoothly.
5.Reflections on Developing Health Standards Based on Evidence-based Concept

National health standards involve all kinds of technical requirements formulated and numbered in accordance with the procedures and formats stipulated in the standardisation system for the implementation of national health and hygiene laws, regulations and policies, and the protection of human health. The establishment of health standards in China should align with our legal framework, including laws, regulations, departmental rules, and health and hygiene policies. During the development of these standards, a comprehensive approach is advocated, encompassing in-depth investigations, rigorous demonstrations, and extensive stakeholder engagement. However, the process of standard formulation may suffer from insufficient research evidence. The evidence-based concept emphasizes the significance of evidence. Therefore, integrating evidence-based concept into the process of developing health standards can enhance the quality and scientific basis of these standards. This article systematically elucidates the current status and development process of health standards in China, explores the necessity and feasibility of incorporating evidence-based concept into the development of these standards, analyzes the challenges and opportunities, and presents reflections and suggestions.
6.Comparison of Wild and Cultivated Paeoniae Radix Rubra Based on Traditional Quality Evaluation

ObjectiveTo compare wild and cultivated Paeoniae Radix Rubra(PRR) in three aspects, including character, microscope, determination of primary and secondary metabolites. MethodSeventeen batches of wild and nine batches of cultivated PRR were collected,their character data were measured by vernier caliper and scales, and their paraffin sections were made by safranin-fixed green dyeing for the observation of microscopic features. The content of ethanol-soluble extracts and total tannin from wild and cultivated PRR was determined by the method of general principle 2201 and 2202 in the 2020 edition of Chinese Pharmacopoeia, the content of polysaccharides was determined by phenol-sulfuric acid method. Anthrone colorimetry was used to determine the content of starch, and Van Soest method of washing fiber was used to determine the content of fiber. The contents of fructose, glucose and sucrose in wild and cultivated PRR were determined by ultra-high performance liquid chromatography evaporative light scattering detection(UPLC-ELSD), and the secondary metabolites(gallic acid, methyl gallate, catechin, oxypaeoniflorin, albiflorin, paeoniflorin, ellagic acid, 1,3,4,6-tetragalloylglucose, galloylpaeoniflorin, 1,2,3,4,6-O-pentagalloylglucose, naringenin, benzoylpaeoniflorin and benzoylalbiflorin) were determined by UPLC. Principal component analysis(PCA) and orthogonal partial least squares-discriminant analysis(OPLS-DA) were used to analyze the data of wild and cultivated PRR, the contribution of different factors to the difference was determined according to the variable importance in the projection(VIP) value>1 and P<0.05. ResultIn term of characters, wild PRR showed the traditional characteristic of Zaopi Fencha, its outer skin was loose and easy to fall off, its surface had longitudinal furrow and wrinkle, but the outer skin of cultivated PRR was not easy to fall off, and its surface was relatively smooth. The radial texture of xylem of wild PRR cross-section was more obvious, showing radial striations, vacuoles and more cracks, while the radial texture of xylem of cultivated PRR cross-section was not obvious, dense and some had cracks. Microscopically, the number of radial vessels arranged in the xylem of wild PRR was more than that of cultivated PRR, the number of calcium oxalate clusters in the phloem and xylem of wild PRR was more than that of cultivated PRR, while the number of starch grains was significantly higher in cultivated PRR. In terms of the content of primary chemical constituents, the contents of polysaccharides and starch of cultivated PRR were significantly higher than those of wild PRR(P<0.05), while the contents of cellulose, lignin, fructose and glucose of wild PRR were significantly higher than those of cultivated PRR(P<0.05). The results of determination of 13 secondary metabolites showed that the contents of paeoniflorin, methyl gallate, catechin and oxypaeoniflorin in wild PRR were significantly higher than those in cultivated PRR(P<0.05), while the contents of albiflorin, gallic acid, ellagic acid, naringenin, benzoylpaeoniflorin and benzoylalbiflorin were significantly lower than those of cultivated PRR(P<0.05). A total of 10 variables contributing to the differentiation between wild and cultivated PRR were screened, including albiflorin, cellulose, benzoylpaeoniflorin, oxypaeoniflorin, naringenin, ellagic acid, starch, lignin, paeoniflorin and total tannins. ConclusionThere are significant differences between wild and cultivated PRR in characters, microscopic characteristics, contents of primary and secondary metabolites. It is suggested that the content ratio of paeoniflorin and albiflorin, the contents of oxypaeoniflorin and cellulose can be used as indicators to characterize production methods of PRR so as to improve the quality standard of PRR. This study can provide reference for the improvement of quality standard of PRR and the guidance of high quality production of PRR.
