Objective To investigate the pathogenesis of avascular necrosis of the femoral head(ANFH) and to observe the effects of uhrashortwave diathermy treatment of ANFH through animal experiments.Methods A total of 30 New Zealand white rabbits were randomly divided into three groups:a control group,a model group,and a diathermy group.All the groups were injected with horse serum and methylprednisolone to induce ANFH.The path- ological effects were observed.Results The amount of osteoblast in the model group was significantly less than in the control group,while in the diathermy group it was significantly increased compared with the control group.The a- mount of osteoclast in the model and diathermy groups was significantly higher than in the control group,and in the diathermy group it was significantly more than in the model group.The thickness of femoral head cartilage in the mo- del and diathermy groups was reduced compared with the control group,while it was thicker in the model group than in the diathermy group.The empty cartilage cell lacunae ratios of the model and diathermy groups were significantly higher than for the control group,and the diathermy group showed significant degradation compared to the model group.The density of blood vessels under the cartilage in the model group was significantly less compared with the control group,while in the diathermy group it was significantly increased compared with the control group.The width of bone trabeculae in the model and diathermy groups was significantly less compared with the control group,while they were significantly wider in the diathermy group compared with the model group.The diameters of fat cells in the model and diathermy groups were increased compared with the control group,while they were significantly smaller in the dia- thermy group compared with the model group.The adipocyte area rates in the model and diathermy groups were signifi- cantly elevated compared with the control group,and rates in the model group were significantly elevated compared with the diathermy group.Conclusion Ultrashortwave diathermy is an effective treatment for early stage ANFH.

BACKGROUND:During artificial knee replacement for al kinds of severe knee disease or conservative treatment for end-stage knee disease, Deluxe-PS knee prosthesis and PFC Sigma RP knee prosthesis have their advantages.
OBJECTIVE: To compare and analyze the matching of the Deluxe-PS and PFC Sigma RP knee prostheses with bone cross section.
METHODS:According to the use of the knee prosthesis, 87 gonarthritis patients undergoing bilateral knee replacement were divided into two groups. 46 cases in the test group received Deluxe-PS knee prosthesis replacement. 41 cases in the control group received PFC Sigma RP knee prosthesis replacement. Operation time and intraoperative blood loss were compared between the two groups. American Knee Society score, knee score of American Special Surgical Hospital and knee joint range of motion were used to evaluate knee joint function before replacement, 3 months after replacement and during final folow-up. Anteroposterior X-ray films were obtained after replacement to measure valgus angle and flexion angle of femoral prosthesis.
RESULTS AND CONCLUSION: (1) Operation time and intraoperative blood loss were significantly better in the test group than in the control group (P< 0.05). (2) No significant difference in American Knee Society score, knee score of American Special Surgical Hospital and knee joint motion was detected between the two groups before replacement and 3 months after replacement (P > 0.05). Above indexes were significantly improved in both groups at 3 months after replacement and during final follow-up (P < 0.05). (3) No significant difference in valgus angle and flexion angle of femoral prosthesis was identified between test and control groups during final follow-up (P > 0.05). (4) These findings indicate that effect of Deluxe-PS type artificial knee prosthesis was identical to that of PFC Sigma RP knee prosthesis in knee osteoarthritis patients receiving bilateral knee joint replacement. Deluxe-PS type artificial knee prosthesis can perfectly realize high-accuracy combination of the prosthesis and the bone cross section, and enhance the fusion suitability of the prosthesis and the bone surface. Deluxe-PS type artificial knee prosthesis has the advantage of operation. This advantage is possibly because Deluxe-PS type artificial knee prosthesis is more in line with the anatomical structure of the knee joint of Chinese.

Objective To discuss the quantitative diagnosis of stagnation of Qi and blood stasis syndrome in chronic prostatitis. To make diagnosis chart and ensure diagnosis threshold level which provide statistics evidence for syndrome differentiation of TCM. Methods By the statistical ways, 168 cases of chronic prostatitis belong to stagnation of Qi blood and stasis syndrome and 198 cases of non-stagnation of Qi and blood stasis syndrome were investigated. To make a diagnosis chart and ensure diagnosis threshold level by applying the method of the maximum likelihood discriminatory analysis. Results The quantitative diagnosis chart was made and diagnosis threshold level was 26. According to the retrospective and prospective test, its sensitivity, especially degree, coincidence rate, error rate and positive likelihood ratio were 94.64%, 88.89%, 91.53%, 8.47%, 8.52 and 94.28%, 90.32%, 92.42%, 7.58%, 9.74. Conclusion The indexes of the quantitative diagnosis have good objectivity. According to the retrospective and prospective test, the diagnosis chart was proved to be practical.

Objective To observe the effect of ultrashort wave diathermy on collagen type I expression in early stage of hormon-induced avascular necrosis of the femoral head (ANFH) in rabbit.Methods A total of 40 New Zealand white rabbits were randomly divided into two groups:a control group (n=10) and a treatment group (n =30).All the animals in the treatment group were injected with horse serum and methylprednisolone to establish the ANFH model,and then divided into 2 subgroups:a model group and a ultrashort wave diathermy group,which were treated accordingly.After 12 weeks of treatment,all the animals were sacrificed and collagen type I expression in the femoral head was observed.Results It was shown that the expression of the collagen type I was significantly lower in the model animals than that in the controls as indicated by the stronger immunohistochemistry staining for rabbit collagen type,while that of the ultrashort wave diathermy group was significant higher than the control group ( P

OBJECTIVETo compare the clinical effect and complications of subfascial breast augmentation and submammary breast augmentation.

METHODFrom Sept. 2009 to May 2012 , 25 patients with subfascial breast augmentation and 31 patients with submammary breast augmentation were observed. The postoperative results including visible implant edge or ripple, upper pole of the implant and long-term implant ptosis were compared respectively. The complications including hematoma, infection and capsular contraction were also recorded.

RESULTS56 cases were followed up for 2 months to 26 months. The incidence rate of visible implant edge or ripple was 4.0% (1/25 ) in the subfascial group and 29.0% (9/31) in the submammary group, showing a significant difference between them ( PC 0.05). The incidence rate of convex upper pole of the implant was 8.0% (2/25) in the subfascial group and 35.5% (11/31) in the submammary group, showing a significant difference between them ( P < 0.05). Long-term implant ptosis was not found in the two groups. The incidence rate of hematoma was 4.0% (1/25) in the subfascial group and 6.5% (2/31) in the submammary group, infection was not found. The incidence rate of capsular contraction was 8.0% (2/25) in the subfascial group and 12.9% (4/31) in the submammary group, showing no statistical difference between them ( P > 0.05 ).

CONCLUSIONSSubfascial breast augmentation has more clinical advantages compared with submammary breast augmentation, but no evident difference was found in the common complication rate, such as capsular contraction.

Objective To investigate the inhibitory effect of dexamethasone on residual Lewis lung cancer cells in mice after palliative surgery.Methods The model of residual Lewis lung cancer cells in C57BL mice after palliative surgery were established,then accordance with the random number table,18 mice were randomly divided into 3 groups with 6 animals in each group:the normal saline group,cisplatin group,and dexamethasone group.After operation,the subcutaneous tumor nodules of each mouse were measured on days 4-10 using vernier calipers (accuracy of 0.l mm).The expressions of hypoxia induction factor-1α (HIF-1 α) and mean vascular density (MVD) in the normal saline group,cisplatin group and dexamethasone group were assessed by paraffin immunohistochemical assay.The expressions of HIF-1α,VEGF and PCNA mRNA and protein in the three groups were assessed by real-time quantitative PCR and Western blotting.Results Tumor growth curve showed that the tumor volume in cisplatin group (200.34 ± 20.94) mm3 and in dexamethasone group (436.58 ± 37.94)mm3 were obviously decreased compared with the normal saline group (1 398.81 ± 192.85) mm3,with statistically significant differences (t =-1201.75,P ＜ 0.001;t =-921.52,P ＜ 0.001).As Paraffin immunohistochemical assay showed,in cisplatin group and dexamethasone group,the expressions of HIF-1 α(2.67 ± 0.43,1.67 ± 0.43) and MVD counts (17.01 ± 3.24,9.89 ± 2.25) were decreased significantly compared with the normal saline (4.21 ± 0.35,29.75 ± 5.64),with statistically significant differences (t =-1.55,P＜0.001;t=-1.83,P＜0.001;t=-12.68,P＜0.001;t=-18.35,P＜0.001).The results of real-time quantitative PCR showed that the expressions of HIF-1α (0.56 ±0.11),VEGF (0.61 ±0.18) and PCNA mRNA (0.38 ± 0.07) in dexamethasone group were obviously reduced compared with the normal saline group (1.21 ±0.13,1.13 ± 0.26,1.06 ± 0.08),with statistically significant differences (t =-0.55,P ＜ 0.001;t=-0.62,P＜0.001;t=-0.69,P＜0.001).The expressions of HIF-1α (0.31 ±0.12),VEGF (0.30 ± 0.13) and PCNA mRNA (0.18 ± 0.06) in cisplatin group were also obviously reduced compared with the normal saline group,with statistically significant differences (t =-0.73,P ＜ 0.001;t =-0.76,P ＜ 0.001;t =-0.81,P ＜ 0.001).The results of Western blotting showed that the expressions of HIF-1α (85.98 ± 20.86),VEGF (173.28 ± 30.98) and PCNA protein (228.96 ± 22.97) in dexamethasone group were decreased significantly compared with the normal saline group (198.98 ± 29.89,378.98 ± 28.98,357.98 ± 35.98),with statistically significant differences (t =98.78,P ＜ 0.001;t =85.68,P ＜ 0.001;t =120.86,P ＜ 0.001).The expressions of HIF-1 α (65.78 ± 18.62),VEGF (109.43 ± 19.86) and PCNA protein (176.86 ± 22.76) in cisplatin group were decreased significantly compared with the normal saline group,with statistically significant differences (t =132.86,P ＜ 0.001;t =108.68,P ＜ 0.001;t =154.74,P ＜ 0.001).Conclusion Dexamethasone can effectively inhibit the growth and angiogenesis of the residual Lewis lung carcinoma after palliative surgery in mice,and may also provide a new method of postoperative adjuvant therapy for patients,especially who received palliative surgery.

OBJECTIVETo compare the difference of anterior knee pain after total knee arthroplasty (TKA) between the ways using periosteal dissector and electric scalpel to release medial collateral ligament and pes anserinus.

METHODSFrom September 2009 to September 2012, 220 patients with unilateral osteoarthritis were treated with primary TKA in hospital 301. All the patients were randomly divided into periosteal dissector group (110 cases) or electric scalpel group (110 cases). In the periosteal dissector group, there were 47 males and 63 females,with an average age of (58.8 +/- 17.2) years old; the degree of genuavarus was (14.0 +/- 3.5) degrees; the weight was (65.6 +/- 12.8) kg; the body mass index (BMI) was (26.6 +/- 3.6) kg/m2. In the electric scalpel group,there were 49 males and 61 females,with an average age of (59.6 +/- 16.7) years old;the degree of genuavarus was (15.0 +/- 4.7) degrees; the weight was (66.4 +/- 13.4) kg; the BMI was (27.4 +/- 4.1) kg/m2. The mean follow-up period was 24.6 months. The AKS, VAS and HSS were used to evaluate clinical results.

RESULTSAll incisions healed at the first stage;no deep vein thrombosis of lower limbs or pulmonary embolism occurred. Knee infection occurred in 3 cases (1 in the periosteal dissector group and 2 in the electric scalpel group), and the 3 patients received stage 2 total knee revision using antibiotic bone cement and TC3 prosthesis. No recurrence of infection occurred during follow-up. Among the 20 patients who had anterior knee pain, 16 patients were in the periosteal dissector group and 4 patients were in the electric scalpel group. The occurrence rate of anterior knee pain in the electric scalpel group was lower than that in the periosteal dissector group. The AKS knee score and HSS score after total knee arthroplasty in the electric scalpel group were all higher than those in the periosteal dissector group, and the VAS in electric scalpel group was lower than that of periosteal dossector group.

CONCLUSIONCompared with using electric scalpel,using periosteal dissector used to release medial collateral ligament and pes anserinus may cause more anterior knee pain after total knee arthroplasty.
