1.Three-dimensional finite element model for biomechanical analysis of stress in knee inversion and external rotation after posterior cruciate ligament rupture

BACKGROUND:The incidence of posterior cruciate ligament injury combined with other knee ligament injuries is relatively high, but studies on the internal dynamic stress change of these ligaments are stil rare. OBJECTIVE:To simulate three-dimensional finite element model of the normal human knee and the complete posterior cruciate ligament rupture (III injury), calculate and analyze the stress distribution and changes of the medial collateral ligament, the lateral collateral ligament and the anterior cruciate ligament in the normal knee model and the complete posterior cruciate ligament rupture model. METHODS:We collected CT and MRI images of the healthy male right knee, and established three-dimensional finite element model of the normal human knee by using E-feature Biomedical Modeler, and ANSYS software. The simulation of a flexion moment at 0°, 30°, 60°, varus/valgus and internal/external rotation torque was applied on the established three-dimensional finite element model of the normal knee. The stress of posterior cruciate ligament, lateral collateral ligament, anterior cruciate ligament and medial col ateral ligament was analyzed. During simulation of complete posterior cruciate ligament rupture, the stress of lateral coll ateral ligament, anterior cruciate ligament and medial col ateral ligament was analyzed. RESULTS AND CONCLUSION:Three-dimensional finite element biomechanical analysis data revealed that after posterior cruciate ligament rupture, the stress of lateral coll ateral ligament during inversion and external rotation of the knee joint obviously increased, but the stress of anterior cruciate ligament and medial collateral ligament slightly altered. These findings suggested that posterior cruciate ligament rupture may cause secondary injury to other ligaments of the knee joint, and further support the importance of the dynamic stability of the knee joint.
2.Surveillance of antimicrobial resistance in the clinical isolates from Chuzhou First Hospital, Anhui Province in 2015

Objective To investigate?the?distribution?and?antibiotic?resistance?profile?of?clinical?isolates?in?2015?for?improving?antimicrobial treatment in Chuzhou First Hospital. Methods Bacterial identification and antimicrobial susceptibility testing were?performed?with?MicroScan?WalkAway?96?PLUS?bacterial?identification?system.?All?the?data?were?analyzed?by?WHONET5.6?software. Results A total of 1499 clinical isolates were collected during 2015, of which gram positive organisms and gram negative organisms accounted for 24.7% and 75.3%, respectively. Prevalence of MRSA among S. aureus was 37.0% and MRCNS among CNS 86.3%. All (100%) Staphylococcus and E. faecalis isolates were susceptible to vancomycin, daptomycin and linezolid. About 2.8% of the E. faecium isolates were resistant to vancomycin. All the Streptococcus pneumoniae isolates were penicillin-susceptible, mainly isolated from children. The prevalence of extended-spectrum beta-lactamases (ESBLs) positive strains was 52.9% in E. coli, 30.8% in Klebsiella spp. (K. pneumoniae and K. oxytoca) and 25.0% in P. mirabilis. The ESBLs-positive strains showed higher resistance rate than ESBLs-negative strains to most antibiotics. The percentage of the P. aeruginosa isolates resistant to meropenem and imipenem was all 29.4%. The percentage of the Acinetobacter spp. (A. baumannii accounted for 85.5%) strains resistant to imipenem and meropenem was 63.0% and 64.5%, respectively. Conclusions Antibiotic resistance is still serious in Chuzhou First Hospital in 2015, especially the emerging carbapenem-resistant Enterobacteriaceae, which is of great concern.
3.Therapeutic Observation of Acupuncture-moxibustion for Chronic Simple Rhinitis Due to Yang Deficiency

Objective To observe the clinical efficacy of XIE’s Wen Du Qu Mai (Warming the Governor Vessel to expel smog) needling method plus heat-sensitive moxibustion in treating chronic simple rhinitis due to yang deficiency. Method Eighty patients with chronic simple rhinitis due to yang deficiency were randomized into a treatment group and a control group, 40 in each group. The treatment group was intervened by acupuncture plus heat-sensitive moxibustion, while the control group was by medication. The symptom and sign score was observed before and after 1-week treatment, and the clinical efficacy was compared. Result The total effective rate was 85.0%in the treatment group versus 55.0% in the control group, and the difference was statistically significant (P<0.05). The symptom and sign scores were changed significantly after treatment in both groups (P<0.01,P<0.05). After treatment, the symptom and sign score in the treatment group was significantly different from that in the control group (P<0.05). Conclusion XIE’s Wen Du Qu Mai needling method is effective in treating chronic simple rhinitis due to yang deficiency.
4.Analysis of the curative effects of reoperation on recurrent calculus of intrahepatic duct

7.Clinical application of axial joint space in prediction and evaluation of cervical vertigo

Objective To explore the value of the joint space in the prediction and evaluation of cervical vertigo(CV).Methods 45 CV patients were selected in our hospital from May 2011 to March 2015 as the observation group,and 45 healthy people in our hospital were selected as control group in the same period.The differences of atlas axis joint space including the atlantodental interval(ADI),LADI and VBLADI between the two groups were measured by three dimensional CT,evaluated the difference of indexes of joint space,CV symptom and function score of patients in observation group before treatment and 3 weeks after treatment were observed by CV symptom and function assess-ment scale,and analyzed theirs correlation.Results The differences of ADI and VBLADI between the two groups were statistically significant(t =3.430,87.753,all P <0.05 ).The vertigo,neck pain,headache,daily life and psychological of work and social adaptive capacity scores,ADI and VBLADI levels in the observation group were significantly decreased after treatment(t =67.236,31.795,25.198,18.494,19.180,98.194,10.972,75.895,all P <0.05).Correlation analysis showed that the ADI,VBLADI and the indexes scores of CV symptoms and function were positively correlated(r =0.456,0.829,0.578,0.456,0.378,all P <0.05;r =0.512,0.398,0.782,0.412, 0.477,0.891,all P <0.05).Conclusion Axial joint disease is an important factor in the pathogenesis of CV,and there is a significant correlation between the axial joint space and CV,which can be used as a predictive index for the assessment and treatment of patients with CV.
8.Specific expression of nucleolin in cap-stage tooth germ of mouse

Objective To investigate the specific expression of nucleolin in the development of cap-stage tooth germ of mouse embryo,and detect the possible biological function of nucleolin. Methods In situ hybridization and immunohistochemistry were performed to detect the expression of nucleolin mRNA and protein in the development of tooth germ at embryo day 15(E15).Double staining for apoptotic cells and nucleolin was employed to explore the relationship between nucleoin and apoptosis in tooth germ. Results The expression of nucleolin mRNA and protein was mainly detected in the cervical loop,inner epithelium and the underlying dental papilla.Moreover,nucleolin protein was also located in the primary enamel knot and basement membrane.It was revealed by double staining that the fluorescence images of nucleolin and the location of apoptotic cells were not overlapped. Conclusion The expression pattern of nucleolin mRNA do not completely coincide with that of nucleolin protein at the cap stage of tooth germ.The location for nucleolin protein in the basement membrane suggests that nucleolin may be involved in the reciprocal interactions between the inner epithelium and dental papilla mesenchyme,subsequently affect the morphogenesis of tooth germ.
9.The effect of motor imagery therapy on lower extremity function in stroke patients with hemiplegia

Objective To study the effect of motor imagery therapy combined with conventional physical training on hemiplegia lower extremity function in stroke patients with hemiplegia.Methods Thirty hemiplegic patients meeting the eligible criterions were randomly divided into two groups: a treatment group (n = 13 ) and a control group ( n = 17).All subjects accepted conventional rehabilitative treatment.Patients in treatment group were treated with motor imagery therapy after physical training, while the patients in control group were treated with physical training only.Motor function was measured by the percentages of changes in maximum loading on affected lower limb (PL%), Barthel index (BI), 5m maximum back and forth walking speed (5m MBFWS), Berg balance scale ( BBS), Fugl-Meyer motor assessment ( lower limb) (FMA-L) before the beginning of training and at the end of sixweek training.Results In both groups, all measurements at the end of training improved significantly ( P < 0.01 )compared to that at the beginning of training.After the whole training course, PL% and 5m MBFWS (m/s) in treatment group were significantly better compared to those in control group( P < 0.05 ), and the other measurements were not statistically significant between two groups( P > 0.05 ).Conclusion Motor imagery therapy had positive effect on hemiplegia lower extremity function of sub-acute stroke patients.
10.Changes and clinical significance of serum thyroid hormone and TNF-αin severe patients

Objective To explore the changes of thyroid hormone and TNF-α in severe patients which occurring under critical illness and to explore the interrelation of these endocrine factors in order to direct clinical therapy. Methods Serum thyroid hormone and TNF-αin 102 severe patients and 80 healthy volunteers were detected by radioimmunoassay and enzymoimmunoassay. The results were analyzed and compared with normal subjects,groups of different prognosis,MODS group and no MODS group. Results TT3,FT4,FT3 levels in severe pa-tients were lower than that in normal subjects(P <0. 01),By contrast,TNF-α level were significantly higher than that of control group (P<0. 01). As the situation became worse,the above changes were more obvious in severe patients as compared with normal subjects. For TT3 and TNF-α level,the differences in the early stages of the disease (3 ~7rd) in the survival group and death group were significant (P<0. 05). Compared with non-MODS group,TT3,FT4,FT3 and TNF-α changed significantly in MODS group(P<0. 05). Conclusion TNF-α and thyroid hormone levels play important roles in the severe patients. The changes of TNF-αand thyroid hormone levels can be used for severe patients prediction and treatment.
