2.Comparison of effect of standard Gamma nail and proximal femoral nail anti-rotation for geriatric intertrochanteric fractures

[Objective]To investigate and compare the effects of standard gamma nail (SGN) and proximal femoral nail anti-rotation (PFNA) in the treatment of osteoporotic intertrochanteric fractures in the elderly.[Method]From May 2002 to September 2005,55 patients with intertrochanteric fractures were treated with SGN.Sixty-eight patients were treated with PFNA from November 2005 to June 2008.The effects of the two implants about perioperative complications,bone healing and the function of the operated hip joint were compared.[Result]No statistically significant difference was found between the two groups in blood loss,blood transfusion,infection,deep venous thrombosis and hospital stay.One patient developed femoral head avascular necrosis postoperatively in Gamma nail group.The incidence of postoperative hip pain in Gamma nail group was higher than that in the PFNA group,but there was no significant difference in the Harris score.[Conclusion]Gamma nail and PFNA nail are reliable methods to treat geriatric intertrochanteric fractures,with advantages of minimal iatrogenic injury,stable fixation and few complications.
3.Effect Comparative Analysis of Sodium Hyaluronate and Chitosan in the Prevention of Thyroid Postopera-tive Adhesion

Objective:To investigate and compare the effect of sodium hyaluronate and chitosan in the prevention of thyroid post-operative adhesion. Methods:Totally 84 patients with thyroid operation in our hospital from August 2012 to August 2013 were select-ed, and according to the admission time, the patients were respectively divided into group A and B with 42 cases each. In order to pre-vent postoperative adhesion, group A was treated with sodium hyaluronate, and group B was treated with chitosan. The clinical curative effect of the two groups was compared. Results: The adhesion rate in group A was 14. 3%(6/42), which was higher than that in group B (3/42 7. 1%), while the difference was not statistically significant (P>0. 05). All the patients showed no bleeding, aller-gies and other adverse reactions, only in group A, 2 mild infection (4. 8%) occurred. The adverse reactions in the two groups were without significant difference (P>0.05). Conclusion: Sodium hyaluronate and chitosan both have the preventive effect on thyroid postoperative adhesion with similar efficiency and adverse reactions.
4.Preparation and application of multicellular spheroid in cancer therapy

Multicellular spheroid (MCS) can simulate many aspects of the in vivo physiological and pathological conditions in many aspects,better reflect the in-vivo behavior of cells in tumors.So it's increasingly accepted as a valuable tool for evaluating the efficacy of therapeutic intervention including chemotherapy,radiotherapy,immunotherapy and combined therapy.Various spheroid co-culture approaches have been presented to study heterologous cells interaction in solid tumors.The present review briefly introduces the methodology and applications of MCS with focus on the up-to-date information.
6.Treatment of fresh subtrochanteric fracture combined with old femoral neck fracture with hemiarthoplasty through anterolateral approach.

OBJECTIVETo explore the clinical effects of one-stage hemiarthroplasty in treating fresh subtrochanteric fracture combined with old femoral neck fracture.
METHODSFrom October 2003 to October 2013, 7 patients with fresh subtrochanteric fracture and old femoral neck fracture were treated with hemiarthroplasty in one stage. There were 5 males and 2 females, aged from 69 to 80 years old with an average of 74.5 years. According to the Garden classification, 2 cases were type IV and 5 cases were type V. According to the A0 classification of subtrochanteric fracture, all cases were type A1 (long oblique femoral subtrochanteric fractures). Biological coating long handle femoral prosthesis was used in the patients. The hip joint functions were assessed according to Charnley scoring system after all hemiarthroplasty.
RESULTSSeven patients were followed up from 12 to 24 months with an average of 18 months. There were 1 case with urinary system infection and 1 case with deep venous thrombosis. Of all, 5 cases recovered the walk ability at 6 months after operation and 2 cases could walk with the help of the crutch at 1 year after operation. According to Charnley scoring system to assess the hip joint function, the total Charnley scores reached 15.0±2.5, and 4 cases got an excellent results, 2 good, 1 fair.
CONCLUSIONArthroplasty through anterolateral approach can obtain satisfactory clinical effects in treating fresh subtrochanteric fracture combined with old femoral neck fracture, it has advantages of good joint stability, little pain, less complication, quickly recover, early activities on fields.

7.Optimization of Fermentation Medium for Honggumycin Using Response Surface Methodology

Under the shaking-flask culture, fermentation medium of honggumycin produced by Streptomyces 702 were studied.The experiment was used response surface methodology to optimize the shaking-flask fermentation medium.Firstly, we applied full factorial design to screen important factors soybean meal and industrial peptone which affected hongmycin produced by Streptomyces 702.Furthermore, we designed experiment to obtain the steepest ascent path and optimal level by the central composite design.The optimum medium consisted of (g/L): maize starch 20, maize meal 20, glucose 20, soybean meal 23, industrial peptone 9, KNO_ 3 2.5, (NH_ 4 )_ 2 SO_ 4 2.5 KH_ 2 PO_ 4 0.3, NaCl 3, CaCO_ 3 6, bean oil 5mL/L.Under the optimal medium, the yield of honggumycin was up to 1500 g/mL, which was increased by 308% than the original medium.

Objective To evaluate the protective effect of leukocyte-depleted warm blood cardioplegia on immature myocardium in infant.Methods Thirty infants with congenital heart disease whose American Society of Anesthesiologists(ASA) were Ⅱ-Ⅲ,aortic clamping time and bypass time were more than 30,40 minutes were respectively selected and divided into 2 groups:Experimental group and control group.Experimental group were perfused with leukocyte-depleted warm blood cardioplegia while control group perfused with common warm blood cardiplegia.Under monitoring the hemodynamics at surgery,the serum levels of troponin I(cTnI) and intercellular adhesion molecule-1(ICAM-1) in heparinized anticoagulant blood samples from radial artery at different time points [anesthesia induction and before extracorporeal circulation(T1),30 min after aortic clamping(T2) and aortic declamping 5 min(T3),15 min(T4)]were detected.Three pieces of cardiac muscle were taken from right atrium at different time points and the levels of myeloperoxidase(MPO) were detected.Results 1.The serum cTnI and ICAM-1 levels after aortic declamping were higher than those before operation,and the levels in experimental group were lower than those in control group(t=2.358,2.533,2.30,2.639 Pa
9.Minimally invasive plate osteosynthesis for treatment of proximal humeral fractures through anterolateral acromial approach

Objective:To investigate the feasibility,surgical technique and effect of osteosynthesis for proximal humerus fractures through the minimal incision anterolateral acromion.Methods: In this study,32 patients of proximal humeral fractures were selected and treated through the minimal incision antero-lateral acromion between January 2012 and October 2014.Of these patients,there were 19 males and 13 females,with mean age 46 years old (range: 22-78 years).The mechanism of injury was a trauma of simple fall for 21 patients and motor vehicle accident for 11 patients.According to Neer classification,18 cases had two-part fractures,12 had three-part fractures,and 2 had four-part fractures.All patients were sustained operation of open reduction and internal fixation.The average delay between trauma and surgery was 4 d (0.5-10.0 d).The operations were performed in invariable operating room with general anesthesia.The fractures were reduced and immobilized with anatomical locking plate through the minimal incision anterolateral acromion during operation.Patients were received passive motion after operation.Results: All the 32 cases were followed up ranging from 3 to 34 months with an average of 11 months.All the fractures were healed.There was neither failure of internal fixation nor incision infection case.The functional results of the shoulder,according to Constant-Murley score was on an average 86 (range: 69-100).Conclusion: The treatment for proximal humeral fractures through the minimal incision anterola-teral acromion approach is a feasible and effective method,which shows good results for treating proximal humerus fractures with correct indication.There are several advantages compared with conventional deltopectoral approach.They are minimal soft tissue disruption,minimal blood loss and easily to manipulate technique.But it required selection of indication strictly and surgeon's experience.
10.Clinical effect of Zero-profile anterior cervical interbody fusion on single-segment cervical spondylosis in the older adults

Objective To investigate the clinical effect of Zero-profile anterior cervical interbody fusion on single-segment cervical spondylosis in the elderly, and to compare with the effect of traditional anterior cervical decompression and fusion (ACDF) with titanium plate.Methods Clinical data of elderly patients who underwent anterior cervical discectomy and fusion operation (one-or two-level) in our department from June 2009 to March 2014 were retrospectively analyzed.The 49 patients who took anterior cervical interbody fusion with the Zero-profile cage were considered as the Zero-P group, and the other 60 patients who received ACDF with titanium plate as control (ACDFP group).The operation time, blood loss, Neck Disability Index (NDI) score before and after operation, Cobb angle of the cervical spine and Cobb angle of the operated segment before and after operation, the fusion rate 1 year after operation, the dysphagia rate after operation according to Bazaz index were recorded and assessed.Results The operation time and blood loss had no statistical difference between the two group (P>0.05 for both).The NDI scores were declined in the two group after operation as compared with before treatment (P<0.05 for both), while there were no statistical difference in NDI score between the two groups (P>0.05).The Cobb angle of cervical spine had little improvement 1 day after operation as compared with before operation, and improved significantly 3 month after operation in the two group.The Cobb angle of operated segment (Cobb S) was reduced 3 months in the two group after operation as compared with before treatment, and the Cobb S was declined much more in Zero-P group than in control group (P<0.01).There was no statistical difference in the number of patients with successful fusion between the two group (46 cases vs.56 cases, x2 =0.08, P> 0.05).Conclusions The clinical effect of Zero-profile anterior cervical interbody fusion is equivalent as the traditional ACDFP.Zero-profile anterior cervical interbody fusion has less exposure and blood loss, which is more suitable for the elderly patients with cervical degenerative disease.
