1.The research of the analgesic effects by acupuncture at jiaji points in rat of incisional pain

Objective To observe the effect of the application of acupuncture Jiaji points and continuous acupoints infusion of analgesic drugs at Jiajji in the treatment of postoperative pain on rat behavior through the establishment of a rat model of incisional pain .Methods 60 healthy male SD rats ,were randomly divided into sham control group ,PI group ,CSI group ,EA group and CAI group ,12 in each group .Plantar incision pain model in rats was established according to the method of Brennan ,rats in 5 groups at preoperative 1 h and 6 ,24 ,48 h after operation ,mechanical withdrawal threshold(MWT) and thermal withdrawal latency(TWL) were evaluated .Results Changes in MWT :5 groups of rats with experimental basis of mechanical withdrawal threshold values showed no significant difference among them ;comparison of the mechanical withdrawal threshold in the sham control group at each time point showed no significant difference(P>0 .05);when compared with sham control group ,MWT in PI group ,CSI group ,EA group and CAI group decreased significantly at 6 ,24 ,48 h after operation (P<0 .05);when compared with PI group ,MWT in CSI group ,EA group and CAI group increased significantly at 6 ,24 ,48 h after operation(P<0 .05);when compared with CSI group and EA group ,MWT in CAI group increased significantly at 6 ,24 ,48 h after operation (P<0 .05) .Changes of TWL :when compared with the sham control group ,TWL decreased significantly in PI group at 6 ,24 ,48 h after operation (P< 0 .05);when compared with the sham group ,TWL in CSI group ,EA group ,CAI group increased significantly at 6 ,24 ,48 h after operation (P<0 .05);when compared with CSI group and EA group ,TWL in CAI group increased significantly at 6 ,24 ,48 h after operation (P<0 .05) . Conclusion Electroacupuncture and continuous acupoint infusion of analgesic drugs at Jiaji could produce obvious analgesic effect , the analgesia effect of continuous acupoint infusion at Jiaji is better than subcutaneous analgesia .
2.Expression difference of cerebral cortex and liver glucocorticoid receptors at protein level following severe closed traumatic brain injury in awake mice

Objective To study the relationship of expression of central cortex glucocorticoid receptor (GR) at protein level with GR expression in the liver at protein level and with changes of serum cortisol and adrenocorticotropic hormone (ACTH) following severe closed traumatic brain injury (TBI) in mice. Methods Severe TBI was established in awake mice by using a BIM-Ⅲ biomechanical machine. At 0.5, 2, 8, 24, 48 and 72 hours after TBI, the total cytosolic GR in the cortex and liver were detected with Western blotting. Levels of serum ACTH and cortisol were measured by ELISA technique and radio-immunological assay (RIA) respectively. Results The expression of GR both in the cortex and liver were obviously down-regulated at protein levels at 2-72 hours after TBI and increased slowly eight hours after injury. The GR in the liver showed no recovery at 72 hours after injury and that in the cortex was decreased continually at 24 hours after injury. Serum ACTH and cortisol levels were increased markedly compared with control group, when there were two different peaks in the observation curve.Conclusion There is glucocorticoid resistance both in the central and peripheral tissues after severe closed TBI in the awake mice, which changes in a time-dependent manner.
3.Diagnosis and treatment of anorectal malignant melanoma: a report of 25 cases

Objective To explore the biological characteristics and treatment of anorectal malignant melanoma. Methods A retrospective analysis of the clinical and pathological features and the results of surgical treatment of 25 cases with anorectal malignant melanoma admitted to the Haicheng Second People′s Hospital and General Hospital of PLA from 1985 to 2005 was made. Results There were 10 males and 15 females with their mean age of 59 years. 21 out of 25 cases underwent surgery. In 18 patients curative resection (Miles operation) was performed. Follow-up study was done in 20 cases for 4 months to 6 years. Five year survival rate of 25 cases was 12%. Five year survival rate of radical operation was 16.7%. Conclusion Anorectal melanoma is an aggressive malignant tumor. Prognosis is poor. Regional lymph node metastasis is the major factor in influencing prognosis. Radical operation and lymph nodes eradication are imperative. Early diagnosis and treatment are the key to raise the survival rate.
4.Protective effect of hydrogen-rich saline on liver mitochondrial in-jury in rats with ischemia-reperfusion

Objective:To study the effect of hydrogen-rich saline on the prevention of liver mitochondrial injury induced by isch-emia-reperfusion (I/R) in rats. Methods:Male Sprague-Dawley rats (n=42) were randomly divided into three experimental groups (n=14), namely, sham-operated, I/R plus saline-treated (5 mL/kg, i.p.), and I/R plus hydrogen-rich saline-treated (5 mL/kg, i.p.) groups. Mi-tochondrial oxidative stress and functions, as well as morphologic changes in the mitochondria, were detected 180 min after reperfu-sion. Results:After I/R, mitochondrial swelling and vague cristae were observed. Mitochondrial functions were significantly decreased, but mitochondrial MDA and GSSH contents were increased by I/R. Hydrogen-rich saline reduced these markers, improved mitochondri-al ATP content and respiratory function, and relieved morphological mitochondrial injury (P<0.01). Conclusion:Hydrogen-rich saline attenuated I/R-induced liver mitochondrial damage and reduced mitochondrial oxidative stress.
5.Protective effect of hydrogen-rich saline on ischemia-reperfusion-induced liver injury in rats

Objective To study the effects of hydrogen-rich saline (HS) on the protection of hepatic injury induced by ischemia-reperfusion (I/R) in rats.Methods Male Sprague-Dawley rats (n =60) were randomly divided into three experimental groups:sham-operated group,I/R plus saline treatment group,and I/R plus hydrogen-rich saline treatment group.Reperfusion liver was conducted 180 mins after liver ischemia.Blood samples and liver tissues were collected.Result Serum ALT,AST levels,and MDA content,HMGB1,TNF-α,IL-1β and IL-6 levels in liver tissue were increased,but SOD and GSH activity were decreased significantly by I/R.Hydrogen-rich saline reduced oxidative stress and inflammatory reaction,and relieved morphological liver injury (P < 0.01).Conclusion Hydrogen-rich saline attenuated I/R induced liver damage by reduction of oxidative injury and inflammatory reaction.
6.Nonfunctional malignant adrenal tumor(report of 12 cases)

Objective To review the clinical features,diagnosis,management and prognosis of patients with nonfunctional malignant adrenal tumors. Methods 12 patients admitted with nonfunctional malignant adrenal tumors from 1990 to 2000 were reviewed.Among them 6 cases had symptoms of fever,ostealgia,etc.The tumor measured 3 cm in 1 case,5 to 10 cm in 9 cases and more than 10 cm in 2 cases.Ten cases developed local invasion and (or) matastasis. Results Five cases underwent tumor excision or enucleation and 5 underwent biopsy only.Of these 10 cases,4 were pathologically diagnosed as cortical adenocarcinoma,4 as metastatic carcinoma,1 as mucous liposarcoma and 1 as lymphoma.The remaining 2 cases had no operation.One case died during hospital stay.Ten cases died within 2 years.One case of adrenocortical carcinoma experienced relapse 3 years after initial operation and died next year. Conclusions Nonfunctional malignant adrenal tumor is rare and difficult to diagnosis in early stage,indicating an extremely poor prognosis.Tumor diameter is an important factor to be considered regarding surgical indication.
7.Preservation of cervical plexus in the functional neck dissection of differentiated thyroid cancer

Purpose:To discuss the functional neck dissection preserving cervical plexus in the treatment of differentiated thyroid cancer.Methods:15 cases of papillary thyroid cancer underwent functional neck dissection while preserving the cervical plexus. Results:Localization of positive lymph nodes were Ⅲ, Ⅳ, Ⅵ. The rate of neck node involvement of Ⅵ (+) is 57. 1%. No impairment of sensation of ears and lower necks and upper shoulders were found in the 15 patients. 8.Effect of Epidural Anesthesia With 0.375% Ropivacaine on Hemodynamics in Breast Cancer Radical Operation

Objective To observe the changes of hemodynamics during 0 375% ropivacaine epidural anesthesia period,and 0 375% bupivacaine as control in patients have undergoing breast cancer radical operation.Methods Sixty-one patients were randomly designed to receive 0 375% ropivacaine(groupⅠ,n=30)or 0 375% bupivacain(groupⅡ,n=31)epidural anesthesia respectively during breast cancer radical operation.BP,HR and SpO 2 were assessed at 5min,10min,15min and 20min after first full doses.Results The changes of BP,HR and SatO 2 at 5min,10min,15min and 20min after first full doses were obviously lower in groupⅠ than that in groupⅡ(P0 05).No severe adverse event was found during the observation.Conclusions Interference of epidural anesthesia with 0 375% ropivacaine on hemodynamics in breast cancer radical operation is slighter than that of 0 375% bupivacaine.
9.Influence of Budesonide Suspension on Pneumonic Anaphylacticitis and Level of Serum Interleukin-5 of(Asthma)-tic Rats

Objective To compare the inhibition effect of budesonide suspension and dexamethasone on the rats′ pneumonic anaphylacticitis and serum IL-5,and study the effect mechanism in treatment of asthma with glucocorticoids on systemic administration and with budesonide suspension.Methods Twenty-six health wistar rats were randomly divided into normal group,asthmatic group,budesonide suspension group and dexamethasone group.Asthma models were sensitized with egg albumin.Before sensitized,budesonide suspension group were administered inhalation of budesonide suspension,and dexamethasone group were administered abdominal injection of dexamethasone.The level of IL-5 was measured with enzyme linked immunosorbent assay(ELISA) reagent box,lung was eviscera-(ted after collecting) blood,pathological slice was made.Results The rats′ pathological changes of lung and bronchus were significant between asthma group and other groups,while there were no significant difference between budesonide suspension group and dexamethasone group,but both of two groups showed slight difference with normal group.Compared with normal group,the level of serum IL-5 in asthma group raised obviously,and there was significant difference(P0.05).Conclusions The treatment of inhalation of budesonide suspension can inhibit rats′ pneumonic anaphylacticitis,ameliorate rat anaphylactic asthma symptom.
