1.X-ray and CT manifestation of tuberculosis of ankle joint:an analysis of 5 cases

[Objective] To investigate X-ray and CT manifestations of tobereulosis of ankle joint and to discuss the differentiation with other diseases.[Method]The X-ray and CT findings in 5 cases with pathalogically proved tuberculosis of ankle joint were retrospectively analyzed.The differentiation with other diseases was discussed.[Result]The lesions involved entire right ankle joint in all 5 cases,The typical signs were as follows:(1)indistlnct and erosive joint surface,insect bitten-like of bony destruction ofjotht surface,digging-like shape of cancellous bone's destruction;(2)narrowed or irregular widened joint space;(3)ostooporosis of diseased region and surrounding;(4)extensive periostoal reaction of distal fibia and fibula;(5)to form cold abscess and sinus tract.[Conclusion]The tuberculosis of ankle joint earyies certain characteristic X-ray and CT signs,CT is better than X-ray in diagnosis.Highly alert of ankle joint tuberculosis is the key of early correct diagnosis.
2.Characteristics of albuminuria in hospitalized octagenarian and nanogenarian patients

Objective This study was performed to investigate clinical characteristics of albuminuria in octagenarian or nanogenarian and provide primary study for chronic kidney disease in eldly patients.Methods The data of 218 patients with albumin-to-creatinine ratio (ACR)between 30 ~300 mg/g from first morning urine in hospital from June 2012 to December 2014 were analyzed retrospectively.Consec-utive hospitalized old patients were divided into 80 +group(aged from 80 to 89 years old)and 90 +group(age≥90 years old),who were al-so divided into albuminuria positive group(ACR 30 ~300 mg/g)and albuminuria negative group(ACR <30 mg/g)according to the ACR. Cohort study was performed to investigate albuminuria related characteristics.Results Of the 218 hospitalized old men,53.21% developed albuminuria with average Log ACR (1.78 ±0.85)mg/g.Albuminuria was positive in 45.16% of 80 +group compared to that of69.84% in 90 +group(P <0.05).For albuminuria positive old men average Log ACR (2.03 ±0.62)mg/g,while it was Log ACR(1.03 ±0.44)mg/g in al-buminuria negative old men.Positive rate of albuminuria in 80 + group vs.90 + group accompanied with either diabetes,hypertension or eGFR <60 mL·(min·1.73 m2 )-1 were 50% vs.77.5%(P <0.05),50.38% vs.58.82%(P >0.05),66.21% vs.75%(P >0.05), respectively.Of a few old men who were free from diabetes,hypertension or eGFR <60 mL·(min·1.73 m2 )-1 the corresponding rate of al-buminuria were 35.39% (21 /59),16.00%(4 /25)and 25.84% (23 /89),respectively.Conclusion Occurrence of albuminuria was seen in 53.21% of advanced aged hospitalized men,with tendency of higher occurrence rate in nanogenarian than those in the octogenarian.
3.Transcatheter Arterial Chemoembolization for Hepatic Carcinoma with Blood Supply from the Intercostal Arteries

Objective To study the safety of transcatheter arterial chemoembolization(TACE)via the intercostal artery(ICA)for hepatic carcinoma.Methods A total of 24 patients with hepatic carcinoma(HCC)fed by the ICA underwent TACE via the artery in our hospital.Among the cases,15 were mass type and 9 were nodular type.Plain and enhanced CT or MRI scan were performed before the operation.During the procedure,we carried out intercostal arteriography.Selected catheterization of the feeding branch and then TACE were done.The ICA angiographic features,tumor location,clinical observation,laboratory tests,and imageology were evaluated after the surgery.Results In our cases,the HCCs supplied by the ICA collaterals were located at segments six and/or seven.Angiography showed that all the ICA collaterals originated from the right side at the levels of T8(7.5%,3/40),T9(15.0%,6/40),T10(47.5%,19/40),or T11(30.0%,12/40).The procedure was completed in 22 of the patients.CT scan performed after the procedure showed that the tumor was completely filled with lipiodol in 87.5%(21/24)of the cases.The serum level of AFP decreased significantly in 82.6%(19/23)of the patients.Three patients complained of severe pain at the shoulder after the operation,1 patient had skin itching during TACE,and 5 developed skin erythema after the procedure.Conclusions HCC supplied with ICA collaterals are often detected in the patients who have the tumor at the right posterior segment(S6 and S7)and had received TACE for several times.TACE via the ICA is safe but may leads to skin injuries.
4.Apoptosis and expression of iNOS after spinal cord injury in rats

ObjectiveTo study the characteristic of apoptosis and the expression of inducible nitric oxide synthase(iNOS),as well as relation between them after spinal cord injury (SCI) in rats.Methods84 adult SD rats were randomly divided into three groups,and made into mild,moderate and severe acute SCI models.The rats were killed at 4h,8h,1d,3d,7d,14d and 21d after surgery.The histopathology changes in spinal cord tissue were observed with HE staining microscopic examination.The expression of iNOS was determined by immunohistochemistry and the apoptosis was labeled by terminal deoxynucleotidyl transferase mediated dUTP nick end labeling(TUNEL).ResultsThe histopathology changes in spinal cord tissue deteriorated with increasing in injury degree.A little iNOS immunoreactivity (iNOS-IR) was detected in nerve cells,gliocytes,ependymocytes and blood vessel endotheliocytes of normal and vertebra shelf operated controls,increasing at 8 h, and in flood tide in 7 d. The expression of Bax and the rates of TUNEL positive cells were similar with that of iNOS after SCI. There was positive correlation between expression of iNOS and degree of primary SCI ,apoptosis index(r=0.414,P<0.01;r=0.854,P<0.01).Conclusions Expression of iNOS and Bax increase and a large number of TUNEL positive cells present after SCI in rats. There is positive correlation between expression of iNOS and degree of primary SCI,apoptosis index.
5.Inferior Phrenic Arterial Embolization for Massive Hemoptysis of Tuberculosis

Purpose Bronchial arterial embolization is the preferred hemostasis method for hemoptysis when medical treatment is invalid. This paper aims to discuss the safety and efficacy of inferior phrenic artery (IPA) embolization in the treatment of hemoptysis of tuberculosis when IPA is involved. Materials and Methods Twenty-eight patients who were confirmed that IPA got involved in the hemoptysis due to pulmonary tuberculosis by IPA angiography underwent embolization. CT scan was performed before the procedure and IPAs arteriography were performed during the interventional procedure. Once the blood supply was identified, catheterization and embolization was carried out with gelatin sponge particle, sodium polymannuronate microsphere or microcoil according to the patients' conditions. After the procedure, the IPA angiographic manifestations, clinical efficacy and complications were evaluated. Results Thirty-three IPAs were identified as getting involved in the blood supply in the 28 patients. Among those patients, 12 had left IPA involved, 11 had right IPA involved and the rest 5 had IPAs involved in both sides. The selective IPA angiography showed IPAs had enlargement, with numberous and disordered branches and hypervascularity. IPA-pulmonary artery shunt was found in 22 cases. None of the cases was found extravasation of contrast medium. The hemoptysis reoccurred in 4 patients on the second day, sixth day and 6 months after the first embolization, thus the procedures were performed three times in 1 patient and twice in the other 3 patients. Nine patients had fever as complication, 19 patients had chest pain and 5 suffered from mild dyspnea. These complications usually disappeared in 3-7 days after symptomatic treatment. Conclusion IPA embolization is technically feasible and safe, whose complications are likely to be few and self-restrictive.
6.Evaluation on the chemosensitivity of hepatocellular carcinoma during mimetic transcatheter arterial embolization

Objective To investigate whether 5 different chemotherapeutic drugs and their combination of either two drugs could further promote the inhibition on the cell growth of HCC cell line (HepG2) in vitro in the hypoxic and hyponutritional culture medium (HHCM) mimicking the different scenarios of transcatheter arterial chemoembolization (TACE).Methods The cells were treated by 5 drugs for 2 h, 4 h,6 h and 24 h, which include epirubicin (EPI), cisplatin (DDP), mitomycin-C (MMC), oxaliplatin (OXA) and 5-fluorouracil (5-FU) in four concentrations of HHCM (5%, 10%, 25% and 50%) mimicking the scenarios during TACE and the cell viability was assessed using 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay.The combinations of dual drugs treated for 24 h were also tested.Results The sensitive drugs with inhibition rates more than 30% were EPI, MMC and OXA in 4 different concentrations of HHCM.The sensitivity of the drugs treated for 24 h was significantly increased compared with that for 2 h in 5%, 10% and 25% HHCM.The dual combinations did not increase the chemosensitivity of HepG2 cells.Conclusions EPI, MMC and OXA exhibited cytotoxic activity against HepG2 cells in various hypoxia and hyponutrition states.Prolonging the exposure time could increase the sensitivity of drug in HHCM, and the combination of dual drugs cannot enhance the cytotoxic effect.
7.Preliminary Applicaton of Placing Freka R Trelumina guided by DSA

Objective To evaluate the initial clinical application value of placing the Freka R Trelumina guided DSA. Methods The Freka(R) Trelumina was placed guided by DSA in 10 patients with disease of digestive tract post operation complicated with stomal leak or/and stomal stegnosis or various kinds of patients with gastric emptying disorder. Results The Freka(R) Trelumina could be put 20 cm away from Treize anadesma guided by DSA. The achievement ratio was 90%. The putting time was 1 to 3 weeks. The localization of the Freka(R) Trelumina was well. The process of feeding was successfully. The effectiveness of decompression of stomach intestine was also good. Stomal leak healed up quickly. The appearance of gastric emptying disorder disappeared. Conclusion Placing the Freka(R) Trelumina guided by DSA is a simple, safe and reliable method for jejunum nourishment and gastrointestinal decompression.
8.Human Schwann cells from normal nervous tissue cultured in vitro

BACKGROUND:Peripheral nerve tissue engineering needs a large number of Schwann cells.In previous studies,lack of normal human nervous tissue,so animal(rat,rabbit,et al)nervous tissues are commonly used to isolate Schwann cells,but as xeno-cells it is limited in clinical application.OBJECTIVE:To investigate an effective technique for isolation,cultivation and purification of human Schwann cells of normal peripheral nerves cultured in vitro.METHODS:Normal peripheral nerves were obtained from the surgery of cerebral palsy patients.Schwann cells were cultured with enzymatic digestion culture method and differential attachment method.Tissues were cut into pieces and incubated in medium supplemented with fetal bovine serum,collagenase and Oispase enzyme,centrifuged.Tissue blocks were placed in the medium,triturated into monoplast suspension,and then moved into a DMEM Petri dish containing polylysine,supplemented with basic fibroblast growth factor.When adherent cells were confluent about 85% 90%,cells could subculture.Schwann cells were counted by Trypan Blue coloring method at 2,3,4,5,6,7,8,9,10.The purity of Schwann cells was identified through S-100 protein immunohistochemistry staining.RESULTS AND CONCLUSION:More than 0.5×10~8/L Schwann cells were detected after four days under a microscope.Following the third passage,the number of Schwann cells was over 9×10~8/L.The purity of Schwann cell population was up to 85%.Results suggested that plenty and purified human Schwann cells could be obtained by enzymatic digestion culture and differential attachment methods in a short time,which can be used for the source of peripheral nerve tissue engineering.
9.Low testosterone levels are inversely correlated with carotid artery plaque formation in elderly women.

OBJECTIVETo study the relationship between serum testosterone levels and the plaque formation of the carotid artery in a population-based cohort of independently living healthy women above 60 years of age.
METHODSAnalysis of the healthy elders from a population-based cohort study in 9 communities of Beijing. Carotid intima-media thickness and atherosclerotic plaques were determined ultrasonographically. Serum testosterone levels were measured by immunoassay. The data were analyzed with ANOVA and logistic regression analysis.
RESULTSThere was an inverse correlation between testosterone and plaque formation in old females (P < 0.01), while no association was found in males. Female with testosterone levels in the lowest quartile (< 0.49 nmol/L) had more risk of plaque formation (OR = 3.805, P < 0.01) after adjusted with age and other traditional factors of atherosclerosis.
CONCLUSIONTestosterone concentrations are negatively associated with carotid artery atherosclerosis in old women in Beijing, experimental and prospective studies are needed to determine the possible therapeutic role of testosterone in atherosclerosis.

10.Adipose derived mesenchymal stem cell transplantation for Alzheimer's disease

Alzheimer's disease is an irreversible neurodegenerative disease characterized by progressive neuronal loss. To date, there has been no effective medicine or therapy for neurodegenerative disease. With development of stem cell technique and theory, neural stem cell transplantation has been found to be prospective in Alzheimer's disease treatment. However, it was challenged by the deficiency of autologous neural stem cell, which can bypass immunological barrier. Compared with neural stem cells, mesenchymal stem cells exhibit extensive resources, such as liver, bone marrow and adipose, and multiple differentiations into bone, muscle or adipose. Considering the easy access, the minor trauma to the patients, and the neuron differentiation potential of adipose derived mesenchymal stem cells (A-MSC), we hypothesize that A-MSC graft is a potential and innovative strategy for the treatment of Alzheimer's disease.
